맥진기 컨텐츠 개발을 위한 통합 데이터베이스 설계 및 구축 (Design and Construction of Integrated Database for Contents Development of Pulse Analysis System)

  • 인구의 고령화와 의료서비스 수요의 증가로 한의학의 이론을 기초로 한 의료기기 개발이 증가하고 있다. 개발한 의료기기의 임상적 유효성 확보를 위한 다양한 임상시험들이 수행되고 있으며, 이 과정에서 다양하게 수집되는 정보들의 체계적인 관리의 중요성이 증가하고 있다. 본 논문에서는 한의학의 대표적인 진단 방법인 맥진을 기기화 한 맥진기를 이용하여 임상시험을 진행하는데 있어 환자의 의학적 정보, 맥진기의 측정 정보들을 체계적으로 관리할 수 있는 데이터베이스를 설계하고 구축하였다. 구축된 데이터베이스는 임상시험에 대한 다양한 정보 및 측정된 정보의 질관리를 통해 기존 알고리즘 검증 또는 새로운 알고리즘 개발 시 효율적인 대응이 가능하고, 임상 데이터의 통제 측면에서도 여러 장점들을 가지고 있다. 본 연구의 결과는 이종의 맥진기나 다른 생체신호 측정기와의 정보 공유가 가능한 통합 데이터베이스 관리 시스템 구축을 위한 한의 의료기기 데이터 표준안 구성에 기여할 수 있을 것이다.

  • 본 연구는 오늘날 한국에서, 현실적으로 많은 치과의사, 한의사, 물리치료사들이 관심을 갖고 치료를 시도하고는 있으나 그 학문적 근거가 미약하여 논란의 소지가 있는, 교합과 측두하악장애 및 관련된 전신 증상과의 관계를 규명하기 위한 사전 작업으로 진행된 문헌 검토에 그 바탕을 두고 있다. 가장 대표적인 다섯가지 이론인 (1) Craniosacral mechanism (2) Osteopathy (3) Myodontics (4) Chirodontics (5) Dental Distress Syndrome and Quadrant Theorem 들에 대한 조사를 토대로 각 이론에 대한 대략적인 개요를 소개하며, 향후 계속적으로 발표될 논문에서 각각의 이론에 대한 구체적인 배경과 근거를 알아보고자 한다.

장석순의 간에 대한 천발과 임상응용 (The Jang suk-sun's explanation and clinical application about liver)

  • The theory of liver by Jang suk-sun[張錫純] is that first, although liver exists at right side of body and spleen at left anatomically, function of liver presents at left side of body and function of spleen at right based on principle of 'interdependence between eum and yang' and 'join strength with elasticity', and in the relation between liver and spleen, if gi of spleen ascends, gi of liver also ascends, and if gi of stomach descents, gi of gall bladder also descents. So. care of spleen and stomach becomes main point in treating disease of liver. The meaning of 'the liver being in charge of the evaporation'[肝主氣化] is that first, it raises the primordial gi and forms the 'great g' for it's circulation of whole body. Second., it excretes the functional activity of gi and connects heart with kidney and guides the evaporation of the whole body by connecting innate nature with acquired nature. Third, 'the liver being in charge of the evaporation' is realized by the help of spleen and stomach. And he said that this functional activity of gi is one of distinctive features that distinguish Oriental medicine from Western medicine. He discoursed upon physiology of 'the liver being in charge of the evaporation' and 'the Body belonged to yin and the Use belonged to yang' after seeing an evil of abusing drugs that calm the liver and check hyperfunction of liver by contemporary doctors. And he established a treat of 'warming and recuperating the gi of liver' [溫補肝氣法] and used it for symptoms of 'deficiency of liver-gi'[肝氣虛], 'exhaution of liver-gi'[肝氣脫], and 'the liver-cold'[肝寒證].

혈전증(血栓症) 및 고점도혈증(高粘度血症)에 대(對)한 당귀음(當歸飮)과 이진탕(二陳湯)의 실험적(實驗的) 연구(硏究) (Studies on the Effects of Danggieum and Ijintang on Thrombosis and Elevated Blood Viscosity)

  • I selected the theory of blood stasis and wetness-phlegm among the major 4 causes of Joong Poong(C.V.A.)(wind. fire. deficiency of vital energy and wetness-phlegm) and recent etiology of blood stasis. An experimental studies were done to investigate the effects of Danggieum (removing blood stasis and promoting blood circulation) and Ijintang(dissipating wetness-phlegm) on thrombosis and elevated blood viscosity. The results were as follows; 1. The number of platelets were significantly increased in only Danggieum group. 2. Related to the amount of fibrinogen. only Danggieum group revealed some increase. but both groups revealed no significance. 3. Related to the prothrombin time. Danggieum and Ijintang groups had significant decrease. but Danggieum group revealed more significance. 4. Related to the degree of concentration of FDP, only Danggieum group had significant decrease. 5. Whole blood viscosity and plasma viscosity in lower shear rates. both groups of Danggieum and Ijintang had significant decrease. 6. Related to the amount of RBC. both groups of Danggieum and Ijintang had significant decrease. 7. Related to the change of hematocrit. only Danggieum group, and the change of hemoglobin. only Ijintang group had significant decrease. 8. Related to the change of body temperature. only Ijintang group had significant raise. 9. Related to the changes of WBC. glucose, albumin, total protein and body weight. Danggieum and Ijintang groups had each increase or decrease. but no significance. According to the above results, Danggieum had more significant effect than Ijintang on the thrombosis. and Ijintang had more significant effect than Danggieum on the elevated blood viscosity. And it is considered that it might be more effective in treating Joong-Poong(C.V.A) to take medicines regarding patient's constitution and symptoms as well as the causes of Joong-Poong(C.V.A)

  • 명대 장경악은 기상 현상에 관하여 "오운육기를 토대로 기화를 추측하면 피차의 영허에 대해 십중칠팔은 들어맞는다"고 하였는데, 이를 착안하여 논자는 "황제내경"에 기재된 기상에 관하여 운기의 기후 특징을 한반도 서울을 중심으로 실제 기상관측 자료와 비교 분석하였다. 기상청 자료를 토대로 서울의 평균 풍속 평균 기온 강수량 평균 습도를 운기 이론 가운데 대운과 비교하였다. 또한 과거 1564년에서 1863년까지 한반도의 이상 기후 발생 빈도와 운기성쇠 운기동화 운기순역과 비교하였을 때 운기동화의 경우와 이상 기후 발생 빈도와 일치하였다. 다만 본 논문의 실제관측 자료와 통계는 한국의 서울을 중심한 분석이기에, 중국과의 어느 정도 오차를 고려하여야 할 것이다.

  • 1. In the recohnition of cosmos true form, It is compared to the Boundless(無極) the Great Absolute(太極),Yin and Yang(兩儀) throungh the fertilazation process of spermatozoon and ovum. 2. It is explained that principle of unchange through the Form and Action(體 用) relation of the outer appearances and Number (象數) with matching the number of nine and ten to HaDoo(河圖) and RakSye(洛書). 3. Eigth divinations(八卦) being compared to the human body, Care presevation of pregnancy(養胎) is explained that head forms firstly(Gun-I 1乾一), secondly lung(Tae-E 兌二), heart(E-Sam 離三), liver(Jin-Sa 震四), gall bladder(Son-Oo 巽五), kidney(Gam-Yuk 坎六), intestines and stomach(Gan-Chill 艮七), lastly flesh forms(Gon-Pal 坤八). 4. It is explained that process of physiological change of $\ll$Nei Ching The Natural Truth in Ancient Times$\gg$(內經 上古天眞論) by matching boy at the age of 8 to Gan-divination(艮卦), and girl at the age of 7 to Tae-divination(兌卦). 5. The theory of six sons from Gun-Gon(乾坤六子論) is explained by relation of Apriority Eight-divination(先天八卦) obedience and disobedience-left and right. 6. It is explained that form of the human-body and the relationship of the Heart - the Kidney through the Gam(坎) Li(離) - divination 7. The effort of interpretating time and space of the Twelve Horary signs is explanined by season, direction, Five elements(五行), rise and decline, the Three Sum(三合), the Six Sum (六合), the six crash(六衡)'s relation. 8. the process of change from apriority(先天) to postery(後天) in the book of Changes(周易) is explanined by comparing to the phenomenum of nature and the human body. 9. According to the Hyo-position(爻位) match the ages, body form and drug by matching Ehight-divination(八卦) to the human body form and function in medicine and the book of Changes(周易) application emphasize the human body Ehight-divination(人身八卦). 16. Throgh the Order-divination(序卦) explain the rightness of Divination Image(卦象) arrangement and all things take shape by cosmo-energy conseusus(宇宙氣交). 17. Throgh the Mixing-divination(難卦) supply the vacancy of medicine and the book of Changes(周易) relationship in the foreword explian the human energy movements, sleep, vomitting, the energy arrival(逮氣), heart pericardium(心包), lung membrane(肺膜) etc.... Like the above sentence medicine and the book of Changes(周易) theory of scholar Tang on the viewpoint of easten-the way Western appliance(東道西器) researching abyss of medicine impart to descendants, so I think that the achievement of medicine and the book of Changes(周易) study is very excellant and I expect that the study Korean Oriental Medicine(韓醫學) theory by means of medicine the book of Changes(周易) reference, will be accelarated.

《상한론(傷寒論)》 태음병(太陰病) 제강(提綱)을 침구학적(鍼灸學的) 분증(分證) 및 분경(分經)의 운용방법(運用方法)에 관(關)한 연구(硏究) (The study on Acupuncture Operation Method of 《Sang Han Lun(傷寒論)》 Liu-Jing-Bian-Zheng(六經辯證) - Based on the Study of Tai-Yin-Bing(太陰病) -)

  • The following study was undertaken in order to seek the acupuncture operation method of $\ll$Sang Han Lun(傷寒論)$\gg$ Liu-Jing-Bian-Zheng(六經辯證). Based on the documents quoted in $\ll$Sang Hang Za Bing Lun Xu Wen(傷寒雜病論 序文)$\gg$ of "Zhang, Zhong-Jing(張仲景)", the relativity of the theory of Jing-Mai(經脈) and Liu-Jing-Bian-Zheng of convalescence, and from the Liu-Jing-Bing(六經病), the origin and implication that caused Tai- Yin-Bing(太陰病) to form was studied on the basis of acupuncture medicine publications and the commentary writing of $\ll$Sang Han Lun$\gg$. As a result of the foregoing study, the author has written the following acupuncture operation on the basis of a summarized Tai-Yin-Bing for $\ll$Sang Han Lun$\gg$ Liu-Jing-Bian-Zheng. Results : $\ll$Sang Han Lun$\gg$ Liu-Jing-Bian-Zheng has succeeded and was developed based on Liu-Jing-Fen-Zheng(六經分證) of $\ll$Su Wen Re Lun(素問 熱論)$\gg$. In addition, the summary of Liu-Jing-Bing became the general principle of Fen-Jing(分經) and Ding-Zheng(定證) that may be applicable to Fenghan(風寒), Wenre(溫熱), Lili(疫려) and Zabing(雜病). 2. Most commentators of $\ll$Sang Han Lun$\gg$ in the Song, Ming and Ching Dynasties of $\ll$Sang Han Lun$\gg$ interpreted the Tai-Yang-Bing in physiological and pathological aspects of Rong-Wei(榮衛) as the disease of the spleen meridian. 3. From the Liu-Jing-Bing of $\ll$Sang Han Lun$\gg$, the region of acupuncture treatment of Tai-Yang-Bing is treated with the needle from the point of view of Bing-Zheng-Lun-Zhi(辨證論治) with the basis of the important region of acupuncture of the spleen meridian.

계측기(計測器)를 이용(利用)한 O-Ring Test법(法)의 검증(檢證)에 관(關)한 연구(硏究) (The Study of Verification Bi-Digital O-Ring Test by gaugus)

  • Using Bi-Digital O-Ring Test which was developed by Ohmura Toshiaki, constitution classification by the vegetables' was measured by various muscle power measurement meters and the results are as follow : 1. Pinch guage (Model : pc5030HPG, Japan) is the guage to measure finger power between the thumb and second finger, Grip Strength Dynamometer (Model : T.K.K. 5101, Japan) is to measure the hand power (hand dynamometer), Back Strength dynamometer (Model : T.K.K. 5102, Japan)is to measue back muscle strength, Vertical Jump Meter (Model : T.K.K. 5106, Japan) is to measure the height of jump. The above guages were used and its result found that the radish, potato, carrot and cucumber can influence to the muscle strength was not true. 2. When the physical constitution is distinguished vy the O-Ring Test method, Taeyangin's rate appeared as average 21% although it was insistedthat there will be only 0.03-0.1%. This means that the influence power of vegetable doest not appear according to the physical constitution but it appears accidently according to the examinee's emotion about the material such as vegetable etc. as favor or unfavor. 3. It was found that the result of O-Ring Test is the same at any time and at any place was not true. there is no reemergence character. 4. The import of O-Ring Test method to the physical discriminatio disregarded that the mental facor influences absolutely to the physical heath in the ideological physical constitution medicla science. 5. 'O-Ring Test method is a objective judgement method'. is wrong judgement. As you see on the above result, Bi-Digital O-Ring Test set the changeable voluntary muscle as the standard of the judgement, that was first mistake logically, second in spite of leass influence of mental influence by the examiner and examinee than the vegetable discrimination influence, the test disregarded the influence. Thire, only grasp of some material on hand can influence to the voluntary was a wrong theory disregarding the physiology. Finally the misunderstaning his subjective view as an objective view in spite of examiner and examinee's strong influence. Therefore such kind of physical descrimination method must be sublated.

사상체질(四象體質) 진단법(診斷法)의 문헌적(文獻的) 고찰(考察) - 외형(外形), 심성(心性), 증(證)을 위주(爲主)로 - (The Bibliographical Investigation of Sasang Constitution Diagnosis)

  • 1. 연구목적 동서양과 고금을 통해 많은 체질 이론들이 있었지만 그 중에서도 이제마의 사상의학은 이론과 임상이 체계적으로 연계되어 있어서 널리 사용되고 있다. 이제마는 동의수세보원의 사단론(四端論), 변증론(辨證論) 등에서 체질진단의 근거로 외형(外形), 심성(心性), 증(證)의 세 가지를 제시하였으나 세 가지 기준별 임상적 중요도에 대한 연구가 이루어진 바가 없어서 임상에서 세 가지 진단 기준을 종합적으로 활용하는데 많은 어려움이 있는 형편이다. 그러므로 향후 세 가지 체질진단 기준을 임상에 적용하여 각각의 기준이 지니는 임상적 중요도를 검증하는 연구가 필요하다고 보며, 본 논문에서는 이러한 임상적 연구에 앞서 기존의 체질진단 연구를 외형(外形)과 심성(心性), 증(證)의 세 가지로 분류, 고찰하는 기초적 연구를 하고자 한다. 2. 연구방법 <동의수세보원(東醫壽世保元)>과 사상의학회지 및 사상의학 관련 도서를 중심으로 문헌적 고찰을 하였다. 본론에서는 그간의 연구들을 세 가지로 분류하여 열거하였으며. 고찰에서는 임상적으로 활용이 가능하거나 이미 완료된 연구 위주로 논하였다. 3. 연구결과 (1) 외형(外形)에 관한 연구 경험과 직관적 관찰을 통한 기상(氣像)과 사기(詞氣)위주의 연구가 선행되었고 이를 바탕으로 체형(體形)과 용모(容貌)의 실제 계측을 통한 각 체질별 이미지 정형화 연구가 진행되고 있다. 체질진단의 정확률 제고를 위해서는 이 두 가지 방법을 균형있게 활용해야 한다. (2) 심성(心性)에 대한 연구 각종 연구나 진단의 객관화 등을 위해 자기보고식 설문조사 기법이 체질진단에 응용되고 있으며, 현재까지 개발된 설문지를 이용함에는 사상변증내용설문조사지(I)의 판별식을 이용한 결과와 QSCCll의 결과를 함께 참고하는 것이 적절하다. (3) 증(證)에 대한 연구 체질증과 체질병증에 대한 연구는 원리론적 연구와 체질별 임상증상 유형에 관한 연구, 기존의 증치의학적 병증관을 사상의학적 병증개념에 적용시켜 해석하고자 하는 연구가 진행되어 왔다. 향후 현대적인 의미의 다양한 병증을 사상의학적 관점에서 해석하고 진단하기 위한 객관화 연구가 필요하며, 또한 체질의 편차를 야기시키는 근본적 문제인 장국(臟局)의 대소(大小) 및 장리(臟理)에 대한 연구도 필요하다.

피(皮)의 분(分)과 부(部)에 대한 연구 - "황제내경(黃帝內經)"을 중심으로 - (Study on Portions and Layers of the Skin - based on "Naejing(內經)" -)

  • By studying the portions and layers(分部), left and right, superior and inferior, location of yin and yang, and beginning and end of diseases of skin, which is the core point of the theory of cutaneous region(皮部論) in "Naejing(內經)", in the physiological and pathological perspective, based on opinion of historic memorial doctors, arrived to the conclusion as below. Cutaneous region means not only the distribution of three yin and three yang(三陰三陽) of the surface, but also inside and outside, shallowness and depth, and it is the system which unites meridians, networks, and vessels. It is divided into portions and layers. The origin and beginning of diseases and the rule of favorable pattern and unfavorable pattern can be known through it. The portion of skin is not only the area that meridian vessels belongs to skin, but also the area that activation of twelve meridian vessels are expressed in the surface. The layer of skin is consisted in order of skin-tertiary collateral vessel-collateral vessel-meridian vessel-bone. In "Naejing", there are two preconditions to divide three yin and three yang into yin and yang. The first is standing while looking the south, and second is the quotation "outside is side of yang(外者爲陽 內者爲陰)." According to this preconditions, yang of outside of yang brightness, lesser yang, and greater yang is the whole body, except inside of hand and foot which yin of lesser yin, pericardium, and greater yin. Superior and inferior of the portions and the layers is designated as hand and foot, theological basis of which superior and inferior work in same diagnostic method can be found in the root and the basis(標本) and the origin and the insertion(根結). In conclusion, cutaneous region not only manages layer of the skin, but also it is divided into layers and portions, so it has close relations between meridian vessels and collateral vessels. The in-depth study of cutaneous region and meridians should be progress, in order to practice of diagnosis and acupuncture and moxibustion more.
