Study on Deficiency-Excess Pattern Questionnaire Development Possibility (허실 변증 설문지 개발 가능성에 대한 고찰)

  • Deficiency and excess pattern differentiation is unique characteristic of traditional oriental medicine on diagnosis and treatment. This differentiation is accomplished through pulse, tongue diagnosis and question examination, but most of these processes need to be objectified for efficacious treatment and traditional medicine development. In this study, we developed the deficiency and excess pattern questionnaire for objectification of question examination. The deficiency and excess pattern questionnaire was made out through The Traditional Oriental Medical Literature with Delphi Technique. Patients who visited oriental medical hospital filled out the questionnaire by themselves. Diagnosis of deficiency and excess pattern are conducted separately by oriental medical doctors with more than 5 years' clinical experience. Various physical condition factors were derived for the deficiency and excess pattern questionnaire. (Ordinary health degree, pain pattern, fatigue, weight change, sweating, uncomfortable awareness on chest and abdomen) Deficiency symptoms group acquired internal consistency, but excess symptoms group did not. (Cronbach's ${\alpha}$ > 0.6) There were significant associations between doctor's diagnosis and deficiency and excess symptoms in 'ordinary heath degree', 'voice weakening', and 'chest distress' (p-value < 0.1) There were significant differences between deficiency and excess syndrome patients groups in deficiency questionnaire score but there were no significant differences between deficiency and excess syndrome patients groups in excess questionnaire score. We acquired the internal consistency and significant result of deficiency pattern questionnaire, but we can find out some difficulties in development of the excess pattern questionnaire. These difficulties are associated with insufficiency description of traditional literature and small number of patients diagnosed as excess pattern.

  • Objectives : This study researched into clinical statistics for patients recieved oriental medicine treatments In the Um-sung Gun Dae-so Myun health care center during eight months from May 1 2002 to December 31 2002. The number of object was 246 cases of 234 persons. Results & Conclusions : Analyzing these 234 persons, the rate of males to females was 1 to 2.1. As for the distribution of age, the age of 61 to 70 occupied 44.02 % and the age of 61 to 90 occupied 76.5 % ih the Whole patients. Therefore female was very highly more than male and patients were for the most part, old ages (61 years old and over) Musculoskeletal disease of 202 cases occupied 82.13 %, were the highest rate. At the duration of disease 1 year to 5 years occupied 26.92 %, was the highest. The Grade II was the most of the sign of patients at first visit, it occupied 47.86 %, The Fair(+) and over effect of treatments occupied 96.84 %, the failure(-) Of effect treatments occupied 3.16 %. As for the duration of treatment, the case of 2 times to 5 times visit occupied 38.03 %, was the highest. By medication of treatment, O-juk-san(五積山) occupied 35.10 %, was the highest rate. The average 8.67 persons treated with oriental medicine a day, the average 1.36 person received the first medical examination a day.

  • This study is to analyze of senile disease status and the social problem according to increased old ages, and then to find distributions of old man's diseases and health status efficiency of oriental-occidental contemporary health examination. And it is the first oriental-occidental contemporary health examination of old man performed by JeonJu Woosuk University Oriental Medicine Hospital and Woosuk-Clinic in nation. Methods The objects in this research are 641's old man of KimJe Gun's over 60's years performed medical examination at JeonJu Woosuk University Oriental- Mmedicine-Hospital and Woosuk-Clinic by oriental-occidental medical contemporary exam., from 1994, 24th June till 1994. 13th July. The 1st occident medical examination methods were consisted of chest x-ray check. blood and urine exam., measurement of blood pressure, visual power and audiometry. The Oriental medical examination methods were consisted of four diagnostics (望,聞,問,切), present illness. chief complaint, past history, families history, social history by question and SA Sang constitution test index. The results and conclusions The results and conclusions are the next: 1. In order of distribution. the athletic disease (75.8%),the digestive disease(43.4%), the circulatory disease(41.5%), the respiratory disease(22.3%), EENT disease(8.1%), the endocrinopathy(5.6%), and the genito-urinary disease(5.3%) are the results of the object about 641's old man, by the oriental-occidental medicine's contemporay exam. 2. Distribution of disease distiction are lumbago. gastritis and peptic ulcer. knee joint pain. heart disease. hypertension. chronic bronchitis. asthma. anemia. DM. Tbc. visual disturbance. CVA. etc in order. 3. Disease distribution according to age is almost high incident in 60-75years. Disease incidence is decreased except E.E.N.T disease in over 76years. 4. The relationships of disease and family history are: the 25.0% of CVA pts. has family history and the 11.6% of hypertension pts has family history. so they showed high relative family history. In addition the 5.6% of TBC pts. and the 2.6% of DM pts. have family history. 5. The relationships of disease and drinking are: Drinking proportion is the 36.4% in respiratory disease pts. the 34.7% in hypertension pts. the 33.3% in heart disease pts.. the 28.4% in digestive disease pts.. but because of no surveying drinking amount we can't know the absolut relationships of disease and drinking. 6. The relationships of Disease and smoking are: Smoking proportion is the 44.1% in respiratory disease pts.. the 38.0% in Heart disease pts.. the 29.8% in Hypertension pts.. but because of no surveying of smoking amount. we can't know the absolut relationships of disease and smoking. 7. Distribution of Sasang constitution is : Tae-eum-in 44.8%. So-yang-in 30.7%. So-eum-in 24.6%. Tae-yang-in 0.0%. And disease distribution of Sasang constitution distinction is ; Tae-eum-in has high incidence of circulation disease(50.0%) and respiratory disease(23.1%).So-yang-in has high incidence of athletics disease(77.7%) and EENT disease(12.2%), So-eum-in has high incidence of digestive disease(65.8%). 8. Distribution of abnormal result in occidental medical examination and oriental-occidental contemporal exam result is considerably different. This is the reason of needing oriental medicine exam, for characteristics of Senile. I think that the oriental-occidental contemporary examination in old man is much more effecient than only occident medical examination. This oriental-occidental contemporary examination has many defects because it is the first practice. To participate in the public health program efficiently. I think that we must improve lots of problems and present the model of the oriental-occidental contemporary examination and the project of oriental medicine's for public health.

An analysis of the impact of Korean medicine health counseling on health perception change and satisfaction (한의건강상담이 건강인식 변화에 미치는 영향 및 만족도 분석)

  • Objectives : The purpose of this study was to examine the changes of health perception, satisfaction survey and lifestyle in Korean medicine counseling for Daejeon University faculty members. Methods : In this study, the demographic questionnaire, lifestyle habit questionnaire, Mibyeong questionnaire, quality of life questionnaire(SF-12, EQ5D), pattern-based questionnaire, constitutional questionnaire(KS-15) and instrumental examination were performed on the first and second visit. We carried out 1:1 consultation at every visit. Results : A total of 189 participants participated in this study. 66% of respondents answered that they felt the change in health perception and more than 93% of the respondents felt satisfaction due to the Korean medicine health counseling. In the demographic characteristics, it was found that women were more satisfied with the Korean medicine health counseling than men(p <.05), the higher the education level(p <.05) and the higher the age(p <.05), managed their health continuously after receiving Korean medicine health counseling.(p <.05) In addition, in the change of lifestyle habit, the group who smoked and drank in the past answered that the consciousness to manage and improve health was higher than other group after Korean medicine health counseling.(p <.05) Conclusions : Korean medicine health counseling had a positive effect on overall health perception change and satisfaction was high. Since the Korean medicine health counseling has different effects on lifestyle and health perception change and the satisfaction of Korean medicine health counseling is different according to demographic characteristics and existing lifestyle habits. In the future, it will be expected that the effect of Korean medicine health counseling program on health care and disease prevention will be further enhanced if the Korean medicine health counseling program reflecting such characteristics is implemented.

Comparison Study between Quality of Life and Body Mass Index, Age, Gender in Sasang Constitution among Healthy Subjects in One Korean Medicine Hospital (사상체질별 체질량지수, 나이, 성별과 삶의 질에 대한 상관성 연구 - 1개 한방병원의 건강검진 내원자를 중심으로)

  • Objectives : This work aimed to reveal the relations between Health-related Quality of Life and Body mass index, age, gender in Sasang Constitution. Methods : 1108 subjects who visited Korean medicine hospital in Jeonju for regular health examination surveyed SF-12. Their Sasang constitution were diagnosed by 1 Sasang constitution medicine specialist. ANOVA (Analysis of variance) was carried out to compare SF-12 sub scales differences among each constitutions. Regression analysis was performed to reveal effects in changes of BMI, age, and gender to SF-12 sub scales. Results : TE type gets more BMI, has more physical limits, even those of SE and SY type are not limited by these reasons. Contrary, SE type feels more vitality when they get higher BMI. SE type women have low mental health status than that of SE type men, though they don't have any specific disease. Conclusions : This is the first work of analysis between quality of life and Sasang Constitution's BMI, age, and gender. To reveal factors which affect quality of life in each Sasang constitutions, more rigorous works are required.

A Study on Korean Oriental Medical Doctors' Use of Uninsured Herbal Extracts and How to Promote the Insurance Coverage of Such Herbal Extracts (한의사의 복합과립제 사용 실태 조사 및 복합과립제 건강보험 급여 시행방안에 대한 연구)

  • Objectives: To research Korean oriental medical doctors' use of uninsured herbal extracts and how to bring about the insurance coverage of such herbal extracts. Methods: We surveyed Korean oriental doctors about the following issues from October 17th to November 15th, 2008: (1) Korean oriental medical doctors' knowledge about herbal extract insurance and the circumstances of oriental medicine in Korea, (2) their opinion on the coverage of currently uninsured herbal extracts and the dispensing of herbal extracts from pharmacies rather than from their clinics, (3) their use of herbal extracts, and (4) how to bring about insurance coverage of uninsured herbal extracts. Results: Over 70% of the respondents said that herbal medicine prescriptions have been reduced recently and that the existence of herbal medicine is in danger. In addition, 63.64% respondents agreed with expanding insurance coverage to include currently uninsured herbal extracts in spite of the fact that patients might have to obtain herbal extracts from pharmacies rather than from Korean oriental medical clinics. The average patient number per month of uninsured herbal extracts was 13.64 people, the average dosage was 5.64g, the average cost per day was 3,859 won, and the average prescription period was 2.65 days. Korean oriental medical doctors asked an average of 12,486 won for the medical examination-prescription fee and 3,292 won in fees for prescriptions obtained outside the hospital. If insurance coverage expands to include these herbal extracts, their usage is expected to increase 2.31 times. Conclusions: This study shows Korean oriental medical doctors' use of herbal extracts and their opinions about execution of herbal extracts' insurance. A periodic study such as this one will hopefully aid in establishing polices for uninsured herbal extracts' insurance.

Evaluation of Relationship between Depression and Anxiety, Self Esteem, BMI, Blood Types, Blood Pressure of University Students (대학생의 우울경향과 불안감, 자아존중감, 비만도, 혈액형, 혈압과의 연관성 평가)

  • Objective : This study aimed to evaluate the relation ship between depression and anxiety, self esteem, BMI, blood types, blood pressure, scoliosis of spine. Method : We analyzed the results Health examination of 5619 university students (included BDI, BAI, SES, BMI, Blood Types, Blood Pressure, X-ray of whole spine). All the data were analyzed statistically by descriptive statistics, t-test, ANOVA and regression analysis. Result : Female's BMI score and BAI score are higher than Male's. And Senior Group showed higher score than Freshman Group. There were positive correlation between BAI, and BDI. but negative correlation between SES, Blood Pressure and BDI. BMI, Blood types and Scoliosis of spine were not related to depression. Conclusion : There were correlation between BAI, SES, Blood Pressure and BDI. It is necessary to study about depression with many other effective factors of various psychological and physical subjects.

Correlation between Rouleau Condition of Red Blood Cell on Live Blood Analysis and Degree of Fatigue (생혈액검사상 적혈구의 연전현상과 피로도와의 상관관계)

  • Objectives : It is known that rouleau condition of red blood cell on live blood analysis is related to degree of fatigue. This study was planed to prove the correlation between rouleau condition of red blood cell and degree of fatigue by using the questionnaire 'Symptom Table on Fatigue Perception' that had been verified before. Methods : We analyzed the correlation between rouleau condition and degree of fatigue by calculating ratio of rouleau of red blood cell after measuring degree of fatigue by the questionnaire to the people who had not evidence of illness on health examination and did not take any medicine. Results and Conclusions : This study showed a significance of positive correlation(0.464) between physical fatigue and rouleau condition of red blood cell.

A Study on Comparison of Yangseng (Traditional Health Behavior), Depression, Anxiety and Cognitive Function between the Elderly Women Living Alone and the Aged in the Living with Family (독거 여성 노인과 가족 동거 여성 노인의 양생 실천 수준과 불안, 우울, 인지기능에 대한 비교 연구)

  • Objectives: The purpose of this study was to investigate the levels of Yangseng (Traditional Health Behavior), depression, anxiety, cognitive function between the elderly women living alone and the aged in the living with family. Methods: The subjects of this study were 137 elderly women aged between 65 and 80. They were evaluated with the Tool in Measuring Yangseng (TMY), Korean Geriatric Depression Scale (KGDS), State-Trait Anxiety Inntory (STAI) and Korean of the Mini-Mental State Examination (MMES-K). Depending on whether or not they lived alone, they were divided into two groups; group living alone (L.A.) and group living with family (L.F.). Results: In TMY score, there were statistically significant differences in characteristics of Mind, Diet, Sleep and total score between L.A. (63 persons) and L.F. (74 persons). Group living alone (L.A.) had significantly higher score in KGDS and STAI compared with other group (L.F.). The values of KGDS and STAI comparing to the values of Mind, Diet, Sleep, Exercise and total score in this study resulted in negative correlations which showed statistically significant. Conclusions: Above results suggest that between L.A. and L.F, there are significant difference in physical and mental health. TMY is enough to estimate health of the aged. These results can be used for Yangseng health promoting program based on Korean traditional medicine.

The Evaluation of Sasang Constitution with Use of Health Examination Data in Workplace (산업장내(産業場內) 건강검진(建康檢診) 자료(資料)를 활용(活用)한 사상체질(四象體質)의 평가(評價))

  • The subjects were 420 workers who received oriental and western health examination both in the workplaces for 1 month. This study was planned for the reflection of western health examination results through evaluation of Sasang constitution with use of data from western health examination, and for the presentation of basic data applicable to constitutional medicine with preventive medicine. Data that were collected in field research were analyzed into frequencies, One-Way ANOVA, Chi-square testing by the study purposes, and the research results were as follows: 1. The distribution of Sasang constitution in 420 subjects was Taeumin > Soumin > Soyangin, and the general characteristics of subject by Sasang constitution were no significant difference. 2. For the measured values for body by the subject of Sasang constitution, Taeumin had higher value for height, body weight, systolic and diastolic blood pressure than other 2 constitutions. Soumin had the lowest values in average for all items, and on the other hand, Soyangin had high pulse pressure compared to other constitutions. It was significant difference between constitutions. 3. For the hematological average, Taeumin had high average for FBS, Total-C, GPT, and Taeumin ,and Soyangin had high in Hb compared to Soumin. Soyangin had the highest average for ${\gamma}-GPT$, and there were significant differences between FBS, Total-C, Hb, GPT, ${\gamma}-GTP$, excepting for GOT. 4. For the result of health examination by the Sasang constitution of subject, Taeumin showed the highest percentage in the items of obesity, primary care, secondary suspicion, and showed significant difference by the constitution. For the distribution of the past disease history by the health examination result of Sasang constitution, Taeumin and Soumin had high prevalence rate for hypertension, liver disease, and Soyangin had high prevalence rate for liver disease, hypertension. For the requirement of lifestyle improvement to the subject, the highest requirements by the Sasang constitution were physical exercise for Soumin, regulation of body weight for Taeumin, and it showed significant difference. For the requirement of primary care by the constitution, the highest requiremetns were obesity control for Taeumin, blood pressure control for Soumin, Soyangin, and liver suspicion was the highest distribution for each constitution in the case of having decision for secondary suspicion.
