Efficacy of Aloclair$^{TM}$ for Treatment of Intraoral Ulcer Caused by Orthodontic Appliance (교정장치에 의해 유발된 구내궤양 치료에 대한 Polyvinylpyrrolidone-sodium hyaluronate (Aloclair$^{TM}$)의 임상적 효과)

  • This experimental study was undertaken to evaluate the efficacy of polyvinylpyrrolidone-sodium hyaluronate(Aloclair$^{TM}$) in decreasing pain in orthodontic procedures. A total of 60 patients who were use Aloclair$^{TM}$ ordered to were included. These patients were categorized by 3 cases; 1. Patients who were bonded with their first brackets. 2. Patients who were in pain with additional appliance such as anchorage devices. 3. Patients who got ulcer caused by appliances. We collected information about pain and assessed it using visual analogue scale. According to questionnaire, it took 3.5 min. for pain to subside and pair relief durated for 2.7 hours. Most patients in the study reported rapid pain relief and found Aloclair$^{TM}$ easy and pleasant to use than wax. Accordingly Aloclair$^{TM}$ provided raid relief of resting and provoked ulcer pain.

  • The incidence of oral complications among adult cancer patients undergoing chemotherapy varies from 12 to 80%. Adequate oral hygiene has been shown to be important in prevention of oral complication and an essential role is reserved for the nursing staff. These considerations prompted the decision to survey by means of a questionaire, the nurses who give care to cancer patients. The Questions were included multidisciplinary treatment, inspection skill, nursing intervention, nursing education, problem in mouth care, solution for problem solving. - Results are fellow : 1. A total of 116 of the nurses returned the questionaire 2. According to 88.2% of the respondents, the policy with regard to oral-hygine is determined by the physician and the nurse. 62.1% of nurses do not consult the dentist When oral complication is occured. 3. In only 34.5% of case was a penight used to provide the necessary extra illumination nursing Inspection of oral cavity. 4. Frequency of oral complications observed by the respondents is that they observed complications in < 25% of patients. The nature of the complication varied from ulcer, stomatitis, infection, dry mouth, candidiasis, herpes simplix, bleeding. 5. Percentages of respondents who use the intervention indicated 1) to prevent oral complication : 0.9% normal saline gargling(44%), 0.02% chlorhexidine gargling, oral dressing(38.8%), observation, nutrition, restriction of alcohol and tabaco(23.2%) 2) to deal with the early symptoms 0.9% normal saline gargling (47.4%), cryotherapy(37.9%), 0.02% chlorhexidine gargling(20.7%) 3) to help alleviate severe complications : dental consult, holding the chemotherapy(34.5%), 0.9% normal saline gargling(31.1%), cryotherapy(18.0%) 6. According to 70% of the respondents, insufficient attention is given to oral complication during nursing education classes only 8.6% said that both the theory and the practical aspects had been deal with in sufficient detail during their training. The results of the survey indicate thatoral care in cancer patients undergoing chemothrapy has a number of problem. There are not enough dentist to provide the necessary care for patients undergoing chemotherapy. The expertise of the nurses with respect to the pathogenesis of the complication is limited. In the training of nurses, additional attention to oral examinations and oral hygine is warranted. The care of patients should be the responsibility of a multidisciplinary team approach. The nurse occupies a key position with in this team, which includes the medical oncologist, a dentist.

Curcumin Induces Recovery from Indomethacin-Induced Gastric Mucosal Lesions in Rats (커규민의 인도메타신 유도 위점막 손상에 대한 치료 효과)

  • In the present study, the curative effect of curcumin on indomethacin-induced gastric mucosal lesions in rats was investigated. Indomethacin is a nonsteroidal anti-inflammatory drug (NSAID), with serious side effects, including erosion, ulcerative lesions, and petechial bleeding in the mucosa of the stomach. Gastric mucosal lesions were caused by oral administration of 25 mg/kg of indomethacin. Various doses (10, 50, and 100 mg/kg) of curcumin were treated for 3 days by oral gavage. Indomethacin clearly increased the gastric ulcer area in the stomach, and curcumin significantly decreased the gastric ulcer area in a dose-dependent manner. Curcumin also markedly inhibited lipid peroxidation in the gastric mucosa and activated radical scavenging enzymes, such as superoxide dismutase (SOD), catalase, and glutathione peroxidase, in a dose-dependent manner. These results suggest that curcumin can induce recovery from indomethacin-induced gastric mucosal lesions through inhibition of lipid peroxidation and activation of radical scavenging enzymes, such as SOD, catalase, and glutathione peroxidase. Curcumin appears to be a powerful free radical quencher, and it may offer an attractive strategy for healing gastric mucosal lesions in humans.

Evaluation of Taste in The Patients with Oral Mucosal Disease by Electrogustomter (전기미각측정기를 이용한 구강점막질환 환자의 미각평가)

  • The purpose of this study was to investigate the taste sensitivity in patients with oral mucosal diseases(Oral lichen planus(OLP), Recurrent apthous ulcer(RAU)) using electrogustometer. One hundred and seventy three subjects were included for the study and they were categorized into 2 groups(control 100, patient 73) and patient group was investigated in the Department of Oral Medicine, Pusan National University Hospital from April, 2005 until January, 2007. Control group was investigated in the clinics at Inchen Metroplitan city and Cheongju city from February to August, 2006. The electrical taste thresholds were measured by using an electrogustometer for the 4 different sites(tongue tip, tongue lateral, circumvallate papilla and soft palate) in oral cavity. The results were as follows ; 1. The electrical taste threshold in patient group was significantly decreased than that in the control group(p<0.001). 2. The electrical taste threshold, in terms of the chronicity and lesion multiplicity, was not significantly changed in patient group. 3. The electrical taste threshold was not significantly changed in Dexan only and Dexan+Prs combination treated group. 4. The electrical taste threshold, in terms of treatment progress(no response vs half response vs complete response), was not significantly changed. However subjective index which was determined by NAS(Numerical Analogy Scale) was significantly increased in no response group but significantly decreased in complete response group(p<0.001)

Evaluation of Factors that Can Affect Protective Immune Responses Following Oral Immunization of Recombinant Helicobacter pylori Urease Apoenzyme

  • Helocobacter phylori is the major cause of gastritis, peptic ulcer, and a principal risk factor for gastric cancer. As the firs step towards a vaccine against H. pylori infection, Hy.pylori urease was expressed and purified as a recombinant apoenzyme (rUrease) in E. coli. In order to develop an effective immunization protocol using rUrease, the host immune responses were evaluated after the oral immunization of mice with rUrease preparations plus cholera toxin relative to various conditions, such as the physical nature of the antigen, the frequency of the booster immunization, the dose of the antigen, and the route of administration. The protective efficacy was assessed using a quantitative culture following an H. pylori SS1 challenge. It was demonstrated that rUrease, due to its particulated nature, was more superior than the UreB subunit as a vaccine antigen. The oral immunization of rUrease elicited significant systemic and secretory antibody responses, and activated predominantly Th2-type cellular responses. The bacterial colonization was significantly reduced (~100-fold) in those mice immunized with three or four weekly oran doses of rUrease plus cholera toxin (p<0.05), when compared to the non-immunized/challenged controls. The protection correlated well with the elicited secretory IgA level against rUrease, and these secretory antibody responses were highly dependent on the frequency of the booster immunization, yet unaffected by the dose of the antigen (25-200$\mu\textrm{g}$). These results demonstrate the remarkable potential of rUrease as a vaccine antigen, thereby strengthening the possibility of developing an H. pylori vaccine for humans.

Pharmacokinetics of a New Antigastritic Agent, Eupatilin, an Active Component of StillenE®, in Rats

  • Pharmacokinetics of eupatilin (an active components of $Stillen^{\circledR}$, a new antigastritic agent) were investigated after both intravenous and oral administration at a dose of 30mg/kg to rats. After intravenous administration, the plasma concentrations of unchanged eupatilin declined rapidly with a mean terminal half-life of 0.101 h. Eupatilin was eliminated fast in rats; the total body clearance was 121 mL/min/kg. Eupatilin was mainly metabolized in rats; the percentage of intravenous dose of eupatilin excreted in 24 h urine and feces as unchanged eupatilin was only 2.5 and 0.919%, respectively. Eupatilin was mainly metabolized to form its glucuronide conjugate; after intravenous administration, 15.9 and 51.7% of intravenous dose was excreted in 24 h urine and feces, respectively, as eupatilin plus its glucuronide. After oral administration, the absolute bioavailability was only 3.86% based on $AUC_{0-24h}$ of eupatilin plus its glucuronide. Approximately 68.5% of oral dose was not absorbed from the entire gastrointestinal tract. Therefore, it could be concluded that the superior effect of eupatilin in experimental animal models of gastric ulcer and inflammatory bowel disease after oral administration could be due to the local action of eupatilin. Further pharmacokinetic studies to elucidate the local action of eupatilin are required.

  • Nasal-type extranodal natural killer/T-cell lymphoma (ENKTL) is a rare disease presenting with non-specific symptoms, typically originating in the nasal cavity, palate, or midfacial region. Oral cavity is an extremely rare site for this type of lymphoma. In this report, we present a case of palatal perforation and oro-nasal fistula as a manifestation of recurrent ENKTL. Complicated disease entity should be considered when surgeons deal with palatal perforation and oro-nasal fistula.

  • "Chimumchang" seems to be a kind eczema. The cause and symptom of eczema is similar to that of chimumchang. so, I investigated the literatures of oriental medicin and recent medicine. The results are summarized as follows. 1. Chimumchang seems to be a kind of eczema. Allergic contact dermatitis, atopic dermatitis, seborrheic dermatits, neurodermatitis, nummular dermatitis, xerotic eczema and so on are similar to chimumchang on cause & symptom. 2. The cause of chimumchang are summer vital energy excess(夏脈太過), sehwa excess(歲火太過), herat meridian wind-heat(心經風熱), heart fire(心火), wind taken spleen dampnees(脾濕受風), and so on. The cause of eczema are contact of external agent, immunological problem, heridity, infection, abnormalities of sebaceous glands, neurologic dysfuction, and so on. 3. The symptoms of chimumchang have been described to the three pattern on literature of oriental medicine until now. 1) scratching slightly, It has exudation, and combination. 2) Vital energy on skin is irritable by psychological problem. At the acute stage the patient is complain of itching & pain, and gradually the exudation expands in whole body. 3) early peorid it's size is very small, and at first the patient is complain of itching, later complain of pain. The exudation change to ulcer and expands in the whole body. 4. In the investigation of prescription(Total: 34, Oral medicine: 4, Ointment: 30), the most frequently used oral medicine is sungmatang(4 times), and the most frequently used ointment are kohosan, korvunsan and kegwanhyuldobang.

Gastroprotective Effect of Korean Rice-Wine (Yakju) (한국 전통약주의 위보호 효과)

  • Gastroprotective effects of Korean rice-wine (Yakju) in two types of acute experimental gastric ulcer induced in rats and in mice were evaluated. Korean rice-wine were administered to 24-hr fasted rats 30 min before administration of 60% EtOH in 150 mM HCl or absolute ethanol. Korean rice-wine prevented formation of gastric ulcers induced by 60% EtOH in 150 mM HCl at oral doses of 250-1,000mg/kg and reduced gastric ulcers induced by absolute ethanol at oral doses of 62.5-1,000mg/kg, and inhibitory effect against 30% alcohol treatment for 7 days (twice/day). These results suggest Korean rice-wine have inhibitory effects on gastric lesion and ulceration.

A CLINICAL STUDY ON THE CARE OF ODONTOGENIC INFECTIONS IN THE ADMISSION PATIENTS WITH AGE-RELELATED GERIATRIC DISEASES (노인성 전신질환 입원환자에서 치성감염 관리에 관한 임상적 연구)

  • This is a reprospective study on the care of odontogenic infections in admission patients with geriatric diseases. The study was based on a series of 480 patients at Dong San Medical Center, Wonju Christian Hospital and Il San Health Insurance Hospital, From Jan. 1, 2000, to Dec. 31, 2002. The Obtained results were as follows: 1. The systemic malignant tumor was the most frequent cause of the geriatric diseases with odontogenic infectious diseases, and refractory lung disease, systemic heart disease, type II diabetes mellitus, cerebrovascular disease, bone & joint disease, senile psychologic disease were next in order of frequency. 2. Male prediction(57.5%) was existed in the odontogenic infectious patients with geriatric diseases. But, there were female prediction in senile psychologic disease, systemic heart disease and cerebrovascular disease. 3. The most common age group of the odontogenic infectious patient with geriatric disease was the sixty decade(47.9%), followed by the seventy & eighty decade in order. 4. In the contents of chief complaints on the odontogenic infectious patients with geriatric disease, peak incidence was occurred as toothache(52.7%), followed by extraction wish, tooth mobility, oral bleeding, oral ulcer, fracture of restoration, gingival swelling in order. 5. In the diagnosis group of odontogenic infectious diseases, periodontitis, pulpitis & periapical abscess were more common. 6. In the treatment group of odontogenic infectious diseases, the most frequent incidence(34.2%) was showed in primary endodontic treatment (pulp extirpation, occlusal reduction and canal opening drainage) and followed by scaling, incision & drainage, only drugs, pulp capping, restoration in order.
