The Effect of Depression and Cognitive Function on the Geriatric Oral Health Assessment Index in the Elderly

  • Background: Depression and cognitive function have a positive effect on the improvement of quality of life and extension of lifespan in the elderly. In addition, it appears as a major factor influencing oral health status. Therefore, this study looked at the relationship between the Geriatric Oral Health Assessment Index (GOHAI), depression, and cognitive function in the elderly using the Korea Longitudinal Study of Aging. Methods: In this study, 4,535 elderly people aged 65 years and over were targeted using the 7th data of the 2018 Korea Longitudinal Study of Aging. A t-test and ANOVA analysis were performed to compare GOHAI, depression, and cognitive function by group. In addition, hierarchical multiple linear regression was performed to understand the effect of the elderly's perceived depression scale and cognitive ability on GOHAI. Results: As a result of adding the depression scale and cognitive function variables to Model 2, the explanatory power was 22%. Educational level, marital status, private health insurance subscription, average monthly allowance, subjective health status, use of dentures, smoking status, economic activity, depression scale, and cognitive function were found to have significant influence (p<0.05). In addition, when controlled and viewed with all factors, depression and cognitive function were found to have an effect on oral health-related quality of life. Conclusion: The findings indicate that depression and cognitive function are associated with oral health-related quality of life in the Korean elderly. As the age increases, the quality of life declines due to depression and cognitive function problems, in addition to oral discomfort, eating disorders, and physical discomfort.

일부 치매 및 치매의심환자들의 인지기능에 따른 구강보건인식 조사 (Perceived oral health awareness in dementia and dementia-suspected depending on KMME)

  • Objectives: The purpose of this study is to investigate cognitive function, performance of activities of daily living, and recognition on oral health with the cognitive function testto dementia or dementia-suspected patients in the outpatients. Methods: The subjects were 94 dementia or dementia-suspected patients visiting C University hospital for the dementia test. Study instruments included Korea Mini-Mental State Examination KMMS, The Bayer-Activities of Daily Living Scale; B-ADL, Seoul-Instrumental Activities of Daily Living; S-IADL, Global Deterioration Scale; GDS, Korean Dementia Screening Questionnaire; KDSQ, and underlying diseases. Results: Dementia or dementia-suspected patients were 42 by KMMSE test, 25 patients had impaired functioning of daily living by B-ADL test, 27 patients showed the presence of depression by GDS test, and 45 patients showed impaired functioning of daily living. There was a statistically significant difference in the subjective recognition on oral health conditions. There was a statistically significant difference in the subjective recognition on oral health conditions by ADL. There was a positive correlation between the cognitive function and ADL performance. Higher cognitive function is proportional to ADL performance. Conclusions: The cognitive function was closely associated with ADL and subjective oral health conditions.

  • Isoflurane and enflurane are currently used on orthognathic surgery in Korea. Since starting to use enflurane and isoflurane in orthognathic surgery, we have questioned their effect on liver function. Many studies have reported liver function after enflurane and isoflurane anesthesia. Although both enflurane and isoflurane are less hepatotoxic than halothane, some cases of liver dysfunction have been reported after enflurane and isoflurane anesthesia. And, we know that isoflurane is less hepatotoxic than its predecessors, enflurane. But, fulminant liver failure and necrosis were also reported after isoflurane anesthesia. The purpose of this study was to compare immediate liver function in healthy orthognathic surgical patients receiving enflurane or isoflurane anesthesia. To assess the effect of enflurane and isoflurane on liver function, we measured pre-and post-operative serum concentrations of aspartate aminotransferase(AST), and alanine aminotransferase(ALT), alkaline phosphatase(ALP), total bilirubin(Tbil).

  • A technical refinement for oral commissure reconstruction is presented. The oral commissure has an important role in oral sphincteric function. Once lost, the oral commissure is very difficult to restore and therefore, reconstruction of the oral commissure requires prevention of this function as well as prevention of microstomia. Trauma and tumor excisions are the most frequent reasons that will necessitate reconstruction of the oral commissure. Direct approximation of the wound margins after resection of the corner of the mouth could easily induce microstomia and difficulty in opening. Thus case presented here deals with distortion of noraml mouth angle and acquired microstomia due to burn and tissue deface following shotgun and explosive wounds. The a mucosal flap approach was used to rehabilitate a natural looking mouth angle showing satisfactory results function and esthetic wise.

구강운동촉진기술: 1 부-이론적 배경과 기초 요소 (Oral-Motor Facilitation Technique (OMFT): Part I-Theoretical Base and Basic Concept)

  • 서론 : 구강 운동 기능은 태아기에서 유년기 발달로 이어지는 감각 탐색, 음식 섭취, 의사소통 표현의 기초가 되는 기능이다. 구강 운동 기능에 문제가 발행하면, 구강 내 음식 처리 어려움, 삼킴 및 섭식기능 저하, 의사소통 기능 감소, 구강 위생 능력 저하 등이 나타난다. 이러한 증상 치료를 위한 구강운동 치료는 구강 내 감각 정상화, 자세 조절, 구강 움직임 증진, 구강 운동 기능 향상을 목표로 제공되는 치료법이다. 본론 : 구강운동촉진기술(OMFT)은 1) 구강 및 안면 근육계의 해부생리학적 이해; 2) 감각·적응·행동·인지의 통합적 접근; 3) 도수적 접근을 통한 감각 및 운동 자극; 4) 운동 조절과 운동 학습 이론에 기초한 구강운동의 4가지 주요 특징을 바탕으로 전반적이고 통합적인 구강 기능 증진을 목적으로 개발되었다. OMFT는 근거 중심의 새로운 치료 프로토콜로써 아동 및 성인, 신경계 및 근골격계의 구강 기능에 문제를 보이는 모든 이들에게 적용이 가능한 치료 기법이다. 결론 : 본 논문은 OMFT의 개발에 따른 이론적 배경 및 OMFT의 실제적 적용에 따른 기초 요소를 소개하는 것이 목표로, 본 논문을 통해 임상 전문가들이 정확한 이론적 배경과 치료법을 바탕으로 대상자에 맞는 전문적인 치료 제공에 도움이 되기를 바란다.

  • When full denture is being fabricated, pronounciation, recovery of aesthetics of function and mastificatory function should be satisfies for patients. To satisfy for the function of denture, following is the difference between this new way of fabricating and formerly one of it. 1. The size of fabricating tooth which is harmony of original oral structure for patients is deaded by manufacturing labial index and artificial tongue. 2. By the use of artificial tongue and labial index, the arranges of artificial tooth is to become harmony of oral structure. 3. Formation of gingival decided functional impression which is used by impression paste, is harmony of oral structure. Therefore, this full denture can be satisfied with pronouncing oral function of recovery of aesthetics as well as mastificatory through physiological movement of oral tissue not disturbed with anything but cooperated drastically. In manufacturing of this full denture, both dentist and dental technician under cooperation need to proceed the work on the base of importance of communication.

치매환자의 구강관리가 삼킴기능과 구강관련 삶의 질에 미치는 영향 (Effect of Oral Health Care on Swallowing Function and Swallowing-Quality of Life in Patients with Dementia)

  • 본 연구는 장기 요양시설에 거주하는 치매환자들을 대상으로 구강관리 실태를 평가하고 이것이 삼킴 기능과 구강건강관련 삶의 질에 미치는 영향을 연구하였다. 광주, 전남에 위치한 장기요양기관에 입소한 치매환자 60명을 대상으로 구강관리, 삼킴곤란 임상척도와 구강건강관련 삶의 질을 측정하였다. 결과는 첫째, 장기요양시설을 이용하는 치매환자의 구강관리가 삼킴기능과 구강건강관련 삶의 질에 미치는 영향은 구강관리 점수에 따라 삼킴기능과 구강건강관련 삶의 질에 각각 유의한 차이를 나타내었다(p<.05)(p<.01). 둘째, 치매환자의 구강관리, 삼킴기능과 구강 건강관련 삶의 질의 관계를 알아본 결과, 치매환자의 구강관리는 삼킴기능(r=.405, p<.01), 구강건강관련 삶의 질(r=.462, p<.01) 그리고 삼킴기능은 구강건강관련 삶의 질(r=.351, p<.01)과 유의한 상관관계를 나타내었다. 따라서, 장기 요양시설 이용자의 구강관리는 삼킴 기능과 구강건강관련 삶의 질에 매우 밀접한 관련을 나타내었다.

단치궁 노인의 구강 기능 만족도에 관한 연구 (A STUDY ON THE SATISFIED DEGREE OF ORAL FUNCTION IN GERIATIRIC PATIENTS WITH THE SHORTENED DENTAL ARCH)

  • The purpose of the present study is to inspect the satisfied degree of each oral function in geriatric patients with the shortened dental arch and when their prosthetic treatment is on schedule, provide some references to such treatment. For the approach to such study, 521 subjects were reviewed by grouping them according to the number of their remaining teeth, and masticatory function, phonetic function, facial change, and TMJ disorders were inspected and clarified through some questionnaires. Also through the questionnaires, the correlations between the geriatiric patients with the shortened dental arch and dentition and between the geriatiric patients with the Free-end RDP at the shortened dental arch and their oral function were found out with their satisfied degree of oral function. Results or findings from such study are as follows : 1. With regard to their satisfied degree of oral function, there was a significant difference of satisfaction between or among the group having only the anterior teeth and the group having the part of premolars and the group having even the part of molars, however no significant difference of satisfaction appeard between the group having 1st molars and the group having 2nd molars. 2. With regard to their satisfied degree of phonetic function, no significant difference appeared between or among the group having only the anterior part of teeth and the group having even the part of premolars and the group having even the part of molars, and with regard to their satisfied degree of facial change, no significant difference of satisfaction appeared between the group having the part of premolars and the group having even the part of molars. 3. With regard to their satisfied degree of masticatory function, phonetic function, TMJ disorders, and facial change, no significant difference appeared between the group having both the anterior part of teeth and the part of premolars and the group attached with the Free-end RPD on the same conditions of the afore-said group.

Pulmonary functions of patients with isolated mandibular fractures: a preliminary report

  • Objectives: The aim of this study was to evaluate pulmonary function in patients with mandibular fractures and to determine the pattern of pulmonary functions in these patients. Materials and Methods: This was a cross-sectional study of pulmonary functions in Nigerian non-smoking patients with isolated mandibular fractures managed at our health institution from December 2015 to June 2017. Forced vital capacity (FVC), forced expiratory volume in one second (FEV1), peak expiratory flow rate (PEFR), and ratio of FEV1 to FVC (FEV1/FVC) were measured for all participants using a portable spirometer just before treatment. The pulmonary indices were compared with the predicted reference values for Nigerians to determine the respiratory pattern. Results: Forty participants consisting of six females (15.0%) and thirty-four males (85.0%) with a female to male ratio of 1:5.7 were included in this study. The mean patient age was 34.5±13.1 years (range, 17-63 years). The mean FVC, FEV1, FEV1/FVC, and PEFR were 3.8±1.2 L, 3.0±1.0 L, 74.3%±13.8%, and 5.2±2.2 L/s, respectively. Comparison of data with predicted values revealed that 17 subjects (42.5%) had normal pulmonary function pattern while 23 subjects (57.5%) had features suggestive of obstructive and restrictive pulmonary function patterns. Conclusion: Isolated mandibular fractures presented with abnormal pulmonary function pattern.

노인의 인지기능과 구강건강상태의 관련성: 고령화연구패널조사(Korean Longitudinal Study of Aging)를 중심으로 (Relevance between cognitive function and oral health status in the elderly: Focusing on Korean Longitudinal Study of Aging)

  • Objectives: This study aimed to confirm the association between cognitive function and oral health status in the elderly. Methods: In this study, 5,794 respondents, aged 55 or older, who had participated in the 7th aging research panel survey were used. Statistical analyses were performed using a complex samples cross-tabulation analysis, complex samples general linear model, and a complex sample multinomial logistic regression analysis. Results: After analyzing the cognitive function of the elderly according to their oral health conditions, it was found that suspicions of dementia (19.6%) and cognitive function decline (25.9%) were more common for those who wore dentures than for those who did not wear dentures (p<0.001). Compared to those who did not wear dentures, those who wore dentures were linked to a 1.665 times higher suspicion of dementia (p<0.001). In comparison, cognitive decline was 0.964 times lower when the number of natural teeth increased by 1, and it was 0.941 times lower when the oral health evaluation index for the elderly increased by 1 point (p<0.001). Conclusions: As a result of this study, it was confirmed that cognitive function and oral health status are related in the elderly. Therefore, systematic measures for oral health management and oral health promotion should be prepared in tandem with awareness of possible cognitive decline in the elderly.
