수술실 간호사의 피로 예측요인 (Factors Influencing on Fatigue in Operating Room Nurses)

  • Purpose: The purpose of this study was to measure the fatigue of operating room nurses and examine factors contributing to their fatigue. Methods: The subjects of this study were 115 nurses who worked in operating rooms of three hospitals. Data were collected from September 15, 2009 to September 30, 2009. Fatigue and job stress were measured by using the Symptom's Fatigue Scale and the job stress measurement tools in operating room nurses. Results: The mean score of fatigue level was $3.10{\pm}0.61$. We analyzed fatigue according to characteristics of subjects and found that there were significant differences in marital status, number of children and sleeping hours in general characteristics. Career, daily participation hours in the surgery relating to job related characteristics. Significant positive correlations were found between job stress and fatigue(r=.233, p= .012). Three significant variables influencing fatigue of operation rooms nurses were job stress, daily participation hours in the surgery, and sleeping hours (Adj. $R^2$=0.284, F=4.773, p<.001). Conclusion: An integrative care program that takes job stress, daily participation hours in the surgery and sleeping hours into consideration is essential to reduce fatigue in operating room nurses.

  • 본 연구의 목적은 중소병원 수술실 간호사의 간호근무환경과 직무몰입의 관계에서 심리적 임파워먼트의 매개효과를 규명하기 위함이다. 연구 대상은 전국 24개 중소병원의 수술실에서 근무하는 간호사 150명으로, 구조화된 설문지를 이용하여 2018년 7월부터 9월까지 자료를 수집하였다. 수집된 자료는 t-test, ANOVA, Pearson's correlation coefficients, 회귀분석으로 분석하였다. 연구결과, 직무몰입은 간호근무환경 또는 심리적 임파워먼트와 정 상관관계가 있었고. 심리적 임파워먼트는 간호근무환경과 정 상관관계가 있었다. 심리적 임파워먼트는 간호근무환경이 직무몰입에 미치는 영향을 부분 매개하는 것으로 나타났다 중소병원 수술실 간호사들이 간호근무환경을 긍정적으로 인식하여 직무에 몰입하도록 돕기 위해 심리적 임파워먼트 향상 프로그램을 개발하여 적용하는 것이 필요하다.

  • Purpose: The purpose of this study was to investigate the effects of aromatherapy on stress, fatigue, mood, and vital signs of the nurses in the operating rooms. Methods: The data were collected from September 2007 to February 2008 in the operating room at K hospital located in Seoul, Korea. A total of 48 nurses were recruited. Among them, 25 nurses were randomly assigned to an experimental group and 23 were assigned to a control group. Experimental group was provided with aroma inhalation by necklace which had essential oil mixture of 0.2 cc. For aroma inhalation, Lavender, Zeranium and Mazoram were mixed in ratio of 5:3:2. In order to test the effectiveness of aroma inhalation, the pretest and posttest results for stress, fatigue, mood, and vital signs were compared between the experimental group and the control group. The data were analyzed by unpaired t-test and ANCOVA using the SPSS/WIN 15.0 program. Results: There were statistically significant decreases in the stress and fatigue scores, and increase in the mood scores and showed significant relaxation in the vital signs after the aroma inhalation. Conclusion: The aromatherapy had the positive effects on stress, fatigue, mood, and vital signs.

간호사의 방사선 방어 지식, 태도 및 행위에 관한 연구 : 수술실과 중환자실 중심으로 (A Study on Knowledge, Attitude and Behavior for Radiation Protection of Nurses : Focus on the Operating Rooms and Intensive Care Units)

  • The purpose of this study was to investigate the knowledge, attitudes and behaviors of radiation protection in operating room and intensive care unit nurses. The Participants for this study consisted of 240 nurses working in operating rooms and intensive care units of 7 general hospitals located in Busan, South Korea. The data were collected from Feb. 15 to Feb. 29, 2016. The study results were as follows. First, 72.9% of the nurses had never been educated for radiation safety. Second, the mean score of knowledge for radiation protection was 9.21. The knowledge score of radiation safety educated nurses is higher than uneducated of nurses(t=-5.67, p<.001). Third, The mean score of attitude towards radiation protection was 41.80. The attitude score of radiation safety educated nurses is lower than uneducated of nurses(t=0.02, p<.030). Fourth, the mean score of behavior for radiation protection was 32.57. The behavior score of radiation safety educated nurses is higher than uneducated of nurses(t=0.35, p=.001). Consequently, it was found that nurses had lack of the radiation safety education. Overall knowledge and behavior towards radiation protection was low. Therefore, it is necessary to raise awareness of radiation protection and to educate, manage, and improve processes to increasing compliance with protective measures against radiation.

수술실 간호사의 환자안전 확보 경험 (Operating Room Nurses' Experiences of Securing for Patient Safety)

  • Purpose: This study was done to evaluate the experience of securing patient safety in hospital operating rooms. Methods: Experiential data were collected from 15 operating room nurses through in-depth interviews. The main question was "Could you describe your experience with patient safety in the operating room?". Qualitative data from the field and transcribed notes were analyzed using Strauss and Corbin's grounded theory methodology. Results: The core category of experience with patient safety in the operating room was 'trying to maintain principles of patient safety during high-risk surgical procedures'. The participants used two interactional strategies: 'attempt continuous improvement', 'immersion in operation with sharing issues of patient safety'. Conclusion: The results indicate that the important factors for ensuring the safety of patients in the operating room are manpower, education, and a system for patient safety. Successful and safe surgery requires communication, teamwork and recognition of the importance of patient safety by the surgical team.

수술실 간호사의 직업적 손상 경험 현황과 안전 분위기의 상관관계 (The Correlation between Occupational Injuries and Safety Climate among Operating Room Nurses)

  • Purpose: This study aimed to examine the occupational injuries experienced by operating room nurses and investigate the relationship between these injuries and safety climate. Methods: The study involved 198 operating room nurses working in general and tertiary hospitals with more than 300 beds. Results: Among the four sub-factors of occupational injuries experienced by operating room nurses, risks due to ergonomic hazards were the most prevalent and those due to physical hazards were the least. The scores related to occupational injury showed a significantly negative correlation with the safety climate. Conclusions: The findings indicate that higher operating room nurses' awareness of the safety climate is associated with lower rates of occupational injury. Therefore, organizational efforts and interventions are imperative to improve the safety climate within hospitals. Additionally, in order to investigate the safety of operating room nurses more in-depth, it is necessary to expand not only the sample size but also the geographic scope.

수술실 간호사의 성과 평가에 대한 인식이 조직몰입 및 성과에 미치는 영향 (Relationship between Perception for Appraisal of Perioperative Nurses and Performance and Organizational Commitment)

  • Purpose: This study was an analysis of the relationship between perception for appraisal of staff nurses in operating rooms and performance and organizational commitment. Method: The survey was conducted with 176 staff nurses in operating rooms in 2 hospitals in Seoul. Data were analyzed using frequency, one-way ANOVA, Pearson correlation analysis, and stepwise multiple regression. Result: 1. Perception for appraisal including accuracy and justice was significantly related to organizational commitment (r=.496, P=.000). Perception for appraisal including accuracy and justice was slightly related to performance (r=.220, P=.003). 2. In order to determine the percentage of the variance of performance and organizational commitment that could be predicted by perception for appraisal, perception for appraisal was entered in the regression equation. Accuracy accounted for 25% of the variance in the organizational commitment. To determine the percentage of the variance of performance that could be predicted by perception for appraisal and organizational commitment, the perception for appraisal and organizational commitment were entered in the regression equation. Organizational commitment accounted for 21% of the variance in the performance. Consequently accuracy predicted organizational commitment. Organizational commitment predicted performance. Conclusions: Findings indicate the need to increase accuracy of performance appraisal to promote organizational commitment and performance in perioperative nurses.

일개 병원의 수술실 약제관련 업무 적정화 방안연구: 설문조사를 중심으로 (Evaluating Appropriateness of Medication Use in the Operating Rooms of a Tertiary Hospital: Based on Survey)

  • Background: Since the use of opioid analgesics is frequent in operation rooms (OR), the risk of medication error is high; however the use of medication in the OR has been operating independently with the hospital pharmacy. Therefore, the assessment on management of medication use in operation and the pharmacist's role is needed. Methods: We conducted the literature review and survey from anesthesiologists, operating nurses at Chung-Ang Hospital on management of medication for operation use, awareness on need for medication management efficiency, need for satellite pharmacy in the operating room and its effect. Results: 56% of medical staffs responded that management of medication in the operating room is efficient; however, 82.6% responded that they felt the inconvenience in medication delivery to the OR when additional prescription was ordered. 51.5% also responded that extra time was required for management of narcotics and inventory/record keeping. 80% agreed that there could be lost costs due to prescription missed. Medical staffs responded improving the drug management system could increase the OR efficiency (87%), and eventually bring the increase in hospital revenue (80.4%). Those who responded that implementation of OR satellite pharmacy was needed include physicians (84.6%), nurses (63.6%), and also responded that it'd bring more profit to the hospital by increasing the efficiency in OR (60.9%). Conclusion: For efficient management of medications, implementation of OR satellite pharmacy would lead to improved drug management and increased efficiency in OR and reduced cost and improved patient care.

수술실의 간호조직특성, 조직문화 유형 및 팀 효과성과의 관계 (A Study on the Relationships among Nursing Organizational Characteristics, Types of Nursing Organizational Culture and Team Effectiveness at Operating Rooms)

  • Purpose: This study was a literary investigation in the development of an effective team-raised and cultural nursing organization targeting operating room nurses. This was done by grasping organizational features, different types of nursing organizational culture and team effectiveness, and by analyzing the relationships among variables. Method: The participators were 373 nurses. The data was collected between March 10th and April 28th, 2004. SPSS Win 11.0 program was used for data analysis. Results: Results of this study were as follows: A nursing organization at operating rooms was found to have an organizational culture with the characteristic of a centralized power and hierarchy-oriented organizational culture. With respect to a relationship between operating room organizational characteristics and the types of nursing organizational culture, there was a significant correlation in relationship-oriented culture and innovation-oriented culture. In the relation between the types of operating room organizational culture and team-effectiveness, it showed a significant correlation in team-devotion, peer-groupsatisfactional perception of a team's outcome in decreasing order. Conclusion: The organizational characteristic at operating rooms was power centralization and the culture type was hierarchy-oriented. These are factors that have a negative effect on a team-effectiveness-affecting relationship-oriented culture, innovation-oriented culture and work-oriented culture.

수술실간호사 업무수행평가 도구개발 (Development of Performance Evaluation Tool for Operating Room Nurses)

  • Purpose: The purpose of this study was to develop a performance tool to evaluate operating room nurses. Method: In the first stage, 59 preliminary items were selected after two focus group meetings were carried out using as background a reference review. At this stage 59 items were developed. Next, the selected preliminary items were modified by an expert group of nurses with over 10 years experience, nurses holding doctoral degrees and professors in nursing. In the third and fourth stage, these items were analyzed by the expert group to determine validity and tested by hospital nurses with over 10 years experience to determine reliability Results: For the final tool, the 59 items, 47 items for the performance evaluation dimension and 12 items for the competence evaluation dimension were found to be valid and reliable. Cronbach's $\alpha$ was .9622, a promising alpha value for the total instrument. Conclusion: This tool is an efficient performance evaluation instrument providing meaningful feedback for operating room nurses.
