소음과 수면양상에 관한 연구 - 중환자실을 중심으로 - (The Relationship between Noise and Sleep Patterns in Intensive Care Units)

  • The environment in the ICU leads to negative changes in a patient's usual sleep pattern and so contributes negatively to the patient's health condition as compared to patients in general wards. Therefore, it is thought that an important nursing intervention would be to identify the relation between noise and sleep patterns which play an important role in illness recovery. The purpose of the present study was to explore the relationship between noise in the ICU and the sleep pattern of patients admitted to the ICU. A descriptive correlation design was used to examine the relationship. Thirty-four subjects were recruited from a Medical ICU (MICU), Surgical ICU (SICU) and Coronary Care Unit (CCU) at a large university hospital in Suwon. Data were collected from September 28 to October 31 in 1999. In the present study, noise was categorized into noise level and patients' perception of noise. The objective noise level was measured using the A-Weighted Sound Level Meter. The patients' preception of noise was measured using a self-reported questionnaire developed by the researcher. Sleep patterns in this study includes both quantity and quality of sleep. These were measured using open ended questionnaires and the 'Korean Sleep Scale A' developed by Oh, Song, Kim(1998). The data was analyzed using the SPSS-WIN to test the research question, Pearson product moment correlation coefficient was run. Ancillary analysis were conducted with demographic variables to determine their relation to the main study variables. For the ancillary analysis, t-test and one-way ANOVAs were performed. The results of the present study are summerized as follows : 1. The total mean of objective noise level (10pm-6am) was 56.2dB. The means for night time noise level in individual ICUs for the SICU, MICU and CCU, were 58.7dB, 58.6dB and 48.3dB, respectively. The total mean for patients' noise perception was 42.8 out of a maximum possible score of 76. For item means of noise perception, the one ranked highest was "conversations between doctors and nurses" (3.2). The one ranked lowest was "noise from the radio" (1.2). Regarding the degree of perception for each type of noise source, the one ranked highest was "equipment noise" (2.6), the second was "conversation between medical staff" (2.4), the third was "conversation between patients, caregivers and visitors" (2.3), and the one ranked lowest was "environment noise" (1.8). 2. Looking at quantity of sleep of ICU patients, the mean nocturnal sleep time was found to be 4.9 hours. The total mean of sleep quality for ICU patients was 21.0 out of a maximum possible score of 40. 3. The relationship between perception of noise and quantity of sleep was statistically significant(r= - .41, p<.05). The relationship between perception of noise and quality of sleep was also statistically significant(r= - .47, p<.01). The results of the study indicate that personal perception of noise is related to sleep patterns. Therefore, it is suggested that nursing interventions be developed to reduce the degree of personal perception of noise and, thus, decrease sleep pattern disturbances in patients in the ICU.

이러닝 교수 설계자 인적 자원 유통을 위한 휴먼 클라우드 플랫폼 프레임워크 설계 (A Design of Human Cloud Platform Framework for Human Resources Distribution of e-Learning Instructional Designer)

  • Purpose - In the 21st century, as information technology advances alongside the emergence of the 4th generation, industrial age, industrial environment has become individualized and customized. It is important to hire good quality employees for good service in the industry. The e-learning market is growing every year. Although e-learning companies are finding better quality employees in e-learning, it is not easy to find it. Companies also spend a lot of time and cost to find employee. On the employees side, they want to get a job freely when they want, but they cannot find their job easily. Furthermore, the labor market environment is changing fast. In the 4th generation, industrial age, employers require to find manpower whenever they need and want at little cost. So of their own accord, we have considered the necessity of management of human resources for employees and employers in e-learning. The purpose of this study is to propose a human cloud platform framework for enabling an efficient management of human resources in e-learning industry. Research design, data, and methodology - To pinpoint the items of a human cloud platform framework, the study was initiated according to the following process. First, items of competency relating to e-learning instructional designer was analyzed. Second, based on the items of information from this analysis, selection and validity verification took place with 5 e-learning specialists group. Third, the opinion of experts who were in charge of hiring in e-learning companies were collated with the questionnaire. Lastly, the human cloud platform framework was proposed based on opinion results. Results - The framework was comprised of 7 domains and 27 items in order to develop the human cloud platform for e-learning instructional designer. The analysis results showed that the most highly considered item were 'skill (4.60)' that employee already have the capability. Following this (in order) were 'project type (4.56)', 'work competency (4.56)', and 'strength area of instructional design (4.52)'. Conclusions - The 27 items in the human cloud platform framework were suggested in this study. Following this, we can consider to develop the human cloud platform for finding a job and hiring e-learning instructional designer easily. For successful platform operation, we need to consider reliability between employer and employee. In addition, we need quality assurance system based on operation has public confidence.

입원아동과 간호사가 지각한 돌봄에 대한 연구 (Hospitalized Children and Their Nurses각 Perception of Caring)

  • Caring has been identified as the essence and unifying domin of nursing(Leininger). Many nurses believe that the art of nursing is comprised of actions that are predominantly caring in nature. Although caring has been the traditional ideology of nurses, it is only now beginning to emerge as the central construct for the development of nut sing research, theory and practice. The problem addressed by this study was to identify how hospitalized children and their nurses express the meaning of caring, how they think nurses should care for children and to describe their experiences of being cared for. The purpose was to provide theoretical understanding of caring as perceived in Korea to contribute to the development of Korean nursing knowledge. The subjects were 76 hospitalized children admitted to pediatric units in five teaching hospitals and 66 nurses who were caring for these children. In this descriptive study, data were collected from Nov 11, 1991 to Jan 30, 1992 by interviews and an open-ended questionnaire and analysed by van Kaam's method. Caring themes perceived by the children and their nurses were classified into eight categories, -helping, comfort, love, warmth(only by children), recovery from illness, health maintenance (only by nurses), presence, nurturance and responsibility. Ideal caring behaviors perceived by the children and their nurses were six categories, -to give help, provide comfort, give love, stay with, treat warmly and aid recovery. Subcategories of giving help were promptness and competence, detailed explanations and support and encouragement. Other subcategories of giving help reported only by nurses were individualizing care, recognizing needs and providing a familiar enviornment. Subcategories of maintaining comfort were making comfortable, alleviating pain ; one subcategory reported only by children was consolating. A subcategory of giving love was concern, two subcategories reported only by nurses were compassion and respect. Subcategories of staying with were playing with and touching : only nurses reported empathy, Subcategories of treating warmly were tenderness and kindness. In the experience of caring, there were 4 categories, -to give help, stay with, show concern and provide comfort. Both the hospitalized children and their nurses had experienced caring primarily from their mothers. Mothers' caring behaviors were direct, personal, basic, supportive nursing acts. On the other hand, nurses caring behaviors were task oriented skilled procedures and medically delegated acts. This study contributes understanding of the complexity of caring, more specifically the meaning and experience of caring and ideal caring behaviors. Research may be able to move into verification when instruments are developed to measure the complexity of caring beliefs, values and behaviors in Korea and other cultural settings.

모성간호영역의 환자건강문제에 대한 간호사ㆍ환자간의 일치 (Congruence of Patients문 Health Problems Between Nurses and Patients in the Field of Maternity Nursing)

  • This study was done to identify the degree of congruence between nurses and patients on patients' health problems. The purposes of this study were 1. To compare the health problems of parturient women as identified by interview and in the nursing record. 2. To compare the health problems of patients who have had gynecological surgery as identified by interview and in the nursing record. 3. To compare the health problems of chemotherapy patients as identified by interview and in the nursing record. The design of this study was a comparative descriptive design. The subjects were 205 Obstetric-Gynecologic patients. The tool for this study was an 11 item questionnaire, including one open ended question on the patient's problem during the past one day. Data was collected through interviews and an audit of the nursing records during the period from March 22, 1992 to April 29, 1992. Data was analyzed using by frequencies and percentiles. The result of this study were summarized as follows : Pain was the most prevalent complaint for parturient women by interview(60.3% ) and from the nursing records(83.2%). There was no record in the nursing records about the complaints of lack of information and emotional problems even though there were complaints of communication problems (17.6%) and of emotional problems(3.5%) identified in the interviews but there were more records of cardiopulmonary problems in nurses record(9.1%) than the patient interviews (3.3%). In the nursing records 25.9% of the identified records identified pain problems compared with 23.3% in the interviews. In the nursing records, 22.3% of the records identified nutrition problem as compared with 18.2% in the interview. There were only a very few emotional problem identified in the nursing records (3.7%) as compared to 18.2% in the interviews. There were no comments about communication problems in the nursing records but 5.2% of the subjects mentioned of communication problems in the patient interview. There were problems in five categories for the parturient women ; comfort, communication, activity and rest, elimination, emotions, and there were problems in ten categories for the surgery patients : comfort, elimination, communication, emotions, nutrition, cardiopulmonary, thermoregulation, physical integrity, host defense and activity /rest. There were also problems in the same ten categories for chemotherapy Patients. On the other hand, in the nursing records, only comfort activity /rest, and elimination problems were identified for the parturient women, there were only seven categories of problems : comfort, elimination, cardiopulmonary, activity /rest, and nutrition for the gynecology surgical patients, and for the chemotherapy Patients, comfort, nutrition, physical integrity, cardiopulmonary, activity /rest, thermoregulation, emotion and elimination were the categories identified, and no communication problems were identified. It was found that there was low congruence between the patients' problems as identified through patient interview and as recorded in the nursing records. Therefore it can be concluded that the main content of the nursing records is the physical problems of the patients and this is not in congruence with the patients' reported problems in the emotional and communication domain.

실천공학교육의 전략 탐색을 위한 델파이조사 (Search for Strategies of Practical Engineering Education Using Delphi Method)

  • 본 연구는 '실천공학기술자 실태조사 및 개선방안' 연구과제의 일환으로 수행되었으며, 그 목적은 실천공학기술자 양성을 위한 타당한 교육목표 설정과 실천공학기술자 및 교수의 핵심역량 등을 진단, 평가하고 전문적이고 타당한 판단의 합의를 도출함으로써 우리나라 대표적인 실천공학 중심대학인 한기대의 교육발전 전략을 탐색하는데 있다. 주요 조사내용은 타 공과대학과의 차별화를 위한 방향, 실천공학기술자의 핵심역량, 실천공학기술자를 양성하기 위한 핵심 커리큘럼, 실천공학기술자 양성을 위한 교수의 핵심역량, 한기대의 정체성 확립을 위한 발전전략등 5개 부문이다. 연구방법은 델파이를 적용했는데 패널은 한국실천공학교육학회 이사, 한기대 교수 및 외부전문가로 구성된 32인이 선정되어 조사에 참여하였다. 델파이 조사는 3회에 거쳐 실시되었으며, 1차 조사에서는 개방형 설문지를 통해 5개 부문에 대한 제안사항을 연구진이 73개 문항으로 정리하였다. 그 문항들을 2차와 3차 설문지로 개발하여 이메일을 통해 배부, 회수한 후 각 항목에 대한 적합성이나 필요성 등을 5단계 리커트 척도로 평정, 연구결과를 산출하고 분석하였다.

건축설계사무소의 ISO 9001품질경영시스템 도입의 경영성과분석에 관한 연구 (The Analysis of Management Performance by Introducing ISO 9001 Quality Management System for Architectural Design Offices)

  • 본 연구는 건축설계사무소의 내부적 경영환경 개선, 국제적 표준화 및 개방화등에 대처하기 위하여 건축설계사무소의 설계품질향상과 경영도구로의 도입 확대 적용의 유용성을 내포한 ISO 9001 품질경영시스템 요구사항이 경영성과에 어떠한 영향을 미치는지 실증적으로 분석하는데 목적이 있다. 본 연구는 ISO 9001 품질시스템인증을 받은 서울지역 건축사사무소를 표본으로 설문조사 및 SPSS 10을 이용한 통계분석에 의하여 수행되었으며, 그 결과는 다음과 같다. (1) 품질경영시스템 요구사항이 경영성과에 끼친 영향으로 고객만족부분에서는 자원관리 제품실현, 조직원만족에서는 자원관리, 측정${\cdot}$분석 및 개선, 품질개선에서는 제품실현, 자원관리, 경영자책임, 경제적성과에서는 측정${\cdot}$분석 및 개선이 유의한 영향을 미쳤다. (2) 품질경영시스템 요구사항은 조직원만족에 미치는 영향에 관한 설명력이 가장 높았으며, 경제적성과에 미치는 영향에 관한 설명력은 상대적으로 낮으며 유의한 영향을 주지 못한 것으로 나타났다. (3) ISO품질경영시스템에 대한 조직원의 견해는 매우 긍정적인 것으로 나타났으며 설계사무소에도 취득동기와 취득 후 효과 또한 매우 좋은 결과로 나타났다.

요양병원 의료기관인증제 시행 후 환자안전, 의료의 질, 조직문화 및 병원경영성과의 변화 (Change in patient safety, quality of medical care, organization culture and hospital management performance after healthcare accreditation system for convalescent hospital)

  • 이 연구는 요양병원 의료기관인증제 시행 후 요양병원의 환자안전, 의료의 질, 조직문화 및 경영성과에 미친 영향을 파악함에 목적이 있다. 2015년 9월 말까지 요양병원 의료기관인증을 획득한 대구지역의 36개 요양병원 중 10개 기관을 임의로 선정하였으며 2016년 2월 15일부터 2016년 3월 25일까지 요양병원 종사자들에게 설문조사를 하여 253명을 최종분석 대상자로 선정하였다. 설문조사 내용으로는 일반적 특성, 인증이후 환자안전과 의료의 질 향상, 조직문화, 경영성과의 변화가 포함되어 있다. 연구결과 환자안전과 의료의 질 향상의 변화정도에서는 진료지원체계가 3.93점으로 가장 높았으며, 조직문화의 변화정도에서는 인적자원문화가 3.78점으로 가장 높았고, 요양병원의 경영성과 부분에서는 내부프로세스 관점, 학습 및 성장관점이 3.71점으로 가장 높게 나타났다. 병원경영성과에 영향을 미치는 요인을 파악한 결과 직종이 행정직보다 간호직, 직급이 국장보다 사원이거나 과장, 환자안전과 질 향상에서 기본가치체계가 개선되었다고 생각하는 경우, 조직문화에서 인적자원문화, 개방체계문화, 위계서열문화, 생산중심문화가 개선되었다고 생각하는 경우 병원경영성과를 긍정적으로 변화시키는 영향 요인이라 응답하였다. 요양병원 의료기관인증제 시행 후 환자안전과 의료의 질 향상에는 높은 변화를 보여 긍정적 영향을 미쳤으나, 조직문화의 4가지 유형 중 위계서열문화와 생산중심문화는 보통수준의 변화를 보였으며 병원경영성과 중 재무관점은 다른 관점에 비해 상대적으로 낮은 변화를 보였다.

비시장재화의 가치평가에 있어서 근접효과(Proximity Effects)의 검증에 관한 연구 : 조건부가치평가법을 중심으로 (Tackling Proximity Effects in Nonmarket Valuation Approaches : An Example of Contingent Valuation Method)

  • 자연자원인 갯벌에 비시장가치평가법 중의 하나인 조건부가치평가법을 적용하여 근접효과를 검증하고자 하였다. 전국의 가구 수에 비례하여 1,000가구를 임의추출하였다. 그리고 갯벌로부터의 거주거리에 따라 30분 간격으로 I 구역에서 V구역까지 구역을 나누어서 표본을 재구성하였다. 이러한 근접효과 분석에는 모형의 적합성과 변수의 통계적 유의성을 고려하여 로그선형모형(log-linear)을 적용하였다. 일반적으로는 로짓모형이 많이 사용되지만 소득효과를 제어할 수 없기 때문에, 소득효과를 제어할 수 있는 로그선형 모형을 이용하였다. 결과를 살펴보면, 각 구간별 지불의사액의 차이를 명확하게 보여주었으며(${\theta}_1$ >0. ${\theta}_2$ >0), 또한 모형에서 더미변수들이 통계적으로 5% 유의수준에서 유의하다는 결론($dum1{\neq}0$, $dum2{\neq}0$, $dum3{\neq}0$, $dum4{\neq}0$)을 얻을 수 있었다. 실제로 개방형 설문조사에서도 I 구역 거주자들의 지불의사액이 V구역 거주자들의 지불의사액보다 약 26% 이상 크게 나타났다. 따라서 이렇게 도출된 결과로부터 비시장재화의 특징을 지닌 자연자원에 대하여 비시장가치평가법을 적용하는 경우에는 근접효과가 존재하기 때문에 설문에 이를 충분히 반영(embedded)하여야 한다는 결론을 도출하였다. 이를 통해서 좀 더 명확한 지불의사액 추정이 가능하며, 편익이전시 과소평가되는 문제를 예방할 수 있다.

영재 청소년의 학업탄력성 척도 개발 (Development of Academic Resilience Scale for Gifted Youth)

  • 본 연구의 목적은 영재 청소년의 학업탄력성 척도를 개발하는 것이다. 이를 위하여 158명의 과학영재교육원 학생들을 대상으로 개방형 질문과 선행연구 고찰로부터 101개의 기초 예비문항을 추출하였다. 이를 토대로 다시 240명의 과학영재학생들을 대상으로 문항분석 및 탐색적 요인분석을 실시하였다. 영재 청소년용 학업탄력성 척도 문항 개발을 위한 최종단계로 인천, 경기, 서울, 창원, 사천, 울산 지역의 영재교육원 및 지역영재학급 학생 444명을 대상으로 확인적 요인분석을 실시하였다. 그 결과 영재 청소년의 학업탄력성을 측정할 수 있는 11요인 42문항을 추출하였다. 하위요인은 '목표의식', '자기이해', '교육적 열망', '학업적 자기조절', '과제 집착력', '성공귀인(능력)', '성공귀인(노력)', '대인관계(친구)', '대인관계(부모)', '대인관계(교사)', '낙관성'으로 확인되었다. 척도의 타당도를 확보하기 위하여 확인적 요인분석 결과, 모든 경로계수가 유의하고, 개념신뢰도, 분산추출지수, 결정계수가 기준치 이상으로 나타났다. 또한 영재 청소년의 학업탄력성 척도가 학교적응, 회복탄력성, 학업소진 척도와 유의한 상관을 나타내고 있어 내적 외적 타당도를 확보할 수 있었다. 본 연구는 영재 청소년의 학업탄력성을 측정할 수 있는 도구를 개발하였다는 데 그 의의가 있다.

농촌문화마을 조성 및 활성화 방안연구(1) - 김제시 대산마을(현황분석 및 기본구상)을 중심으로 - (A Study on the Creation and Activation Program of Cultural Rural Village - Focused on the Case in Dae -San Village, Kimje-si, Chonbuk Province -)

  • Now in order to overcome the weakest points of the rural areas of the city of Kimje and, transform them into rural cultural villages which have local governing systems suitable to new localization age and activate this plan, we selected Daesan Village as a model village which had shown a lot of potentials in the basic research and studied it dividing it into the former part and the latter part. We studied Daesan village in the former part focusing on state analysis and basic ideas and in the latter part focusing on master plan and detail planning. We can summarize the conclusion like the followings. 1. Daesan Village located 8 kilometer away from the downtown Kimje and the city of Iksan respectably has comparatively good environment of good sunny place as an open field whose surrounding configuration of the ground consists of farming lands and low hills in front and rear. It has 38 farming households in all. 2. Human environment(인문환경); the village road whose width is about 4 meters is forming a flow system forking off into three. There is a route bus which operates three times a day even into the inside of the village. The main sources of revenue are vegetables in facilities, fruits and floriculture. Their average revenue is about 10.5 million won. 3. Here in DaeSan Village a legend dealing with Teasan literally meaning a big mountain consist of th village's tradition and you can see the tombs of a very faithful son and Anwi an army general in the age of the Japanese Invasion of Korea of 1592 to 1598 inside the village. 4. 85 out of the eitire population 141 whose age are over 20 showed very positive attitudes in a questionnaire about, making the village a cultural one and its development. 5. The basic of planned ideas is to increase the revenue of the farming household by making the village a professional farming one which has a state-of the-art production facility and agricultural technique. It is to make the village the one where people can enjoy the sense of the rural life and the farmer can enjoy their lives through consumptive and consistant leisure and resting activities. 6. We are planning to make entrance space, life space, rest and sport space, and cultural space considering the characteristics of the village and the demand of the resident. We are also planning to make tile entire city of Kimje an information transmitting base in short and long term perspectives. 7. DaeSan Village was planned as a place where tradition and the future exist together. On the basis of this concept we planned future programs for Daesan Village and in the latter part of the study master plans and detail plans will be continued.the regional agricultural condition. The development permissions were only during the period of restricted to use ($1979.12{\sim}1993.11$). We propose that the authority of development permission should be given to the local autonomy government, because the local government has the knowledge of its individual agricultural conditions.
