성별에 따른 급성심근경색증의 특성비교 (Comparison of Presentation in Acute Myocardial Infarction by Gender)

  • Purpose: This study was to explore gender differences on presenting patients with acute myocardial infarction in the emergency department. Methods: The survey was done with 143 emergency medical charts presented to the emergency department and diagnosed with acute myocardial infarction between January 2005 and December 2006. The collected data were analyzed with frequency, chi-square, and t-test. Results: Significant gender differences were apparent in age, route to the emergency department, elapsed time from onset of symptoms to arrival, and initial heart rate. Women were significantly more likely to report hypertension, diabetes, and congestive heart failure than men, but men were significantly more likely to report smoking. Chest pain was the most common initial symptom in both men and women. Women were significantly more likely to report dyspnea and nausea/vomiting than men. Conclusion: Although similarities exist in the associated symptoms of acute myocardial infarction, women might experience different symptoms, compared to men. These findings have implication that patients and health care providers should consider gender difference in presenting symptoms.

  • Natural convection of a fluid with intermediate Prand시 number of Pr=0.2 in a horizontal annulus is considered, and the bifurcation phenomena and chaotic flows are numerically investigated. The unsteady two-dimensional streamfunction-vorticity equation is solved with finite difference method. The steady downward flow with two counter-rotating eddies bifurcates to a simple periodic flow with a fundamental frequency. And afterwards, second Hopf bifurcation occurs, and a quasi-periodic flow with two incommensurable frequencies appears. However, a new time-periodic flow is established after experiencing quasi-periodic states. As Rayleigh number is increased further, the chaotic flow regime is reached after a sequence of successive Hopf bifurcation to quasi-periodic and chaotic flow regimes. A scenario similar to the Ruelle-Takens-Newhouse scenario of the onset of chaos is observed.

  • An algorithm for the sodium boiling model has been developed for calculation of the void reactivity feedback as well as the fuel and cladding temperatures in the KALIMER core after onset of sodium boiling. Modeling of sodium boiling in liquid metal reactors using sodium as a coolant is necessary because of phenomenon difference comparing with that observed generally in light water reactor systems. The applied model to the algorithm is the multiple-bubble slug ejection model. It allows a finite number of bubbles in a channel at any time. Voiding is assumed to result from formation of bubbies that (ill the whole cross section of the coolant channel except for the liquid film left on the cladding surface. The vapor pressure, currently, is assumed to be uniform within a bubble The present study is focused on not only demonstration of the vapor bubble behavior predicted by the developed model, but also confirmation of a qualitative acceptance for the model. As a result, the model can represent important phenomena in the sodium boiling, but it is found that further effort is also needed for its completition.

원주 후류에서의 2차적 불안정성 (Secondary Instability in the Wake of a Circular Cylinder)

  • Secondary instability of flow past a circular cylinder is examined using direct numerical simulation at Reynolds number 220 and 250. The higher-order finite difference scheme is employed for the spatial distributions along with the second order Adams-Bashforth and the first order backward-Euler time integration. In x-y plane, the convection term is applied by the 5th order upwind scheme, and the pressure and viscosity terms are applied by the 4th order central difference. In spanwise, Navier-Stokes equation is distributed using Spectral Method. The critical Reynolds number for this instability is found to be about Re=190. The secondary instability leads re three-dimensionality with a spanwise wavelength about 4 cylinder diameters at onset (A-mode). Results of three-dimensional effect in wake of a circular cylinder are represented with spanwise and streamwise vorticity contours as Reynolds numbers.

  • Background: This study evaluated Somatosensory evoked potentials (SEP), Quantitative sensory testing (QST), and thermography as diagnostic methods for nerve injury. Methods: From 2006 through 2011, 17 patients (mean age: 50.1 years) from ${\bigcirc}{\bigcirc}{\bigcirc}{\bigcirc}$ Hospital who sought care for altered sensation after dental implant treatment were identified. The mean time of objective assessment was 15.2 months after onset. Results: SEP of Inferior alveolar nerve(IAN) was $15.87{\pm}0.87ms$ on the normal side and $16.18{\pm}0.73ms$ on the abnormal side. There was delayed N20 latency on the abnormal side, but the difference was not statistically significant. In QST, the abnormal side showed significantly higher scores of the current perception threshold at 2 KHz, 250 Hz, and 5 Hz. The absolute temperature difference was $0.55^{\circ}C$ without statistically significance. Conclusion: These results indicate that QST is valuable as an objective method for assessment of nerve injury.

MLS 차분법을 이용한 동적균열전파 해석 (Analysis of Dynamic Crack Propagation using MLS Difference Method)

  • 본 논문은 MLS(Moving Least Squares) 차분법을 바탕으로 동적균열전파 해석을 수행하기 위한 알고리즘을 제시한다. MLS 차분법은 절점만으로 이루어진 수치모델을 사용하며, 이동최소제곱법을 이용하여 전개한 Taylor 다항식을 기초로 미분근사식을 유도하기 때문에, 요소망의 제약에서 완벽하게 벗어난 절점해석이 가능하다. 시간항을 포함하는 동적 평형방정식은 Newmark 방법으로 시간적분 하였다. 동적하중을 받는 균열이 전파할 때, 매 시간단계마다 절점모델을 재구성하지 않고 균열선단 주변에서 국부적인 수정을 통해 해석이 가능하다. 동적균열을 묘사하기 위해 가시한계법(visibility criterion)을 적용하였고, 동적 에너지해방률을 산정하여 균열의 진전유무와 그에 상응하는 진전방향을 결정하였다. 모드 I 상태와 혼합모드 상태에서 균열이 진전하는 현상을 모사하였고, 이론해와 Element-Free Galerkin법으로 계산한 결과와의 비교를 통해 개발된 알고리즘의 정확성과 안정성을 검증하였다.

Difference in Muscle Activity of Deltoid Muscle according to Isokinetic Range of Motion Using Proprioceptive Neuromuscular Facilitation Pattern of Upper Extremity

  • Purpose: This study sought to compare the muscle activity of the deltoid muscle according to the range of motion during the proprioceptive neuromuscular facilitation (PNF) upper extremity D2 pattern exercise performed with an isokinetic exercise device. The aim was to provide basic data for selecting an exercise for the relevant segment of the range of motion to enhance function in clinical practice. Methods: In this study, the relevant measurements of the anterior and middle trapezius of 25 healthy adults were taken using a surface electromyography system. The upper extremity pattern exercise was performed in three ranges (0% to 50%, 50% to 100%, and 25% to 75%) using the upper extremity isokinetic device. Results: There was a statistically significant difference in the muscle activity ratios of the anterior and middle deltoid muscles according to the measurement conditions (p<0.05). There was a statistically significant difference in the activity ratio of the middle deltoid muscle according to the measurement conditions in the ratio from the start range to the end range (p<0.05). There was no significant difference in the muscle onset time difference according to the measurement conditions (p>0.05). Conclusion: The muscle activity of the anterior and middle deltoid muscles was analyzed according to the range of motion during the PNF upper extremity pattern exercise performed with an isokinetic device. The results could be used as a basis for selecting exercises for the relevant segment of the range of motion according to the function to be emphasized.

의미적 속성을 가진 시.청각자극의 SOA가 시청각 통합 현상에 미치는 영향 -중복 표적 효과와 시각 우세성 효과를 중심으로- (The Influence of SOA between the Visual and Auditory Stimuli with Semantic Properties on Integration of Audio-Visual Senses -Focus on the Redundant Target Effect and Visual Dominance Effect-)

  • 본 연구는 의미적 속성을 가진 시각과 청각자극 간의 SOA(stimulus onset asynchrony)가 시청각 통합 현상에 미치는 영향을 살펴보고자 하였다. 시청각 통합 현상 중 표적을 의미하는 자극의 양상이 두 개 이상인 경우 표적에 반응이 빠르고 정확한 중복 표적 효과(redundant target effect)와 청각 자극에 비해 시각 자극에 대한 반응이 빠르고 정확한 시각 우세성 효과(visual dominance effect)를 중심으로 살펴보기 위해서 시각과 청각 단일 양상 표적 조건과 다중 양상 표적 조건을 구성하여 조건들의 반응시간과 정확률을 살펴보았다. 그 결과, 시 청각자극의 SOA가 변하더라도 중복 표적 효과는 이에 영향을 받지 않는 것으로 나타났으며, 두 자극 간의 SOA가 100ms 이상인 조건에서는 청각자극의 우세현상이 나타났다. 이러한 결과는 중복 표적 효과의 경우 두 양상자극 간의 SOA가 변하더라도 안정적으로 지속된다는 점을 시사하며, 청각자극의 경우 시각자극에 비해 약 100ms 이상의 시간적 이득조건이 마련되었을 때에 비로소 우세한 정보처리의 행동결과가 도출될 수 있음을 시사한다.

모야모야병 환자의 뼈목동맥관 직경과 임상표현과의 관계 (Relation of Bony Carotid Canal Diameter and Clinical Manifestations in Patients with Moyamoya Disease)

  • Background: Moyamoya disease is characterized by a progressive stenosis or occlusion of the intracranial internal carotid artery and/or the proximal portion of the anterior cerebral artery and middle cerebral artery. Whether the onset time was childhood or adulthood, the bony carotid canal diameter might be different, but reflects the size of internal carotid artery passing through the bony carotid canal. In this study, we aimed to identify the relationship between bony carotid canal diameter and clinical manifestation. Methods: 146 consecutive patients diagnosed with moyamoya disease by brain imaging studies were included. We measured the diameter of a transverse portion of bony carotid canal on bone window of a brain computed tomography(CT) image. Patients were divided into two groups, ischemic or hemorrhagic stroke according to clinical manifestation. As a result, 115 patients were included. The Suzuki stage was used as criteria for disease progression. Results: Bony carotid canal diameter was $3.6{\pm}0.5$ (right) and $3.6{\pm}0.4$ (left) in the hemorrhagic stroke group, and $3.7{\pm}0.4$ (right) and $3.6{\pm}0.4$ (left) in the ischemic stroke group. The bony carotid canal diameter of the moyamoya vessels (3.6 mm) was smaller than the diameter of non-moyamoya vessels (3.8 mm), significantly (p = 0.042). However, there was no difference in the collateral patterns and clinical manifestation in a comparison of both groups. Conclusions: In our study, there was no significant difference of clinical manifestations and collateral patterns depend on the bony carotid canal diameter in patients with moyamoya disease. These findings suggest that the clinical presentations of moyamoya disease are not related to the onset time of the disease.

Identification of an effective and safe bolus dose and lockout time for patient-controlled sedation (PCS) using dexmedetomidine in dental treatments: a randomized clinical trial

  • Background: This study investigated a safe and effective bolus dose and lockout time for patient-controlled sedation (PCS) with dexmedetomidine for dental treatments. The depth of sedation, vital signs, and patient satisfaction were investigated to demonstrate safety. Methods: Thirty patients requiring dental scaling were enrolled and randomly divided into three groups based on bolus doses and lockout times: group 1 (low dose group, bolus dose 0.05 ㎍/kg, 1-minute lockout time), group 2 (middle dose group, 0.1 ㎍/kg, 1-minute), and group 3 (high dose group, 0.2 ㎍/kg, 3-minute) (n = 10 each). ECG, pulse, oxygen saturation, blood pressure, end-tidal CO2, respiratory rate, and bispectral index scores (BIS) were measured and recorded. The study was conducted in two stages: the first involved sedation without dental treatment and the second included sedation with dental scaling. Patients were instructed to press the drug demand button every 10 s, and the process of falling asleep and waking up was repeated 1-5 times. In the second stage, during dental scaling, patients were instructed to press the drug demand button. Loss of responsiveness (LOR) was defined as failure to respond to auditory stimuli six times, determining sleep onset. Patient and dentist satisfaction were assessed before and after experimentation. Results: Thirty patients (22 males) participated in the study. Scaling was performed in 29 patients after excluding one who experienced dizziness during the first stage. The average number of drug administrations until first LOR was significantly lower in group 3 (2.8 times) than groups 1 and 2 (8.0 and 6.5 times, respectively). The time taken to reach the LOR showed no difference between groups. During the second stage, the average time required to reach the LOR during scaling was 583.4 seconds. The effect site concentrations (Ce) was significantly lower in group 1 than groups 2 and 3. In the participant survey on PCS, 8/10 in group 3 reported partial memory loss, whereas 17/20 in groups 1 and 2 recalled the procedure fully or partially. Conclusion: PCS with dexmedetomidine can provide a rapid onset of sedation, safe vital sign management, and minimal side effects, thus facilitating smooth dental sedation.
