  • As the proportion of the elderly population in rural Korea has increased rapidly, the quality of life of the rural elderly has become a major public and policy concern. In this regards, most of the researches on rural elderly have focused mainly on the 'risk factors or problems' of rural areas, ignoring the 'positive or protective aspects' of rural community on the quality of life of elderly. This study attempts to explore the possible positive influence of rural community on the quality of life of elderly and to examine the linkage among ecological characteristics of the communities, social interaction and quality of life of rural elderly. To achieve this goal, a community case study was conducted in four Korean rural villages. The data were collected using participant observation method and in-depth interviews (person-to-person, group interviews). All the interviews were tape-recorded and transcribed verbatim for the analysis. The data was analyzed using the reflective qualitative analytic technique. The major findings of this study are as follows : All of the four villages have strong community ties and provide contexts which make it possible for older people to share their everyday life with neighbors and to actively participate as a community member. Although the four villages were geographically not far from each other, the pattern of social interaction and the nature of everyday-life of the elderly were nevertheless different depending on the ecological environment of each community, the relationship history among the community members, and the shared experience of historical events in recent Korean history. Elderly men have smaller networks and have less frequent contact with community members than elderly women. The political and practical implications of this study are discussed.

  • This study analyzed per capita consumption expenditure by the consumption quintile of Middle and Older Elderly Households and the variables that affected it. Sample of 2,983 households was collected from the 6th KLoSA in 2016. First, when comparing the HH's living expenses by the number of family members, two persons to a person ratio, more than six persons to a person ratio were 2.007 and 4.148, respectively. The ratio increased as the number of family member increased; however, the rate of ratio increase decreased. Second, the per capita living expenses for the first and fifth quintile were 5.11 million won and 16.93 million won at the per capita living expenses that applied the OECD's equivalence scale to the HH's living expenses. The per capita living expenses for the fifth quintile was 3.31 times higher than in the first quintile. Third, among the variables influencing per capita living expenses by the consumption quintile, the marital status was the significant for all consumption quintiles; however, HH's income, HH's total asset, HH's financial asset, pension, subjective perception of the economy class, home ownership, residence (metropolitan or small city), education (elementary school), participation groups were significant in some quintiles among all quintiles. Fourth, of the significant variables influencing per capita living expenses, the most influential variables were first quintile followed by second quintile, third quintile, fourth quintile, marital status, HH's income, home ownership, HH's total asset, participation groups, residence (metropolitan), subjective perception of the economy class, in order.

  • Objectives : To analyze factors associated with elderly falls in a community dwelling, a comparatively important, but somewhat neglected, health mailer. Methods : Data came from personal interview surveys using a questionnaire of 552 people aged 65 or older living in a community. Socioeconomic, and health related characteristics were investigated as independent variables and experiences of falling in the previous year as the dependent variable. Results : 118(21.4%) of the elderly subjects experienced a fall in the previous year. 24 subjects experienced more than two falls, accounting for 20.3% of the elderly fall victims studied. Factors affecting the falls were families living together, level of daily activity, heart disease, and aconuresis;, therefore an elderly person who lives alone, lives independently, has heart disease, or suffers urinary incontinence had a demonstrated increased chance of failing. Conclusion : Groups at risk for luting included elderly persons living alone, living independently, suffering heart disease, or experiencing urinary incontinence. When an elderly fall prevention program is developed and implemented, these results should be considered.

  • Objective: This study was conducted to provide basic data for the establishment of effective health policies for the unmet medical experience that may occur among the elderly depending on whether they live in a singleperson household or not. Methodology: This study used data from the 8th National Health and Nutrition Examination Survey (2019-2020) and excluded cases with missing values in variables for the total number of respondent participants of 15,469. Finally, 2,850 subjects aged 65 or older were selected for final analysis. This study examined the relationship between experiences of unmet medical needs, attempting to confirm the relationship between single-person households and unmet medical needs through subgroup analysis considering gender, age, and household income. Results: According to the results, in the case of single-person households, the odds ratio (OR) for unmet medical needs was significantly higher at 1.60 times (95% CI: 1.16-2.21). Upon conducting subgroup analyses for gender, age, and household income quintiles, the OR was significantly higher at 2.24 times (95% CI: 1.14-4.41) for males and 1.48 times (95% CI: 1.02-2.14) for females, statistically significant in both cases. For individuals aged 65-69, the OR was significantly higher at 1.90 times (95% CI: 1.04-3.47), but for those aged 70-74 and over 75, it was not statistically significant. In the case of households with 'low' income, the OR was higher at 1.62 times (95% CI: 1.16-2.26), and for 'middle' income, it was significantly higher at 3.21 times (95% CI: 1.08-9.51). Conclusion: This study confirmed that the experience of unmet medical care is high among men who make up single-person households and low-income seniors. Therefore, this study suggests that policies to expand medical services and support welfare for single-person households should be established to resolve these problems, showing that health policies that take into account individual and regional characteristics are needed to improve medical accessibility for single-person households.

  • Anatomic and physiological changes of the larynx with advancing age result in morphologic changes of the vocal fold and reduced control of the phonatory mechanism in elderly individuals and are reflected in increased unstability of fundamental frequency (Fo). The purpose of this study is to increase current understanding of acoustic and stroboscopic characteristics of normal elderly persons voices. First, phonated /a/ vowel productions by 40 normal adults (20 to 40 years, 20 men and 20 women) and 40 normal elderly persons (60 to 80 years,20 men and 20 women) were analyzed, using CSL (model 4300B) acoustic analysis software, to obtain acoustic measures related to fundamental frequency stability nd vocal resonance characteristics. Second, stroboscopic images of the vocal fold behavior in all subjects were analyzed by experienced specialists. In the men, fundamental frequency variation (vFe) (p<0.01), jitter. (p<0.05), and shimmer (p<0.05) for the older group were significantly higher than the value for the adult group. In the stroboscopic findings, edema of vocal fold had a significant finding in aged men (15%). In the women, vFo (p<0.05), jitter (p<0.05), and noise to harmonic ratio (NHR) (p<0.05) for the older group were significantly higher than the value for e adult group and first formant frequency (F1) (p<0.01) and second formant frequency (F2) (p<0.01) for. the older group were significantly lower than the value for the adult group. In the stroboscopic findings, vocal fold atrophy had a significant finding in aged women (25%). Frequency stability, as reflected by vFo, jitter, shimmer, and NHR, decreases with advancing age in men and women and spectral analysis of phonated /a/ vowel productions reveals the lowering of the frequency of F1 and second F2 with advancing age, especially in aged women. Change in the mass of vocal folds, due to atrophy or edema, is considered to be the greatest factor in these acoustic changes.

  • Objectives : We examined the association between alcohol consumption and incidence of colorectal cancer in elderly Koreans. Methods : The cohort members (n=14,304) consisted of 4,834 males and 9,470 females derived from the Korea Elderly Pharmacoepidemiologic Cohort (KEPEC), a population-based dynamic cohort. They were aged 65 years old or older and lived in Busan between 1993-1998; they were beneficiaries of the Korean Medical Insurance Corporation (KMIC). Baseline information was surveyed by a self-administered, mailed questionnaire. This study population was restricted to 14,304 participants who reported alcohol drinking habits on the questionnaire and had not been diagnosed with colorectal cancer at baseline. The adjusted hazard ratios (aHR) of status, type, frequency and daily average amount of alcohol consumption were computed with Cox's proportional hazard model, with the never-drinkers as a reference group and controlling for age and gender. Results : After 4.82 person-years of mean follow-up 112 cases of colorectal cancer occurred. The incidence densities of colorectal cancer were 161 (95% CI=123-200) for never-drinkers, 219 (95% CI=125-339) for ex-drinkers, and 137 (95% CI=84-189) for current-drinkers per 100,000 person-year. The status, type, frequency, and daily average amount of alcohol consumption were not significantly related to the incidence of colorectal cancer after controlling for age and gender. Conclusions : There was no significant association between alcohol consumption and colorectal cancer among elderly people after controlling for age and gender.
