아세트아닐리드의 초기 인체위해성 평가 (Initial Risk Assessment of Acetanilide with Respect to Human Health)

  • 아세트아닐리드는 그의 생산 및 이용 공장에서 환경으로 방출된 다음 인체에 노출될 수 있다. 아세트아닐리드는 진통효과클 나타내는 것으로 알려져 과도 노출시에는 건강에 부작용을 초래할 수 있다. EUSES시스템에 의하면 아세트아닐리드는 지역노출의 경우 6$\times$$10^4$을 초과하는 높은 MOS 값(안전성 마진)을 보여주어 공중보건상 충분히 안전한 것으로 나타났다 국지수준(작업장)에서 경피노출에 의한 MOS최저 값은 3$\times$$10^{-4}$으로 추정되었지만 작업장에서 개인장비나 환기와 같은 예방조치에 의하여 그 위험을 부분적으로 경감시킬 수 있다. 아세트아닐리드는 분진 흡입하는 작업자에게 위험 가능성이 나타날 수 있다. 작업장에서 건강보호를 위해서는 산업보건 측면에서의 안전함이 증명될 수 있도록 반복투여독성, 생식독성 및 발육독성에 관한 자료가 보완되어야 할 것이며, 따라서 이에 대한 실험이 진행되어야 할 것으로 사료된다.

소규모 다중이용시설의 실내공기질 실태조사 및 건강위해성평가: 민감군 이용시설을 중심으로 (Indoor Air Quality and Human Health Risk Assessment for Un-regulated Small-sized Sensitive Population Facilities)

  • Objectives: The purpose of this study is to investigate human health risk assessment of indoor air pollutants at small-sized public-use facilities (e.g., daycare centers, hospital and elderly care facilities) that the susceptible population is mainly used. Methods: To assess indoor air quality (IAQ), the concentrations of indoor air contaminants such as HCHO, benzene, toluene, ethylbenzene, xylene, styrene, PM-10, CO, $NO_2$ and $O_3$ in air samples were measured according to the Indoor Air Quality Standard Method. By conducting the questionnaire survey, the major factors influencing IAQ were identified. Human health risk assessment was carried out in the consideration of type of use (user and worker) at 75 daycare centers, 34 hospitals and 40 elderly care facilities. Results: As a result of measurement of indoor air contaminants, the average concentration of HCHO and TVOCs in hospitals was higher than daycare centers and elderly care facilities, about 8.8 and 23.5% of hospitals were exceeded by IAQ standard. In human health risk assessment, for the user of daycare centers and elderly care facilities, the mean carcinogenic risk of HCHO inhalation was higher than acceptable value. Except for HCHO, other values were determined under acceptable risk. Similarly, for the worker of hospitals, the mean carcinogenic risk of HCHO inhalation was higher than acceptable value and other values were evaluated under acceptable risk. In contrast, the risk levels of other contaminants measured in elderly care facilities were acceptable. In the determination of factors influencing IAQ, the construction year, building type, ventilation time, and the use of air cleaner were identified. Conclusions: This study provides the information for establishing the plans of public health management of IAQ at small-sized public-use facilities that have not yet been placed under the regulation. The findings suggest the consideration of human health risk assessment results for the IAQ standards.

흰쥐에 있어서 사염화탄소에 의한 간 손상이 toluene 대사에 미치는 영향 (Effect of CCl4-induced liver damage on the metabolism of toluene in rats)

  • 흰쥐에 있어서 사염화탄소에 의한 간손상이 toluene 대사에 어떠한 영향을 미치는지를 알아보기 위하여 흰쥐에 $CCl_4$를 투여한 후 간기능 검사와 병리 조직 검사를 통하여 간손상이 확인되었다. 이같은 간손상 모델 실험동물에 toluene을 투여한 뒤 24시간 후 처치한 다음 요 중 hippuric acid 함량을 측정한 결과, 간손상이 유도된 후 흰쥐에 toluene을 투여한 실험군에 있어서 요 중 hippuric acid 함량이 toluene만 투여한 군 보다 현저히 감소되었다. 이때 toluene 대사에 관여하는 간조직의 cytochrome P-450에 상응하는 aniline hydroxylase 및 aminopyrine demethylase 활성과 benzylalcoh이 및 benzaldehyde dehydrogenase 활성 역시 간손상이 유도된 흰쥐에 있어서 유의하게 감소되었다. 또한 benzaldehyde dehydrogenase 활성의 kinetics를 관찰한 결과, 간손상이 유도된 실험동물 및 대조군 모두 유사한 $K_m$치를 보였으나 $V_{max}$치는 간손상이 유도된 실험동물에서 낮게 나타났다. 이상 실험결과를 종합하여 볼 때 $CCl_4$에 의한 간손상시 toluene 대사가 억제되며 이는 toluene 대사에 관여하는 효소단백합성 억제에 기인된 결과로 사료된다.

방사선사 핵심 직무역량 모델의 적합성 및 타당성 검증 (A Goodness of Fit and Validity Study of the Korean Radiological Technologists' Core Job Com petency Model)

  • 방사선사는 사람의 생명을 대상으로 하므로 전문적인 직무역량(job competency)이 요구된다. 그럼에도 현재 한국에서는 방사선사의 직무역량을 밝히고 있는 연구가 이루어진 바 없다. 이에 한국 방사선사의 핵심 직무역량 지표를 정의하고 역량군별 요인모델을 제시하고자 방사선사 직무역량에 대한 설문을 실시하여 최종 147명의 응답내용을 취합하였다. 응답 내용은 PASW Statistics Version 18.0와 AMOS Version 18.0을 이용하여 탐색적 요인분석과 확인적 요인분석을 실시하였다. 요인분석 결과 전문직업성', '환자관리', '건강 및 안전', '장비의 운영', '절차와 관리'의 5개 핵심 직무역량군과 17개의 하위 역량으로 구성된 모델을 구성하였다. 5개 핵심 직무역량군의 측정 모델 RMSEA 값은 0.1, CFI와 TLI 값이 0.9에 근사하게 나타나 양호한 적합도를 보였으며, 타당도 분석 결과 평균분산추출 값 0.5이상, 개념신뢰도 값 0.7이상으로 각 하위 역량에 포함된 세부 항목 간에 상관관계도 높게 나타났다. 이 연구의 결과는 한국의 의료 환경에서 방사선사에게 요구되는 직무역량을 명확히 제시하여 역량 중심의 인적 자원 양성과 관리에 필요한 구체적인 정보를 제공해 줄 수 있을 것으로 기대된다.

중국의 지리교육과정 변천과 지리과정표준의 구성체계 (Chinese Geography Curriculum Changes and the Organization of Geography Curriculum Standards)

  • 지리는 동아시아 삼국의 상호이해를 증진하는데 필요하고 충분한 조건을 갖춘 도구 교과이지만, 중국의 지리교육에 대한 국내 연구는 매우 부진하다. 이에 중국의 지리교육과정 변천 과정과 최근의 교육과정인 '지리과정표준'의 구성체계와 그 특징을 살펴보았다. 연구 결과는 다음과 같다. 첫째, 중국의 지리교육과정은 정치적 이념과 사회적 변화의 영향을 크게 받았으며, 번창${\rightarrow}$위축${\rightarrow}$진흥${\rightarrow}$발전의 과정으로 발달하였다. 둘째, 최근의 지리과정표준은 종합성과 실용성을 추구하는 내용표준 중심의 개혁적인 교육과정으로, 학생 중심 그리고 지리적 지식과 기능의 유기적 연계를 추구하는 활동 중심으로의 변화가 가장 큰 특징이다. 셋째, 지리과정표준은 크게 서언, 과정목표, 내용표준, 실시건의의 4부분으로 구성되어 있다. 넷째, 초급중학교 지리교육과정에서는 지구와 지도, 세계지리, 중국지리, 향토지리 4영역의 기초적인 지리 지식과 기본적인 지리 기능을 강조한다. 다섯째, 고급중학교 지리교육과정은 3개의 필수과목과 7개의 선택과목으로 구성되어 있으며, 학생들의 장래 직업과 지역 실정을 고려한 지리교육을 강조하고 있다.

일반시민의 119구급대원에 대한 이미지 (119 Rescuers' image of Citizens)

  • 본 연구는 일반시민을 대상으로 효과적인 구급대원의 이미지 향상을 위한 기초자료를 제공하기 위하여 119 구급대원 이미지 분석을 시도한 서술적 조사연구이다. 본 연구는 2011년 11월 9일부터 2011년 12월 9일까지 일반시민 281명을 대상으로 시행하였으며, 수집된 자료는 SPSS PASW statistics 18.0 program으로 분석하였다. 4개의 이미지 하부영역 중 업무적 이미지($3.17{\pm}0.36$)가 가장 높고 사회적 이미지($2.81{\pm}0.48$)가 가장 낮은 것으로 분석되었다. 대상자의 일반적인 특성 중 연령($p$ <.001)과 직업($p$ <.001) 및 응급처치교육이수 여부($p$ <.05), 119구급대 이용 유무($p$ <.05) 등이 119구급대원의 이미지와 통계적 유의성이 있는 것으로 분석되었다. 다중회귀분석결과 회귀모형에 대한 수정된 $R^2$ 값이 0.120으로 회귀모형이 12.0%적합하였으며, 직업(학생, 주부, 회사원)과, 응급처치교육이수자가 119구급대원에 대한 이미지가 높은 것으로 나타났다. 이상의 결과를 토대로 연구대상자의 일반적인 특성(성별, 연령, 직업, 경제적 수준 등)을 통제한 119구급대 이용 유무 와 구급서비스 만족에 따른 이미지를 비교분석할 것을 제언한다.

수은 폭로 근로자들의 생물학적 폭로지표와 호흡량 및 폭로 기간과의 상관성에 관한 연구 (The Study on the Relationships Between Inhalation Volume and Exposure Duration and Biological Indices of Mercury among Workers Exposed to Mercury)

  • This study was conducted to evaluate the relationships between the environmental exposure and biological monitoring among workers exposed to metallic mercury We interviewed each workers to get the medical history including previous hazardous occupational history. We measured the respiration rate and tidal volume of each worker in order to calculate the 8-hour inhaled mercury of workers. And we wafted to evaluate the effect of exposure duration to mercury concentrations in blood and urine as biologic exposure indices of metallic mercury. The regression and correlation analysis were done to the relationships of 8-hour inhaled mercury and mercury in blood and urine. The results were as follows; 1. The subjects were 35 fluorescent lamp manufacturing workers. The mean age of subjects was .24.8 years old, and the mean work careers of workers was 1.19 years. 89% of the total was consisted man. 2. The correlation coefficients between 8-hour inhaled mercury and mercury in blood and urine were higher than that of only considered air mercury concentration. 3. The correlation coefficients of 8-hour inhaled mercury and mercury in blood and urine were above 0.9 in workers who had exposed to mercury more than 1 year 4. The R-square value and -value of regression analysis between the 8-hour inhaled mercury and mercury in blood and urine was also higher in workers who had exposed to mercury over 1 year than in workers who had less than 1 year working experience. The important results of this study were that relationships between the 8hr-inhaled mercury and mercury in blood and urine was very high than that with air mercury concentration only. And the results were very apparent when considering workers 1 year or more. Therefore we concluded that the work career and respiratory volume of each individuals should be considered in evaluation the, results of biological monitoring of workers exposed to metallic mercury.

청소년의 스마트폰 사용시간 표준화 연구 : 2017년 청소년건강행태온라인조사 원시자료를 이용하여 (Standardization of Time of Smartphone Use in Adolescents : Using Data from Korea Youth Risk Behavior Web-based Survey of 2017)

  • 본 연구는 청소년의 하루 평균 스마트폰 사용시간에 대한 표준화를 조사하고자 하였다. 자료는 2017년 청소년건강행태온라인조사의 원시자료를 이용하였고, 분석된 인원은 54,603명이었다. 통계분석은 빈도분석, 독립 t 검정, 피어슨 상관분석, 일원분산분석을 사용하였다. 연구결과, 스마트폰 사용시간과 나이, 학년 사이에 상관계수는 매우 낮았다(r=.025~.039). 중학생과 고등학생의 스마트폰 사용량은 유의한 차이가 없었다(p>.05). 중학생에서 학년이 높아질수록 스마트폰 사용량이 유의하게 많았다(p<.05). 고등학생에서 2학년은 1학년과 3학년보다 스마트폰 사용량이 유의하게 많았다(p<.05). 본 연구는 54,603명의 청소년에서 성별(남, 여), 사용기간(주중, 주말), 학교(중학교, 고등학교), 학년(1학년, 2학년, 3학년)에 대한 각각의 하루 평균 스마트폰 사용량(분)을 7분위수로 제시하였다. 스마트폰 사용량의 기준치 제공은 청소년의 신체 및 정신 건강관리에 근거자료로 사용될 수 있다. 추후 연구를 통해 스마트폰 과사용을 증명할 수 있는 기준값 제시가 필요하다.

임플란트 식립 부위의 골밀도 평가에 관한 연구 (The study of bone density assessment on dental implant sites)

  • Introduction: Bone density is one of the important factors for the long term success of endosseous implants. The bone density varies from site to site and from patient to patient. A preoperative evaluation of the bone density is quite useful to oral surgeons for planning dental implantation. More accurate information on the bone density will help surgeons identify suitable implant sites, thereby increase the success rate of dental implantation. This study examined the correlation between the bone density measured preoperatively by computed tomography (CT) and the implant primary stability measured by resonance frequency analysis. Furthermore, the effects of the implant sites, gender, age and generalized systemic disorder patients on the bone density and primary implant stability were examined. Materials and Methods: One hundred and fourteen patients were selected. None of the patients had undergone a tooth extraction or bone graft history in the previous year. Preoperatively, the patients underwent CT scanning to evaluate the Hounsfield unit (HU), and resonance frequency analysis (RFA) was used to evaluate the implant primary stability at the time of implant installation. All implants were 4.0 mm diameter and 11.5 mm length US II. All patients were recorded and the HU and implant stability quotient (ISQ) value were evaluated according to the sites, gender and age. Results: The highest HU values were found in the mandibular anterior site ($827.6{\pm}151.4$), followed by the mandibular molar site ($797{\pm}135.1$), mandibular premolar site ($753.8{\pm}171.2$), maxillary anterior site ($726.3{\pm}154.4$), maxillary premolar site ($656.7{\pm}173.8$) and maxillary molar site ($621.5{\pm}164.9$). The ISQ value was the highest in the mandibular premolar site ($81.5{\pm}2.4$) followed by the mandibular molar site ($80.0{\pm}5.7$), maxillary anterior site ($77.4{\pm}4.1$), mandibular anterior site ($76.4{\pm}11.9$), maxillary premolar site ($74.2{\pm}14.3$) and maxillary molar site ($73.7{\pm}7.4$). The mean HU and ISQ value were similar in females and males. (HU: P=0.331, ISQ: P=0.595) No significant difference was also found in the age group respectively. However, the correlation coefficients between the variables showed a closed correlation between the HU and ISQ value. Conclusion: These results showed close correlation between the bone density (HU) and primary stability value (ISQ) at the time of implant installation (Correlation coefficients=0.497, P<0.01). These results strengthen the hypothesis that it might be possible to predict and quantify the initial implant stability and bone density from a presurgical CT diagnosis.

적혈구(赤血球) Protoporphyrin과 철분대사(鐵分代謝)에 관(關)한 연구(硏究) (A Study on Red Cell Protoporphyrin Concentration and Iron Metabolism)

  • The relative state of human iron storage may be ascertained more reliably through determination of the serum iron, iron binding capacity, transferrin saturation and absorption of radioactive iron in conjunction with studies of red cell morphology than from the study of red cell morphology alone. Recent investigations have shown that there is an increase in red cell protoporphyrin concentration in iron deficiency anemia. The significance of the red cell protoporphyrin has been discussed greatly during the years since its discovery. Two of the main factors which appear to influence the amaunt of protoporphyrin are increased erythropoiesis and factors interfering with the utilization of iron in the synthesis of hemoglobin, and iron deficiency. Recently Heller et al. have described a simplified method for blood protoporphyrin assay and this technique could be used assess nutritional iron status, wherein even minor insufficiencies are detectable as increased protoporphyrin concentrations. Based on the evaluation of the relationship between nutritional iron status and red cell protoporphyrin as an index suitable for the detection of the iron deficiency is described in this paper. RESULTS 1. Hemoglobin Concentrations and Anthropometric Measurements. The mean and standard deviations of the various anthropometric measurements of different age and sex groups are shown in table 1. There measurements have been compared with the Korean Standard. In the absence of local standards for arm circumference and skin-fold thickness over triceps, they have been compared with the standard from Jelliffe. Table 2,3, and 4 give anthropometric measurements and frequency (%) of anemia in children surveyed. The mean height of the children studid was 10 to 20 percent; below the Korean Standard. The distribution of height below 80 percent of the Standard was 21.2 percent, however, among anemic group this percentage was 27.7 percent. In general, the mean weight of the children was 10 to 15 percent below the Korean Standard. The percentage of children with weight less than 80 percent of the Standard was about 35 percent. But in the anemic group of the children, this percentage was 44 percent. The mean arm circumference was about 15 percent lower than the Jelliffe's standard. 61.2 percent of the children had values of arm circumference below 80 percent of the standard. Children with low hemoglobin levels, this percentage was 80 percent. The mean skinfold thickness over the triceps of the children studied was about 25 Percent lower than the Jelliffe's standard and 61.2 percent of the children had the value less than 80 percent of the standard. Among anemic children, this percentage was 70.8%. As may be seen from table 5, the mean hemoglobin concentration of the total group was 11.3g/100ml. Hemoglobin concentration was less than 11.0g/100ml. in 65(36.5%) of the 178 children. The degree of anemia in most of these children was mild with a hemoglobin level of less than 8.0g/100ml. found in only one child. In general, the prevalence of anemia was high in female children than male and decreased its frequency with increasing age. Relatively close relationship was observed between hemoglobin level and anthrophometric measurements especially high between arm circumference and skinfold thickness and hemoglobin but very low in height and low in weight and hemoglobin level, estimated by chi-square value. II. Serum iron, Transferrin saturation (1) Serum iron, and transferrin saturation Serum iron, transferrin saturation and red cell protoporphyrin concentrations were estimated in sub-sample of 84 children from 1 to 6 years and 24 older children between 7 and 13 years of age. The findings are presented in table 6. The mean serum iron concentration of the total group was 59ug/100ml. However, the level incrased with age from 36.6ug/100ml. (1-3years) to 80.8ug/100ml. (7-13 years). 60 percent of these children had a serum iron level less than 50ug/10ml. in the 1-3 years age group and 31.4 percent for 4-6 years group. These contrast with the finding of 12.5 percent anemic children in the 7-13 years age group. The mean transferrin saturation for the total group was 18.1 percent and frequency of anemia by transferrin saturation was observed same pattern as serum iron concentration. (2) Red cell protoporphyrin concentrations. (a) Red cell protoporphrin levels of children: Red cell protoporphyrin and other biochemical data are shown in table 4. The mean concentration in red cell of all children was fround 46.3ug/100ml. RBC. and differences with age groups were observed; in the age group 1-3 years, the mean concentration was $59.5{\pm}32.14$ ug/100ml. RBC; 4-6 years $44.1{\pm}22.57$ ug/100ml. RBC. and 7-13 years, $39.0{\pm}13.56$ ug/100ml. RBC. (b) Normal protoporphyrin values in adults: It was observed that in 10 normal adult males studied here the level of protoporphyrin in red cell ranged from 18 to 54 ug/100ml. RBC. and the mean concentration was $47.5{\sim}14.47$ ug/100ml. RBC. Other biochemical determination made on the same subjects are presented in table 8. (c) Red tell protoporphyrin concentration of occupational blood donors: The results of analyses for red cell protoporphyrin as well as serum iron, transferrin saturation and hemoglobin in the 76 blood donors are presented in table 7 and 8. In this experiment, donors were selected at random, however, most of them bled repeatedly because of poor economic situation, I doubt. Table 9 shows the distribution of red cell protoporphyrin concentration and hemoglobin concentration of occupational donors. The mean hemoglobin value for the total was 11.9 g/100 ml. When iron deficiency anemia is defined as a transferrin saturation below 15%, prevalence of anemia was 47.4 percent and the mean serum iron was 27.1ug/100ml. and red cell protoporphyrin, 168.3ug/100ml. RBC. However, mean serum iron and protoporphyrin concentration of above 15% transferrin saturation were 11.6 ug/100 ml. and 58.8 ug/100 ml. RBC. respectively. The mean Protoporphyrin concentration of non-anemic (above 15% transferrin saturation) donors was slightly higher than the results of normal adult males.
