Objectives: This study investigated the pattern identification (PI) and clinical index of Parkinson's disease (PD) for personalized diagnosis and treatment. Methods: This prospective observational multi-center study recruited 100 patients diagnosed with PD from two Korean medicine hospitals. To cluster new subtypes of PD, items on a PI questionnaire (heat and cold, deficiency and excess, visceral PI) were evaluated along with pulse and tongue analysis. Gait analysis was performed and blood and feces molecular signature changes were assessed to explore biomarkers for new subtypes. In addition, unified PD rating scale II and III scores and the European quality of life 5-dimension questionnaire were assessed. Results: The clinical index obtained in this study analyzed the frequency statistics and hierarchical clustering analysis to classify new subtypes based on PI. Moreover, the biomarkers and current status of herbal medicine treatment were analyzed using the new subtypes. The results provide comprehensive data to investigate new subtypes and subtype-based biomarkers for the personalized diagnosis and treatment of PD patients. Ethical approval was obtained from the medical ethics committees of the two Korean medicine hospitals. All amendments to the research protocol were submitted and approved. Conclusions: An objective and standardized diagnostic tool is needed for the personalized treatment of PD by traditional Korean medicine. Therefore, we developed a clinical index as the basis for the PI clinical evaluation of PD. It is important to understand how marbling traits and tenderness differ among beef steaks from the carcass grading site and other regions within the longissimus thoracis (LT) muscle, as these characteristics are closely associated with consumer acceptability and willingness to purchase. Thus, the aim of this study was to compare the marbling fleck traits and objective tenderness parameters in the groups classified by the coarseness index (CI) of marbling fleck (high and low groups) at the carcass grading site ($13^{th}$ thoracic vertebra; 13T) and three different locations (13T, 9T, and 6T) within the LT muscle from well-marbled (marbling score 7 to 9) Hanwoo steer. Image analysis showed that the longitudinal locations had a significant effect on marbling fleck traits. The total area of large marbling fleck divided by the total marbling area (coarseness) was higher at the central region (13T to 12T) compared to the front thoracic region (6T to 5T) in the high CI group (0.23 vs. 0.17, p<0.05), whereas no significant differences were observed in the total number of marbling fleck within the LT muscle in the high or low CI groups (p>0.05). Location effect on objective tenderness parameters within the LT muscle was somewhat limited, although the high CI group had a lower Warner-Bratzler shear force (WBS) value than did the low group (p<0.05). Taken together, the degree of coarseness of marbling fleck decreased from the carcass grading site to the front thoracic site, whereas the objective tenderness parameters, including WBS and hardness, of the grading site did not differ from the other regions within the LT muscle.
The purpose of this study was to examine the differences in satisfaction of leisure life and quality of life using standardized evaluation tools developed by WHO. Resource availability when dining out was measured by objective index(time of dining out, dining out expense) and subjective index(perceived satisfaction with time of dining out and dining out expense). In this study, 500 questionnaires(male & female adults) located in Seoul and the metropolitan area were sampled by convenience sampling and gather in 433 sheets of questionnaire. Data was analyzed by frequency analysis, factor analysis, reliability analysis, correlation analysis and regression analysis using SPSS Win 12.0. As a result there was high positive correlation between time of dining out, dining out expense and perceived satisfaction with time of dining out and dining out expense. Moreover, dining out resources impact on leisure life satisfaction and quality of life. Leisure life satisfaction had fairly high effects on quality of life. Since there is an increase in leisure time the desire to improve quality of life while enjoying dining out as a part of leisure life is increasing and as various forms of consumption takes place, it suggests that segmentation of dining out market is necessary and marketing strategy should be established.
목적 마인드맵을 활용한 회상치료가 여성 치매 의심 노인의 인지기능과 우울 지수 및 삶의 질에 미치는 영향을 알아보고자 하였다. 연구방법 본 연구는 무작위 대조군 전 후 실험설계를 사용하였다. 연구 대상자는 여성 치매 의심 노인으로 실험군 7명, 대조군 7명으로 이루어져 있다. 모든 대상자는 중재 기간 동안 동일하게 20분간 그리기 활동에 참여하였고, 실험군은 추가로 마인드맵을 활용한 회상치료를 적용하였다. 마인드맵을 활용한 회상 치료는 5주간, 주 2회, 1회당 40분씩 총 10회기로 구성하였다. 중재 효과를 검증하기 위해 사용된 평가 도구로 인지기능은 간이 정신상태 검사-한국판(Mini-Mental State Examination Korean Version ; MMSE-K), 우울 지수는 벡 우울증 척도 한국판(Korean Version of Beck Depression Inventory ; K-BDI), 삶의 질은 한국판 세계보건기구 삶의 질 간편형 척도(Korean Version of the World Health Organization Quality of Life Scale Abbreviated Version ; WHOQOL-BREF)를 사용하였다. 결과 분석은 기술 통계와 맨-휘트니 검정(Mann-Whitney U test), 윌콕슨 부호 순위 검정(Wilcoxon signed rank test)을 시행하였다. 연구결과 중재 후 집단 내에서 인지기능, 우울 지수, 삶의 질에 유의한 차이가 있었고, 집단 간 에서는 인지 기능에 유의한 차이가 있었다. 결론 마인드맵을 활용한 회상치료가 여성 치매 의심 노인의 인지기능과 우울 지수 및 삶의 질에 긍정적인 효과가 있음을 확인하였다.
Objective : In period of convalescence and aftereffect, facial palsy patients suffer from social and psychological problems, besides experiencing physical inconvenience. So Quality of life is important Evaluation in treatment or facial palsy. Nevertheless the aims of recent study were only trying to explain about objective symptoms. Therefore, Oriental-Western Medicine was performed, effectiveness of treatment were measured in Quality of life. Methods : Acute facial palsy patients who visiting whin 5days completed questionnaire about Quality of life, if he(or she) participated voluntarily. Questionnaire are comprised of general characteristics, Facial Disability Index(FDI), WHOQOL-BREF, VAS and House-Brackmann grade. Questionnaire used two times, the first medical examination and 4weeks later after starting Oriental-Western Medicine. The statistical analysis was performed by GraphPad Prism 4.0. T-test was used to verify effectiveness between the two groups. Results : 1. When we compared the first medical examination with 4weeks later, score of FDI-Physical function and FDI-Social/Well-bieng function increased but they were not valid statistically. 2. When we compared the first medical examination with 4weeks later, in WHOQOL-Brefoverall domain and physical domain, score increased. In WHOQOL-Bref-psychological, Social, Environment domain, score decreased. but, they were not valid statistically. 3. VAS, House-Brackmann grade decreased, but, they were not valid statistically. Conclusion : The number of subjects with facial palsy in our study(N=5) was too small, and the period of study(4 weeks) was short, too. For this reason, our data were not valid statistically. But Facial palsy Patient's Quality of life has risen.
Purpose: The purpose of this retrospective study is to investigate the degree of coincidence between the peer assessment rating (PAR) index and American Board of Orthodontics objective grading system (ABO-OGS) in the assessment of orthodontic treatment outcomes of Class I malocclusion cases. Materials and Methods: The sample consisted of 26 Class I patients. The PAR index was used for evaluation of pre-(T0) and posttreatment (T1) casts, and the ABO-OGS for assessment of T1 casts. If there was a reduction in PAR scores from T0 to T1 of more than 30%, the label 'PAR+' was given to the case, and if not, it was labeled 'PAR-'. If the ABO-OGS was less than 27, the label 'OGS+' was given to the case and if not, it was labeled 'OGS-'. 'A PAR-only qualified group' (PAR+), 'ABO-OGS-only qualified group' (OGS+), 'both indices qualified group' (PAR+/OGS+), and 'both indices disqualified group' (PAR-/OGS-) were compared with a Wilcoxon rank-sum test, sensitivity/specifi city test and Spearman's correlation test. Result: PAR scores for T0, T1, and percentage reduction were 21.1, 6.4, and 65.9%, respectively, and 35.4 for ABOOGS. The distribution of the 'PAR+/OGS+', 'PAR+', and 'PAR-/OGS-' group was 19.3%, 76.9%, and 3.8%, respectively. The T0-PAR, T1-PAR and PAR point reductions for the 'PAR+' group were significantly higher than those of 'PAR+/OGS+' groups (23.1 vs. 15.6; 6.7 vs. 4.6; and 16.5 vs. 11.0; all P<0.05). However, the PAR-percentage reduction and treatment duration between the two groups were not statistically different (70.0% vs. 67.0%, P=0.4325; 24.1 months vs. 25.0 months, P=0.4057). The T1-ABO-OGS score for 'PAR+' group was significantly higher than that of the 'PAR+/OGS+' groups (38.2 vs. 24.0, P<0.001). Conclusion: Since the fraction of the 'PAR+/OGS+' group was less than 20% and there was no significant correlation between PAR-percentage reduction and T1-ABO-OGS, development of a new index system for the accurate evaluation of treatment outcome is needed.

Objective : It has been reported in recent studies that 50-80% of patients with cervical disc hernia have concomitant sleep disorders. The aim of this study was to evaluate the quality of sleep before and after surgical treatment in patients with cervical disc hernia and to assess the effects on treatment. Methods : The study included 32 patients performed discectomy and fusion with an intervertebral cage through the right anterior cervical approach. Oswestry Disability Index (ODI), Visual Analog Scale (VAS) and Pittsburgh Sleep Quality Index (PSQI) were applied to all patients preoperatively and at one month postoperatively. Results : The postoperative PSQI total points and all the PSQI subscale points, the ODI and VAS scores were significantly reduced compared to the preoperative values. A positive correlation was determined between the preoperative ODI points and the PSQI total points and sleep duration, sleep latency and daytime functional loss subscale points. A positive correlation was also found between preoperative ODI points and VAS points. A positive correlation was determined between the preoperative VAS points and and the PSQI total points and sleep duration, and sleep latency subscale points. A negative correlation was determined between the postoperative ODI and the daytime functional loss subscale points. Conclusion : The results of the study showed that in patients with cervical disc hernia, sleep quality and daytime functionality were negatively affected by severity of pain that limited daily activities. Bringing the pain under control with surgical treatment was observed to increase sleep quality. It can be concluded that when planning treatment for these patients, it should be taken into consideration that there could be a sleep disorder in addition to the complaints and symptoms such as pain, hypoestesia and loss of strength.

Objective: Irregular patterns of marital cohabitation are a common problem in upper Egypt due to employment conditions. The objective of this study was to investigate the effect of irregular marital cohabitation on the quality of life and sexual function of infertile men. Methods: In total, 208 infertile men were included and divided into two groups. The first group included 134 infertile men with an irregular pattern of marital cohabitation and the second group included 74 infertile men with a regular pattern of marital cohabitation. All subjects were assessed through a clinical evaluation, conventional semen analysis, the fertility quality of life (FertiQoL) questionnaire, the International Index of Erectile Function (IIEF-5) score, and the premature ejaculation diagnostic tool (PMEDT). Results: The two groups were compared in terms of conventional semen parameters, FertiQoL, IIEF-5 score, and PMEDT. Infertile men with an irregular pattern of marital cohabitation had significantly lower subscale and total FertiQoL and IIEF-5 scores. Additionally, they had significantly higher PMEDT scores. Erectile dysfunction and premature ejaculation were more common in them than in infertile men with a regular pattern of marital cohabitation. Conclusion: Irregular patterns of marital cohabitation had an adverse effect on quality of life and sexual function in infertile men.
Objectives The purpose of this study is to research current trends of acupuncture treatment of lumbar herniated intervertebral disc using the PubMed database. Methods We set up the search strategy and investigated clinical trials on acupuncture treatment of lumbar herniated intervertebral disc through PubMed search. This study analyzed previous researched papers published from January 1st, 2000 to April 30th, 2014, and classified them by publication year, journal names, types of literature, treatment methods and evaluation scales. To assess the quality of the reviewed literature, randomized controlled trial (RCT) studies were assessed by Cochrane's risk of bias (ROB) tool and non-RCT studies were assessed by risk of bias for non-randomized studies (RoBANS). Results We found 35 studies on the acupuncture treatment of lumbar herniated intervertebral disc. Papers on this topic have been published, on average, three to four times annually in 9 journals since the mid-2000's. The journal with the largest number of publications was Chinese Acupuncture & Moxibustion, and most of articles were classified as RCT. Acupuncture treatment was performed individually or together with other treatments. The most frequently used pain evaluation index was visual analogue scale (VAS). The index of effective rate was used frequently but there was a lack of objectivity. In regards to the quality of the studies, outcome assessment in RCT showed that random sequence generation, allocation concealment, and the blinding of participants and personnel increase potential of risk of bias. For non-RCT assessment, outcome showed that confounding variable, measurement of intervention were at high risk of bias. Conclusions In order to obtain objective clinical evidence of acupuncture treatment of lumbar herniated intervertebral disc, further clinical studies should be designed to minimize the risk of bias, using STRICTA with larger sample sizes.
Using geographical indication, a type of source identification, can effectively promote local specialty agricultural products of superior quality, by identifying the specific geographic location or origin of the produce. Agricultural products can be registered using the geographical indication by describing the product's relation to its geographical origin including the reputation and quality. However, this indication has no objective standards to qualify goods as agricultural specialty products. The purpose of this study is to suggest basic criteria to define the characteristics and criteria of agricultural specialties based on a quantitative evaluation method. To propose this basic standard, we used the proportion of arable land to denote the major production areas and the location quotient (LQ) index to grasp the extent of the specialty of a product. The results show that the average LQ values of registered agricultural products, particularly apples, pears, and garlic, are 3.26, 8.01, and 2.82, respectively. This indicates that they are more specialized than produce from other areas that have not registered for a geographical indication. Low LQ values were found in some areas with registered rice geographical indications, which are also more focused on their historical reputation as the main rice producing areas. Considering the agricultural specialty of products, the recommendation is that the producing proportion should be over 1% of the national scale and over 10% of the province scale, and the LQ value should be over 2.0. This recommendation is not a requirement, but the criteria can prove to be useful in identifying a higher range of specialized agricultural products.
