Associativity Among OTPF and the International Classification of Function, Disorder and Health : Focusing on the Activity and Participation Centered on Life Care (OTPF와 국제 기능·장애·건강 분류간의 연관성 : 라이프케어 중심의 활동과 참여 관점으로)

  • The purpose of this research was to identify the associativity between OTPF and ICF, present the relevance and difference of theoretical concept of OTPF-3 and ICF regarding of the participation and to identify if ICF can explain the professional area of occupational therapy. For this purpose, 4 experts of occupational therapy have made a comparative analysis of the subclassification codes of OTPF-3's work area subsection and of ICF activity and participation. As a result, connecting the most subclassification codes of OTPF-3's work area subsection and of ICF activity and participation, they demonstrated the conceptual similarity and difference. Through this, it is judged to explain the area of occupational therapy with ICF for non-expert and other experts, but it is considered to have limitation to the communication between the experts of occupational therapy. In order to overcome the difference between OTPF and ICF, it would be necessary to have much more researches of comparing widely OTPF and ICF from the perspective of occupational therapy.

Proposed Application Design for Community-Based Rehabilitation Services Access in Community Care System: Occupation and Activity Based (커뮤니티케어 제도 내 지역사회중심재활 서비스 접근을 위한 애플리케이션 디자인의 제안 : 작업과 활동 중심으로)

  • Chronic diseases have been increasing recently as the average life expectancy of humans has been extended, and this trend has caused problems such as the widespread demand for health and rehabilitation services and rising medical costs. In order to solve this problem, the community-based rehabilitation has been developed and strengthened in Korea and gradually promoted since 2019. It is important to secure access to clients who want to use services to revitalize community-based rehabilitation. So in this study, as part of the community-based rehabilitation, intends to devise smartphone applications designs and develop a prototype to secure access to community-based occupational therapy services based on occupation and activities. For Occupational Therapy Practice Framework (OTPF), International Classification of Functioning, Disability and Health (ICF), and Allen Diagnostic Module-2 (ADM-2) were used to devise and categorize occupation and activity based application content, and link OTPF, ICF, and ADM-2 through prior research analysis and expert meetings. The derived content was visualized through literature review and activity analysis, and was implemented to enable direct playback within the application using the YouTube API, and finally developed a prototype application. The Android Studio 3.5.2 for Windows 64-bit was used to build the application prototype. In further research, converging various digital technologies for user convenience and additionally researching community-based occupational therapy service providers opinions and service user satisfaction will improve accessibility to community-based occupational therapy services for clients who have difficulty occupational performance in the community.

Analysis of time use by occupation domain according to the general characteristics and care need reasons of the Korean elderly with disablilities : Focused on the 2019 Time Use Survey (국내 장애노인의 일반적 특성과 돌봄 필요 사유에 따른 작업영역별 시간사용 분석 : 2019년 생활시간조사 중심으로)

  • In this study, after reclassifying the behavioral classification of '2019 Time Use Survey' according to occupation domain of OTPF-4. Time use of occupation domain was examined according to general characteristics and care need reasons were performed Independent t-test and one-way ANOVA. As a result, according to gender, IADL(P<.001), Leisure(P<.001), Social participation(P<.001) and according to marital status, ADL(P<.01), IADL(P<.001), Play(P<.05), Leisure(P<.001) and according to education level, ADL(P<.05), Education(P<.001), Social participation(P<.01) were differences. According to care need reasons, ADL(P<.01), IADL(P<.001), Rest and Sleep(P<.01), Leisure(P<.001), Social participation(P<.001) showed difference. This study will be used to identify time use by occupation domain and to plan interventions for a balanced and efficient lifestyle of korean disabled elderly.

The Effect of Instrumental Activities of Daily Living Program on General Self-Efficacy, Motivation for Rehabilitation, Social Support in a Patient with Subacute Stroke (도구적 일상생활 프로그램이 회복기 뇌졸중 환자의 자기효능감, 재활 동기, 사회적 지지에 미치는 영향)

  • Purpose : The purpose of this study is to examine the effect of instrumental-activity of daily living program on a patient with subacute stroke in the view of self-efficiency, motivation for rehabilitation, social support. Methods : Eight stroke patients who attended instrumental-activity of daily living program at P hospital in Busan Metropolitan City from march 2018 to January 2019 were recruited. Instrumental-activity of daily living program was offered to 8 stroke patients 1 session (at least 1 hours) a day, 5 times a week, for 4~5 weeks. The instrumental-activity of daily living program was based on occupational therapy practice framework (OTPF) and international classification of functioning, disability and health (ICF). We evaluated self-efficacy, rehabilitation motivation, social support before and after intervention. Self-efficacy was evaluated using the general self-efficacy scale. The collected data was processed using SPSS 20.0 and were analyzed using descriptive statistics, Wilcoxon signed rank test, Pearson's correlation coefficient. Results : There were statistically significant differences in self-efficiency, rehabilitation motivation, social support between before and after instrumental-activity of daily living program (p<.05). Examining the correlation between the self-efficiency, rehabilitation motivation and social support of the stroke patient, there was a correlation between the self-efficiency and social support (p<.05). Conclusion : This study showed that the instrumental-activity of daily living program have positive effects on self-efficacy, rehabilitation motivation, social support. When considering the instrumental-activity of daily living program with improved self-efficacy, rehabilitation motivation, social support of the participants, we suggest that further studies will be needed to examine more extensive instrumental-activity of daily living and rehabilitation to the society with a larger sample size.

Occupational Balance and Time Use Satisfaction of Adolescents With Disabilities (장애 청소년의 작업 균형과 시간 사용 만족감에 관한 연구)

  • Objective : This study used the data of the National Statistical Office to analyze the time use of disabled youth and their self-satisfaction with it by their type of occupation. Methods : Using raw data extracted from Statistics Korea, 84 disabled youth from 10 to 24 years old were re-classified into six occupational areas based on Occupational Therapy Practice Framework (OTPF)-3. The occupational types were classified according to the time use by individuals based on data from Statistics Korea. Finally, according to the type of occupation, self-satisfaction with the use of time was identified. Results : The total amount of time spent by disabled youths was the largest in the rest and sleep categories, followed by daily living, education, leisure, social participation, and play. The analysis of occupational types showed that the groups were turned around (10.71% of participants), high activity group (50.00%), low activity (28.57%), and balanced (10.71%). There were significant differences between types in the first five of the aforementioned categories. There was no significant difference in the lack of time use, but there was a significant difference among their satisfaction with their use of time. The balanced type (21.53%) reported the most satisfaction. inadequacy Conclusion : The purpose of the project was to provide the raw data of intervention activities that can improve the quality of life and health of disabled youth. By analyzing their time use and occupations, deficiencies in time management and their satisfaction with their use of time can be noted.

Application of Occupational Therapy Intervention Process Model: A Case of Child With Sensory Integration Dysfunction (작업치료중재과정모델의 적용: 감각통합기능장애 아동 사례)

  • Objective : Purpose of this study is to discuss benefits and implications of the clinical reasoning process and re-evaluation in the OTIPM by introducing a single case that occupational therapy intervention is provided based on the OTIPM. Methods : The case subject is a boy aged 5 years and 10 month who had diagnosed as attachment disorder and anxiety disorder from a pediatric psychiatrist before. The boy is referred to sensory integration therapy clinic and underwent occupational therapy intervention service twice a week for four month. Therapeutic activities for the intervention were consisted of sensory integration activities for restorative model, care-giver education for educational model, and performance skill training for acquisitional model. Measurements used in the initial evaluation are JSI-R, DDST-2, Social Maturity Test, KPPS-R, and observation-based performance task analysis. For the performance task analysis, performance skill items were constructed based on the Occupational Therapy Process Framework (OTPF), and those were assessed by the evaluation system of Assessment of Motor and Process Skill (AMPS) and Evaluation of Social Participation (ESI). Results : The detail process of implementing of the OTIPM in this study is reported by following four phases; 1) establish client-centered performance context; 2) establish baseline and interpret cause (initial evaluation); 3) intervention planning and implementing; and 4) recognize intervention outcome (reevaluation). Conclusion : In this case, occupational therapist could provide the client an occupation-based intervention within comprehensive performance context based on the OTIPM. Therapist could clearly identify the cause of problematic performance skills and behaviors and so provide effective intervention to improve client's occupational performance. Additionally, it was found that client's satisfaction of the intervention can be raised when the concept of 'who is the client' is expanded based on the OTIPM. From this study, it is proposed that OTIPM may be a model educible 'comprehensive' enhancement of 'specific' occupational engagement, as it considers both improvement of occupational performance and satisfaction.
