Changes of Lower Limb Joints Stiffness with Gait Speed in Knee Osteoarthritis (무릎 골관절염 환자의 보행속도에 따른 하지 관절 강성 변화)

  • Spring-like leg models have been employed to explain various dynamic characteristics in human walking. However, this leg stiffness model has limitations to represent complex motion of actual human gait, especially the behaviors of each lower limb joint. The purpose of this research was to determine changes of total leg stiffness and lower limb joint stiffness with gait speed in knee osteoarthritis. Joint stiffness defined as the ratio of the joint torque change to the angular displacement change. Eight subjects with knee osteoarthritis participated to this study. The subject walked on a 12 m long and 1 m wide walkway with three sets of four different randomly ordered gait speeds, ranging from their self-selected speed to maximum speed. Kinetic and kinematic data were measured using three force plates and an optical marker system, respectively. Joint torques of lower limb joints calculated by a multi-segment inverse dynamics model. Total leg and each lower limb joint had constant stiffness during single support phase. The leg and hip joint stiffness increased with gait speed. The correlation between knee joint angles and torques had significant changed by the degree of severity of knee osteoarthritis.

Effects of Aroma Massage on Pain, Activities of Daily Living and Fatigue in Patients with Knee Osteoarthritis (무릎 골관절염 환자에게 시행한 아로마 마사지가 통증, 일상활동 능력 및 피로에 미치는 효과)

  • Purpose: The effects of aroma massage on pain, activities of daily living, and fatigue were investigated in the patients who have knee osteoarthritis. Method: A quasi-experimental design with non-equivalent control group pretest-posttest measures was used. Twenty one and twenty subjects were included in control and experimental group, respectively. Subjects in experimental group had aroma massage which used lavender, chamomile, and ginger oil on painful knee. They were encouraged to implement aroma massage at least two times a day for 2 weeks. Subjects in the control group had conventional oil massage implementing by exactly same method as did in the experimental group. GRS(graphic rating scale), Korean version of WOMAC (Western Ontario and McMaster) osteoarthritis index, and MAF(multidimensional assessment of fatigue) were used to measure the outcome variables such as pain, activities of daily living and fatigue, respectively. Results: After 2 weeks, those in the experimental group reported significantly less pain and fatigue and better activities of daily living than those in the control group. Conclusion: Based on these results, aroma massage could be recommended as a self managed intervention for the patients with knee osteoarthritis.

  • Although arthroscopic surgery has been used conventionally, it has not been widely adopted yet due to the risks of complications, including nerve damage, technical difficulties, and limited indications. As shown in other joints, however, the use of an arthroscope will gradually increased in the elbow joint ('Arthroscopy always wins'). Herein, arthroscopic treatments and arthroscopic ulnar nerve decompression will be discussed in cases of elbow osteoarthritis.

  • Osteoarthritis is one of the most common degenerative disease in the temporomandibular joints(TMJ). Structural changes in the osseous structure is associated with destructive changes such as erosion, flattening and other bony changes. Destructive degenerative changes quite often cause shortening of the condyles which eventually produces the changes in the occulsion and the facial profile which require orthorgnathic surgery and/or orthodontic treatment by the dental professional. The dentists need to understand the nature and the pathophysiology of the osteoarthritis in the TMJ for the better dental treatment, especially in orthodontic and prosthodontic treatment. The possibility of serious complication can not be avoided after any dental treatment is given to the patient if osteoarthritic changes in the TMJ is under progression as undiscovered.

Factors Affecting Depression in Women Patients with Osteoarthritis (골관절염 여성 환자의 우울에 영향을 미치는 요인)

  • Purpose: This study was conducted to determine factors affecting depression in women patients with osteoarthritis. Methods: Raw data from the 6th Korea National Health and Nutrition Examination Survey in 2014 was used. Data for 480 women were included. Variables related to general, physical and psychological characteristics were selected. After a sampling plan compound file was created using IBM SPSS 23.0 program, data were analyzed by giving weights. Results: The results of the study are as follows. 1) Depression was significantly different according to economic status, marital status, knee pain, stress and subjective health status (p<.05). 2) Factors affecting the women included marital status, knee pain, stress, and subjective health status. The explanatory power was 41.0%. Conclusion: Results indicate that there is a need to give comprehensive consideration to the factors identified in this study when designing preventive interventions and management plans to alleviate depression in women with osteoarthritis.

  • Primary osteoarthritis of the ankle without history of trauma is rare. We are reporting the use of a low tibial osteotomy on 2 ankles in one patient. The osteotomy is designed to correct the varus tilt and anterior opening of the distal tibial joint surface where it has been shown by weight-bearing radiographs. Follow-up at 8 months the result showed to be excellent, by Takakura's scoring system. We experienced that slight overcorrection of deformity by low tibial osteotomy is effective in treating intermediate-stage primary osteoarthritis of the ankle.

Thread-Embedding Therapy for Knee Osteoarthritis

  • The aim of this study was to investigate the efficacy of thread-embedding therapy for the treatment of knee osteoarthritis. There were 20 patients treated with thread-embedding therapy at various acupoints on the muscles around the knee. Gender, age, location, morbidity period, numeric rating scale (NRS), Western Ontario and McMaster Universities (WOMAC) index, improvement result, side effects, and patient's satisfaction were investigated. After the treatment, the NRS score (z = -4.07, p < 0.001) and WOMAC (p < 0.001) indices decreased in most patients. The NRS score decreased by more than 2 points in 95% of the patients. The WOMAC index decreased by 6-12 points. There were no serious side effects, although bruising, pain, and edema were observed. Overall, 85% of the patients felt satisfied with the thread-embedding therapy. These findings suggested that thread-embedding therapy was effective and may be used widely for knee osteoarthritis.

  • Anatomical total shoulder arthroplasty (TSA) has been used widely in treatment of glenohumeral osteoarthritis and provides excellent pain relief and functional results. Reverse total shoulder arthroplasty (RSA) was created to treat the complex problem of rotator cuff tear arthropathy. RSA also has been performed for glenohumeral osteoarthritis even in cases where the rotator cuff is preserved and has shown good results comparable with TSA. The indications for RSA are expanding to include tumors of the proximal humerus, revision of hemiarthroplasty to RSA, and revision of failed TSA to RSA. The purposes of this article were to describe comprehensively the conditions under which RSA should be considered in glenohumeral osteoarthritis, to explain its theoretical background, and to review the literature.

Effects of the Progressive Exercise Program on Flexibility, Grip Strength, Pain, and Fatigue for Osteoarthritis Patients (점진적 운동강화 자조관리프로그램이 골관절염 환자의 유연성과 악력, 통증 및 피로에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of a progressive exercise program for osteoarthritis patients. Methods: This study was performed from 24th March. to 7th July. 2009. Thirty six osteoarthritis patients were participated in the progressive exercise program. Outcome measures were shoulder flexibility, knee flexibility, grip strength, pain intensity, number of pain site and fatigue. SPSS/WIN Version 14.0 was used for the data analysis. Results: At the completion of 6 weeks of progressive exercise program, there were statistically significant differences in shoulder flexibility and knee flexibility. But no significant differences were found in grip strength, pain, number of pain site and fatigue between pretest and posttest. Conclusion: Progressive exercise program was proved to be an effective nursing intervention for improving the shoulder and knee flexibility. However, progressive exercise program did not increase pain & fatigue.
