일 대학병원 신생아집중치료실 입원 미숙아의 수면/활동 양상 (The Sleep/Activity Patterns of Premature Babies in the Neonatal Intensive Care Unit)

  • Purpose: The purpose of this study was to investigate the sleep/activity patterns of premature babies. Method: The subjects were 55 premature babies who were hospitalized in the Neonatal Intensive Care Unit (NICU) of one University Hospital in Daejeon from September 1, 2004 to March 31, 2006. The sleep/activity patterns of the premature babies were observed for 24 hours a day for 7 days. The observations were made and recorded by the researcher and 5 trained research assistants using NCASA records developed by Barnard et al. (1979). For statistical analysis, frequency, average, t-test, and Pearson correlation coefficients were used. Results: The infants slept more and had less awake periods during daytime than full-term infants. As the hospital stay of the premature babies increased, the amount of daytime sleep and total daily sleep decreased, and the amount of daytime awake periods increased. Conclusions: The findings suggest that the level of physical and physiological maturity of premature babies is one of the major variables that affect their sleep/activity patterns. Furthermore, the findings of this study will provide information about the sleep/activity patterns of premature babies, and present baseline data to develop the nursing intervention programs which support optimum development for the premature babies.

만성통증을 지닌 노인의 자기효능과 삶의 질에 관한 연구 (A Study on Self-Efficacy and Quality of Life in the Elderly Patients with Chronic Pain)

  • The subjects of this study were 100 elderly people who were over 60 years of age and had chronic pain. The study examined the matter of quality of life and self-efficacy of elderly patients with chronic pain including the influence of related factors on these variables. Also, by examining their relation, it provides basic data for the development of efficient nursing interventions for the elderly patients with chronic pain. The tools were the general self-efficacy scale by Sherer & Maddux(1982) and a specific self-efficacy Scale(Lorig et al, 1989) modified to better suit the subjects of this study. The quality of life scale by You-Ja Ro(1988) modified to better suit the subjects of this study was used. The data were collected from August to October 1999. Four researchers and two research assistants developed a data collection protocol subjected to a pilot study for confirmation of validity. In this study, the research assistants read the questionaire to the subjects and recorded the responses themselves. The interviews lasted 40-50 minutes on an average. The data thus collected were analyzed in terms of t-test, ANOVA, Pearson's correlation coefficient, using the SAS PC program. The major findings are as follows: 1) For self-efficacy it was found that males had higher self-efficacy than females for specific self-efficacy. But for general efficacy, males and females showed similar results. The the quality of life was found to be higher for males than females. 2) A positive correlation was identified between the specific self-efficacy, general self-efficacy and quality of life. 3) The general characteristics affecting the self-efficacy of elderly patients with chronic pain were employment, age, level of education, spouse, economic status and income. Self-efficacy was higher among those who were employed, aged between 66-70, well-educated, married and rich or having their own income. 4) The general characteristics affecting the quality of life of elderly patients with chronic pain were lodging with children, religion, level of education and economic status. The quality of life was higher among those who lived with their children and were Buddhists, were well-educated and rich.

연하장애를 가진 노인에 대한 노인요양시설 간호사의 경험: 포커스 그룹 연구 (Nurses' Perception and Experiences at Nursing Home Residents with Dysphagia: Focus Group Interviews)

  • Purpose: This study aimed to describe Korean nursing home nurses' perceptions on dysphagia management and their working experiences of older adults with dysphagia. Methods: Using a purposive sampling design, 23 eligible nurses were interviewed as four focus groups from 4 facilities out of nursing homes. The qualitative data from focus group interviews were analyzed using thematic analysis to classify common themes into larger categories. Results: Two main themes on the attributes of caring nursing home residents with dysphagia were 'Need for nursing resources' and 'Need for support'. In addition, four subthemes emerged as a result of analysis: 'need for nursing protocols for assessing and managing dysphagia', 'need for emergency care skills of nurses and nurse assistants', 'need for prompt and accurate management by cooperation of physicians and therapists', and 'need for partnership upon comprehension of visiting family members concerning dysphagia'. Conclusion: Nursing home nurses fully recognized the importance of dysphagia management; however, they experienced substantial barriers due to lack of adequate nursing protocols or partnership with family caregivers, insufficient training for emergency care, and deficient support from medical staff. Development of nursing guidelines tailored to the nursing home context and based on partnership among medical experts and family caregivers is needed.

'간호관리 및 윤리' 교과목의 문제중심학습 패키지 개발 및 평가 (Development and Evaluation of a Problem-based Learning in Nursing Management and Ethics)

  • Purpose: This study was aimed to develop and evaluate of a Problem-based Learning (PBL) in the course of Nursing Management and Ethics. Method: The design of the study was both methodological and one group only pre-post design. The sample included 61 senior students who are currently enrolled in Nursing management and Ethics course in college of nursing. Data regarding PBL evaluation were collected on the critical thinking and clinical reasoning using structured questionnaires during March to June, 2005. Data were analyzed using descriptives and paired t-test. Results: A total of three PBL packages was developed by the two faculty members and two teaching assistants who are majoring in nursing management. PBL packages that had been developed was applied to 61 senior students for three months. Critical thinking and clinical reasoning were measured twice pre and post the application of PBL packages. There were statistically significant differences in the critical thinking and clinical reasoning between the pre and post PBL application. Conclusion: PBL was considered to be effective in understanding the learning concepts in the Nursing Management and Ethics. Further research on the facilitative strategies and development model considering the characteristics of Nursing Management and Ethics course is needed.

표준화환자를 활용한 당뇨식이교육 동영상학습이 간호학생의 임상수행능력과 학습만족도에 미치는 효과 (Development of a Multimedia Learning DM Diet Education Program using Standardized Patients and Analysis of Its Effects on Clinical Competency and Learning Satisfaction for Nursing Students)

  • Purpose: The purpose of this study was to develop a multimedia learning program for patients with diabetes mellitus (DM) diet education using standardized patients and to examine the effects of the program on educational skills, communication skills, DM diet knowledge and learning satisfaction. Methods: The study employed a randomized control posttest non-synchronized design. The participants were 108 third year nursing students (52 experimental group, 56 control group) at K university in Seoul, Korea. The experimental group had regular lectures and the multimedia learning program for DM diet education using standardized patients while the control group had regular lectures only. The DM educational skills were measured by trained research assistants. Results: The students who received the multimedia learning program scored higher for DM diet educational skills, communication skills and DM diet knowledge compared to the control group. Learning satisfaction of the experimental group was higher than the control group, but statistically insignificant. Conclusion: Clinical competency was improved for students receiving the multimedia learning program for DM diet education using standardized patients, but there was no statistically significant effect on learning satisfaction. In the nursing education system there is a need to develop and apply more multimedia materials for education and to use standardized patients effectively.

발반사요법이 항암 화학요법을 받는 유방암환자의 오심, 구토 및 피로감에 미치는 영향 (The Effects of Foot Reflexology on Nausea, Vomiting and Fatigue of Breast Cancer Patients Undergoing Chemotherapy)

  • purpose: The purpose of this study was to identify the effects of foot reflexology on nausea, vomiting and fatigue in breast cancer patients undergoing chemotherapy. Method: The research was a quasi-experimental study using a non-equivalent pre-post design and was conducted from Jan. 26, to Mar. 20, 2004. The subjects consisted of 34 patients with 18 in the experimental group and 16 in control group. A pretest and 2 posttests were conducted to measure nausea, vomiting and fatigue. For the experimental group, foot reflexology, which was consisted of 4 phases for 40 minutes, was given by a researcher and 4 research assistants. The collected data were analyzed by repeated measures ANOVA using the SPSS WIN 10.0 program. Results: There was a statistically significant decrease in nausea, and vomiting in the experimental group compared to the control group over two different times. In addition, there was a statistically significant decrease in fatigue in the experimental group compared to the control group over two different times. Conclusion: Foot reflexology was effective on nausea, vomiting and fatigue in breast cancer patients receiving chemotherapy in this study. Therefore, foot reflexology can be usefully utilized as a nursing intervention in the field of cancer nursing for breast cancer patients receiving chemotherapy.

검사 전 교육과 건강 계약이 백혈병 아동의 척수 검사 순응 정도와 불안에 미치는 효과 (Effect of Preeducation and Health Contract on Adaptability and Anxiety during Spinal Puncture in Children with Leukemia)

  • Purpose: This study was done to verify the effects of preeducation and health contract on anxiety and adaptability of children with leukemia when undergoing a spinal puncture. Methods: Children in experimental group 1 (10) were given preeducation prior to the procedure and children in experimental group 2 (13), a health contract. Children in the control group (16) received the regular hospital preparation. Observation tools were used to evaluate adaptability and anxiety. Heart rate and oxygen saturation was also used as a measure of anxiety. The data were analyzed using the SPSS program. Results: Compared with the control group, experiment groups 1 and 2 showed significant difference in adaptability regarding the number of assistants and test posture. Experiment groups 1 and 2 showed significant difference in voice tone and movement level when compared with the control group. Elevation level in heart rate showed significant differences between the three groups. Conclusions: Preeducation and nursing mediation through health contracts were shown to be effective methods to reduce anxiety and increase adaptability in children with leukemia who were undergoing a spinal tap.

Workplace Violence and Safety Issues in Long-Term Medical Care Facilities: Nurses' Perspectives

  • Background: Workplace violence (WPV) is becoming an issue that needs immediate attention in the United States, especially during this period as more states are adopting the "stand your ground laws to promote worker protection." This study was conducted to investigate how WPV has contributed to an unsafe environment for nurses and nursing assistants who work in long-term medical care facilities. Methods: A structure questionnaire was used to collect data for the study. Three facilities were sampled and 80 nurses and certified nursing assistants participated in the study. Ninety-two percent (n = 74) were female and 8% (n = 6) were male. Approximately 62% were black or African American, approximately 33% were Caucasians, and only 2% were from other ethnicities. Results: We found that 65% of the participants had experienced WPV while 41% believed that management shows little or no concern for their safety. Approximately 23% of respondents believed that reporting supervisor's WPV act is an unsafe action. In addition, 22% of those who reported that they have experienced WPV believed that the work environment is not safe to perform their duties. This significant difference in perception of workplace safety between those who had experienced WPV and those who had not was significant (t = 3.95, df = 158, p < 0.0001). Conclusion: WPV is an epidemic problem that affects all health-care professionals. The findings of this study could help long-term medical care facilities' management identify the areas to focus on mitigating, controlling, and/or eliminating incidents of WPV.

노인의 운동행위변화단계에 따른 변화과정, 의사결정평가, 자아효능감 및 주관적 건강지각 (The Process of Change, Decision Making, Self-efficacy and Perception of Subjective Health by the Stage of Exercise Behavior among Older Adults)

  • Purpose: The purpose of this study was to examine the differences of the process of change, decision making, self-efficacy and perception of subjective health according to the stages of exercise behaviors suggested by transtheoretical models among older adults. Method: The subjects consisted of 291 older adults. Trained research assistants interviewed the older adults using structured questionnaires. ANOVA analyses with post hoc test were used to examine the mean differences for each stage of exercise behavior using SPSS for windows. Result: The subjects were divided into five stages of exercise behavior: 24.4% precontemplation, 12.4% contemplation, 17.9% preparation, 12.4% action, 33.0% maintenance stage. There were significant differences in process of change, decision making, self-efficacy and subjective health perception according to the exercise stage of change. 'Self-liberation' and 'stimulus control' were important strategies for intention of exercise, while 'reinforcing management' for the actual practice of exercise. 'Self-efficacy' was an effective strategy to change older adults from pre-contemplation to preparation stage. Conclusion: For exercise intervention, older adults should be classified into groups according to their stages of exercise behaviors and provided effective individualized intervention depending on their stage.

농촌 골관절염 여성의 질병 경험과 관리 실태조사 (A Survey on Experience and Illness Management of Rural Women with Osteoarthritis)

  • Purpose: This descriptive survey was done to assess how rural women with osteoarthritis perceive their symptoms, what are their treatment preferences, and how they manage their illness. Method: Individual interviews were carried out with 205 women with osteoarthritis living in rural areas. Structured questionnaires were used and the interviewers were 7 trained research assistants. Descriptive statistics were used for data analysis with SPSS win 11.5 program. Results: Of the respondents, 68.8% were overweight and 15.1% were obese, 86.9% reported pain in the knees, low back, and shoulders and the average pain score was 66.06. Those knowing the exact diagnosis accounted for 73.7% of the respondents but only 49.7% reported receiving medical treatments, and 40.2% who received treatment adhered to their prescription. Folk medicines were used by 14.1% of the respondents, but 65.2% among them reported that folk medicines were not effective. Exercise 2.03 times per week was reported by 17.1% of the respondents and 48.6% reported preferring to walk. Conclusion: Women with osteoarthritis in rural areas need a rehabilitative care program focused on exercise. The community health care system in the rural area should deliver optimal health care for this population. The results of this study can be used as a basis for planning intervention programs for women with osteoarthritis in the rural area.
