The Effects of a Health Mentoring Program in Community-dwelling Vulnerable Elderly Individuals with Diabetes (취약계층 당뇨병노인을 위한 건강멘토링프로그램의 효과)

  • Purpose: This study aimed to estimate the effects of a health mentoring program on fasting blood sugar, total cholesterol, triglyceride, physical activity, self care behavior and social support changes among community-dwelling vulnerable elderly individuals with diabetes. Methods: A non-equivalent control group pre-post-test design was used. Participants were 70 community-dwelling vulnerable elderly individuals with diabetes. They were assigned to the experimental (n=30) or comparative (n=30) or control group (n=28). The experimental group participated in the health mentoring program, while the comparative group participated in health education program, the control group did not participate in any program. Data analyses involved a chi-square test, Fisher's exact test, a generalized linear model, and the Bonferroni correction, using SPSS 23.0. Results: Compared to the control group, the experimental and comparative groups showed a significant decrease in fasting blood sugar, total cholesterol, and triglyceride. Compared to the comparative and control groups, the experimental group showed significant improvement in self care behavior. However, there were no statistical differences in physical activity or social support among the three groups. Conclusion: These findings indicate that the health mentoring program is an effective intervention for community-dwelling vulnerable elderly individuals with diabetes. This program can be used as an efficient strategy for diabetes self-management within this population.

Effects of Clinical Practicum Reinforcement Program on Nursing Student's Clinical Competency and Satisfaction in the ICU (중환자실의 현장실무 강화교육 프로그램이 간호학생의 임상수행능력과 임상실습교육 만족도에 미치는 효과)

  • Purpose: To identify the effects of clinical practicum reinforcement program on nursing student's clinical competency and satisfaction in the ICU. Methods: The study utilized a quasi-experimental pretest-posttest research design. The participant was 76 senior nursing students, who were engaged in two week-long clinical practicum in ICU at the C university hospital in a G city, Korea. The clinical practicum reinforcement program was applied to the experimental group (n=39), while the control group (n=37) was involved in the conventional practicum program. The collected data were analyzed using descriptive analysis, $x^2$/Fisher's exact test, and t-test, ANCOVA with SPSS/WIN 18.0 program. Results: After 2 weeks' clinical practicum, the experimental group showed a statistically significant increase in the clinical performance abilities by self evaluation, when compared to the control group. Clinical instructor's evaluation on the student's clinical performance revealed that the experimental group showed higher level in nursing activity than that of the control group (p<.001). However, there was no significant difference between the two groups in terms of the satisfaction on the clinical practicum program except the learning guidance and the evaluation. Conclusion: The clinical practicum reinforcement program is an effective one for improving the student's clinical competency.

Development and application of a couple-centered antenatal education program in Korea

  • Purpose: This study was conducted to develop a couple-centered antenatal education program and to test the program's feasibility. Methods: With a preliminary-experimental study design, 33 pregnant couples who were expecting their first child participated in this study. The program consisted of four sessions (1 hour/session/week) of education and counseling. Data were collected before and after the intervention from September 2018 to April 2019 at a women's hospital in Daejeon, Korea, with demographic data forms, the Edinburgh Postnatal Depression Scale, Perceived Stress Scale, Maternal-Fetal Attachment Scale, Korean Newborn Care Confidence Scale, Wijma Delivery Expectancy/Experience Questionnaire, and Dyadic Adjustment Scale-10. Results: The pregnant women and their husbands were on average 32.30±3.10 and 33.21±6.25 years old, respectively. The mean marriage duration was 2.34±1.63 years, the gestational age was 31.30±2.66 weeks, and 78.8% of the couples had a planned pregnancy. After the program, both the pregnant women and their husbands showed significant improvements in attachment to the fetus and confidence in providing infant care. Prenatal depression, prenatal stress, and fear of childbirth in pregnant women significantly decreased after completing the program. However, the dyadic adjustment score did not change significantly either in the pregnant women or their husbands. Conclusion: A couple-centered antenatal education program seems to be effective for couples adjusting to parenthood, but further studies should explore ways to have a positive impact on couples' relationships.

The Effect of Urinary Elimination Facilitation Program for Stroke Patients During Rehabilitation: Residual Urine, the Number of Urinary Catheterization, Urinary Discomfort, and Urinary Tract Infections (배뇨중재 프로그램이 뇌졸중 재활대상자의 잔뇨량, 도뇨횟수, 배뇨불편감, 요로감염에 미치는 효과)

  • Purpose: The aims of this study were to develop and apply the urinary elimination facilitation program for stroke patients with urinary disorders and evaluate the effectiveness of the program. Methods: This study was conducted using the non-equivalent control group pre- posttest design. The participants included 23 patients in the control group and 22 in the experimental group. Data were collected from Nov. 25, 2013 to Nov. 25, 2014 in acute rehabilitation inpatient wards. For the final analysis, 35 patients' data were utilized 17 in the control group and 18 in the experimental group. The developed urinary elimination facilitation program was continued for 7 days and the program consisted of logging urination, monitoring timely voiding, assisting urinary elimination, and stimulating urination using sound. The data were analyized using the SPSS-win (version 21). Results: The number of urinary catheterization and the urinary discomfort were significantly improved in the experimental group compared to the control group. However residual urine and the occurrence of urinary tract infections were not significantly different between the experimental and control groups. Conclusion: The results of this study indicated that the urinary elimination intervention program for stroke patients with urination disorder could be useful in decreasing the number of urinary catheterization and the urinary discomfort.

The Effects of an Individual Cognitive Improvement Program on the Elderly with Mild Cognitive Impairments (경도인지장애 노인의 개인별 인지기능강화 프로그램적용의 효과)

  • Purpose: This study was conducted to investigate changes in cognitive functions, depression and life-satisfaction for elderly participants with mild cognitive impairments participating in an individual cognitive improvement program. Methods: A quasi-experimental with non-equivalent control group pretest-posttest design was implemented. The participants were senior citizens over 65 years of age who had been clinically diagnosed with mild cognitive impairments in G City and J Province (experimental group: 29, control group: 27). The experimental group participated in the individual cognitive improvement program based on Korean traditional tales and games for nine weeks. Results: After the intervention, only the participants in the experimental group reported significant improvement in K-MMSE (t=-2.5, p=.016), MoCA-K (t=-2.6, p=.008), depression (t=3.51, p=.001), and life satisfaction (t=-2.75, p=.008) when compared to those in the control group. Conclusion: These results indicated that the individual cognitive improvement program was effective in improving cognitive functions, depression, and life-satisfaction among the elderly with mild cognitive impairments. Therefore, the application of this individual cognitive improvement program developed in this study by visiting nurses may strengthen the cognitive functions of seniors with mild cognitive impairments.

Effects of an Education Program on Prevention of Malpractice using Precedent Cases related to Patient Safety in Safety Perception, Safety Control, Autonomy and Accountability in Clinical Nurses (판례를 이용한 환자안전관련 간호과오 예방교육이 간호사의 안전지각, 안전통제감, 자율성 및 책임수용성에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effects of an education program on safety perception, safety control, autonomy and accountability in clinical nurses. Precedent cases related to patient safety were used in the education program. Methods: A quasi-experimental design with pretest and posttest measures was used. Participants in the study, 72 nurses in the experimental group, 71 nurses in the control group, were enrolled for 3 months. The education program was composed of the 20 precedent cases related to patient safety from home and foreign countries. Results: The major findings of this study were as follows: Safety perception (p=.000), Safety control (p=.000), attitude toward autonomy (p=.000), and attitude toward accountability (p=.000) improved after the education program. Conclusion: The findings from this study indicate that an education program using precedent cases is an efficient method to improve behavior and change attitudes towards protecting patients' safety and preventing malpractice claims against nurses.

The Effect of a Preoperative Patient-Controlled Analgesia Education Program on Postoperative Pain Control in Older Patients with Spine Surgery (수술 전 자가통증조절기 교육 프로그램 적용이 노인 척추 수술 환자의 통증 조절에 미치는 효과)

  • Purpose: This study aimed to investigate the effectiveness of preoperative patient-controlled analgesia(PCA) education program on older patients with spine surgery. Methods: A quasi-experimental research with a non-equivalent control group pretest-posttest design was conducted to investigate the impact of a PCA education program before surgery on postoperative pain, pain knowledge and attitudes, and frequency of additional analgesic use. The sample size for experimental and control group was 55 respectively. Results: The experimental group, which underwent the PCA education program, had lower postoperative pain scores compared to the control group. Furthermore, the experimental group exhibited a higher level of knowledge on PCA (p<.001) and more positive attitudes toward analgesic use (p<.001). While there was a significant difference in the use of opioid analgesics for additional pain relief between two groups (p<.001), there was no significant difference in the use of non-opioid analgesics. Conclusion: The implementation of the PCA education program was found to increase knowledge and positive attitudes on the use of PCA. Moreover, it significantly alleviated pain, particularly during physical activity, within initial 48 hours after spinal surgey in older patients. Therefore, the findings of this study supported that the PCA education program could be used as a preoperative intervention to alleviate postoperative pain for older patients with spinal surgery.

The Effects of Hybird simulation practice program for Nursing students using Complex Scenario (간호대학생을 위한 Hybrid 시뮬레이션 실습교육 프로그램의 효과: 복합 시나리오 적용)

  • The study attempted to examine the effects of hybrid simulation practice program on critical thinking disposition, self-efficacy, communication competency, and clinical competency of nursing students. The study was one group pre-test and post-test design. Data were collected between April 24 to May 5, 2023 from 35 nursing students. The collected data was analyzed using the SPSS 25.0 program, frequency analysis, mean, standard deviation, and paired t-test. Research results showed that nursing students' critical thinking disposition(t=7.01, p<.001), self-efficacy(t=2.17, p=.037), communication competency(t=2.70, p=.011), and clinical competency(t=6.60, p<.001) were improved after the simulation program. The hybrid simulation practice program is significant in that it applies various learning tools, including high-fidelity-low-fidelity-role play to strengthen the connection between nursing students' theory and practice.

Comparing Effects of Tai-Chi Self-help Program and Tai-Chi Exercise in Osteoarthritis Patients (골관절염 환자를 위한 타이치 운동그룹과 타이치 자조관리 그룹의 효과 비교분석)

  • Purpose: The purpose of the study was to compare the effects of Tai Chi self-help program and Tai Chi exercise in patients with osteoarthritis. Methods: quasi experimental design was utilized for the study to compare the effects after implementing tai chi self-help program and Tai Chi exercise for 6 weeks. The subjects of the study was osteoarthritis patients who visited to two Public Health Centers in Gyunggi-do from August to December, 2005. Total of 25 OA patients agreed to participate in the study, and 10 tai chi self-help program group and 9 Tai Chi exercise group were included for the final analysis after 6 weeks. Tai Chi self-help program was provided twice a week, 2 hours per session (one hour for tai chi exercise, one hour for self help program) for 6 weeks, and Tai Chi exercise group was provided once a week, 2 hours per session for 6 weeks. Outcome measures include flexibility, 6 m walking, and balance. Mann-Whitney U test of SPSS WIN 11.0 was utilized to compare the group differences. Results: The subjects of both program participants were not improved in their arm flexibility, and the difference was not significant. Tai chi self-help group improved more in their 6 m walking ability than Tai Chi exercise group, but the difference was not significant (F=.000, p>.05). Tai chi self-help group improved more in their balance than Tai Chi exercise group, but the difference was not significant (Rt: U=.24, p>.05; Lt: F=2.60, p>.05). Conclusion: The provision of both Tai Chi self-help program and Tai Chi exercise can lead improvement of 6 m walking, and balance, but the group difference was not significant. A rigorous research design should be further utilized to identify the potential benefits for this population.

Comparing Effects of Tai Chi Exercise, Tai Chi Self-help Program, and Self-help Management Program for Osteoarthritis Patients (골관절염 환자를 위한 타이치, 타이치-자조관리 및 자조관리과정의 효과 비교)

  • Purpose: The purpose of the study was to compare the effects of Tai Chi exercise, Tai Chi self help program, and self help management program on arthritic pain, stiffness, fatigue and difficulties in performing daily activities in patients with osteoarthritis. Method: The pre-post experimental design was utilized with convenient sampling at Public Health Centers. Total of 38 subjects agreed to participate in the study with the inclusion criteria of being diagnosed as osteoarthritis and no previous regular exercise. The 12 subjects were assigned to Tai Chi exercise group, 13 subjects to Tai Chi exercise combined with self help management program, and the other 13 to self help management program. Tai Chi exercise group performed exercise twice a week for one hour per each session. Tai Chi self help management group came to the class twice a week, one hour for Tai Chi exercise, and another hour for self help management program. The third group came to the class once a week for one hour to participate self help management program. At the completion of 6 week programs, 10 subjects from Tai Chi exercise, 9 from Tai Chi self help, another 10 from self help management groups completed the post-test measures. The data were analyzed by SPSSWIN 11.0. The homogeneity tests for three experimental groups were conducted by $X^2-test$, Fisher's exact test, Kruskal-Wallis test. The final analysis of the post-test measures was conducted by ANCOVA with fatigue, pain, stiffness, and difficulties in daily activities of the pretest measures as covariates. Results: The results were as follows. There were no significant differences in fatigue, pain, and difficulties of daily activities among three groups for the pretest measures. The score of arthritic stiffness was significantly different among groups after controlled for covariates, but the post-hoc tests showed no group differences. Conclusion: The nursing interventions applied for osteoarthritis patients have been previously tested to show the cost effectiveness of symptoms management of this population.
