Impact of Premarital Coaching (PMC) Intervention using the Psycho-educational Approach on Parenting Self-efficacy of Newly-married Underage Couples: A Narrative Literature Review of Evidence and Feasibility in Indonesia

  • Purpose: The purpose of this study was to identify the latest evidence for interventions involving adolescent newlyweds, and the feasibility of premarital coaching (PMC). Methods: A narrative literature review design carried out through related studies was used for the present literature review. A diverse search from several databases was performed to create a critical appraisal and to develop a more integrated model for the PMC program. Studies that empirical, written in English, peer-reviewed, and published during 2008~2017 were included. Results: Six articles were selected for the review. The principal findings identified three major themes including underage period as a high-risk for marriage, development tasks of the newly married spouse, parenting self-efficacy and the psycho-educational approach of PMC to enhance parenting knowledge, skills, and readiness. Conclusion: It was found that content of PMC as the preparation of underage married couples were formulated based on coaching technic, various spouse and parenting educations, and adjusted based on couples's preferences. Rigorous studies with measurement of long-term retentions are needed.

  • Physical and psychological events can produce stress response in various degrees. Stress affects many aspects of physiology including both brain and peripheral elements which is represented as hypothalamus-pituitary-adrenal axis. Brain elements consist of corticotropin-releasing hormone(CRH), locus ceruleus(LC)-norepinephrine(NE)/autonomic system. Peripheral elements include pituitary-adrenal axis and the autonomic nervous system, which coordinate the stress response. Current trend of the stress researches is emphasizing the mechanisms of the stress response which is adaptive or become maladaptive. This review introduces 1) the concepts of stress, 2) physiological and behavioral aspects of stress responses, 3) the consequences of stress response, 4) the measurements of stress and 5) stress management for those interested in stress research.

정보제공이 외래 의식 하 수술, 시술 환자의 불안에 미치는 영향: 체계적 문헌고찰 (Effects of the Provision of Information on Anxiety in Patients during Outpatient Surgery: A Systematic Review)

  • Purpose: The purpose of this study was to investigate the research methods, contents, and effects of providing information to alleviate patients' anxiety during outpatient surgery. Methods: The PICOTS-SD (Participants, Interventions, Comparisons, Outcomes, Time, Setting, Study design) was used and 11 randomized controlled trials studies that met the inclusion and exclusion criteria were selected. For assessing the quality of the studies, the Cochrane Risk of Bias tool was used. Results: In the 11 selected studies, intervention methods included verbal, written paper, video, and websites, and the outcome variable was anxiety. Regarding the effectiveness of providing information on anxiety, the result of the present study was ambivalent. Conclusion: This study showed the current trends in providing information as an intervention. However, randomized controlled trials are further needed in Korea, to verify the effects of such an intervention on patients' anxiety.

섭식장애 청소년을 위한 인지행동치료 프로그램 개발 (Development of a Cognitive Behavioral Therapy Program for Adolescents with Eating Disorders)

  • Purpose: This study was undertaken for developing a cognitive behavioral therapy (CBT) program for adolescents with eating disorders. Methods: The specific process for the development of this program involved establishing a program goal, analyzing existing CBT programs, and constructing a CBT program. Results: The program consisted of 12 sessions, which is shorter than typical programs conducted for people with eating disorders, in order to reduce the dropout rate. Each session was 40~60 minutes long, and included the use of the psychoeducation, Dysfunctional Thought Record, and activity planning. The participants attended 5 sessions individually and 7 with their family members. Conclusion: This study presented a 12-session CBT program for adolescents with eating disorders. These results will provide valuable data for developing effective programs to improve the quality of life of these adolescents.

  • Purpose: The purpose of this study was to provide foundational knowledge on nursing tasks performed on patients with COVID-19 in a nationally-designated inpatient treatment unit. Methods: This study employs both quantitative and qualitative approaches. The quantitative method investigated the content and frequency of nursing tasks for 460 patients (age ≥ 18 y, 57.4% men) from January 20, 2020, to September 30, 2021, by analyzing hospital information system records. Qualitative data were collected via focus group interviews. The study involved interviews with three focus groups comprising 18 nurses overall to assess their experiences and perspectives on nursing care during the pandemic from February 3, 2022, to February 15, 2022. The data were examined with thematic analysis. Results: Overall, 49 different areas of nursing tasks (n = 130,687) were identified based on the Korean Patient Classification System for nurses during the study period. Among the performed tasks, monitoring of oxygen saturation and measuring of vital signs were considered high-priority. From the focus group interview, three main themes and eleven sub-themes were generated. The three main themes are "Experiencing eventfulness in isolated settings," "All-around player," and "Reflections for solutions." Conclusion: During the COVID-19 pandemic, it is imperative to ensure adequate staffing levels, compensation, and educational support for nurses. The study further propose improving guidelines for emerging infectious diseases and patient classification systems to improve the overall quality of patient care.

여성의 유산경험에 대한 현상학적 연구 (Women s Experience of Abortion : Phenomenological Perspectives)

  • Women’s experience of abortion are not only widespread but also significant event in their lives. But this experience has not been a concern for professional care by health care providers, especially nurses. As professional nurses should have holistic approaches to clients, need to understand the lived experience of abortion from women’s perspectives. These Nurses must identify unique means for improving the quality of life of women. This study identified the meaning and structure of the lived experience of abortion. The participants were residents of Seoul and Kwang ju who were recruited through personal recommendation. Colaizzi’s method was used for the phenomenological analysis. The research question was aimed at revealing the covert meaning of abortion. The 14 women who had a spontaneous abortion and /or an artificial abortion and who argeed to participate in the study were informed of the purpose of the study, the possible risks and benefits of participation, the data generation method and they were assured of privacy and confidentiality. The investigator conducted in-depth unstructured interviews which were audiotaped with the permission of the participants. The investigator read the data repeatdly to identify and categorize themes and basic structural elements. Eight themes of abortion as experienced by these participants were : 1) complicated feelings about the anticipated abortion 2) feelings of fear and anxiety about the operation and complications 3) grieving related loss and injury 4) feelings of lonliness in perceiving no supportive person, 5) attributing the cause of the abortion to siginificant others or to self, 6) guilt feelings and resentment, 7) vacillating between regretting and not regretting, 8) struggling to a responsible person. Five basic structures identified were : 1) Complicated feelings 2) Feelings of loss and need for caring 3) Attributings to cause to significant others 4) Paradoxical emotion 5) Struggling to be a responsible being The significes of this study for nursing are : 1) It enables nurses and other health care providers understand more clearly the lived experience of abortion 2) It provides information which can be used in for women who experience abortion.

성인 발열환자의 발열관리 실태 조사 (A Descriptive Study for Fever Management in Adult Febrile Patients)

  • Purpose: The specific aims of this study were to define fever from the nurse's and physician's perspectives, describe fever management method and decision-making, and identify differences of definition of fever and fever managements between nurses and physicians. Methods: A cross-sectional survey using a 23-item questionnaire was used. A total of 305 nurses and physicians were surveyed, and 246 useful questionnaires were returned. Results: Body temperature of $36.5{\sim}38.3^{\circ}C$ were considered as adult febrile, and fever managements were initiated when the temperature was $37.0{\sim}38.0^{\circ}C$. More than half of the nurses and physicians, made a clinical decision regarding fever managements based on their own experiences. The most frequently used interventions in nursing practice were antipyretics, intravenous hydration, and external cooling. Nurses provided external cooling to lower body temperature than physicians (Z=-4.67, p<.001). Most participants reported that they needed a fever management protocol. Conclusion: Based on the information obtained, nurses respond with sensitivity to lower body temperature. We found significant differences among nurses, between nurses and physicians in approaching care of adult febrile patients.

건강증진 중심의 학부 교과과정에 관한 평가연구 (Study for Curriculum Evaluation of the College of Nursing, Seoul National University: Focusing on the Health Promotion)

  • The purpose of this study is to evaluate the revised curriculum of the college of nursing, Seoul National University. This curriculum was consisted of three conceptual framework: health promotion, illness care and professional development. Especially it is intended to enhance the ability in solving health problem by focusing on the topic of health promotion. Further, the curriculum is strengthening on the professional development through encouraging scholarly inquiry of students. The result of survey was as follows: 1) Is the curriculum focusing on the health promotion generally? - 81.2% of graduates answered positively. 2) Is the curriculum reflecting on the six aims of the education for the college if nursing? - 64.6% of graduates answered positively. 3) Is the curriculum reflecting on the concept of health promotion properly? - 72.8% of faculty answered positively. 4) Did the curriculum achieve the six aims of college of nursing effectively? - 82.6% of faculty answered positively. 5) Was the lecture of each subject in the new curriculum given with focus on the concept of health promotion? - 60.9 % answered affirmatively 6) Did the optional subjects achieve the goal of professional development effectively? - 83% of faculty answered affirmatively.

임상실습을 경험한 간호대학생의 감염관리 활동의 관찰과 수행을 통한 감염관리에 대한 인식 및 심리적 갈등 (Perception and Inner Struggle Experienced by Nursing Students in Relation with Infection Management through Observation and Performance of Infection Control Activities)

  • Purpose: To study the internal psychological conflicts among nursing students during an infection control protocol carried out in the hospital by measuring their observation skills and performance during clinical training. Methods: Investigation of both pre- and post- infection control was conducted using questionnaires for clinical infection practices. We identified and evaluated the students' observation skills, clinical performance, clinical perception, and internal conflict regarding clinical infection control. We also interviewed the students as part of our study. Results: Among parameters such as clinical performance, observation skills, clinical perception, and internal conflict, the average observation skills (t=5.49, p<.001) were significantly lower, while internal conflict among students (t=-7.23, p<.001) was significantly higher than expected prior to clinical training. Generally, there was a negative correlation between observation skills and internal conflict in every aspect of infection control practice (r=-.281, p=.031). Internal conflict was significantly higher than expected in the context of hand hygiene (t=-2.135, p=.037), personal hygiene (t=-3.48, p=.002), and ventilator management (t=-3.69, p<.001). Clinical performance of students in the context of hand hygiene (t=4.69, p<.001), personal hygiene (t=2.06, p=.044), and ventilator management (t=2.68, p<.001) was significantly lower than expected prior to clinical training. Conclusion: Our findings showed that internal psychological conflict is higher when infection control practices are observed or performed to a lesser degree. Therefore, reinforcing education regarding infection control among students, such as developing a systematic program, or consecutive training and monitoring, is suggested.

일 의료원의 간호조직 효과성에 관한 연구 (Study on the Effectiveness of Nursing Organization of one tertiary Hospital)

  • This Study was dosigned to test the relationship between effectiveness of nursing organization and related variables of organization on the perspectives of total quality management, The data were collected through self reported questionnaires from 138 nurses working in, 288 patients hospitalized in one tertiary hospital in Seoul from May 30 to October 10 in 1996. The data were analyzed by using the pc-SAS program. The information was obtained of descriptive statistics, correlation analysis, multiple regression, ANOVA & Tukey's multiple comparison test. Results showed that the managing change, the managing job design, the managing conflict, the formation and the centralization of nursing organization structure were shown to be relating variables for nurses' job satisfaction. Especially the managing change was the predictor of nurses' job satisfaction, It explained 57% of total variance. According to clustor analyses of the variables of the managing change, the managing job design, the managing conflict, the nursing units were divided three clusters and according to ANOVA, there was significint difference in nurses' job satisfaction The nurses' job satisfaction was higher in the nursing units revealed to have high job management score. But there was no significant difference in patient satisfaction among three clusters.
