간호간병통합서비스 도입에 따른 서울의료원 병동부 시설환경변화 연구 (A Study on the Change of Physical Environment in Seoul Medical Center by Providing Comprehensive Nursing Service)

  • Purpose: The research looks at differences between the 'General ward' and the 'Comprehensive nursing ward' in Seoul Medical Center, regarding the facility improvements and changes in nursing services. It investigates and analyzes spatial problems and improvement needs through a survey and conduct investigation of staffs. It is to propose the primary data for the architectural planning of the future ward with the comprehensive nursing service. Methods: Targeting the comprehensive nursing ward, changes of the physical environment and spatial problems are analyzed through a field survey, behavior investigation, present-condition investigation, and floor plan analysis. Results: The workforce is increased by approximately twice the amount of the nursing staffs in the comprehensive nursing ward, compared to the pre-general ward. When utilizing the general ward, various spatial problems arise due to the restrictions of the facility condition. Because Sub N.S is an important facility as a part of the nursing work function in the ward of the comprehensive nursing service, the opinions of staffs must be considered when selecting a location and composing a space. Implications: It can be used as a primary data for the comprehensive nursing ward when architectural planning of a new hospital.

요양병원의 의료서비스 품질이 관계의 질 및 환자충성도에 미치는 영향 (Effect of Medical Service Quality in Nursing Hospital on Relationship Quality and Patient Loyalty)

  • 본 연구는 국내 요양병원의 의료서비스 품질과 관계의 질 및 환자 충성도 간의 영향 관계를 검증하는 데 목적이 있다. 이를 위해 총 324명의 환자를 대상으로 설문조사를 실시하였으며, 조사 결과는 다음과 같다. 첫째, 요양병원 의료 서비스품질이 관계의 질에 미치는 영향을 분석한 결과, 의료 서비스품질변수 중에서 요양시설장비, 요양의료진, 요양서비스시스템, 요양시설 평판은 관계의 질에 유의미한 정(+)의 영향을 미치는 것으로 나타났으며, 진료비는 유의미한 영향이 없었다. 둘째, 요양병원 환자와 병원 간의 관계의 질이 환자 충성도에 미치는 영향을 분석한 결과, 관계의 질 변수 중에서 신뢰와 관계몰입은 모두 환자 충성도에 유의미한 정(+)의 영향을 미치는 것으로 나타났다. 셋째, 요양병원 의료 서비스품질이 환자 충성도에 미치는 영향을 분석한 결과, 의료 서비스품질 변수 중에서 요양시설장비, 요양의료진, 요양서비스시스템, 요양시설 평판은 환자 충성도에 유의미한 정(+)의 영향을 미치는 것으로 나타났으며, 진료비는 유의미한 영향이 없었다. 넷째, 요양병원 의료 서비스품질과 환자 충성도의 관계에서 관계의 질의 매개효과를 분석한 결과, 요양 진료비를 제외한 요양시설장비, 요양의료진, 요양서비스시스템, 요양시설 평판 모두 환자와 병원 간의 관계의 질을 매개로 하여 환자 충성도에 긍정적인 영향을 미치는 것으로 나타났다.

노인요양 시설 입소 노인 삶의 질 구조모형 (A Structural Equation Model of Quality of Life in Nursing Home Residents)

  • Purpose: This study was done to develop a predictive model for quality of life of elderly residents in long-term care facilities (LTCF). The study was based on Brofenbrenner's ecological system theory and a literature review. Methods: Data were collected using a convenience sample of 205 elderly residents in 2 nursing homes located in D city and 1 nursing home located in K province. The exogenous variables were individual factors, family support, and facility environmental factors. The endogenous variables were self-esteem, accommodation adaptation and quality of life of elderly residents in LTCF. Collected data were analyzed through structural equation modeling using AMOS 20.0. Results: Eleven of the twelve hypotheses were supported, but the hypothesis that facility environment factors effect self-esteem was not supported. Quality of life of elderly residents in LTCF was explained first by facility environmental factors, followed by self-esteem, individual factors, accommodation adaptation, and family support with an explanatory power of 83.0%. Conclusion: To improve the quality of life of elderly residents in LTCF, the service and environment preparation provided by facilities is important, and it is necessary to provide emotional counseling to improve the self-esteem of these elders.

경수완전손상환자의 재활병동생활 경험 - 퇴원을 중심으로 - (Experience of Discharge from Rehabilitation Facility)

  • Purpose: The purpose of this study was to explore and describe the essence of hospital life concerning discharge from rehabilitation facility as defined by clients with complete quadriplegia. Method: This was a qualitative research using Giorgi's descriptive phenomenology. Four participants with complete quadriplegia were participated. Results: The lived experience of discharge from rehabilitation facility among clients with complete quadriplegia was structured by 10 components and was defined as a life event which those patients felt fears and worries to leave the therapeutic environment and to return to their community with various changes due to their disability. During their hospital stay, the patients with quadriplegia were concerning to prepare proper living conditions and their health conditions so that they could keep their life independent and worthy. They also cherished a hope for cure for their cervical injury even at the moment of discharge. Conclusion: This study suggests developing a discharge program which relieves the clients' emotional burden for discharge from inpatient rehabilitation facility and helps them to take an active role in preparing for returning to their daily life.

암 전문병원 간호사의 간호역할 수행도와 중요도 (Nurse's Role Performance and Perception of the Importance of Role among Nurses Practicing in a Cancer Care Facility)

  • Purpose: The purpose of this study is to examine the nurse's role performance and the perception of the importance of role among nurses practicing in a cancer care facility. Methods: A descriptive study design was applied with convenient sampling of 175 nurses working at a cancer care facility in Daegu, Korea. Data was collected using a structured questionnaire from July 1, 2013 to July 5, 2013. Results: Cancer care facility nurse's role performance score was measured on average $3.23{\pm}0.64.$ Oral medication was the most frequently performed role, followed by encouragement, care related to medication, and pain management. The perception of the importance of nurse's role was measured on average $3.31{\pm}0.35$. Care related to medication was regarded as the most important, followed by transfusion, oral medication, and intravenous & intramuscular medication. Nurse's role performance was different according to nurse's age (F=3.21, p=.024) and current practice area (F=3.73, p=.012). The perception of the importance of nurse's role was different only in relation to current practice area (F=6.82, p<.001). Conclusion: Nurses practicing in oncology setting frequently involve encouraging patients and pain management. Training programs designed to facilitate frequently performed and highly regarded nurse's roles are required for nurses practicing in oncology setting.

유료요양원의 수요와 시장세분화에 관한 연구 (A Study on Demand and Market Segmentation in Nursing Homes)

  • The purpose of this study is to analyze the consumers' demand pattern and the feature of the market for nursing homes, the number of which is tending upwards. The survey data were obtained from the interview of 500 elderly people living in Seoul and Kyung-Ki provincial area. All respondents were 60 years of age and above. The main findings were summarized as follows: 1. The respondents who are less aged, highly educated comparatively, and living with spouse show positive response for the use of nursing homes. The aged living independently and the aged living with unmarried children show higher demand for this facility. Also, the respondents who prefer independent living away from their childrenn, urban areas as their residence and flat-type housing show more interest for the facility. The respondents who are self- supportive, who has no financial supporter, no caretaker, and no domestic helper demonstrate strong inclination to the use of the facility. The respondents who are interested in this kind of facility, acknowledge the necessity of it show strong intention of moving into it. 2. Logistic regression analysis was conducted to understand factors related to the intention of moving into the nursing homes. The group who wish to live separated from their children in the future give 1.78 times more favorable response than the opposite. The group who have an interest in the facility for elderly has 2.02 times higher intention of moving than the opposite. The group who have an intention of using the facility for elderly it is 7.34 times more likely to move into it. 3. The respondents who are the potential consumers for nursing homes can be subdivided. Within the positive group, it could be divided into the group of living independently with the preference of flat-type housing, the group living independently with the preference of separate housing, and the group wishing to live with their children. Within the negative group, the factor of the division is their concern to the facility. Following this study, it can be said that old age people, who have been regarded as one homogeneous group so far, should be recognized as one characteristic individual. This study also shows that the demand aspect yet in its initial stage shold be researched in anticipation of rapid increase. The understanding of diciding factors, the segmentation of potential market will help work out proper strategy, which will contribute to providers' benefit.

병원 웹사이트 품질 인식(사용용이, 의료정보, 심미성, 안전성)이 고객충성도에 미치는 영향(신뢰를 조절변수로) (The Effect of Quality Cognition(usage, medical information, aesthetic, safety) of the Web Site of the Facility Hospital on Customer Loyalty(moderating role of trust))

  • The research was to investigate the effect of quality cognition(usage, medical information, aesthetic, safety) of the web site of the facility on customer loyalty and moderating role of trust variable. The respondents was 201 patients and caregivers. Data were collected from March 1 to 31, 2010 at C university hospital in G city. In relationship between quality cognition of facility web site and customer loyalty, the results of quality cognition on customer loyalty showed 43.1% (F=36.912) in model 1, 46.3%(F=33.454) in model 2, and 46.8%(F=18.580) in model 3. In relationship between web site quality cognition and customer loyalty, moderate effect of trust was not considerable. As results, quality is the leading fact for customer loyalty more than trust. Therefore, customer loyalty leaves its possibility of increment when facility web site is designated depending on medical information, usage and aesthetic.

한국 노인요양시설 거주환경의 특성에 대한 연구 (A Survey on the Characteristics of Living Environment on Nursing Homes for the Elderly in Korea)

  • The purpose of this study is to analyze the characteristic of living environment on nursing homes and to examine the current status and problems of the nursing homes by looking into the differences in the spatial uniqueness of living environment in relation to the characteristics of the nursing home. This study utilized a mail survey during July 29 - August 31, 2006 that included a structured questionnaire. The questionnaire largely dealt with the general feature of the facility and the elderly who live there, and the spatial feature of the facility. The study limited its survey to those facilities that were registered to the Ministry of Health and Welfare, which opened after 1995 and that had the capacity of more than 30 people. As a result, the questionnaire was sent to the directors of 344 facilities who were in charge of the nursing home operation and planning. 182 facilities returned the questionnaire and the study used'182 questionnaires for data analysis. The study used the SPSS 12 program to draws the frequency, percentage of the answers and utilized the cross-tab, t-test, ANOVA to analyze the differences.

장기요양시설 노인에 대한 간호서비스 제공시간 분석 (Care Time of Elderly in Long-Term Care Facilities)

  • Purpose: This study was to define the care time of elderly in long-term care facilities and to compare and analysis the care time by staff and facility types. Method: The data were collected from 530 elderly over sixty, residing in two long-term care hospitals for dementia, three long-term care hospitals for the elderly and two skilled nursing facilities. Care time for individual residents was measured the total time spent by nurses, aides. Result: The average care time measured by nursing staff was 158.6 minutes a day. The average care time for one resident by nurse was 40.4 minutes, and by aides, 118.2 minutes. The difference of the care time showed statistical significance between long-term care facility types(p<0.001) : average care time for dementia hospital(199.1min) was twice as long as that of skilled nursing facility(94.1min). Conclusion: The results of the study showed that the care time differentiates care time provided to elderly in long-term care facilities in Korea. The study suggests the need to emphasize the importance of standardization of level of staff and service programs by the long-term facilities.

요양시설 규모에 따른 직원의 감정노동, 직무만족이 조직몰입에 미치는 영향 (The Effects of Emotional Labor and Job Satisfaction on Organizational Commitment in Staffs by the Size of Facility)

  • Purpose: The purpose of this study was to identify the factors influencing organizational commitment of staffs according to the size of long-term care facility. Methods: A cross-sectional descriptive study was designed. Data collection was conducted for a total of 315 employees in long-term care facilities located in Seoul, Gyeonggi, Gangwon, Gyeongbuk, and Chungnam. Data were collected from July 2018 to October 2018 using questionnaires which included emotional labor, job satisfaction, organizational commitment, and general characteristics. In order to confirm the differences in the size of the facility, the facilities with less than 30 beds, those with 30-99 beds, and those with more than 100 beds were analyzed. Data were analyzed using descriptive statistics, t-test, ANOVA, Mann-Whitney U test, Kruskal-Wallis H test, Pearson's correlation analysis, and multiple regression. Results: The job satisfaction and organizational commitment were significantly different according to the size of long-term care facility. Organizational commitment was influenced by 'external job satisfaction' in less than 30 beds, was influenced by 'external job satisfaction, and attentiveness to required display rules of emotional labor' in 30~99 beds, and then was influenced by 'type of job, and internal job satisfaction' in more than 100 beds. The predict variables accounted for 23.0%, 41.0%, and 34.0% of organizational commitment respectively. Conclusion: These findings show that tailored interventions should be provided depending on the size of facility in order to increase organizational commitment. In addition, organizational commitment programs should be developed by considering strategies to reduce the emotional labor and to increase job satisfaction.
