Health Status and Daily Life Style of the Elderly (노인의 건강상태 및 일상생활 양식)

  • This study was planned and carried out in an attempt to investigate health status and life style. Using a guidline developed by the researcher open-ended interviews were done with a hundred and sixty Korean elderly people, from April 17, through June 2, 1988. The data were analyzed by frequencies, means, percentile scores and Chisquare-test. Major health deviations identified are ; arthritis, blood pressure discrepancies, neuralgia, and powerlessness. Discomfort in relation to elimination(75%) and to rest-activity, especially to the activities of daily living(78%) are experienced ; 11% revealed to be in need of assistance from others. One or more sensory discrepancies are complained by 75% of the subjects. Only 20% of the subjects reported being satisfied with the life-time achievements. Factors revealed that 'descendants' were the major source of pleasure. Leisure hours were spent mostly 'watching TV' 'Healthy life', 'harmonious family life' and 'frequent communication with the family' were the three most frequent wishes towards their family members. 'Insurance for ageing', 'free health care', 'work', 'cultural facilities' and 'warm attention' were the request toward the society.

Factors Influencing the Development of Pressure Ulcers in Surgical Patients (수술환자의 욕창발생 예측요인에 관한 연구)

  • Purpose: The purpose of this study was to identify the influencing factors on the development of pressure ulcers in patients undergoing surgery which lasted more than two hours. Method: One hundred nineteen surgical adult patients were included in the study. Data was measured on each participant from December 2003 to February 2004. It was collected using a structured researcher-administered sheet and analyzed by descriptive statistics, t-test, chi-square test and logistic regression analysis. Result: The prevalence of a perioperative pressure ulcer was 26.1%. The level of moisture, friction and shear, length of surgery, and perioperative irrigation were significantly higher in the pressure ulcer group than those in the non-pressure ulcer group. The level of activity and level of consciousness were significantly lower in the pressure ulcer group than those in the non-pressure ulcer group. Significant influencing factors on the development of pressure ulcer were 'moisture' and 'irrigation' and those variables explained 23.1% of varience in the development of a pressure ulcer during surgery. Conclusion: It is necessary to develop a strategy to prevent pressure ulcer by taking 'moisture' and 'irrigation' into account during the preoperative, perioperative and postoperative period.

Development and Application of a Health Promotion Program for Marriage Immigrant Women (여성결혼이민자를 위한 건강증진 프로그램 개발 및 적용)

  • Purpose: This study was to develop a health promotion program for marriage immigrant women and to evaluate its efficacy. Methods: The health promotion program was comprised of eight 100-minute weekly sessions. Each session included understanding of health responsibility, nutrition, physical activity, interpersonal relations, stress management, and self actualization. The research was conducted under the principles of nonequivalent control group pretest-post test design. The outcome variables were health promoting behavior, health status, acculturation, self efficacy, and perceived barrier. The participants were 15 immigrant women in the experimental group and 16 in the control group. Data was analyzed using ${\chi}^2$ test, Fisher's exact test, Mann Whitney U-test and Wilcoxon signed rank test. Results: There was a significant improvement in health promoting behaviors (U=-3.08, p=.002), left shoulder flexibility (U=-3.02, p=.003), right shoulder flexibility (U=-3.02, p=.003), low back flexibility (U=-3.37, p=.001), social health status (U=-3.38 p=.001) and subjective health status (U=-2.17 p=.030) in the experimental group compared to the control group. Conclusion: The health promotion program for marriage immigrant women was an effective intervention for improving health promoting behavior, physical health status, social health, and subjective health status. Therefore, the developed health promotion program needs to be applied to married immigrant women in other kinds nursing care settings in future research.

Meta-analysis about the Effectiveness of Intervention Programs for Married Immigrant Women (결혼이주여성에게 적용한 중재 프로그램의 효과성에 대한 메타분석)

  • Purpose: The purpose of this study was to test the effect size of intervention programs for married immigrant women as well as to suggest the basic data for health care practices for married migrant women. Methods: Meta-analysis was conducted with 27 articles from domestic Korea master's and doctorate degree dissertations and Korea academic journals from 2007 to 2014. Results: Overall average effect size was 1.17 and 'parental education' of Intervention Variables was the biggest effect size. Intervention Variables were also identified to be the most desirable in cases when total sessions were applied with 10-18 sessions, 1 session per week, 90-120 minutes per session activity time and with less than 10 subjects in group size. Regarding effect variables, psychological variable group was identified to show the biggest effect size and in sub-variables, self-efficacy was identified to show the biggest effect size. Conclusion: If intervention programs for married immigrant women is expanded and conducted based on the results of this study, the program would have significant affect psychological, social and physical health of the married migrant women who currently occupy the important status in our society.

A Meta-analysis of the Risk Factors related to Falls among Elderly Patients with Dementia (치매노인의 낙상위험요인에 관한 메타분석)

  • Purpose: The purpose of this study was to provide data about the risk factors related to falls among elderly patients with dementia using meta-analysis. Methods: Key words used for search through electronic database (CINAHL, PubMed, Ovid-MEDLINE, RISS, KISS, DBPIA, National Assembly Library) included 'dementia', 'Alzheimer', 'fall'. Twenty studies met the inclusion criteria for the meta-analysis and 'R' version 3.2.2 was used to analyze the correlated effect size. Results: Study results showed that risk factors related to falls were identified as the demographic (age, gender, education), dementia-related (disease duration, cognition), physical (body mass index, walking, balance, activity of daily living, use of walking aids, number of medications including psychotropic drugs, musculoskeletal problems, parkinsonism, comorbidity), psychological (neuropsychiatric symptom, depression), environmental (Physical environment), and fall-related (fall history, high risk group of fall) factors. The effect size of risk factors such as high risk group of fall (r=.35), use of walking aids (r=.33), depression (r=.31), psychotropic drugs (r=.27), Musculoskeletal problems (r=.25) were higher than the other risk factors. Conclusion: Based on the findings of this study, strategies to improve elderly patient's depression, intensive care for high risk group of fall, and adequate training with walking aids are needed for prevention of falls in elderly patients with dementia.

Nursing Home Environment with Positive Distraction for Reduction of Chronic Pain and Healing (만성통증의 경감과 치유를 위한 노인요양시설의 긍정적 관심 전환 환경)

  • Majority of the seniors living in nursing homes suffer from persistent chronic pain, which may cause depression and compromised quality of life if untreated. The environment should support them to lift their focus from current pain and worries to the positive feelings and the delight of life. The purpose of this research is to classify the healing environment elements for positive distraction, and analyze 6 international cases to see the current situation. The elements were categorized as follows, based on literature review from both healing spaces and elderly care field: spatial elements(view, natural elements, artificial elements, exercise space, garden), psychological elements(grooming area, space for privacy, meal/drink area, elements for recollection, religious space), social elements(common living area, activity/hoppy room, family/visitor area, information area, local community program space). Analysis on 6 facilities showed that each elements were reflected to designed relatively well. New inventions from workers who think distraction is important were also introduced. Healing environment for positive distraction requires delicate touch, derived from understanding characteristics and situation of the residing elderly individuals. Technology update is also significant, from audio books to virtual reality devices, since cultural life of nursing home is far behind from what the others enjoy now.

The Effects of Health Behavior and Health Status on Heath-related Quality of Life in Older People: Gender Analysis by using the 2012 Korea Health Panel Data (건강행위와 건강상태가 노인의 건강 관련 삶의 질에 미치는 영향: 2012년 한국의료패널 자료를 이용한 성별 분석)

  • Purpose: The purpose of this study is to measure health-related quality of life (HRQoL) and to investigate the affecting factors according to gender in older adults by using the 2012 Korea Health Panel data. Methods: The Korea Health Panel data 2012 were collected from February to August 2012 by Korea Institute for Health and Social Affairs and National Health Insurance Corporation and the number of respondents were 1,921. The data were analyzed by t-test, chi-square, Pearson correlation coefficient and multiple regression using SPSS/WIN program. Results: The limitation of activity was the most influential factor for health-related quality of life of both male and female. For male, the affecting factors on HRQoL were hearing problems and frustration. For female, the affecting factors on HRQoL were eating problems, depression and suicidal impulses. Drinking, moderate physical activities, vision problems and stress had a significant effect on HRQoL for both male and female. Conclusion: Health care providers should consider the differences between male and female when they design a program in order to improve HRQoL of older people.

Factors Affecting the Sexual Function of Pregnant Women (임부의 성기능 영향요인)

  • Purpose: To determine whether maternal attitude toward sex during pregnancy and health-related quality of life could influence sexual function of pregnant women. Methods: In this study, 138 second and third trimester pregnant women completed self-report questionnaires during their visits to women's hospitals or community health centers to assess their general characteristics, attitude toward sex during pregnancy (PIES-M), health-related quality of life (EQ-5D), and sexual function (FSFI-6K). Multiple regression analysis was performed to test the research model with SPSS version 23. Results: Pregnant women who had discontinued their sexual life after recognizing their pregnancy accounted for 27.5% of women questioned. The average sexual function score of pregnant women was higher in the second trimester than the third trimester. Attitude toward sex during pregnancy (${\beta}=-.38$, p<.001), maintaining sexual life (${\beta}=.20$, p=.028), health-related quality of life (${\beta}=.18$, p=.030), and adverse symptoms during sex (${\beta}=.18$, p=.042) were determinants of sexual function during pregnancy. Conclusion: Nurses in antenatal care units need to help pregnant women maintain a positive attitude toward sexual activity during pregnancy and manage their health-related quality of life to maintain their sexual life during pregnancy.

The effects of AI Robot Integrated Management Program on cognitive function, daily life activity, and depression of the elderly at home (AI로봇 통합관리프로그램이 재가노인의 인지기능, 일상생활활동, 우울에 미치는 효과)

  • This study was conducted using non-face-to-face care technology for the elderly with mild dementia and the physically weak living in the community, as various methods of care for the elderly have been raised due to the prolonged COVID-19. The purpose of this study is a similar experimental study before and after the inequality control group to compare cognitive function, daily living activities, and the degree of depression by applying an AI robot integrated management program using. The data was collected from June 4 to September 17, 2021, and the survey results of 17 people in the experimental group and 18 in the control group were analyzed using the SPSS 25.0 program. As a result of the study, the experimental group was significant in language function, activities of daily living, and depression. In particular, the results showed a decrease in moderate to severe depression and mild depression. Cognitive function was significant with long-term care grade and daily living activity with family living together. Therefore, if such non-face-to-face care technology is introduced to the elderly care field in the 'With Corona era', it is thought that it will contribute to cognitive function training and depression reduction of the elderly.

Predicting Factors of Post-Stroke Depression (뇌졸중 후 우울에 영향을 미치는 요인)

  • Predicting factors of post-stroke depression were investigated. The subjects were 254 stroke patients who had been discharged and visited regularly the outpatient clinic for follow up care. The influencing factors were classified into five categories : demographic, disease-related, current state, social support, and illness intrusiveness. Sex, age, job, and educational level were defined as the demographic factor. The disease-related factors included stroke type, illness duration, attack frequency, and NIH score. ADL, cognitive function, and social activity were considered as the current state factors. The social support was measured as the perceived amount of social support. Illness intrusiveness means the perceived illness induced life style disruptions. Demographic, disease-related, current state, and social support were hypothesized to directly and indirectly affect post-stroke depression through illness intrusiveness. The illness intrusiveness was hypothesized to directly affect post-stroke depression. The hierarchial multiple regression was used to identify significant factors. The result showed that this model explained 43.3% of variance of post-stroke depression. And the prevalence of post-stroke depression was 38.8%. Among the demographic factors, job was identified as a main contributor to indirectly increase the post-stroke depression. Among the disease-related factors, stroke type, attack frequency, and NIH score were found to indirectly affect the post-stroke depression. Among the current state factors, ADL and social activity indirectly affected the post-stroke depression through illness intrusiveness. Social support and illness intrusiveness were identified to directly affect the post-stroke depression. This study has proved the factors likely to be implicated in the development of post-stroke depression. Based upon these results, it is recommended that the nurses who take care of post-stroke patients consider the risk factors such as social support, illness intrusiveness et al. Also programs which decrease the illness intrusiveness and increase the social support to reduce post-stoke depression recommended to be developed.
