  • This study aimed to explore the status of food service outsourcing behavior of long-term care institutions (LTCIs) through a cross-sectional survey using a questionnaire administered between July 16th and August 7th, 2020. The survey respondents were either dietitians or facility managers, who worked at 731 nursing homes, 477 group homes, and 673 day-care centers. Approximately 25.9% of nursing homes, 11.7% of group homes, and 33.1% of day-care centers used a managed-services company to operate their food service units. The main reason for outsourcing food service by nursing homes was related to the staffing of dietitians and cooks, whereas group homes and day-care centers outsourced food services due to factors relating to meal costs and the cooking process. Almost all the LTCIs entered into private contracts for outsourced food services. Only a few food service contracts included the types of meals, nutrition standards such as protein and calories per meal, and the parameter or ratio of food cost. Of the respondents, 84.5% from nursing homes, 87.5% from group homes, and 87.1% from day-care centers agreed that the quality of outsourced food services of the LTCIs should be regulated. Meals are essential for maintaining the health and functional status of LTCI users. As more LTCIs outsource their food services, we suggest the following: (1) Increasing the minimum dietitian staffing standards for LTCIs as per the Welfare of Senior Citizens Act and requiring at least one dietitian for every nursing home, (2) Making it mandatory to use a standard food service contract template when drafting food service contract, and (3) Developing realistic standards for food service operations considering the size and operation type of the LTCIs.

  • Purpose: This study aimed to identify the effects of preparedness towards delegation, critical thinking disposition, and communicative competence on job satisfaction among comprehensive nursing care service ward nurses and general ward nurses. Methods: The participants in this study were 218 nurses from two university hospitals. Data were collected using self-administered questionnaires and were analyzed using SPSS Statistics/WIN 21.0, including descriptive statistics, x2 test, Fisher's exact test, t-test, ANOVA, Scheffeé test, Pearson's correlation coefficient, and multiple linear regression analysis. Results: For comprehensive nursing care service ward nurses, the significant influencing factors on job satisfaction were age (β=-.37, p=.041), marital status (β=.35, p=.001), critical thinking disposition (β=.29, p=.003), and communicative competence (β=.28, p=.002) with the explanatory power was 39.0% (F=12.00, p<.001). On the other hand, for general ward nurses, the significant influencing factors on job satisfaction were advanced degree (β=.29, p=.047), critical thinking disposition (β=.23, p=.040), and communicative competence (β=.42, p<.001) with the explanatory power was 33.0% (F=11.78, p<.001). Conclusion: The results of this study suggest the importance of developing and applying education programs to improve critical thinking disposition and communicative competence of nurses. This would enhance the comprehensive nursing service and increase overall nursing manpower resources. In addition, further studies are needed to improve nursing work environments and to develop strategies for enhancing preparedness towards delegation, which would increase the job satisfaction of nurses in the comprehensive nursing care service wards.

  • The study is to identify the status of the ward of public hospital for integrated nursing care service. Integrated nursing service has expanded nationwide from 2016 but 10 out of 34 public hospitals are currently unable to operate as nurses due to the shortage. Analyze each floor plan and space program of ward and provide the basic data for the construction of the architecture plans for Integrated nursing care service. For this study, selecting 14 wards of public hospital in Korea and then analyzes space organization, distance from NS to bedroom and NS to core(Elevator) and spatial hierarchy of them. Among the 14 public hospitals, there are 7 types of double-loaded corridor, 4 types of double corridor and 3 types of single corridor. If NS is placed centrally, the control of the outsider and the patient's control would be efficient, but most of NS are lean on one side of floor. the placement of NS should be independent of individual arrangements, but it should be smooth alignment with patient spaces, staff spaces, and public spaces. This study could serve as basic research for the architectural plan for future integrated nursing care in ward of public hospital.

Methods: Data were collected from September 3 to October 9, 2012 targeting 196 baby boomers residing in Seoul and Gyeonggi-do with the measurement of long-term care service needs having five domains. Collected data were analyzed using the SPSS 20.0 program. Results: Demand for long term care service in those who preferred nursing homes was highest as $4.40{\pm}0.69$ in the safe environment domain. In case of those preferring home care services, demand was highest as $4.37{\pm}0.56$ in the social interaction domain. People who preferred nursing homes had higher needs in personal health care and improvement of the service quality domains. Those who preferred home care services showed diverse needs according to their characteristics. Conclusion: The baby boom generation had high needs in all the domains of long-term care services and such needs were diverse according to their characteristics. However, long-term care services had limitations that they provided standardized and uniformed services only. Therefore, health care services and improved quality services should be provided in a way of meeting the users' needs, and tailored services should be provided in consideration of the users' characteristics.

  • Purpose: The purpose of this study was to evaluate the effectiveness of case management for patients with hypertension on their health status and medical service utilization. Methods: This study was a secondary analysis of data collected for a larger study of chronic disease management in 2008 using the National Health Insurance Corporation database. A total of 12,944 patients who received case management for hypertension were included in this analysis. The subjects of case management were classified into subgroups, namely, over-use, under-use, and non-use groups according to the amount of medical service utilization. To compare the medical service utilization, a control group was selected randomly. The data were analyzed through descriptive statistics, McNemar test, and ANOVA. Results: All the subgroups displayed significant differences in blood pressure, self-management, social support, and their characteristics of medical service utilization. The total medical expense of the under-use and non-use groups increased after case management. However, there was no decrease in the medical expense of the over-use group. Conclusion: This finding suggests that there is a need to re-examine why patients overuse medical services and to supplement specific strategies for encouraging appropriate medical service utilization, and enhancing case management efforts for the over-use group.

Methods: A cross-sectional descriptive study was conducted with an online survey of 234 service workers using volunteer panels. Data were collected using a self-administered questionnaire from July 16 to 24, 2017. Major variables included work-life balance, resilience, emotional intelligence, job stress, and fatigue. Using the SPSS 24.0 program, descriptive statistics, t-test, ANOVA, correlation analysis, and multiple regression analysis were conducted. Results: The mean score of the work-life balance level of female service workers was $2.9{\pm}0.85$ on a total score of 6. A multiple regression analysis revealed that smoking, resilience, job stress and fatigue were statistically significant factors in the model (p<.001), with an explanatory power of 50.3%. Emotional intelligence was not significantly associated with work-life balance. Conclusion: The level of work-life balance was considerably low in female service workers. This study suggests that nursing interventions to enhance work-life balance are required at both the individual and organizational levels.
