선택진료제도 개편에 대한 중요도-성취도 분석(IPA) - 서울시내 상급종합병원 의료종사자 인식을 중심으로 - (An Importance-Performance Analysis of the Healthcare Reform on a Doctor-Designation System - Focused on the Perception of Medical Providers' at Tertiary Hospital in Seoul -)

  • Purpose: The purpose is to understand the perception of medical providers at tertiary hospitals in Seoul on the importance-performance of the reform on a doctor-designation system, and to provide the policy suggestion of the perception on such a system. Methodology: To achieve the purpose, this study utilized structured survey tools to conduct a questionnaire survey of nursing, administrative and medical technology professionals at six tertiary hospitals in Seoul. An importance-performance analysis was utilized for an analysis that showed the perception of the reform on a doctor-designation system. Findings: First, it was identified that those medical providers had the highest perception of the importance about the performance of the reform on a doctor-designation system. Second, according to the result of action grid of an importance-performance analysis (IPA), it can be identified that in the 2nd stage of 'Concentrate here' included are the items as to design and effect of medical care quality and subsidies & patient infection and safety and control fees, effect of income security of the method for compensating for loss established by the government and offering sufficient information on general medical services and doctor-designation medical services. In the 1st stage of 'Keep up the Good Work', and the 2nd stage of 'Concentrate here' included is the item as to the performance of patients of the reform on a doctor designation system. Practical Implications: There could be identified the effect of the reform bill on the perception of medical providers. It is expected that a better environment would be provided for patients to use a hospital and for medical providers to offer medical services, if the goverment makes efforts to improve methods for compensating for loss and continuous monitoring of the performance of patients.

Impact of Leadership, Empowerment, and Organizational Effectiveness on Turnover Intention for Employees of Korean Dental Clinics

  • This study inquires into employee's intent to leave in South Korean dental clinics. A systematic research was undertaken on theories of leadership types, job satisfaction, organizational commitment and empowerment, and then interaction between these variables was verified based on quantitative data by using Structural Equation Model (SEM). The findings suggest that making improvements of empowerment, job satisfaction, and organizational commitment demand individualized consideration and charisma, the components of transformational leadership. As a way to reduce turnover intention, the role of job satisfaction and organizational commitment was more influential than that of empowerment. Also, a combination of charisma of transformational leadership and contingent reward of transactional leadership turned out to be the ideal leadership type to reduce turnover intention. Lastly, the harmony of individual goal with that of organization, apparent documentation of reward system, and stress control were recommended as practical solutions to improve organizational profitability and effectiveness and to reduce turnover rate in dental clinics.

노인장기요양보험 관련 미디어 내용 분석 (Contents Analysis of Media on Long-term Care Insurance)

  • Objectives : This study confirmed the limitation of long-term care insurance by analyzing media contents. Methods : Articles and reviews were searched with the article searching system (KINDS) from July 2008 to December, 2015. Results : Among the 155 articles examined, 61.1% highlighted the faults of suppliers, and 25.2% indicated the responsibility of the insurer. As for their purpose, 56.8% reported on accidents, and 32.3% provided information. Furthermore, 74.2% reported on negative contents and only 25.8% on neutral contents. The negative contents consisted of requesting false insurance benefits, amending the range and price indicating the very low salaries of the care givers, limitations on the care grade assessment, and problems related with assistive devices. The majority of neutral articles is for providing information. Conclusions : There were many problems starting from the early stage of the insurance. We must pay attention to these problems. Moreover, we should try to handle and prevent these problems with supportive responses from authorities.

병원 간호사의 직무스트레스에 관한 연구 동향 분석 (Research Trend Analysis on Job Stress of Hospital Nurses)

  • 1) Purpose : The purpose of this research is to identify the trend of job stress of hospital nurses based on last 10 years of domestic research and to suggest directions for systematic future research. 2) Methodology : 101 studies between 2006 and 2016 were selected using key words such as registered nurse and job stress from Academic Information Service(Riss4u), the National Assembly Library and Korean studies Information Service System(KISS). 3) Findings : The researches on job stress of hospital nurses were increasing. The result showed that job stress increased for shift nurses. Especially, nurses of small-medium sized hospital showed remarkable effects to organization such as turnover intention. In addition, although the importance of approach to management is emphasized, there is little research on these aspects. 4) Practical Implications : It is necessary to develop systematic research on job stress directly effecting turnover intention of hospital nurses from hospital management aspect. The development of job stress measuring tools reflecting the characteristics of the medical institution, and the human resources management plan to resolve the imbalance in manpower for nurses should be required.

환자안전보고학습시스템 자료를 활용한 의료정보기술 및 전자의무기록시스템 관련 환자안전사건 분석 (Analyzing Health Information Technology and Electronic Medical Record System-Related Patient Safety Incidents Using Data from the Korea Patient Safety Reporting and Learning System)

  • Purpose: At present, there are a variety of serious patient safety incidents related to problems in health information technology (HIT), specifically involving electronic medical records (EMRs). This emphasizes the need for an enhanced electronic medical record system (EMRS). As such, this study analyzed both the nature of and potential to prevent incidents associated with HIT/EMRS based on data from the Korea Patient Safety Reporting and Learning System (KOPS). Methods: This study analyzed patient safety incidents submitted to KOPS between August 2016 and December 2019. HIT keywords were used to extract HIT/EMRS incidents. Each case was reviewed to confirm whether the contributing factors were related to HIT/EMRS (HIT/EMRS-related incidents) and if the incident could have been prevented (HIT/EMRS-preventable incidents). The selected reports were summarized for general clarity (e.g., incident type, and degree of harm). Results: Of the 25,515 obtained reports, 2,664 incidents (10.4%) were HIT-related, while 2,525 (9.9%) were EMRS-related. HIT/EMRS-related incidents were the third largest type of incident followed by 'fall' and 'medication incidents.' More than 80% of HIT/EMRS-related incidents were medication-related, accounting for approximately one-third of the total number of medication incidents. Approximately 10% of HIT/EMRS-related incidents resulted in patient harm, with more than 94% of these deemed as preventable; further, sentinel events were wholly preventable. Conclusion: This study provides basic data for improving EMR use/safety standards based on real-world patient safety incidents. Such improvements entail the establishment of long-term plans, research, and incident analysis, thus ensuring a safe healthcare environment for patients and healthcare providers.

스마트 헬스케어를 위한 프라이빗 블록체인과 생체인증기반의 만성질환관리 원격의료시스템 (Private Blockchain and Biometric Authentication-based Chronic Disease Management Telemedicine System for Smart Healthcare)

  • 고령화 사회로 인한 만성질환자의 증가로 그들의 질병 예방과 관리가 시급하다. 이러한 문제를 해결하기 위한 생체인증 방법과 원격의료시스템들이 소개되고 있으나 의료정보와 개인인증의 보안성 문제를 해결하는데에는 어려움이 있다. 스마트 헬스케어는 대상자의 개인 의료정보를 포함하고 있으므로 무엇보다 개인정보의 보안이 중요한 분야이다. 따라서 본 논문에서는 프라이빗 블록체인 환경에서 손목 밴드 형태의 스마트 웨어러블 디바이스 ECG와 얼굴 개인인증을 활용한 원격의료시스템을 제안하고자 한다. 이 시스템에서는 다양한 의료인과 전 지역 만성질환자를 대상으로 하였으며, 데이터의 무결성과 투명성을 높일 수 있는 프라이빗 블록체인을, 위변조가 어렵고 개인식별성이 높은 ECG와 얼굴인증을 활용하여 보안성과 신뢰성이 높일 수 있는 시스템을 구성하였다. 이를 통해 재가 만성질환자의 질병 예방과 건강관리에 힘써 만성질환 관리의 효율성을 높이는데 기여하고자 한다.

  • 최근 고령화로 인한 치매환자의 증가로 그들의 배회행동과 실종예방을 위한 대책이 시급하다. 이러한 문제를 해결하기 위해 다양한 인증 방법과 위치 탐지 기법들이 소개되고 있으나 개인인증의 보안성 문제와 실내·외를 전반적으로 확인할 수 있는 시스템은 찾아보기 어려웠다. 본 연구에서는 프라이빗 블록체인 환경에서 손목 밴드 형태의 웨어러블 디바이스를 활용해 개인인증, 기본적 건강 상태 파악 및 실내·외의 전반적인 위치를 파악할 수 있는 시스템을 제안하고자 한다. 이 시스템에서 개인인증은 위변조가 어렵고 개인식별성이 높은 ECG를, 실내는 저전력, 비접촉 및 자동 송수신 방식으로 사용이 용이한 블루투스 비콘을, 실외는 GPS 위성의 의사거리 오차를 보정한 DGPS를 활용하여 치매환자의 위치를 파악함으로써 배회행동 및 이상징후를 탐지하고자 한다. 이를 통해 재가나 요양시설 등에서 생활하는 치매환자의 배회행동 및 이상징후 시 신속한 대처와 실종예방에 기여하고자 한다.

보건진료원 직무수행에 영향을 미치는 요인에 관한 연구 - 경기도 관내 보건진료원을 중심으로 - (A Study on the Factors which influenced the Performance of Community Health Practitioners' Function -Around the CHPs in Kyonggi-province Area-)

  • This study was done in order to analyze the factors which influenced the performance level of community health practitioners' task. Interview survey was done during the period from August to October, 1986. Interviewee were 166 CHPs among total of 217 CHPs in Kyonggi province area. Multiple stepwise regression and canonical correlation analysis were used to identify major factors influenced to perform community health practitioners' task. The results of this study were summarized as follows: 1. General characteristics of CHPs 1) Personal characteristics The average age of CHPs was 37.8 years and their marital status was $77.6\%$ of married, educational back-ground was $65.3\%$ of junior college graduation. Their job career was $38.6\%$ of between 1-3 years, $33.3\%$ of between 3-5 years, $22.2\%$ of less than 1 years. Most of CHPs$(62.8\%)$ were fully satisfied with their job, $33.3\%$ were moderately, and $3.8\%$ were not satisfied. 2) Working environmental condition Only $31.7%$ of CHPs were satisfied with their working condition of primary health post, $26.6\%$ were not satisfied. Half of CHPs$(52.5\%)$ replied having good cooperation with health center, $10.1\%$ replied bad. Cooperation with health subcenter was good in $32.9\%$, and bad in $21.9%$. Cooperation with private health institutions was good in $34.2\%$, bad in $21.6%$. 2. Performance level of community health practitioners' task Among a total of 52 contents of their functions medical history taking. physical examination, referral of diagnostic laboratory work-up($(86.4\%)$, health assessment of pregnant women$(82.1\%)$, development of health information system$(79.4\%)$, supervision of health workers $(78.4\%)$, follow-up of family planning acceptors$(77.3\%)$, and follow-up of family planning acceptors' side effects$(77.3\%)$ were actively performed. Diagnosis of pregnancy$(62.1\%)$, sampling of drinking water for quality test$(52.5\%)$, making list of equipment' & supplies $(51.5\%)$, evaluation of primary health post activities $(37.6\%)$, organization of village health workers$(32.4\%)$ and management of village health workers $(30.1\%)$ were poorly performed. 3. Stepwise multiple regression analysis of job function The factors which influenced the performance level of community health practitioners' function were age, marital status, educational level, job career, job satisfaction, satisfaction of working environment of primary health post, cooperation of health center, cooperation of health center, cooperation of private health instiutions in orders. These 9 variables were able to explain job function from $25.7\%$ of program planning to $6.7\%$ of management of common disease. 4. Canonical correlation analysis between the performance of function and general characteristics of CHPs. Cooperation of private health institutions was found to be the factor influencing task performance of community organization, management of primary health post, technical supervision of health personnels. Job satisfaction of CHPs was also found to be the factor influencing task performance of family planning, management of common disease and maintenance of health information system.

임상병리사의 직무스트레스의 수준에 따른 하위영역의 실태조사 (Investigation of Subcategories according to the Level of Job Stress in Medical Technologist)

  • 본 연구는 임상병리사를 대상으로 직무스트레스의 정도를 파악하고, 이들 간의 상관성을 규명함으로써 직무스트레스의 감소를 위해 건강을 높일 수 있는 방안을 마련하는데 기초자료를 제공하고자 수행한 서술적 조사 연구이다. 임상병리사의 직무스트레스는 최대 100점에 49.74점으로 나타났고, 하부영역별로 직무불안정이 64.42점으로 가장 높았고, 조직체계가 52.46점, 보상부적절이 52.30점, 물리적환경이 51.15점, 직무 요구도는 49.57점, 직무자율성결여는 48.43점, 관계갈등은 39.78점 순이었고, 직장문화는 39.77점으로 가장 낮게 나타났다. 본 연구결과를 통해 임상병리사의 직무스트레스에 관련한 요인들의 상관성을 확인하였고 직무스트레스가 높은 군에서는 직무불안정을 감소시키고 건강증진에서의 신체활동을 증가시키는 활동이 요구되고, 이를 통해 직무스트레스 감소를 위해 절실한 프로그램의 개발이 요구되고. 건강상태 인자에 대한 정보를 향후 조사해야 할 것으로 사료된다.

요양시설 내 재활대상 노인의 지지가 죽음불안에 미치는 영향 (The effect of social support on elderly people for rehabilitation at nursing homes)

  • 본 연구는 요양시설 입소 노인의 죽음 및 불안과 관련하여 사회적 지지와의 상관성을 알아보기 위해 대구 경북에 소재하고 있는 노인 요양시설 6개를 임의 모집하여 노인환자 300명(남성: 183명, 여성:117명)을 대상으로 조사하였다. 연구의 결과는 사회적 지지에서 물질적 지지와 정보적 지지가 죽음 불안과 관련이 있는 것으로 나타났고 죽음 불안과 일반적 특성의 연관성에서는 고령일수록, 입소기간이 길수록 증가되는 것으로 나타났다(p<.05). 따라서 요양시설 입소 노인이 느끼는 죽음·불안에 대한 두려움을 개선하도록 하며 자아 존중감, 자아 통합감을 향상시키고 우울을 해결할 수 있도록 전문적인 상담과 프로그램을 개발하여 실시하는 것이 필요하다.
