암환자의 요구 조사 도구 개발 (Development of Needs Assessment Instrument for the Patients with Cancer)

  • 목적 : 본 연구는 암환자의 요구를 포괄적으로 포함하면서 실용적으로 측정할 수 있는 도구를 개발하기 위해 시도되었다. 방법 : 기존의 문헌들과 함께 Wingate와 Lackey(1989)의 내용분석 결과를 근거로 정보요구, 이용가능한 자원, 신체적 요구, 정서적 요구, 영적 요고, 그리고 법적/경제적 요구의 6개 영역을 선정하였고, 각 영역에 대한 문항 내용은 암환자 대상의 요고, 조사연구들를 참고로 하여 작성한 후 간호학 교수 3인의 자문을 받아 문항의 내용을 수정, 추가 또는 삭제하면서 예비문항을 작성하였다. 이를 통해 정보요구 7문항, 이용가능한 자원 5문항, 정서 요구 4문항, 영적요구 5문항, 법적/경제적 요구 4문항, 신체적 요구 7문항의 총 32문항의 예비도구를 작성하였다. 예비 도구의 내용 타당도를 확인하기 위해 강원도 소재 대학병원의 간호사 중 평균 경력 10년 이상인 암환자를 간호하고 있거나 간호한 경험이 있는 수간호사와 책임 간호사 9명을 대상으로 실시하였고, 구성타당도와 신뢰도를 검증하기 위해서는 강원도 충정도 및 경기도 소재 종합병원에 근무하고 있는 간호사 116명을 편의표출하였다. 수집된 자료를 SPSS 10.0 WIN을 이용하여 요인분석 및 Cronbach ${\alpha}$값을 구하였다. 결과 : 1) 내용 타당도 분석 결과, 전문가 합의률이 55.4%로 낮은 '유언장 쓰는 방법을 알고싶다'는 문항이 삭제되어 31문항이 선정되었다. 2) 요인분석을 실시한 결과 신체적 요구, 정보 요구, 영적 요구, 정서 요구, 이용가능한 자원, 그리고 법적/경제적 요구의 6요인이 추출되었고 전체 설명력은 61.840%였다. 제 1요인인 신체적 요구와 제 2요인의 정보요구는 각각 25.354%와 10.903%의 설명력을 나타내어 암환자 요구의 주요 영역으로 규명되었다. 3) 도구의 신뢰도는 내적 일관성을 나타내는 Cronbach ${\alpha}$값은 .90이었다. 결론 : 본 연구에서 개발된 도구는 간호사로 하여금 암환자의 요구를 포괄적으로 사정할 수 있게 하고 궁극적으로는 암환자의 요구에 맞는 총체적이며 포괄적인 간호중재를 마련하는 근거로 활용될 것이다.

융합적 시대에서 종합병원 간호사의 임종기 치료, 연명치료, 사전의료의향서에 대한 지식과 사전의료의향서 작성에 대한 교육요구도의 관계 (Relationship of Knowledge of End-of-life care, Life-Sustaining Treatment, and Advance Directive with the Educational Needs regarding Advance Directives Writing among Nurses in General Hospital in Convergence Era)

  • 본 연구는 일 종합병원 간호사의 임종기 치료, 연명치료, 사전의료의향서에 대한 지식과 사전의료의향서 작성에 대한 교육요구도의 상관관계를 확인하는데 그 목적이 있다. 연구의 대상자는 서울시 소재 일 종합병원에 근무하는 임상경력 3개월 이상의 간호사 169명이다. 자료수집기간은 2018년 7월 19일부터 7월 26일까지였고, 수집된 자료는 SPSS/WIN 23.0을 이용하여 분석되었다. 간호사의 임종기 치료에 대한 지식은 8.56±1.61점(11점 만점), 연명치료에 대한 지식은 5.11±0.94점(6점 만점), 사전의료의향서에 대한 지식은 8.02±1.33점(9점 만점)이었고, 사전의료의향서 작성에 대한 교육요구도는 4.31±0.58점(5점 만점)이었다. 사전의료의향서 작성에 대한 교육요구도는 연명치료에 대한 지식수준이 높을수록 높았고(r=.182, p=.018), 사전의료의향서에 대한 지식수준이 높을수록 높은 것으로 나타났다(r=.234, p=.002). 본 연구결과는 향후 사전의료의향서 교육프로그램 개발을 위한 기초자료를 제공하는데 그 의의가 있다.

입원중인 조기 진통 임부의 신체적, 정서적 상태와 간호요구에 대한 연구 (A Study on the Physical and Emotional Status, and Nursing Needs of the Pregnant Women Hospitalized by Premature Labor.)

  • The purpose of this study was to find out physical and emotional status, and nursing needs of the pregnant women who were hospitalized by premature labor. The research respondents were 96 from four university hospitals located in Seoul, from June 30, 1996 to September 15, 1995. The research instrument was consisted of 14 items of physical status(discomforts) (Cronbach's=0.86), 17 items of emotional status (Cronbach's=0.89), 33 items of nursing needs (Cronbach's=0.94), and they were measured by 5 level of Likert Scale. The data were analyzed by frequency, percentage, mean standard deviation, ANOVA, Pearson correlation coefficient as the statistical techniques in the program of SPSS/$PC^+$. The findings were as follows : 1. The perception of physical status was mainly about physicl discomforts during the hospital stay. It included four categories about 'absolute bed rest' 3.48, 'hospital foods' 3.38, 'health care teams' 2.93, 'hospital environment' 2.83 in order of mean of discomforts. The most discomfortable one was "malodor by not doing personal hygiene." The next one was "urination and defecation on the bed using bedpan." 2. The perception of the emotional status was about negative mood related to 'fetus', 'hospitalization' perse, 'personal situation.' The highest score of negative mood was "I am afraid that the baby's condition will be bad if I deliver it before full term." The next one was "I am anxious about whether my baby will be in incubator if I deliver it before full term." 3. The highest mean score among items of nursing needs was "Nurses observe whether the labor come or not with concerns." The next one was "Nurses observe the fetal movement and check up the fetal heart sound." The lowest one was "Nurses help me when I need bedpan." 4. Nursing needs were consisted of four categories : professional, educational, emotional, and physical. The mean score of them was high in professional, educational, emotional, and physical need in order. 5. The physical status was related to "Experience of treatment for maintenance of pregnancy" and "Experience of hospitalization by premature labor". The emotional status was related to "Type of delivery" and "Type of habitation." 6. In the correlation of physical and emotional status, it showed positive correlation between them. The higher score of physical discomfort, the higher score of negative mood(r=0.5113, p=0.0001).

유방암 환자의 정보요구 사정 (Information Needs of Women with Breast Cancer)

  • Nurses play an important role in patient education, including providing patients with useful and appropriate information. The purpose of this study was to explore what particular types of information were important to women diagnosed with breast cancer. Sixty seven women with breast cancer an outpatient clinic in W Christian Hospital responded to the structured questionnaires developed by the investigator. Interviews and mail surveys were used in this study. The findings in this study showed that six kinds of information needs arose, ranked as the signs and symptoms of recurrence, the possibility of recurrence, the possibility of metastasis, treatment, diet and physical activities in daily living. Marital status, level of education, and level of monthly income were not related to level of informational need. Younger women rather than older women had a greater need to seek information(r=-.471, p<.01). There was a significantly negative relationship between duration of disease and the score on information needs(r=-.32, p<.05). The findings have implications that the opportunity to ask questions and have accurate information on the recurrence should be provided to women with breast cancer.

  • This study was designed to investigate present conditions and needs for ICT-based elderly group home remodeling by applying kinetic principle. to provide useful planning factor to make elderly housing more suitable for supporting elderly residents remodeling. This study surveyed 20 elderly care assistants and nurses who have employed at elderly group homes and presented them with 7 categories of 44 elderly home remodeling planning elements to check currently existing services in their elderly housing and evaluate their need for each service with regard to their residents' needs.

제왕절개 기왕력이 있는 질 분만(VBAC)에 관한 간호사의 인식정도와 교육안 (Korean Nurses Knowledge Concerning Vaginal Birth After Cesarean(VBAC) and its Educational Material)

  • The purpose of this study are 1. Evaluate the degree of knowledge of Vaginal Birth After Cesarean(VBAC) of Korean nurses. 2. To gather and develop educational material for VBAC. The sample was surveyed Korean Nurses knowledge about VBAC, from November 1998 through March 1999. For the data analysis, the SPSS computer program percentage and frequency were used for descriptive statistics. The x2 and the t-test were used to compare the results of the two sample groups. Open questions asked in the survey were sorted out by content, then displayed in chart form. For the education material, the Internet was the main source of information. Information on the Internet was provided by professional doctors and prenatal educational nurses. The results of the survey are as follows: 1. Out of 97 Nurses 15.3% answered that cesarean deliveries do not need to be performed after previous cesarean sections : however. 46.4% answered that cesarean sections must be performed after previous cesarean sections. 2. Of the nurses surveyed 14% had no knowledge or had never heard of VBAC. 3 Nurses did not have questions from patients concerning VBAC was 34.7%. This led to the conclusion that patients either do not have knowledge about VBAC or patients have no interest in the trial of labor. 4, Nurses indicated that their information about VBAC originated from other people's experiences (31%), Nursing School (25%), Media information (9%), and through literature review (6%). This data led to the conclusion that the knowledge about VBAC may not be extensive enough to counsel and guide patients who are willing to endure the trial of labor. 5. Nurses preferred hospital education programs to develop their knowledge concerning VBAC. Based upon survey, the conclusion was made that General Nurses and Maternity Nurses did not have knowledge about VBAC success rates and the possibility of a trial of labor. In order for nurses to help patients make decisions concerning VBAC, nurses have to gam more knowledge through hospital educational programs. Further more, the study suggests that through hospital educational programs, the possibility and importance of VBAC must be emphasized to nurses who work in maternity areas. Second, through prenatal educational programs, the possibility and importance of VBAC must be explained and emphasized to patients who had previous cesarean sections. Third, the clinical pathways of VBAC need to be developed. Fourth, each hospital needs to develop multi-disciplinary teams, consis-ting of obstetricians, risk management/quality management, staff registered nurses, and the director for perinatal services. This team can review cesarean section rates and help to increase the practice of VBAC.

가정간호 가족 돌봄 제공자의 요구도 (Identifying the Needs of Home Care Patient's Family Caregivers)

  • Purpose: The purpose of this study was to identify the needs of family caregivers of home care patients. Methods: A total of 40 caregivers had been selected from a hospital-based home care agency in Gyunggi province. The instrument developed by Hileman, Lackey, & Hassanein(1992) was modified to 55 items and used in this study. The instrument consists of 6 categories: informational, household, patient care, personal, spiritual, and psychological needs. Out of 29 analysed with descriptive statistics, Mann-Whitney U test, and Spearman correlation test using SPSS 14.0. Results: Most caregivers were females, with a mean age of $60.0{\pm}15.5$, 32.1% were spouses, and 92.9% were living with patients. Patient's activities of daily living score was very low, and 44% of patients had cerebrovascular disease. Caregiver's needs were moderate, and the greatest being personal need. There was a significant difference between caregiver's monthly income and needs. Conclusion: Home care nurses need to teach and support family caregivers with specific programs and services to meet the identified and unmet needs of caregivers of home care patients. In-home respite and institutional respite are recommended for family caregivers taking care of patients with chronic disease.

기본간호실습 교육 현황 및 실습교육 필요도 (Content and Educational Needs for Fundamental Nursing Practice)

  • Purpose: The purpose of this study was to identify content of Fundamental Nursing Practice (FNP) and the educational needs for nursing practice items in schools of nursing. Methods: Participants were 81 professors who were teaching FNP and 166 clinical nurses. The research questionnaire included 148 practice items, which were selected by content analysis of 7 FNP textbooks. Data were collected from May 20 to July 27, 2011 and analyzed using descriptive statistics and t-test with SPSS 17.0. Results: The research findings showed that : 1) the seven items included in the education content of all schools of nursing were putting on sterile gloves, checking radial pulse, checking blood pressure in the arm, Foley catheterization, injection preparation with ampule, intramuscular injection in the buttocks, and mixing intravenous fluid. Fifty items were included in the content in more than 80% of the schools. 2) Educational domains needing emphasis were medication, infection control, health assessment, oxygenation, urinary elimination, activity/positioning, and nutrition. There were significant differences in reported educational needs between professors and clinical nurses for 62.8% of practice items. Conclusions: Results indicated that standardization of FNP education contents using the findings of this study should lead to improvement in quality of FNP education.

주임간호사가 경험하는 직무와 관련된 어려움 - Parse의 인간되어감 연구방법 적용 - (Job-related Difficulties of Charge Nurses - Applied Parse's Human Becoming Research Method -)

  • Purpose: This study was done to identify the core concepts and structure of difficult experiences of charge nurses in general hospitals. Methods: Data were collected from August 1 to September 5, 2014. Participants were 10 nurses from general hospitals in D and S cities. Parse research method was applied to examine lived experiences of the participants. The procedure of study was based on dialogical engagement and data were analyzed by extraction-synthesis and interpreted heuristically. The research question was "Would you tell me about your difficult experiences as a charge nurse?" Results: Job-related difficulties of the charge nurses were conflicts due to ineffective communication, heavy workload, not feeling respected, role conflict, and struggling to overcome difficulties with sense of responsibility to the work. The conceptual integration was: to be in the process of being originated and transformed through connecting-separating and enabling-limiting with valuing. Conclusion: Structural conversion of these concepts reveals that charge nurses suffer from the job-related difficulties identified in this study, but they endure to proceed with a sense of mission. It is suggested that supportive interventions should be made to let charge nurses express their feelings, identify their needs, and develop self-care or coping skills for themselves.

암환자를 돌보는 일부 종합병원 간호사의 보완대체요법에 대한 인식도 (Survey of Perception of Complementary and Alternative Therapies (CAT))

  • Purpose: This study was done to investigate oncology nurses' perception, attitude, knowledge and practice of CAT and to describe nurses' needs for education on CAT Method: A cross-sectional descriptive study was conducted. A total of 270 oncology nurses working at four medical centers in Seoul and Kyungki participated in this study. They completed the questionnaire between December 2005 and February 2006. Results: Oncology nurses perceived CAT as possible nursing interventions, but felt that there was no conclusive evidence on effectiveness/safety. Of the participants 44.4% have used CAT for themselves, and 73.9% of them were satisfied after using CAT. Two biggest reasons for CAT use were symptom control and emotional support. However, 80.8% of the participants had not had any training related to CAT Both nurses who have used CAT and those who have been trained to practice CAT represented a more positive perception toward CAT (p=.000). The biggest perceived obstacles to the practice of CAT were concerns about side effects (71.2%) and lack of knowledge and skill (69.6%). The most favorable CAT modalities in both practice and training were mind-body medicine. Conclusion: Overall oncology nurses' perception toward CAT was encouraging. However a high number of the nurses reported lack of knowledge and skill in CAT Education and training strategy for CAT need to be developed.
