Objectives : The aim of His study is to investigate the properties of the patients treated by fasting therapy. Methods : We measured PIBW, BMI, BFR and WHR of the patients and investigated the purpose of fasting therapy, the cause of weight increase, the times of weight increase, the duration of fasting therapy and the number of times of fasting therapy. Result : 1 . The major age group was females in twenties. 2. The PIBW, BMI, BFR and WHR were highest in fifties. 3. In the purpose of fasting therapy, 'for beauty' was the most. 4. In the cause of weight increase, diet disorder was the most. 5. The number of times of fasting therapy was the most in thirties. Conclusions : Above study shows that evaluation of obesity is important and more researches into fasting therapy were required.
Purpose: This study aimed to investigate the prognostic value of lymph node ratio (LNR) in patients with locally advanced gastric cancer who received neoadjuvant chemotherapy. Materials and Methods: We retrospectively enrolled gastric cancer patients treated with neoadjuvant chemotherapy and curative surgery at the First Affiliated Hospital of Zhejiang University from 2004 to 2015 as the study cohort. Patients with the same inclusion criteria treated in 2016-2017 were enrolled as the validation cohort. Kaplan-Meier curves were assessed using the log-rank test to analyze the differences in overall survival (OS). Multivariate survival analysis was performed using the Cox proportional hazards model. The areas under the receiver operating characteristic curve of ypN and LNR categories for predicting the actual 3-year OS were compared. Results: A total of 265 patients were included in the proposal cohort. The median number of retrieved lymph nodes (rLNs) was 32. The number of positive lymph nodes (pLNs) increased as rLN increased (P=0.037), but the LNR remained relatively constant (P=0.462). The LNR was categorized into 4 groups according to the prognosis: ypNr0, node-negative with rLN>25; ypNr1, node-negative with rLN≤25 or 00.3. In the validation cohort of 43 enrolled patients, there was a clear distinction in OS that significantly (P<0.001) varied depending on the LNR values and LNR was the only independent prognostic factor in multivariate analysis (P<0.001). Conclusions: LNR was an independent prognostic factor for survival of patients with gastric cancer after preoperative chemotherapy and might be an alternative predictor for ypN stage.
Objective : We investigated the actual condition of oriental medical office in a public health center. Methods : We classified patients who visited an oriental medical office iin a public pealth center of Dalseong-gun, Daegu Metropolitan City during recent 3 years by KCPJMAIN(data processing system), according to location, age, time(month, year), sex, disease, insurance, etc. Results : As a local category, the residents in Hyeunpung-Myeon higher than 80% of the patients who visited the public health center. The patients aged over 60 occupied 80% of the patients. The frequency of the treatment was more than 10 in those patients aged over 60. The number of the patients was the highest in May when there was a change for the past 36months, whereafter the number has sustained decline for 3-5 months and repeated decreasing. As a gender category, the female inpatients were 10732(82%). This statistics shows that these musculoskeletal system disease occupied large part in them. As a heath insurance category, the patients who were insured by health care were 12454(96.30%). Conclusion : It should need to enable the rural residents who have difficulty benefitting from medical service to reach the service by making their access to the oriental medical office in public health center easier. Most of all, the support from both government and municipality should be urged to accomplish it. plus, it should be included not only boosting doctors' reliance but also improving the capability and services of doctors in public heath center. In conclusion, the treatment service in public health center should be diversified beyond musculoskeletal system disease and the identity transform of public health center should be needed to appeal to young generation.
Background: Survival of breast cancer patients depends on a number of factors which are not only prognostic but are also predictive. A number of studies have been carried out worldwide to find out prognostic and predictive significance of different clinicopathological and molecular variables in breast cancer. This study was carried out at Nuclear Medicine, Oncology and Radiotherapy Institute (NORI), Islamabad, to find out the impact of different factors on overall survival of breast cancer patients coming from Northern Pakistan. Materials and Methods: This observational retrospective study was carried out in the Oncology Department of NORI Hospital. A total of 2,666 patients were included. Data were entered into SPSS 20. Multinomial logistic regression analysis was performed to determine associations of different variables with overall survival. P values <0.05 were considered significant. Results: The mean age of the patients was 47.6 years, 49.5% being postmenopausal. Some 1,708 were ER positive and 1,615 were PR positive, while Her 2 neu oncogene positivity was found in 683. A total of 1,237 presented with skin involvement and 426 had chest wall involvement. Some 1,663 had > 5cm tumors. Lymph node involvement was detected in 2,131. Overall survival was less than 5 years in 669 patients, only 324 surviving for more than 10 years, and in the remainder overall survival was in the range of 5-10 years. Conclusions: Tumor size, lymph node metastases, receptor status, her 2 neu positivity, skin involvement, and chest wall involvement have significant effects whereas age and menopausal status have no significant effect on overall survival of breast cancer patients in Pakistan.
Objectives : This study was performed to investigate the characteristics and ADL(Activities of Daily Living) associated factors of elderly inpatients in long-term care hospitals. Methods : Data were collected from the nationwide data of 'Survey of Patients (2013-2014)' administerd by the Ministry of Health & Welfare. The data included in this study consisted of 27,606 cases of elderly inpatients in long-term care hospitals. Results : The survey scores for the elderly inpatients were as follows: 57.6% 'Needed much and total help' with ADL, followed by 26.6% who 'Needed much help', and 15.8% who 'needed minimal supervision' in long-term care hospitals. The ADL score was high in the following categories: women, old age, referred visit, health insurance type, not-recovered & death, transferred, corporate hospitals, small hospital size, low number of physicians per 100 beds, and high number of nursing staff per 100 beds. The inpatients with 'diseases of the nervous system', 'diseases of the circulatory system' and 'diseases of the genitourinary system' were more likely to have high ADL scores. Conclusions : The results of this study suggest that long-term care hospitals should provide active and proper care for patients with high ADL scores and improve medical personnel training as well provide more medical care.
Background/Aims: Intralesional steroid injections have been administered as prophylaxis for stenosis after esophageal endoscopic submucosal dissection. However, this method carries a risk of potential complications such as perforation because a fine needle is used to directly puncture the postoperative ulcer. We devised a new method of steroid intralesional infusion using a spray tube and evaluated its efficacy and safety. Methods: Intralesional steroid infusion using a spray tube was performed on 27 patients who underwent endoscopic submucosal dissection for superficial esophageal cancer with three-quarters or more of the lumen circumference resected. The presence or absence of stenosis, complications, and the number of endoscopic balloon dilations (EBDs) performed were evaluated after treatment. Results: Although stenosis was not observed in 22 of the 27 patients, five patients had stenosis and dysphagia requiring EBD. The stenosis in these five patients was relieved after four EBDs. No complications related to intralesional steroid infusion using the spray tube were observed. Conclusions: Intralesional steroid infusion using a spray tube is a simple and safe technique that is adequately effective in preventing stenosis (clinical trial number, UMIN000037567).
Objectives: This study was performed to emphasize improvement of the perception of dental hygienists' environmental working conditions by analyzing various factors. Methods: Two hundred ninety-eight female dental hygienists working at 35 medical institutions in Daegu City and Gyeongsangbuk-do Province participated. A self-recording type survey was done in October 2016. Results: Factors influencing the perception of infection control was length of career. Dental hygienists with more experience have a higher level of perception. Factors influencing the perception of working posture was the number of patients per day and career level. Dental hygienists seeing 50 or fewer patients per day and senior staff showed higher levels of this perception. Factors influencing the perception of noise in the working area were length of career and lunchtime break. The perception level was high in the group with longer careers and dental hygienists who rest at lunch time. Factors influencing chemical exposure during work were the number of patients per day and the type of institute. Dental hygienists working in dental clinics or dealing with more patients had lower levels of perception. Conclusions: The sociodemographic and working characteristics of dental hygienists influence the perception of dental hygienists' environmental working conditions.
Purpose: The aim of this study was to identify the effect of Dongchimi juice containing kimchi Lactobacillus as an oral hygiene agent and to compare it with that of chlorhexidine solution (0.12% dilution). Methods: This study employed a pretest-posttest experimental design in which a single group of patients was exposed to two different treatments over a period of time. The study included 32 patients hospitalized at a longterm care hospital in Korea. Data were collected between August 12, 2016 and September 28, 2016. The patients first used chlorhexidine solution as an oral care agent for 1 week. After an interval of 2 weeks, they used Dongchimi juice for 1 week. Each agent was applied 2 times a day depending on the protocol. The oral status of the patients was measured using Beck's Oral Exam Guide (OEG) scores. The number of pathogens in the oral cavity was counted by culture, and the patients' subjective satisfaction score for each oral agent was measured using a visual analogue scale. T-test and Mann-Whitney test were performed to identify significant differences between Dongchimi juice and chlorhexidine solution by using PASW Statistics for Windows, Version 18.0 (SPSS Inc., Chicago, IL, USA). Results: The OEG score was not statistically different with the use of chlorhexidine solution and Dongchimi juice. However, decreasing number of pathogens and the subjective satisfaction score were higher with Dongchimi juice than with the chlorhexidine solution. Conclusion: These findings support the use of Dongchimi juice containing kimchi Lactobacillus as an oral hygiene agent for Korean patients.
Background: Readmissions related to lack of quality care harm both patients and health insurance finances. If the factors affecting readmission are identified, the readmission can be managed by controlling those factors. This paper aims to identify factors that affect readmissions of convalescent rehabilitation patients. Methods: Health Insurance Review and Assessment Service claims data were used to identify readmissions of convalescent patients who were admitted in hospitals and long-term care hospitals nationwide in 2018. Based on prior research, the socio-demographics, clinical, medical institution, and staffing levels characteristics were included in the research model as independent variables. Readmissions for convalescent rehabilitation treatment within 30 days after discharge were analyzed using logistic regression and generalization estimation equation. Results: The average readmission rate of the study subjects was 24.4%, and the risk of readmission decreases as age, length of stay, and the number of patients per physical therapist increase. In the patient group, the risk of readmission is lower in the spinal cord injury group and the musculoskeletal system group than in the brain injury group. The risk of readmission increases as the severity of patients and the number of patients per rehabilitation medicine specialist increases. Besides, the readmission risk is higher in men than women and long-term care hospitals than hospitals. Conclusion: "Reducing the readmission rate" is consistent with the ultimate goal of the convalescent rehabilitation system. Thus, it is necessary to prepare a mechanism for policy management of readmission.
Objectives: The purpose of this study is to analyze the recent trends in patients with work-related musculoskeletal disorders in South Korea and to check the major results by reviewing the literature on the risk factors and prevalence of musculoskeletal diseases related to work. Methods: Industrial disaster data from the Ministry of Employment and Labor from 2012 to 2021 were used, and the literature was reviewed regarding risk factors for musculoskeletal diseases related to work using PubMed and RISS. Results: The trend of patients with work-related musculoskeletal disorders has increased overall since 2017 after declining until 2016, with a particularly notable increase in the average annual number of patients with physical burden work. The average annual rate per ten thousand people for patients with body burden work, non-accidental lower back pain, and carpal tunnel syndrome among work-related diseases was high in the mining industry. The average annual rate per ten thousand people for patients with accidental lower back pain was the highest in the fishing industry. Within the manufacturing field, it was the highest in the shipbuilding and ship repair industry. As a result of the literature review, the search rate for work-related musculoskeletal disease papers in unstructured work was high. In addition, physical stress factors were high among risk factors, and pain areas showed a high rate for the waist. Conclusion: Even after the institutional implementation of a hazard investigation system related to musculoskeletal diseases is implemented, the number of patients with occupational musculoskeletal disorders continues to increase. Therefore, it is necessary to conduct regular surveys and implement effective improvement activities for vulnerable industries or occupations.
