  • Motion of the metatarsophalangeal joints is essential for the normal gait. Therefore it is important to recover the motion of normal joint in the treatment of stiffness of the metatarsophalangeal joints. However, there have been no report about the treatment of stiffness of the four lateral lesser metatarsophalangeal joints yet. We report an experience that good clinical and radiographic results were obtained after resection arthroplasty for the post-traumatic stiffness of the four lateral lesser metatarsophalangeal joints.

Effects of Random Visual and Auditory Stimulation on Walking of Healthy Adults (무작위 시청각자극이 건강한 성인의 보행에 미치는 영향)

  • Purpose: The purpose of this study is to investigate the effect of visual and auditory stimulation randomly applied to healthy adults on walking. Design: Randomized Controlled Trial. Methods: Twenty-six healthy students in S college were randomly divided into visual feedback group (n=13) and auditory feedback group (n=13). The visual feedback group walked using four conditions. 1) In the red screen was shown, clap twice to the right, 2) In yellow screen, clap twice to the left, 3) In green screen, clap twice over head. 4) Do not clap in purple screen. The auditory feedback group walked using four conditions. 1)in red, clap twice to the right, 2) In yellow, clap twice left, 3) In green, clap twice over your head. 4) Do not clap in purple. All subjects measured gait variables before and after the test using G-walker. Result: The visual feedback group showed a significant decrease (p<.05) in the number of steps per minute, walking speed, and step length compared with that of normal walking. The auditory feedback group showed a significant decrease (p<.05) in the number of steps per minute, walking speed, and step length than that of normal walking. Conclusion: The results of this study suggest that visual and auditory stimulation applied to healthy adults may have significant effects on walking.

Change of lower limb muscle activation according to the use of arm sling in normal subjects (정상인의 팔걸이 사용에 따른 보행 시 하지 근 활성도의 변화)

  • Background: The purpose of this study was to investigate the change of lower limb muscle activation according to the use of arm sling in normal subjects. Design: Cross-sectional Study Methods: Seven healthy subjects (6 males and 1 female, 25.42 years, 173.57 cm, 71.71 kg) were recruited on a voluntary basis. To measure the lower limb muscle activation during walking with and without arm sling, we used a wireless surface electromyography (sEMG) (FreeEMG1000, BTS Bioengineering, Milano, Italy). Six wireless sEMG electrodes were attached to the following three major muscle groups of the both side lower limb: rectus femoris, biceps femoris, medial gastrocnemius. All subjects wore arm sling on their right side during measurement. Results: In the stance phase, there was a significant increase in right side rectus femoris muscle activation in walking without arm sling compared to the walking with arm sling (p<.05). Additionally, In the stance phase, there was a significant increase in left side tibialis anterior muscle activation in walking without arm sling compared to the walking with arm sling (p<.05). Conclusion: The results of this study suggest that there is a significant association between the arm swing restriction and lower limb muscle activation. Therefore, it seems that it can be applied as basic data for gait training with an arm slings.

A Familial Case of Tetramine Intoxication from Neptunea (고둥 섭취 후 발생한 일가족 테트라민 중독 1례)

  • Certain parts of shellfish contain poisonous substances and cause intoxication. Tetramine toxin is found in the salivary gland of Neptunea. Three family members were admitted to the hospital with chief complaints of dizziness and blurred vision, gait disturbance, and spasms of the lower extremities after ingesting Neptunea. Physical examination revealed sluggish pupil light reflexes, but laboratory studies were normal. Symptoms were completely resolved within 24 hours after injection of atropine. We report a case of three patients with dizziness and blurred vision, gait disturbance, and spasms of the lower extremities due to Neptunea tetramine toxin.

  • People with somatoform disorder have a number of different symptoms that typically last for several years. Their symptoms can't be traced to a specific physical cause. In people with somatoform disorder, medical test results are either normal or don't explain the person's symptoms. The symptoms of somatoform disorder are similar to the symptoms of other illnesses. People with this disorder may have several medical evaluations and tests to be sure that they don't have another illness. In this case, we described a 56-year old man who was diagnosed as R/O) ALS, because he has many similar symptoms of ALS (amyotrophic lateral sclerosis) such as gait disturbance, dysarthria, dyspnea. But we diagnosed his case as somatoform disorder and treated with herbal medication, acupuncure treatment ,negative cupping and relaxation training. Through these treatment, the patient showed improvement of chief complains and accessory symptoms.

  • Measurements of plantar pressure provide an indication of foot and ankle function during gait and other functional activities because the foot and ankle provide necessary support and flexibility for weight bearing and weight shifting while people are performing these activities. Plantar pressure is being increasingly used in both research and clinical practice to measure the effects of various footwear and physical therapy intervention. The influence of walking speed and ground inclination on plantar pressure parameters However has not been evaluated in detail. So, in this study to determine the effect of changes in walking speed and ground inclination on plantar pressure treadmills with different walking speeds and inclination were used. Plantar pressure parameters were measured with the Parotec system using the walking and running in 20 healthy participants(10 male, 10 female) aged $20{\sim}28$(mean 22.22, S.D.2.26 years) when slow walking and running. The result of this study with increased die walking speed, the peak pressure of 1st, 5th metatarsal head and total contact time and impulse total at the forefoot was affected by walking speed; however, die peak pressure, contact time and impulse total at the forefoot was not affected by ground inclination.

Wearable and Motorized Crutch Control System (착용형 전동 목발 제어시스템)

  • This paper proposes a wearable and motorized crutch control system for the patients using the conventional crutches. The conventional crutches have a few disadvantages such as the inconvenience caused by the direct contact between the ground and the armpit of the patients, and unstable gait patterns. In order to resolve these problems, the motorized crutch is designed as a wearable type on an injured lower limb. In other words, the crutch makes the lower limb to be moved forward while supporting the body weight, protecting the lower limb with frames, and rotating a roller equipped on the bottom of the frames. Also the crutch is controlled using the electromyography and two force sensing resistor (FSR) sensors. The electromyography is used to extract the walking intention from the patient and the FSR sensors to classify the stance and swing phases while walking. As a result, the developed crutch makes the patients walk enabling both hands to be free, as if normal people do.

  • In this study, a transfemoral prosthesis system of which both stance phase and swing phase are controllable has been developed for the recovery of the biomechanical function of the amputated leg. It consists of a 5 bar link mechanism, a hydraulic-rubber knee damper for stance phase control and a pneumatic cylinder controlled via a microprocessor for stance phase control. The mechanical characteristics of the knee damper which absorbs the impact energy generated at the heel contact were investigated. The characteristics of the pneumatic cylinder essential for the speed adaptation of the prosthesis during swing phase were also studied for its mechanical characteristics. The prosthesis was subject to the clinical tests, and the gait characteristics obtained were very close to those of normal subjects. The stance and swing controlled prosthesis that were developed in this study showed good stability during the stance phase and showed good controllability during the swing phase.

Three-Dimensional Contact Dynamic Model of the Human Knee Joint During Walking

  • It is well known that the geometry of the articular surface has a major role in determining the position of articular contact and the lines of action for the contact forces. The contact force calculation of the knee joint under the effect of sliding and rolling is one of the most challenging issues in this field. We present a 3-D human knee joint model including sliding and rolling motions and major ligaments to calculate the lateral and medial condyle contact forces from the recovered total internal reaction force using inverse dynamic contact modeling and the Least-Square method. As results, it is believed that the patella, muscles and tendon affect a lot for the internal reaction forces at the initial heel contact stage. With increasing flexion angles during gait, the decreasing contact area is progressively shifted to the posterior direction on the tibia plateau. In addition, the medial side contact force is larger than the lateral side contact force in the knee joint during normal human walking. The total internal forces of the knee joint are reasonable compared to previous studies.

  • The prevalence of knee osteoarthritis was higher people with lower limb amputation. This was identified that transfemoral amputees have a greater external knee adduction moment than ablebodied subjects by biomechanical studies. Therefore, they need rehabilitative intervention for prevention and reduction of knee osteoarthritis. The purpose of this study was to determine the effect of lateral wedge insole used in the treatment of knee osteoarthritis. This study was participated in fourteen unilateral transfemoral amputees and we were analyzed the difference gait variables between without lateral wedge insole and with $5^{\circ}$ and $10^{\circ}$ lateral wedge insole during gait. Our results showed that step length ratio was more symmetrical and, hip adduction and ankle inversion angle were more close to normal value, and knee adduction moment was decreased as the wedge angle increases. We proposed that these data would be utilized conservative treatment of knee osteoarthritis in lower limb amputees.
