Objective: This study was performed to evaluate functional changes of occlusion after orthodontic treatment by measuring the occlusal force (OcFr) and occlusal contact area (OcAr), and to compare OcFr and OcAr change according to premolar extractions. Methods: Data were obtained from 74 patients who had finished orthodontic treatment using fixed appliance aged between 18 and 40 years. Subjects were divided into groups who had four premolars extractions or non-extraction (Male extraction-16, Male nonextraction-18, Female extraction-19, Female nonextraction-21). All subjects were asked to bite pressure-sensitive sheets into maximum intercuspation with maximum bite force, and OcFr and OcAr were evaluated by measuring the sheet with a CCD camera. Records were taken right after debonding, 1 week, 1 month, 3 months, 6 months and 1 year after debonding. Results: OcFr and OcAr increased gradually in all groups during the 1 year retention period (p < 0.05). Male groups showed higher OcFr and OcAr than female groups throughout the retention periods (p < 0.05). There were no statistically significant differences of OcFr and OcAr between extraction and non-extraction groups in both males and females (p > 0.05). Conclusions: Occlusion was improved functionally throughout the 1 year retention, and premolar extraction did not induce a decline in the functional aspect of occlusion.
Alveolar crest is the section of interproximal alveolar bone which includes the free edge of the alveolar process. An increase of the normal forces within limits of tolerance leads to deposition of new bone. If forces are beyond the limits of tolerance, resorption of bone will result whether the force produces pressure or tension. This study was designed to evaluate changes of alveolar bone levels in mesial and distal surface of the left, right first molar, by using pre-treatment, post-treatment panorama films. Two hundreds sixteen subjects were divided into adolescent group of 104 subjects and adult group of 112 subjects, to which orthodontic treatment with a bicuspid extraction (adolescent group-50 subjects, adult group-50 subjects) or without a nonextraction (adolescent group-54 !subjects, adult group-62 subjects) was applied by fixed appliances. Pre- and post-treatment Panorama films were traced, and alveolar crest height was measured. Amounts of changes in alveolar crest height by treatment were calculated md compared in terms of side of tooth, extraction, age. The results were as follows ; 1. When pre-treatment alveolar crest bone levels were compared, levels of adult group were significantly lower than those of adolescent group. 2. Post-treatment alveolar crest bone levels were significantly lower than pre-treatment levels. 3. When changes of alveolar crest height were compared, between adolescent and adult group were not significantly. 4. When changes of alveolar crest height were compared, significantly larger changes were noticed in ex윤action than nonextraction cases. 5. When changes of alveolar crest height were compared, significantly larger changes were noticed in maxilla than mandible. 6. When mesio-distally compared, significantly larger changes were observed in the distal than mesial sides of adult group.
Objective: This study aimed to investigate the current data regarding age, sex, and Angle Classification of Korean orthodontic patients and influence of these factors on the tendency to undergo extraction and orthognathic surgery. Methods: The recent trends of Korean orthodontic patients were assessed using questionnaire survey. The questionnaires were e-mailed to orthodontists who met the study criteria; 58% of the orthodontists opened the e-mails, and 27.7% replied to the e-mails. In all, the medical records of 11,340 patients who underwent orthodontic treatment at private clinics and small dental hospitals in Korea were analyzed. Results: The percentage of female patients in the study sample was 69.6%, and the average age of the patients was 19.87 years. The percentage of patients who were older than 19 years was 50.2%. Class II and Class III malocclusions were noted in 33.6% and 23.6% of patients, respectively. Extraction and orthognathic surgery were performed in 60.4% and 6.9% of patients, respectively. Conclusions: The results showed that there were a high percentage of adult, Class II malocclusion and extraction patients in private practices and small dental hospitals during the study period. Further, a relatively high proportion of adult patients opted to undergo orthognathic surgery.
Certain malocclusion are associated with specific "facial type," and it is important for the clinician to classify the common facial characteristic of each patient. Because the reaction to treatment mechanics and the stability of the denture is depended upon the analysis of the facial pattern. Basically, there are 3 district facial types or patterns under which almost all malocclusion can be classified. 1. mesofacial is the most average growth. 2. brachyfacial which is a horizontal growth pattern has a week muscle, with dental arch, deep bite. 3. dolichofacial which is a vertical growth pattern has a strong muscle, narrow dental arch, open bite. Brachyfacial pattern show a resistant to mandibular rotation during treatment can accept a more protrusive denture and are prominantly nonextraction, whereas dolichofacial patterns tend to open during treatment require a more retracted denture in order to assure post-treatment stability. Brachyfacial pattern would better treat to use extrusive force system, whereas dolichofacial pattern treat to use intrusive force system with head gear and intermaxillary elastics.
Most of orthodontic cases are treated with extraction of certain teeth, which influence the pre-eruptive movement of the lower third molar The purpose of this study was to evaluate the positional change of lower third molar following orthodontic treatment. Pre- and post-treatment pantomograms of 163 orthodontic patients (77 nonextraction group, 78 first premolar- extraction group, 8 second molar- extraction group) were analyzed in terms of the mesiodistal and buccolingual angles of lower third molar. The results were as follows. 1. The change of the mesiodistal angle of lower third molar by orthodontic treatment was significant in second molar-extraction group. 2. The mesiodistal angle of lower third molar in pre-treatment was significantly correlated to the mesiodistal angle in post-treatment and/or the change of the mesiodistal angle by treatment. 3. The change of the buccolingual angle of lower third molar by orthodontic treatment was significant in non -extraction group or first premolar-extrction group. 4. The change of the buccolingual angle of lower third molar by treatment was significantly correlated to the mesiodistal angle in post-treatment, the change of the mesiodistal angle by treatment, the buccolingual angle in pre-treatment or the buccolingual angle in post-treatment.

Objective: The purpose of this study was to compare the skeletal, dental, and soft-tissue treatment effects of nonextraction therapy using the modified C-palatal plate (MCPP) to those of premolar extraction (PE) treatment in adult patients with Class II malocclusion. Methods: Pretreatment and posttreatment lateral cephalographs of 40 adult patients with Class II malocclusion were retrospectively analyzed. The MCPP group comprised 20 patients treated with total arch distalization of the maxillary arch while the PE group comprised 20 patients treated with four PE. Fifty-eight linear and angular measurements were analyzed to assess the changes before and after treatment. Descriptive statistics, paired t-test, and multivariate analysis of variance were performed to evaluate the treatment effects within and between the two groups. Results: The MCPP group presented 3.4 mm of retraction, 1.0 mm of extrusion, and $7.3^{\circ}$ lingual inclination of the maxillary central incisor. In comparison, the PE group displayed greater amount of maxillary central incisor retraction and retroclination, mandibular incisor retraction, and upper lip retraction (5.3 mm, $14.8^{\circ}$, 5.1 mm, and 2.0 mm, respectively; p < 0.001 for all). In addition, the MCPP group showed 4.0 mm of distalization and 1.3 mm of intrusion with $2.9^{\circ}$ distal tipping of the maxillary first molars. Conclusions: These findings suggest the MCPP is an effective distalization appliance in the maxillary arch. The amount of incisor retraction, however, was significantly higher in the PE group. Therefore, four PE may be recommended when greater improvement of incisor position and soft-tissue profile is required.
Objective: The purposes of this study were to provide an epidemiologic data base related to the orthodontic treatment need and to know the changing trends about treatment modality of private orthodontic clinics. Methods: Distribution, trends and orthodontic treatment plan of malocclusion patients were investigated in 1,620 consecutive patients who had been visited and examined in 4 private orthodontic clinics located in Seoul from 2003-2006. Results: Percentage of male and female patients was 26.9% and 73.1% respectively Age distribution had shown that percentage of the patients above 13 years was 78.9%, and above 19 years was 59.0%. Average age of whole patients was 20.5 years. With regard to Angle classification, each percentage of Class I, Class II division 1, Class II division 2 and Class III malocclusion was 38.9%, 34.8%, 2.3% and 24.0%. The percentage of extraction cases(00.9%) outnumbered nonextraction cases(39.1%) and 46% of extraction cases were upper and lower 1st premolar extraction cases. Patients who had chose treatment with fixed appliance and orthognathic surgery was 10.2%. Conclusions: Because the high percentage of adult, Class II malocclusion and orthognathic surgery cases in patients of private orthodontic clinics were shown in this study, orthodontic education program and national health policy in Korea need reformation.

Objective: The purpose of this study was to examine the effectiveness and mechanism of clear aligner therapy for the correction of anterior open bite in adult nonextraction cases. Methods: Sixty-nine adult patients with anterior open bite were enrolled and classified into Angle's Class I, II, and III groups. Fifty patients presented with skeletal open bite (mandibular plane angle [MPA] ≥ 38°), whereas 19 presented with dental open bite. Fifteen cephalometric landmarks were identified before (T1) and after (T2) treatment. The magnitudes of planned and actual movements of the incisors and molars were calculated. Results: Positive overbite was achieved in 94% patients, with a mean final overbite of 1.1 ± 0.8 mm. The mean change in overbite was 3.3 ± 1.4 mm. With clear aligners alone, 0.36 ± 0.58 mm of maxillary molar intrusion was achieved. Compared with the Class I group, the Class II group showed greater maxillary molar intrusion and MPA reduction. The Class III group showed greater mandibular incisor extrusion with no significant vertical skeletal changes. Conclusions: Clear aligners can be effective in controlling the vertical dimension and correcting mild to moderate anterior open bite in adult nonextraction cases. The treatment mechanism for Class III patients significantly differed from that for Class I and Class II patients. Maxillary incisor extrusion in patients with dental open bite and MPA reduction with mandibular incisor extrusion in patients with skeletal open bite are the most significant contributing factors for open bite closure.
This study was investigated to assess the difference of facial height and occlusal plane inclination between pre- and post-treatment in Class I malocclusion group The subjects consisted of 35 extraction patients and 30 nonextraction patient,;, and was subdivided into Group I(overbite<0mm), Group II(04mm) in reference to overbite, and adolescent group and adult group in reference to age. Lateral cephalogram was taken with standard method, traced, and digitized for each subject. The computerized statistical analysis was carried out with SAS program The results wolf as follows. 1. In both groups of extraction and nonextraction group the anterior facial height increased after orthodontic treatment but there was no significant difference(p>0.05) between each goup. 2. There was no statstical significance in change of occlusal plane inclination in adolescent group, but significant difference(p<0.05) among three subgroup in adult group. 3. In adolescent-extraction and adolescent-nonextraction group there was significant increase of anterior facial height and posterior facial height, and was superior to adult groups in posterior facial height increment. 4 In all groups upper and lower molars were uprighted to occlusal plane. This had statistically significant effect.
One of the strenuous problems in orthodontic procedures is postretention stability and retention against relapse. Many investigative trial had been done to disclose the factors associated with relapse and effective prescription to stave off, however, the nature of these jeopardies remained obscure. The objective was to investigate the long-term stability and quantitative changes of dental arches subsequently after Class I nonextraction treatment. Study models,cephalometric headfilms of 26 samples which were taken before, after teatment and postretention were employed to measure the interdental width of corresponding buccal teeth,overbite,overjet and the inclination of incisors and molars. Statistical analysis was carried to compare each measurements across the time period, and followings were brought around. 1. The quantitative amount of relapse in overbite presented positive correlation with the amount of changes through the treatment. 2. Stability of intercanine width was so far secure in the case the expansion had been done through. 3. The amount of changes in intercanine width of the lower regardless of expansion or contraction manifested less than the upper, however, the relapse ratio got high. 4. The upper and lower incisors were likely to be labioversive, and remained stable after retention. 5. The first molars of the upper and lower were conceivably tipped back immediately after treatment and returned to the original angulation. The expansion of intermolar width stayed stable across the time scheme after treatment It was suggested that the maintenance of intercanine width of lower was pertinent to perform the postretention stabilityv and the expansion of dental arch shoed be confined within physiologic boundaries of the patients.
