Granuloma Formation, a Rare Complication after PDO Threads Lifting, and Adjuvant Treatment Using Dual-Frequency Ultrasound (LDM®-MED)

  • Thread-lifting is a minimally invasive procedure that shows good results and fewer complications as compared with those results and complications of standard surgery. Many procedures and techniques have been developed to create a younger appearance of facial/neck skin for aging people, and the demand for an improved aesthetic appearance is increasing. Since the incidence of side effects is much less than that of non-absorbable threads, which can lead to complications such as foreign body reactions, polydioxanone (PDO) threads are predominantly used for face lift procedures. A 66-year-old woman presented to our clinic with inflamed palpable masses. She had undergone a face lift with absorbable threads in our clinic 5 months previously. Excisional biopsy was performed with the patient under local anesthesia. During the operation, any threads were not detected and there was both fibrotic scar tissue and granulomatous tissue. For effectively promoting healing and managing the scars, treatment with LDM®-MED was performed on the day after surgery. The treatment was performed according to the author's protocol. Although foreign body granuloma as a complication after using non-absorbable thread types have been previously reported, it is relatively rare to find this type of complication after using absorbable thread. In this report, we present a case in which a 66-year-old female with foreign body granuloma after undergoing a face lift using absorbable threads was treated with the application of dual-frequency ultrasound, which promoted wound healing.

적외선 체열진단을 이용한 안면마비와 안면과 상지에 분포한 경혈위와의 관계에 대한 임상고찰 (A clinical study on the relation between facial paralysis and acupoints on the face and the upper limbs by the use of DITI diagnosis)

  • Objectives : this study is to see if there is a significance in thermal differences of acupoints in diagnosis and treatment period of facial paralysis and to substantiate the validity of acupuncture and moxibustion treatment for it. Methods : 1. By using DITI, thermal differences of acupoints on the face and the upper limbs of 13 Bell's palsy patients were measured around 3 days after an attack of the disease. These 13 patients, whose treatment progress was monitored up to 6 months after attack, were among the inpatients and outpatients of oriental internal medicine of National Medical Center from July 1 to August 31. 2. The patients were divided into 1month, 2-3months, 4months, 6months groups according to the occasion of improvement and thermal averages of each treatment period measured. Results : When it takes within 1 month for the condition of facial paralysis to change for the better, DITI image shows the temperature of the affected face parts and arms is higher than that of the non-affected parts. However, when it takes more than 4 months, the temperature of the affected face parts and arms on DITI image is lower than that of the non-affected parts. Conclusions : Hereby, prognosis of the disease and necessary time for the treatment can be presumed through DITI screening after an occurrence of facial paralysis. Also, condition of the disease is reflected by thermal differences of acupoints for Bell's palsy treatment that are in accordance with the theory of meridian on the face. This supports the efficacy of acupuncture and moxibustion treatment for this disease.

  • 2020년 2월 COVID-19의 1차 대유행 이후 국내외적으로 원격의료에 대한 수요가 크게 증가하였고, 이에 대부분의 국가에서는 원격의료 관련 규제를 완화하였다. 우리나라 역시 2월 이후 한시적으로 '비대면진료'라는 이름으로 원격의료를 허용하고 있다. 원격의료는 만성질환 관리에 매우 효과적이고 정보통신기술의 발전에 따라 적용 가능한 진료과목이 점차 확대되고 있어, 의료서비스 질 향상 및 새로운 진료모형의 창출에 기여할 것으로 기대된다. 그러나 원격의료 참여를 매개로 한 의료공급자 간 차별화 효과를 낳을 수 있고 이는 대형병원 쏠림의 왜곡된 의료서비스산업구조의 문제점을 더욱 악화시킬 우려가 있다. 따라서 일차 진료기관을 중심으로 한 만성질환 관리 주치의제도의 정착을 통해 원격의료의 확대와 지역의료기반의 안정화를 동시에 도모하고, 상급종합병원 및 외국인환자유치기관에 대해서는 재외국민과 외국인 대상 원격의료 제공 능력을 지원하는 이원적 지원 정책 추진이 필요하다.

웃음 치료 훈련을 위한 웃음 표정 인식 시스템 개발 (Development of a Recognition System of Smile Facial Expression for Smile Treatment Training)

  • 본 논문은 실시간 카메라 영상으로부터 얼굴을 검출하고 얼굴 표정을 인식하여 웃음 치료훈련을 할 수 있는 시스템을 제안한다. 제안된 시스템은 카메라 영상으로부터 Haar-like 특징을 이용하여 얼굴 후보 영역을 검출한 다음, SVM분류기를 이용하여 얼굴 후보 영역이 얼굴 영상인지 아닌지를 검증한다. 그 다음에는 검출된 얼굴 영상에 대해, 조명의 영향을 최소화하기 위한 방법으로 히스토그램 매칭을 이용한 조명 정규화를 수행한다. 표정 인식 단계에서는 PCA를 사용하여 얼굴 특징 벡터를 획득한 후 다층퍼셉트론 인공신경망을 이용해 실시간으로 웃음표정을 인식하였다. 본 논문에서 개발된 시스템은 실시간으로 사용자의 웃음 표정을 인식하여 웃음 양을 화면에 표시해 줌으로써 사용자 스스로 웃음 훈련을 할 수 있게 해 준다. 실험 결과에 따르면, 본 논문에서 제안한 방법은 SVM 분류기를 통한 얼굴 후보 영역 검증과 히스토그램 매칭을 이용한 조명정규화를 이용하여 웃음 표정 인식률을 향상시켰다.

  • The O2O services in the healthcare sector have only been in full swing for about three years, and unlike existing O2O consumer goods, the scale and scope of the dispute are more complicated due to restrictions on medical treatment. In this study, O2O service platform operators and medical institutions' roles and responsibilities were redefined as a countermeasure for resolving disputes in healthcare O2O services and the laws for changing the transaction environment. A change in institutional mechanisms was proposed. This study looked at the types of consumer disputes related to healthcare O2O services as insufficient information problems, problems in the course of medical service implementation, problems with immunity provisions for platform operators, cancellations, and non-compliance with refunds. All the information generated during transactions in the healthcare sector was extensive in scale and included the most sensitive information among personal information, stressing the importance of ensuring security. The area that started in the O2O range before the medical institution visit also proposed a plan to establish a system for the delivery of proven information as a pre-medical person. The scale and growth will grow faster, given that consumers can experience the information they want anytime, anywhere they want. However, the platform broker's role, a link player, will become more important because consumers who use the service will have their first meeting with non-face-to-face product providers. On the other hand, service providers may have side effects of misleading consumers by providing false information or misleading consumers through exaggerated advertisements. The O2O service market is expected to expand beyond distribution and dining out to the entire industry. However, since it is challenging to check accurate statistics on the detailed market, various disputes and consumer protection measures will be required for each detailed market, and comprehensive leading solutions will be essential in the future.

대학생의 일반적 특성 및 교정치료에 따른 자존감 비교연구 - 대구, 경북지역 대학생을 중심으로 - (A Comparative Study of University Student's Self-Esteem According to General Characteristics and Orthodontic Treatment)

  • Purpose: The goal of this study is to investigate the correlation of orthodontic treatment with self-esteem and how orthodontic treatment has an effect on university student's self-esteems by investigating and analyzing the difference in self-esteem of each general characteristic and the difference in self-esteem according to the existence or nonexistence of malocclusion and orthodontic treatment for adult university students. Methods: This study conduced a survey for 420 university students including dental related department, department of health and department of non-dental health in two universities located in Taegu, Gyeongbuk-do province from May 15, 2012 to May 30, 2012, and in 420 questionnaires of the investigated 420 target students, total 400(95.2%) questionnaires except for the questionnaires with falsely or erroneous responses were used for study analysis. Results: In the relations of general characteristic with self-esteem, for self-esteem by each gender, male student showed higher, for self-esteem by residential area, the university student residing at metropolitan area showed higher, for self-esteem by financial status, high class and middle class students in home financial status showed higher, for self-esteem by face shape, the student with triangular face shape showed higher, and in the relation of self-esteem with orthodontic treatment, the university student who is getting orthodontic treatment showed higher self-esteem statistically significantly. Conclusion: For gender, male university student, for residential area, the university student residing at metropolitan area, the high class university student in financial status, the university student with triangular face shape, and the university student who is getting orthodontic treatment showed statistically significantly higher self-esteem.

  • 4차 산업혁명의 발전으로 저 전력 근거리 무선통신을 활용한 기술들이 의료정보와 연계 적용되면서 의료정보에 관한 관심도가 증가하고 있다. 스마트 장비의 사용 연령층이 높아지고 보편화 되면서 모바일 기기와 다양한 사물, 장비 등을 연동하는 등의 관련 연구가 지속적으로 이루어지고 있다. 또한 각종 전염병을 비롯한 코로나바이러스 감염증-19(Coronavirus Disease 2019;COVID-19)이 유행함에 따라 언택트(비대면)에 대한 관심이 증가하고 있다. 이로 인한 조치로 사회적 뿐만 아니라 생활 속에서도 거리가 필요한 실정이다. 본 연구에서는 저 전력 근거리 무선 통신 기술인 비콘을 이용하여 IoT 디바이스를 만들어, 의료정보 시스템의 웹 서버와 통신하여 의료기관 방문 시 보다 쉽게 진료 접수 할 수 있도록 연구하였다.

Analysis of the Status of Artificial Medical Intelligence Technology Based on Big Data

  • The role of artificial medical intelligence through medical big data has been focused on data-based medical device business and medical service technology development in the field of diagnostic examination of the patient's current condition, clinical decision support, and patient monitoring and management. Recently, with the 4th Industrial Revolution, the medical field changed the medical treatment paradigm from the method of treatment based on the knowledge and experience of doctors in the past to the form of receiving the help of high-precision medical intelligence based on medical data. In addition, due to the spread of non-face-to-face treatment due to the COVID-19 pandemic, it is expected that the era of telemedicine, in which patients will be treated by doctors at home rather than hospitals, will soon come. It can be said that artificial medical intelligence plays a big role at the center of this paradigm shift in prevention-centered treatment rather than treatment. Based on big data, this paper analyzes the current status of artificial intelligence technology for chronic disease patients, market trends, and domestic and foreign company trends to predict the expected effect and future development direction of artificial intelligence technology for chronic disease patients. In addition, it is intended to present the necessity of developing digital therapeutics that can provide various medical services to chronically ill patients and serve as medical support to clinicians.

  • The outcomes of first-generation EGFR-TKIs (Gefitnib and Erlotinib) have shown great advantages over traditional treatment strategies in patients with non-small cell lung cancer (NSCLC), but unfortunately we have to face the situation that most patients still fail to respond in the long term despite initially good control. Up to now, the mechanism of acquired resistance to EGFR-TKIs has not been fully clarified. Herein, we sought to compile the available clinical reports in the hope to better understanding the subsequent treatment choices, particularly on whether restoring after a drug holiday or switching to another EGFR-TKI is the better option after failure of one kind of EGFR-TKI.

  • The stethoscope has long been used for the examination of patients, but the importance of auscultation has declined due to its several limitations and the development of other diagnostic tools. However, auscultation is still recognized as a primary diagnostic device because it is non-invasive and provides valuable information in real-time. To supplement the limitations of existing stethoscopes, digital stethoscopes with machine learning (ML) algorithms have been developed. Thus, now we can record and share respiratory sounds and artificial intelligence (AI)-assisted auscultation using ML algorithms distinguishes the type of sounds. Recently, the demands for remote care and non-face-to-face treatment diseases requiring isolation such as coronavirus disease 2019 (COVID-19) infection increased. To address these problems, wireless and wearable stethoscopes are being developed with the advances in battery technology and integrated sensors. This review provides the history of the stethoscope and classification of respiratory sounds, describes ML algorithms, and introduces new auscultation methods based on AI-assisted analysis and wireless or wearable stethoscopes.
