We design a reconfigurable optical node for metropolitan WDM networks, and numerically study the capability of the node in the optical signal level. Unlike a long-haul WDM system, major limitations of metropolitan WDM systems are power loss, fiber dispersion and optical signal-to-noise ratio(OSNR) degradation due to EDFAs. Therefore, we include the behaviors of transmitter and receiver, and fiber, EDFAs, and optical filters(MUX/DeMux) in numerical simulations with varying parameters over wide range. From simulation results, we can identify the maximum span numbers for OC-48 and OC-192 to achieve $BER<10^{-12}$ using the node structure at various received powers and residual dispersions.

Cognitive radio has been recently proposed to dynamically access unused-spectrum. The CR users can share the same frequency band with the primary user without interference to each other. Usually each CR user needs to determine spectrum availability by itself depending only on its local observations. But uncertainty communication environment effects can be mitigated so that the detection probability is improved in a heavily shadowed environment. Soft detection is a primary user detection method of cooperative cognitive radio networks. In our research, we will improve system detection probability by using optimal cooperative node selection algorithm. New algorithm can find optimal number of cooperative sensing nodes for cooperative soft detection by using maximum ratio combining (MRC) method. Through analysis, proposed cooperative node selection algorithm can select optimal node for cooperative sensing according to the system requirement and improve the system detection probability.

This paper proposes an low-complexity soft-output multiple-input multiple-output (soft-MIMO) detection algorithm for achieving soft-output maximum-likelihood (soft-ML) performance under max-log approximation. The proposed algorithm is based on a parallel tree-search (PTS) applying a channel ordering by a sorted-QR decomposition (SQRD) with altered sort order. The empty-set problem that can occur in calculation of log-likelihood ratio (LLR) for each bit is solved by inserting additional nodes at each search level. Since only the closest node is inserted among nodes with opposite bit value to a selected node, the proposed node insertion scheme is very efficient in the perspective of computational complexity. The computational complexity of the proposed algorithm is approximately 37-74% of that of existing algorithms, and from simulation results for a $4{\times}4$ system, the proposed algorithm shows a performance degradation of less than 0.1dB.
In this paper, we present a distributed topology control algorithm for constructing an optimized topology having a minimal search-time in unstructured peer-to-peer network. According to the proposed algorithm, each node selects the best nodes having higher hit-ratio than other nodes as many as the number being exponentially proportional to the hit-ratio of the node itself, and then it connects to them. The ensemble behavior of the proposed algorithm is very similar to the recrystrallizing phenomenon that is observed in nature. There is a partial order relationship among the hit-ratios of most nodes of constructed topology. Therefore once query message visits a node, it has a higher hit-ratio than the node that was visited last by the message. The query message even sent from freeloader can escape to the node having high hit-ratio by one hop forwarding, and it never revisits any freeloader again. Thus the search can be completed within a limited search time. We also propose the Chain-reactive search scheme using the constructed topology. Such a controlled multicasting reduces the query messages by 43 percent compared to that of the naive Gnutella using broadcasting, while it saves the search time by 94 percent. The search success rate of the proposed scheme is 99 percent.

Background: This study aimed to characterize the histopathological pattern of lymph node pathology among Saudi patients and to highlight the age and gender variations of these lesions as base line data. Materials and Methods: We retrospectively analyzed the data from lymph node biopsy specimens received at the Department of Pathology, King Fahad Hospital, Madinah, Saudi Arabia from January 2006 to December 2013. Results: Of the 289 lymph node biopsy specimens received, 154 (53.3%) were from males and 135 (46.7%) from females giving a male: female ratio of 1.14:1. Age of the patients ranged from 2.5 to 96 years with a mean age 33.9 years. The commonest lymph node group affected was the cervical (30.4%) followed by axillary (9.7%) and inguinal (8.7%). Malignant lymphoma [71 Hodgkin's disease (HD), 57 non Hodgkin's lymphoma (NHL)] 128 (44.3%), reactive hyperplasia 68 (23.5%), and tuberculosis 41 (14.2%) were the common causes of lymph node enlargement. While HD, reactive hyperplasia and tuberculosis were commonest in young adult patients (10-29 years old) and rare above the age of 50 years; NHL was the predominant cause of lymph node enlargement above 50 years. Conclusions: Lymph node biopsy plays an important role in establishing the cause of lymphadenopathy. Among the biopsied nodes, lymphomas were the most common (44.3%) followed by non-specific reactive hyperplasia (23.5%) and tuberculous lymphadenitis (14.2%).
Background: Ten to 30% of early breast cancer (EBC) patients develop brain metastasis (BM) during their follow-up. In this study, we aimed to evaluate importance of the lymph node ratio (LNR) in development of BM in EBC cases. Materials and Methods: Ninety patients whom had axillary metastases in lymph nodes at their initial diagnosis and developed BM during 5-year follow-up were detected in 950 EBC patients. LNR values were calculated for all patients and after categorization into 4 molecular sub-types as luminal A, luminal B HER-2 (+), HER-2 overexpressing and basal- like. Comparison was with control group patients who had similar characteristics. Results: In the comparison of all molecular sub-types of LNR, 54.9% and 28.4% values were found in patients with and without BM respectively (p<0.001). In the comparison of the LNR with control groups, a statistically significant differences were found with luminal A with BM (p=0.001), luminal B HER-2 (p=0.001), HER-2 overexpressing (p=0.027) and basal-like groups (p<0.001). In the evaluation of patients with BM, the highest ratio was found in the basal-like group (67.9%) and there was a statistically significant difference between this group and the others (p=0.048). Conclusions: EBC patients developing BM within 5 years follow-up had significantly higher LNRs for all molecular sub-types, especially in the basal-like group. Larger scale studies are now needed for evaluating LNR prognostic importance for EBC regarding BM development.
Background : The 2015 American thyroid association (ATA) guidelines greatly expanded section on risk stratification of thyroid cancer. Definition of "Low risk of recurrence" has expanded, by inclusion of small volume lymph node involvement, such as less than 5 lymph node metastases each smaller than 2mm in central compartment. Purpose : We evaluated the number of positive nodes, Positive node ratio (PNR), recurrence, and radioablation therapy. Also, evaluated the safety of omitting strategy of radioablation after total thyroidectomy with PTC, especially on low-PNR N1a patients compared with high-PNR N1a patients. Methods : Consecutive 147 N1a and 216 N0 patients who underwent total thyroidectomy with central neck dissection between 2003 and 2004 were enrolled. We divided 147 N1a patients into two groups, such as 96 high-PNR versus 51 low-PNR group according to 50% of PNR, and compared these two groups with N0 group. Results: 7.2% (26/363) recurrences were occurred, and 21/147 (14.3 %) recurrences were on N1a patients, and 5/216 (2.3 %) were on N0 patients. Of these 21 recurrences in N1a stage patients, 20 (95.2 %) recurrences were occurred in high-PNR N1a group and only 1 (4.8 %) recurrence was in low-PNR N1a group. The recurrence of low-PNR N1a group was significantly lower than high-PNR N1a group (Log-rank p value = 0.003), but significantly not different from N0 group (Log-rank p value = 0.889). Although this study was a retrospective non-randomized trial with small number of patients, the 10-year recurrence of omitting RAI in low-PNR N1a patients with less than 50% of PNR were shown to be comparable with 216 N0 low risk patients. Conclusion : Positive node ratio could be a useful predictor of recurrence and useful guidance postoperative management -rather than absolute number of positive node.

In this paper, the capacity criteria have been proposed in order to select a cooperative relay node in WSNs, under the environment where direct path has a poor link gain. This process may ensure the efficiency improvement of signal transfer between source and destination and reduction of energy consumption as well. Two criteria are incorporated to select a cooperative relay node. Firstly, calculate the energy gain ratio between the relay path and the direct path. Secondly, investigate the effects of relay node's usage in WSNs through the simulation in terms of energy consumption. In the simulation, the relationship between energy consumption and direct path gain, uniformly generated in the certain range and its positive effects have been identified.

Data routing in wireless sensor networks must be energy-efficient because tiny sensor nodes have limited power. A cluster-based hierarchical routing is known to be more efficient than a flat routing because only cluster-heads communicate with a sink node. Existing hierarchical routings, however, assume unrealistically large radio transmission ranges for sensor nodes so they cannot be employed in real environments. In this paper, by considering the practical transmission ranges of the sensor nodes, we propose a clustering and routing method for hierarchical sensor networks: First, we provide the optimal ratio of cluster-heads for the clustering. Second, we propose a d-hop clustering scheme. It expands the range of clusters to d-hops calculated by the ratio of cluster-heads. Third, we present an intra-cluster routing in which sensor nodes reach their cluster-heads within d-hops. Finally, an inter-clustering routing is presented to route data from cluster-heads to a sink node using multiple hops because cluster-heads cannot communicate with a sink node directly. The efficiency of the proposed clustering and routing method is validated through extensive simulations.
