A Study on the Sleep/Activity Pattern in Newborn Baby (대전 지역 신생아의 수면/활동 양상에 관한 연구)

  • The purpose of this study is to identify the sleep/activity patterns of neonates and to provide a baseline data in developing an effective maternal-child nursing intervention strategies. The subjects of this study were 57 normal newborn babies from birth to 4 weeks of age, who visited the Postpartum Care Center and two General Hospitals and One Local clinic in Taejon. The data were collected from October 28th, 1999 to April 28th, 2000. The instrument used in this study was NCASA developed by Barnard and validated by the Academic Society of Parent-Child Health. The data were analyzed with the SPSS PC+ 7.5 for window program using means, standard deviation, one-way ANOVA. The results of this study were as follows: 1. The mean amount of daytime sleep was 7.45 hours, nighttime sleep was 4.27 hours, and the total daily sleep was 11.72 hours. The mean amount of longest sleep period was 4.32 hours. The mean regularity of daytime sleep was 35.55%, the nighttime sleep was 44.82%, and the total daily sleep was 39.02%. The mean frequency of nighttime awakenings was 2.44 times. 2. The mean amount of daytime awake periods was 8.54 hours, the nighttime awake periods was 3.72 hours, and the daily total awake periods was 12.27 hours. The mean amount of the longest awake period was 3.98 hours. 3. The mean frequency of daytime feeding was 5.18 times, the nighttime feeding was 2.50 times, and the total daily feeding was 7.49. The mean of regularity of total daily feeding was 61.34%. 4. According to an analysis of sleep patterns based on newborn's age, there were some statistically significant differences noted in the following factors ; amount of nighttime sleep (F=7.65, p=.002), longest sleep period(F=5.84, P=.006). 5. According to an analysis of activity patterns based on newborn's age, there were some statistically significant differences noted in the following factors ; amount of nighttime activity (F=7.64, P=.002), longest activity period (F=4.34, P=.020), frequency of nighttime feeding(F=3.89, p=.029), frequency of total daily feeding(F=3.76, P=.033), and regularity of daily feeding(F=4.66, P=.016). In conclusion, the newborn baby slept more during the daytime than nighttime and more active during the daytime. And there were some irregular sleep/activity pattern noted during 1 to 2 weeks of newborn baby. The results of this study will contribute to maternal-child health nursing practice and nursing research, and provide an information to parents about what to expect their newborn baby.

South Korean nursing students' experiences of clinical practice in the newborn nursery and neonatal intensive care unit: A phenomenological study

  • Purpose: While clinical practice is crucial for nursing students to acquire the skills needed to provide professional, high-quality nursing care, further studies on improving undergraduate nursing programs are needed to provide a supportive clinical learning environment for student nurses. This study aimed to understand nursing students' clinical experiences in newborn nurseries and neonatal intensive care units and to provide basic data for the establishment of strategies to promote effective clinical education. Methods: Interviews were held with 15 nursing students at J University who had clinical practice experience in the newborn nursery and neonatal intensive care unit. The collected data were analyzed using the phenomenological analysis method developed by Colaizzi (1978). Results: The nursing students' experiences were grouped into four categories: "expectations for and anxiety about clinical practice", "acquisition of a wide range of knowledge regarding neonatal nursing", "challenges faced in clinical practice", and "experiencing interpersonal changes". Conclusion: The current neonatal practice nursing education system provides students with positive learning experiences. However, the lack of practice opportunities, insufficient instruction, and the theory-practice gap were identified as major issues hindering students' learning needs. These study results are expected to provide basic data for curriculum development to improve undergraduate nursing education.

A Study on the Home Health Care Need of Postpartum Mothers (산욕기 산모의 가정간호 요구도 조사)

  • The purpose of this study was to provide the basic data for developing a program for effective intervention for home health care need of postpartum mothers and newborn babies. The subjects were 104 women. The data were collected from march, 2003 to June, using a 81 item questionnaire and analyzed by SPSS program for frequency, Mean, ANOVA. The results were as follow. 1. The mean of care needs of newborn babies was higher than that of physical demand of Postpartum mothers. The mean of physical demand of Postpartum mothers was $3.99\pm.42$. The mean of care needs of newborn babies was $4.11\pm.50$. 2. The most highest mean of physical demand of Postpartum mothers was wound care for caesarean section and episiotomy($4.53\pm.66$), and then breast engorgement care($4.38\pm.71$). The most highest mean of care needs of newborn babies was emergency care methods($4.58\pm.52$), and then infection control $4.51\pm.56$). 3. 66.3% of postpartum mothers positively desired consultation hospitals centered home care need during postpartial periods. 4. Influential variables of home health care need was postpartial periods. they wanted the first week after delivery, more freqently visiting of home care nurse. 37.5% of postpatial mothers wanted visiting within 1 weeks after delivery. 31.7% wanted 2 times/week. In conclusion, it is necessary to study to make a program in nursing of home health care for postpartum mothers, and to keep on studying repeatedly in order to raise the number of objects and to find related variables.

Adequacy of Medical Manpower and Medical Fee for Newborn Nursery Care (신생아실 의료인력의 적정성 및 신생아관리료의 타당성 분석)

  • To assess the adequacy of medical manpower and medical fee for the newborn nursery care, the author visited 20 out of 24 hospitals with the pediatric training program in Youngnam area between July 29 and August 14, 1991. Total number of newborn, both normal and sick, admission and discharge in 1-30 June 1991 was obtained from the logbook of nursery. Head nurse and staff pediatrician of the nursery were interviewed to get the current staffing for the nursery and their subjective opinion on the adequacy of nursery manpower and the difficulties in recruiting manpower. Average medical fee charged for the maternity and normal newborn nursery care was obtained from the division of self-audit of medical insurance claim of each hospital. Average minimum requirement of nursing care time for one normal newborn per day was 179.5 (${\pm}58.6$) minutes; 202.3(${\pm}50.7$) minutes for the university hospitals and 164.2(${\pm}60.5$) minutes for the general hospitals. The ratio of minimum requirement of nursing care time and available nursing time was 1.42 on the average. Taking the additional requirement of nursing care for the sick newborns into consideration, the ratio was 2.06. The numbers of R. N. and A. N. in the nurserys of study hospitals were 31%, and 17%, respectively, of the nursing manpower for the nursery recommended by the American Academy of Pediatrics. These findings indicate that the nursing manpower in newborn nursery is in severe shortage. Ninety percent of the head nurses and 85% of the staff pediatrician stated that the newborn nursery is short of R.N. and 75% of them said that the nurse's aide is also short. Major reason for not recruiting R.N. was the financial constraint of hospital. For the recruitment of nurse's aide, short supply was the second most important reason next to the financial constraint. However, limit of quarter in T.O. was the mar reason for the national university hospitals. Average total medical fee for the maternity and newborn nursery cares of a normal vaginal delivery who stayed two nights and three days at hospital was 219,430won. Out of the total medical fee, 20,323won(9.3%) was for the newborn nursery care. In case of C-section delivery who stayed six nights and seven days, total medical fee was 732,578won and out of the total fee 76,937won (12.0%) was for the newborn care. Cost for a newborn care per day by cost accounting was 16,141won for the tertiary care hospitals and 14,576won for the all other hopitals. The ratio of cost and the fee schedule of the medical insurance for a newborn care per day was 5.0 for the tertiary care hospitals and 4.9 for the all other hospitals. Considering the current wage level of the medical personnel, capital investment for the hospital facilities and equipments, and the cost for hospital maintenance, it is hard to expect adequate quality care in the newborn nursery under the current medical insurance fee schedule.

A Study on the Washing and Satisfaction of Newborn Clothing - Focused on Innerwear - (신생아복 세탁 실태 및 만족도에 관한 연구 -배냇저고리 중심으로-)

  • This study analyzed satisfaction with and washing of newborn clothing (especially the innerwear washing behaviors) which are considered to be the most basic items among newborn clothing.The survey was conducted on mothers with children under 24 months old. Using an average of 6.7 innerwear items that were changed average 2.3 times a day with baby formula and breast milk representing the main contaminants. When washing clothes for a newborn, respondents were mainly concerned with: rinsing residual detergent, removing stains and contaminants, and sterilization and disinfection. Items were washed by a laundry label to prevent clothes damage. A baby-friendly or environmentally-friendly detergent was used to wash before being worn after purchase was separated and washed in various washing courses of the washing machine every day, boiled, rinsed an average 3.6 times, and dried mainly in the sun. Environmentally friendly cotton (59.5%) and organic cotton (41.6%) products were mainly used; however, items were washed separately from regular laundry. Respondents were satisfied with methods of washing. Consumers are concerned about washing because they think that washing affects a newborn's health. Therefore, it is necessary to provide consumers with appropriate information on washing and establish regulations for the use of harmful substances in newborn clothing and detergents.

A Case Analysis of Home Health Care for Cesarean Postpartum Women and Their Newborns (제왕절개분만 산욕부와 신생아의 가정간호 사례분석)

  • This study was done to provide a basis for home health care management for women following Cesarean delivery. Furthermore it was initiated as an possible application of home health care in the future. In this study, client selection criteria was developed by the researcher and assessment tools for home health care, recording system and problem oriented recording system were revised from Jun's(1993) methods. The selection criteria tool for home health care for women who had a Cesarean delivery was structured and consisted of five areas : physical status, functional status, psychological-emetional status, educational needs status and environmental status. The structured assessment tool consisted of general items, obstetric history, past medical history, methods of feeding, medications taken before ad-mission, laboratory results, discharge instructions, discharge medications, family tree, economic status, environmental status, a map, health assessment of women and their newborns. The visit note consisted of the date : nursing problems : nursing process including initial assessment : nursing goals : visit plan : health status of the postpartum women and their newborn : nursing diagnoses : nursing implementation evaluation : summary : next visit plan and revision. The problem oriented recording system consisted of the date, problem numbers, nursing diagnoses, problem appearance date, problem resolution date. The results of the research are as follows : The seven cases having had a Cesarean delivery were discharged on an average on the 5th day after the Cesarean birth. The total number of home visits was 13. According to Cordon's functional health patterns the total possible nursing diagnoses was 34 diagnoses for the methers and their newborns. Among the 34 diagnoses, there were 13 diagnoses in the health perception /management pattern, 7 in the psychosocial health perception / management pattern, 8 in the psychosocial self-perception, 2 in the nutrition / metabolism pattern of physical function, 2 in the knowledge deficit of newborn management, anxiety related to newborn management, knowledge deficit related to disease process of new-born, anxiety related to disease process of newborn anxiety related to prognosis of baby's condition, knowledge deficit related to newborn jaundice each appeared once. The changes in the number of nursing diagnoses was related to not the number of visits but to the number of nursing diagnoses decreasing. The con-tent of the home health care was categorized ac-cording to assessment, direct care, counseling, education, family care. The recommendation based on the results of this research are Home health care nurses for Cesarean postpartum women and their neonates requires comprehensive knowledge of pregnancy, delivery, and the postpartum period and of the neonate so that they can provide appropriate care and holistic views. Most of cases terminated after the second visit, this outcome may be related to the subjects being discharged on the 5th day after delivery. Therefore, study done with earlier discharge after delivery may have different outcome. It is very hard to assess psychological aspects that need follow-up and to develop communication channels.

A Study on the Effects of Father Class upon Father's Level of Knowledge and Confidence about Postpartal Woman and Newborn Care (산후 아버지 교실이 산욕기관리와 신생아 돌보기의 지식정도와 자신감에 미치는 영향에 관한 연구)

  • The purpose of this study is to find out how the Father Class, which is being practiced at a hospital located in Seoul, influences on fathers' level of knowledge and confidence of postpartal mother and newborn care. Research design was quasi-experimental study using non equivalent experiment and control group. The data, questionaires, were collected from Sep. 13, 2001 to Oct. 31, 2001 through e-mail, telephone and mail, after discharge from S hospital located in Seoul, where the experimental group was educated at the Father Class and control group didn't take part in the class. The objects were the fathers who have the first baby through normal delivery or C-sec delivery including experimental group 48 persons and control group 52 persons. Four instruments were the revised ones of Lee Mi Kyoung's(1990) and Kwak Yon Hee's tool based on the resources of direct interviews of fathers and one expert, professor. The reliability of four instruments were Cronbach's $\alpha$ scores .85 -93. Data were analyzed by SPSS 10.0 program using $x^2$-test, and t-test and, One- way ANOVA, and Pearson correlation, etc. The results of this study are as follows; 1.The two groups were confirmed as a homogeneous group by showing no significant difference statistically at the level of 5%. 2.The results of proving the hypothesis are followings: 1)The first hypothesis, "the fathers who participated in the Father Class will have the higher level of knowledge about postpartal mother care than the fathers who didn't" was accepted by significant difference statistically (p=0.000). 2)The second hypothesis, "the fathers who took part in the class will have the higher confidence in postpartal mother care than the fathers who didn't", was supported by significant difference statistically (p=0.000). 3) The third hypothesis, "the fathers who attended the class will have the better knowledge about taking care of newborn than the others", was accepted by significant difference statistically (p=0.000). 4)The fourth hypothesis, "the fathers who participated in the class will have the higher confidence in newborn care than others," was supported by significant difference statistically (p=0.000). 3.The further analysis showed whether they participated in Prenatal Birth Preparing Class influenced on the father's level of knowledge and confidence in postpartal mother and newborn care. In conclusion, the Father Class is the effective nursing intervention strategy that can help new fathers attain father and husband roles by enhancing the level of confidence in and knowledge of the postpartal mother and newborn care.

Congenital epulis in a newborn

  • Congenital epulis (CE) is an extremely rare benign tumor of the gingiva that is found on the alveolar ridge of newborns, and the main treatment option is simple excision. Postoperative prognosis is very good, and spontaneous regression may occur despite incomplete excision. This report presented a rare case of CE and its healing process after surgery under local anesthesia. The treatment plan was decided upon through consultation between a medical team and the patient's family, with surgical excision for the main lesion, which benefitted from surgery, and follow-up for a very small-sized lesion, which was thought to be appropriate for a newborn. No recurrence was found after its removal, and favorable healing was observed.

Effects of Newborn Care Education Program on Child-rearing Knowledge, Child-rearing Stress, and Child-rearing Self-efficacy of Immigrant Pregnant Women (신생아돌보기프로그램이 결혼이주여성 임산부의 양육지식, 양육스트레스 및 양육효능감에 미치는 효과)

  • Purpose: This study was designed to evaluate the effects of a newborn care education promotion program on child-rearing knowledge, child rearing stress, and child rearing self-efficacy of immigrant pregnant women. Methods: The subjects were 23 immigrant pregnant women who voluntarily participated in the study in D city. The program was provided, once a week for 90 minutes for a period of 3 weeks. The outcome variables were child-rearing knowledge, child-rearing stress, and child rearing self-efficacy. Data was collected from April 4 to June 24, 2011, through self-administered questionnaires. Results: The program significantly improved child rearing knowledge, child rearing stress, and newborn care self-efficacy for married immigrant pregnant women who participated in the program compared to women who did not participate. Conclusion: The results suggest that the reproductive health promotion program has an affirmative effect on child-rearing knowledge, child rearing stress, and child rearing self-efficacy of married immigrant pregnant women. It is suggested, therefore, that the educational program be presented to married immigrant woman at public health centers to enhance their confidence in child-rearing.

A Study on Protection of Maternal Antibody against Hantavirus in Rats

  • The etiologic agents of haemorragic fever with ranal syndrom (HFRS) in Korea are Hantaan and Seoul virus in the genus Hantavirus, family Bunyaviridae. In order to elucidate the role of maternal immunity to Hantavirus infection in rats, the protective effect of the maternal antibody were studies by using rats experimentally infected with Seoul virus strain HR80-39. Antibody titers of sera and viral antigen against Seoul virus were investigated by indirect immunofluorscence antibody technique (IFA). The dam sera had IFA antibody titers ranging from 1:128 to 1:1,024 after parturition. In fetuses, IFA antibody titers ranged from 1: 16 to 1:64 just after birth, increased to peak titers ranged from 1:256 to 1:1,024 in the 2nd week after birth. Challenged newborn rats had IFA antibody titers ranging from 1:64 to 1:1,024 after inoculation. No viral antigen was detected in lungs or other organs of the newborn rats. The maternal antibody to Seoul virus was transferred prenatally through placenta and postnatally via colostrum from immune dams to their offspring. These results demonstrated that maternal antibody to Seoul virus was quite effective in protecting newborn rats against same virus infection.
