Anti-allergic Effects of Shinbijen (신비전(新鼻煎)의 항알레르기 효과)

  • The discovery of drugs on the treatment of rhinitis and allergic disease is a very important subject in human heath. The Shinbijen has been used for centruries as a traditional medicine in Korea and is known to have an anti-inflammatory effect. However, its specific mechanism of action is still unknown. In this report, we investigated the effect of hot water extract from Shinbijen on OVA-mediated allergic reaction and studied its possible mechanism of action using fluorescenc microscopy and RT-PCR analysis. Shinbijen inhibited OVA-induced rhinitis, total cells in BFA and lymphocyte related to inflammation in mice. Shinbijen decreased immuno response, which activated by IL-4, COX-2 and iNOS expression. in tissue. Shinbijen reduced inflammatory molecule release from mice lung tissue and CD4/CD8 cells activated by cardiac blood. Shinbijen decreased OVA-induced IL-4 and iNOS levels in pulmonary alveoli. Our findings provide evidence that Shinbijen inhibits OVA-induced allergic reactions, and also demonstrate the involvement of inflammation and allergic disease in these effects.

Effects of Treadmill Exercise on the Recovery of Dopaminergic Neuron Loss and Muscle Atrophy in the 6-OHDA Lesioned Parkinson's Disease Rat Model

  • This study was to determine the effect of exercise on the recovery of dopaminergic neuron loss and muscle atrophy in 6-OHDA-induced hemi Parkinson's disease model. Exercise was loaded twice per day for 30 minutes each time, at 5 days after 6-OHDA lesioning and continued for 16 days using a treadmill. Exercise significantly increased the number of tyrosine hydroxylase positive neuron in the lesioned substantia nigra and the expression level of tyrosine hydroxylase in the striatum compared with the control group. To examine which signaling pathways may be involved in the exercise, the phosphorylation of $GSK3{\beta}$ and ERK were observed in the striatum. In the control group, basal level of $GSK3{\beta}$ phosphorylation was less than in both striatum, but exercise increased it. ERK phosphorylation decreased in the lesioned striatum, but exercise recovered it. These findings suggest that exercise inactivates $GSK3{\beta}$ by phosphorylation which may be involved in the neuroprotective effect of exercise on the 6-OHDA-induced cell death. In the exercise group, weight, and Type I and II fiber cross-sectional area of the contralateral soleus significantly recovered and expression of myosin heavy chain and Akt and ERK phosphorylation significantly increased by exercise. These results suggest that exercise recovers Parkinson's disease induced dopaminergic neuron loss and contralateral soleus muscle atrophy.

A Study of the Relationship between Granulocyte/Lymphocyte Ratio and Dysmenorrhea, History of Gynecological Disease, Heart Rate Variability in Some Young Women (과립구/림프구 비율과 일부 젊은 여성들의 월경양상, 산부인과적 질환 과거력 유무, HRV와의 관계)

  • Objectives: The purpose of this study is to investigate the relationship between granulocyte/lymphocyte ratio and dysmenorrhea, history of gynecological disease, heart rate varibility(HRV). Methods: From May 1st 2011 to July 10th 2011, a total of 40 female subjects (age 20-39) were recruited. We evaluated the menstruation condition by questionnaires including VAS(Visual Analog Scale), VRS(Verbal Rating Scale) and MVRS (Multidimensional Verbal Rating Scale), also measured subject's WBC differential count and HRV. Results: 1. There was no statistically significant difference in granulocyte/lymphocyte ratio according to menstruation cycle, duration, amount, color, and blood clot. 2. There was no statistically significant difference in VAS, VRS, MVRS mean scores between normal group and abnormal group according to granulocyte/ lymphocyte ratio. 3. The past prevalence of gynecological disease of abnormal group showed significantly higher compared with that of normal group($x^2$=6.578, p=0.010). 4. LF/HF ratio and granulocyte ratio significantly showed positive correlation ($r_s$=0.311 p=0.048) and LF/HF ratio and lymphocyte ratio significantly showed negative correlation($r_s$=-0.319 p=0.045). Conclusion: These findings suggest that the gynecologic diseases are related to disorder of autonomic nervous system, but not dysmenorrhea. And the balance state of sympathetic/parasympathetic nerve infered through granulocyte/lymphocyte ratio and HRV seems to be consistent. However, reconfirmation through further studies is needed.

The Effectiveness of Acupuncture Treatment for Patients with Alzheimer's Disease: A Meta-Analysis of Randomized Controlled Trials

  • The purpose of this study was to evaluate the effectiveness of acupuncture treatment for the symptoms of Alzheimer's disease (AD). There were 11 databases searched for randomized controlled trials using acupuncture treatment for AD. The risk-of-bias assessment tool of the Cochrane Library was used to evaluate the quality of each study. Using the Review Manager (RevMan), a meta-analysis was performed using risk ratio, mean difference, 95% confidence interval, and random effect model. There were 32 studies selected, all of which were conducted in China. There were 16 studies where acupuncture was used as the intervention, and 23 studies used Donepezil as the control group. Acupoint GV20 was most frequently selected during the treatment. The Mini Mental State Examination (MMSE) was the most frequently used outcome variable. A period of 12 weeks was the most common treatment duration. As a result of meta-analysis (n = 25), acupuncture improved the MMSE and activities of daily living scores. Electroacupuncture improved the Alzheimer Disease Assessment Scale-Cognitive Subscale, and scalp acupuncture improved the MMSE score for AD. Acupuncture alleviates the symptoms of AD. However, further research is necessary to provide a better level of evidence.

A Review of Acupuncture and Moxibustion for the Treatment of Parkinson's Disease (파킨슨병의 침구치료 동향에 대한 고찰)

  • Objectives : The purpose of this study was to report possibility of acupuncture or moxibustion for the treatment of Parkinson's disease (PD) by reviewing literature about its effectiveness. Methods : In this review, PubMed, SCOPUS, Science Direct and CINAHL of EBSCOhost were used to search medical journals, using keywords "Parkinson's disease and acupuncture" and "Parkinson's disease and moxibustion". The search range included randomized controlled trials (RCT) about Parkinson's disease combined with another disease and other treatments with acupuncture or moxibustion. Non-randomized controlled trial (nRCT), case study, animal experiment, human experiment, review, survey, essay, letter, and protocol for review were excluded. Results : From 311 studies, 111 were selected during the title and the screening. Finally, 16 RCTs (15 for acupuncture research and one for moxibustion) were included in this review, after scanning and matching the inclusion and exclusion criteria. The number of patients varied between 5 and 88. A total of 12 studies using electroacupuncture (EA) were classified into acupuncture studies. The body acupuncture studies numbered 4, scalp acupuncture 4, body and scarp acupuncture mixed studies 4, and bee venom, ear and abdomen acupuncture were each one study. In evaluation methods, total effective rate method was used in 9 studies, the Unified Parkinson's Disease Rating Scale (UPDRS) was used in 8, and the Webster score in 2. In addition, the Berg balance scale (BBS), 30 m walking time, steps to walk 30 m, PD motor function score, and Motor Performance Series by Schoppe (MLS) method were used for evaluation. In 15 of the 16 studies, the verum acupuncture group showed significant improvement compared with the control. In 9 studies using total effective rate method, the effective rate was reported as 80.0-97.3% in verum acupuncture groups and 52.6-86.4% in controls. Conclusions : Acupuncture may be a plausible alternative method to care for the long term symptoms and treat movement impairment of Parkinson's disease. However, to confirm this result, high quality studies including randomized, placebo-controlled double-blind trials are warranted.

Effect of Stopping Drinking, Using Alcoholic Liver Disease Questionnaire, DSOM and SF-36 (알코올성 간질환 변증 설문, DSOM, SF-36을 이용한 알코올성 간질환 환자의 금주 효과 연구)

  • Objectives : This study was done to evaluate the effect of stopping drinking, using alcoholic liver disease questionnaire, Diagnosis System of Oriental Medicine (DSOM) and Health Related Quality of Life (HRQOL). Methods : 49 men who satisfied the requirement participated in this trial. They stopped drinking for 6 weeks. They were analyzed using DSOM, alcoholic liver disease questionnaire and SF-36. The data were classified by age (<47,$\geq$48) and alcoholic intake per day (<100g,$\geq$100g). For HRQOL, the SF-36v2 Health Survey was used and Quality Metric Health Outcomes Scoring Software 2.0 (QualityMetric, Lincoln, RI, USA) was applied for the analysis. Results : The alcoholic liver disease questionnaire had a partial correlation with DSOM. Generally stopping drinking decreased Heat (熱). Especially in the group drinking over 100g per day, the correlation was high. In the group over 48 years old, spleen (脾) was improved comparatively. In the group with low HRQOL (PCS<31.43, MCS<23.33) deficiency (虛) was improved. Conclusions : We found that stopping drinking can improve pathogenic factors of alcoholic liver disease and the alcoholic liver disease questionnaire be a useful diagnostic method on alcoholic liver disease by comparison with DSOM.

Brief comparison of the mechanism of modern medicine and traditional medicine in neuronal cell death

  • Medicine has a past, a present, and will have a future; the same can be said for many diseases. Even with the surprising development of modern medicine, traditional medicine, especially eastern Asian traditional medicines still exist and are widely used in those regions. But modern medicine and western pacific traditional medicines have different theories and applications for the same disease. In this review, traditional medical theory, used together with modern medicine, can be combined to shed light on the area of neuronal death.

  • Serotonin has been implicated in the etiology of many disease states and may be particularly important mental illness, such as depression, anxiety, schizophrenia, sleep disorders, suicide, eating disorders, obsessive compulsive disorders, migraine and others. Many currently used treatments of these disorders are thought to act by modulating serotonergic function. The identification of many serotonin subtypes, most of which have been shown to have functional activity and differential distribution, has stimulated considerable effort into synthesizing selective ligands(drugs) to help understand their significance. This should understand the role of serotonin in mental disorders and these new drugs can be studied alone and in combination with other treatments in order to clarify the parameters of drug use for the clinical effect.

  • We have known that "Chimgudaeseong(鍼灸大成)" had been written by Yanggyeju(楊繼洲) in Ming(明) dynasty. And it had been the only text book of acupuncture & moxibustion for 300 years. This book is composed of 10 chapters dealing almost all the medical theories of that times. This book is so enormous that it is hard to understand essential ideas of author. The reading medical records is one of the best way to develop one's abilities of curing a disease without clinical practice. so we can't help dealing with medical records, because it is one of important method of understanding Oriental Medicine. On this study, we investigate a objective method on understanding medical records in "Chimgudaeseong(鍼灸大成)".

Polysomnographic Evauation for Disorders of Initiating and Maintaining Sleep (불면증 평가를 위한 수면다원검사)

  • Disorders of intitating and maintaining sleep(DIMS : insomnia) may be a symptom of some other disease or the basic, root problem in some patients. DIMS is usually treated by private physicians or psychiatrists, initially. Many physicians can do to improve the quality of life of the sleep disordered patients by appropriate evaluation and management For the effective management of DIMS patients, the accurate differential diagnosis is essential. In polysomnographic evaluation for insomnia, the physician should consider the aspects of cost-effectiveness to provide the maximal benefits for the patients. For the purpose of this, evaluation of DIMS complaints outside the sleep disorder clinic, indication for referral to a sleep disorder clinic, the role of polysomnopraphy in evaluating DIMS, and polysomnographic findings of different types of DIMS are discussed, together with possible way to diagnose them.
