Measurement of Spatial Dose Distribution for evaluation operator dose during Neuro-interventional Procedures (두경부 질환의 인터벤션 시술 시 시술자의 피폭선량평가를 위한 공간선량측정에 관한 연구)

  • The spatial dose distribution was measured with ionization chamber as preliminary study to evaluate operator dose and to study dose reduction during neuro-interventional procedures. The zone of operators was divided into four area (45, 135, 225, and 315 degree).We supposed that operator exist on the four area and indicated location of critical organs(eyes, breast, gonad). The spatial doses were measured depending on distance( 80, 100, 120, and 140 cm) and location of critical organs. The spatial doses of area of 225 degree were 114.5 mR/h (eyes location), 143.1 mR/h (breast location) and 147 mR/h (gonad location) in 80 cm. When changed location of x-ray generator, spatial dose increased in $18.1{\pm}10.5%$, averagely. We certified spatial dose in the operator locations, Using the results of this study, It is feasible to protect operator from radiation in neuro-interventional procedures.

Endovascular Treatment of a Large Partially Thrombosed Basilar Tip Aneurysm

  • Despite the remarkable developments in neurosurgical and neuro-interventional procedures, the optimal treatment for large or giant partially thrombosed aneurysms with a mass effect remains controversial. The authors report a case of a partially thrombosed aneurysm with a mass effect, which was successfully treated by stent-assisted coil embolization. A 41-year-old man presented with headache. Brain computed tomography depicted an $18{\times}18$ mm sized thrombosed aneurysm in the interpeducular cistern. More than 80% of the aneurysm volume was filled with thrombus and the canalized portion beyond its neck measured $6.8{\times}5.6$ mm by diagnostic cerebral angiography. Stent-assisted endovascular coiling was performed on the canalized sac and the aneurysm was completely obliterated. Furthermore, most of the thrombosed aneurysm disappeared in the interpeduncular cistern was clearly visualized follow-up brain magnetic resonance imaging conducted at 21 months. The authors report a case of selective coiling of a large, partially thrombosed basilar tip aneurysm.

Reducing Radiation Exposure Dose on Operator by Using Lateral Protection in Neuro-Intervention (뇌혈관 중재적시술에 있어 측방향 차폐체를 이용한 시술자 피폭 선량 저감화 방법 연구)

  • The bi-plane cerebrovascular angiography radiation is done the radiation exposure at the forward and lateral direction as opposed to the one of the source. So, the exposure dose of radiation workers increases further. Therefore, the medical diagnostic radiation workers as well as patients is interested to ways to reduce the dose. The exposure dose of cerebral angiography and interventional radiology must be considered the primary radiation of X-ray tube directly, scattered primary radiation between lateral tube and lateral detector and relatively small secondary scatter radiation in the walls of room. The aim of study is that the exposure dose of primary and scatter radiation reduce as much as possible to install protection device of lateral protection than common shielding of table and ceiling. As a result, the dose of fluoroscopy was reduced approximately 3.64 times the gonads, thyroid approximately 3.13 times, 4.42 times around eyes. And the dose of DSA was reduced approximately 4.98 times the gonads, thyroid approximately 3.00 times, 1.67 times around eyes. Consequently, medical practitioners can be helpful for radiation dose-exposure for the lateral protection of bi-plane cerebrovascular angiography more than the common shield method in cerebrovascular angiography and interventional radiological procedures.

Study on Exposure Dose and Image Quality of Operator Using Shielding Material in Neuro Interventional Radiology (뇌혈관 중재적 시술에서 차폐체를 이용한 시술자의 피폭선량과 화질에 관한 연구)

  • Although interventional procedures use very low tube currents, there is a high risk of exposure to radiation as well as the operator due to long-term radiation exposure. The purpose of this study is to investigate the effects of radiation dose on the quality of the operator by measuring the dose received by the operator in the interventional procedure of the cerebral vascular system and finding the shielding material and shielding method which can effectively shield the exposure from the medical radiation. And to find a way to minimize it to the extent that it does not. As a result, when the newly designed shielding system with Nano Tungsten material was used, it was confirmed that the mean dose was reduced by 7.95% on average by the operator. Also, the PSNR results were measured to be 38.44 dB when using the designed shielding material, and it was confirmed that Nano Tungsten does not affect the image quality. In conclusion, the Nano Tungsten shielding material proved to be capable of significantly reducing the operator radiation dose, without affecting the image quality. The use of the above materials is expected to solve the problems related to the harmfulness and economical efficiency of the human body and the environment, which have recently become an issue of shielding materials.

Distribution and Management of Spatial Dose Rate in Neuro Angio Room (두개부 혈관조영실에서 공간산란선량의 분포와 관리)

  • This study is performed in the intervention unit, during interventional procedures and in accordance with the direction and distance during the exposure indoor space is to measure the dose. I was classified at an angle of $45^{\circ}$ counterclockwise from the phantom. Seven(A, B, C, D, E, F, G) were classified as direction. Length was measured from the center of the phantom. Each direction 50cm, 100cm, 150cm, 200cm were classified. I was analyzed by measuring of frontal, lateral, Bi-plan fluoroscopic Spatial dose rate in all 28 points. Measured dose was the highest at 50cm and over 200cm, dose was rapidly decreasing as increased distance. Dose was different more than nine times depending on the distance and direction, Installation of shielding wall can reduce exposure about 84.52% to 93.54%.
