  • Squamous cell carcinoma(SCC) associated antigen is a subfraction of TA-4, a tumor-associated antigen first described by Kato and Torigoe in 1977. TA-4, obtained from squamous cell carcinoma cancer tissue of the uterine cervix, has been characterized as a glycoprotein with a molecular weight of approximately 45,000 daltons. SCC antigen has been studied in other squamous cell malignancies including lung, esophagus, head and neck, anal canal, and skin. SCC antigen is shed naturally through sweat, saliva and other body fluids. Contamination of specimens, tray, bead dispenser or other accessories with sweat, saliva or aerosols can cause falsely elevated values. To reduce the possibility of contamination, gloves should be worn in all phases of assay preparation, and when handling specimens, accessories or reagents that will be used in SCC and Thyroid function test(TFT).

  • 현재 일반적으로 Neurilemmoma 또는 Schwannoma라고 부르는 일련의 종양들은 1908年 Verocay에 의해 처음으로 기술되었으며, 1910년 그는 이것을 "Neurinoma"라고 명명하였다. Neurilemmoma라는 명칭은 1935년 Stoutdp 의하여 처음으로 소개되었다. Neurilemmoma는 신경초(Newe Sheath)를 소유하고 있는 어떤 신경에서나 발생할 수 있는 종양으로서 비교적 희유한 양성종양이며 연령, 성병, 그리고 발생장소에 따른 어떤 우월성은 없었으나 이 종양의 약 25% 이상이 두경부에 발생한다. 다른 부위로 전이를 일으키지 않으며 완전 제거 후에는 재발되지 않으며 방사선에 대한 감수성은 거의 없었다. 저자들은 과거 5년간 본 병원 이비인후과령역에서 수술제거한 5예의 Neurilemmoma를 문헌고찰과 함께 보고하는 바이다.

  • Surgical excision along with use of postoperative radiotherapy forms an integral management of sinonasal teratocarcinosarcoma (SNTCS). However, given the rarity of the tumor, no standardised guidelines, dose, technique and target delineation exist especially in the era of modern radiation delivery techniques. This is a case of 55-year-old male diagnosed as SNTCS treated with radical ethmoidectomy followed by volumetric modulated radiotherapy, showing good local control and acceptable toxicity profile.

  • Schwannoma is a benign, slow-growing, tumor of the peripheral nerves without specific symptoms, so that early diagnosis may be difficult. Though approximately 25~40% of all schwannomas occur extracranially in the head and neck region, only 1% of schwannomas are reported in the oral cavity. An 18-years-old female patient visited our clinic with a mass on the middle-right-dorsal surface of the tongue slowly growing for 1.5 years. The patient underwent the surgical removal of the neoplasia under general anesthesia. The mass was well capsulated and a cleavage plane was easily found. There was no recidivation during the course of a one-year follow-up. The treatment for schwannoma is surgical excision of the lesion and recurrence after excision of schwannoma is rare. The final diagnosis is made after a histological examination. Differential diagnoses must be made in relation to malignant tumors and in relation to numerous benign neoformations based on epithelial and connective tissues.

소아 Lipoblastoma의 임상 양상 (The Clinical Manifestations of Lipoblastoma in Children)

  • Lipoblastoma is a rare benign soft tissue tumor occurring in infancy and early childhood. It is characterized by fat lobules with varying degrees of maturity, multivaculoated lipoblasts, fibrocapillary networks and myxoid stroma. Lipoblastoma has a good prognosis with no metastases despite its potential for local invasion. From Jan, 1990 through April, 2007, 12 children underwent the operation for lipoblastoma, 7 boys and 5 girls, diagnosed at median 22 months (5 ~ 43 months). Median follow up was 6 year 7 months. Primary sites included back (n = 5), intraabdominal (n = 2) and one in each of buttock, chest wall, neck, nose and scalp. Tumors presented with a growing mass in 9 patients, abdominal distension in 2, and an incidental finding on chest radiography in one. Complete excisions were done in all patients. There was one recurrence in a patient with a scalp mass. After reoperation, he has been doing well without evidence of recurrence. Lipoblastoma has a favorable prognosis, but recurrence can occur even with complete excision. Regular follow up is necessary to detect recurrences.

  • Nodular fasciitis is a reactive, non-neoplastic lesion that is most commonly found in the subcutaneous or superficial fascia of the extremities and trunk. Head and neck lesions are relatively uncommon and reports vary from 7% to 15% depending on the authors. Nodular fasciitis grows quickly, and shows a pleomorphic spindle cell pattern with increased mitotic activity. Such factors lead to cases where the lesion is mistaken for a malignancy such as fibrosarcoma and the case may end up with unnecessarily aggressive treatments. The intent of this paper is to report a relatively rare case of nodular fasciitis occurring in the periorbital area and also to highlight the importance of accurate diagnosis and non-aggressive management of this benign lesion.

구강 및 구인두종양수술시 구순-하악골이단 접근법의 이용에 관한 임상적 연구 (A CLINICAL STUDY ON THE APPLICATION OF LABIO-MANDIBULOTOMY APPROACH FOR ORAL AND OROPHARYNGEAL TUMORS)

  • The labiomandibulotomy approch to the oral cavity and oropharygeal region was first described by Roux in 1836 and become popular for oral and oropharyngeal tumors in cases where there are no clinical and radiological signs of mandible invasion. Anterior labiomandibulotomy and swing procedure provides excellent access and facilitate a mandibular resection and subsequent repair. In last two years, 8 cases of oral and oropharyngeal tumors were treated by this approach at the Dept. of Oral & Maxillofacial Surgery, Holy Family Hospital, Catholic University of Korea. And we analyzed postoperative complications as well as functional evaluations, and the results were as follows; In 4 cases, marginal mandibulectomy were combined with labiomandibulotomy and in case of malignancies, neck dissection was performed simultaneously. In almost case, plate and miniscrew fixation was used for osteotomy sites. Histologic evaluation of the resection margins of the specimens revealed tumor free in all cases. The postoperative complications were occured in 3 cases, one case of nonunion, one case of orocutaneous fistula, and one case of wound dehisence. Occlusal stability, jaw movement and swallowing function were acceptable postoperatively in 3 months. From above results, we concluded that, this approach not only provides wide exposure, permitting radical removal of benign and malignant lesions but also preserves function with minimal complications.

비골 골-피부 유리 피판을 이용한 상악동 암종 재건 1례 (A Case of Maxillary Carcinoma Recontruction with a Fibular Osteocutaneous Free Flap)

  • A radical maxillectomy causes a defect of the alveolar bone, gingiva, palate, and orbital floor and causes cosmetical problems and masticatory and phonatory functions. Defect after a radical maxillectomy was reconstructed with skin or dermis graft was introduced, but recently wide resection of the tumor and functional reconstruction with free flap was introduced by several methods. The defect due to radical maxillectomy was reconstructed with scapula, iliac crest, radius. But reconstruction with a fibular osteocutaneous free flap was rarely introduced to defect of radical maxillectomy. The fibular osteocutaneous free flap was firstly introduced by Taylor. The fibular osteocutaneous free flap has several advantages. We experienced the first case of radical maxillectomy and reconstruction with the fibular osteocutaneous free flap, so we reported that case with literatures. The patient has a right maxillary sinus squamous carcinoma (T2N0M0), and performed a radical maxillectomy with right supraomohyoid neck dissection, and reconstruction with fibular osteocutaneous free flap. Donor site morbidity was little, and phonatory and masticatory function were nearly normalized. And cosmetical result was very acceptable.

Massive Hemoptysis due to Endotracheal Hemangioma: A Case Report and Literature Review

  • Tracheal hemangioma is a rare benign vascular tumor in adults. We reported a case of massive hemoptysis caused by a cavernous hemangioma in a 75-year-old man. This is the first report, to our knowledge, of a tracheal cavernous hemangioma that presented with massive hemoptysis. The lesion was removed with a $CO_2$ laser under rigid laryngoscopy. Endovascular tumors, such as tracheobronchial hemangiomas, should be considered a diagnostic option in cases of massive hemoptysis without a significant underlying lung lesion.

A rare case of isolated schwannoma in infraorbital nerve

  • A schwannoma is a benign tumor that develops from Schwann cells. It is known to occur more frequently in women than men, and about one third of schwannoma cases occur in the head and neck area. It is also known to originate mainly in the auditory nerve. However, it is rarely associated with the trigeminal nerve, and especially, schwannomas related to the infraorbital nerve are very rare. we report a rare case of a schwannoma involving the infraorbital branch of the trigeminal nerve in a 45-year old male adult. The patient underwent physical examination and magnetic resonance imaging. The mass was approached through subciliary approach that is familiar to the plastic surgeon and completely resected. Histopathological findings showed pointed to a benign schwannoma. Infraorbital nerve schwannoma is difficult to distinguish from other diseases by means of clinical symptoms, physical findings, or imaging. In spite of its rarity, infraorbital nerve schwannoma may be considered a possible diagnosis in the case of mass on cheek. Assessment by computed tomography or magnetic resonance imaging is necessary for proper diagnosis. About the surgical approach, excision through the subciliary approach should be considered rather than the direct transfacial approach in view of stability, cosmetic effects, and familiarity.
