Background: Individuals with mechanical neck pain show biomechanical and neurophysiological changes, including cervical spine muscle weakness. As a result of deep muscle weakness, it causes stability disability and reduced upper thoracic spine mobility, which finally leads to functional movement restriction such as limited range of motion and dysfunction. Recent studies have shown that thoracic spine manipulation and mobilization could reduce symptoms of mechanical neck pain in patients. Objects: The purpose of this study was to investigate the effects of thoracic mobility exercise on cervicothoracic function, posture feature, and pain intensity in individuals with mechanical neck pain. Methods: The study subjects were 26 persons who were randomly assigned to the experimental (with thoracic mobility exercise) and control groups (without thoracic mobility exercise), with 13 subjects in each group. The cervicothoracic function (neck functional disability level and cervicothoracic range of motion), posture feature, and pain rating (using a quadrupled visual analogue scale [QVAS]) were measured before, after 3 weeks, and after 6 weeks. Results: Statistically significant group-by-time interactions were found with repeated analyses of variance for the Korean neck disability index (KNDI), all cervical range of motion (CROM), all thoracic range of motion (TROM), cranial rotation angle, sagittal shoulder posture (SSP), and QVAS (p<.05). All groups showed significant improvements from all times in all the evaluated methods. The KNDI, CROM, TROM of left rotation, and SSP in the experimental group showed significant improvements after 3 weeks, and the TROM of the right rotation and QVAS in the experimental group showed significant improvements after 6 weeks when compared with the control group. Conclusion: Thoracic mobility exercise during 6 weeks might be effective intervention to improve the functional level, posture feature, and QVAS pain rating for managing individuals with mechanical neck pain.
Purpose : To identify the effects of Hold-Relax of proprioceptive neuromuscular facilitation (PNF) on the functional improvement of patients with neck myofascial pain syndrome. Methode : The present research investigated 60 patients with neck myofascial syndrome, dividing them into a group doing Hold-Relax of proprioceptive neuromuscular facilitation and a group doing Stretching, This study examined degree of recovery from neck pain by comparing their neck myofascial pain syndrome before and after the treatment, and compared two groups to find difference in the degree of recovery from myofascial pain syndrome. Results : 1. For Hold-Relax of proprioceptive neuromuscular facilitation group, the pain rating score (PRS) were significantly decreased for six weeks treatment. 2. For Stretching group, the pain rating score (PRS) did not indicate significant decrease for six weeks treatment. 3. For Hold-Relax of proprioceptive neuromuscular facilitation group, range of motion of flexion, left rotation, right rotation indicated significant difference after pre test and after two week but no significant difference after four week. 4. For Stretching group, range of motion of flexion, left rotation, and right rotation except extension indicated significant difference after pre test and after two week but no significant difference after four week. Conclusion : The Hold-Relax of proprioceptive neuromuscular facilitation is considered to be effective in increasing joint range of motion of patient and decreasing pain. Accordingly physical therapists has to learn a wide range of patterns and to develop various treatment techniques.
Purpose : Neck pain is commonly experienced by both adolescents and adults. The purpose of this study was to measure neck disability index(NDI) and the cervical range of motion(CROM) following spinal manipulation and mobilization techniques. Methods : Thirty participants(manipulation=15, mobilization=15) with mild neck disability volunteered for this study. It was used to measure NDI with NDI questionnaire and six motions(neck flexion and extension, left and right lateral flexion, left and right rotation) with the CROM instrument. Independent t-test and paired t-test were used to estimate NDI and CROM, and compared left with right motions. Results : A significant decrease in NDI were found after spinal manipulation and mobilization treatment(p<.05). A trend toward increase in all cervical motions(flexion, extension, left and right lateral flexion, left and right rotation), after interventions, were revealed(p<.05). The significant differences were not found on NDI and CROM between the group(p<.05). It was presented asymmetric motions in cervical lateral flexion(p<.05) before and after treatment. Conclusions : Our results suggest that manipulation and mobilization probably provide at least short-term benefits for patients with mild neck pain. It is needed to be studied the factors and preventive methods influencing the asymmetric cervical motion.
Chyle fistula is one of the complications of neck dissections. Although immediate surgical repair seems to be the best choice when chyle leakage is observed during the operation, some operators can be embarrassed when chyle leakage is heavy and not controlled during surgery. In this case, chyle leakage was occurred after extensive resection of lymph nodes in left level IV, and was not controlled in any way. The clavicular head of sternocleidomastoid muscle was dissected and inferior-based muscular flap was rotated to cover the suspected region of fistula orifice. Amount of drainage was checked less than 20 ml per day in the following days, and drain tube was taken out on the 3rd postoperative days. We present the technique using the inferior based sternocleidomastoid muscle flap for intraoperative management of chyle leakage not easily controlled.
Pain is the most common symptom that brings a patient to the hospital. Repetitious stress and sprain injury result in various pains, and so we tried to improve cervical movement and release from pain by using taping technique of actual agonists and postural muscles in addition to psychological relaxation. The 4 patient with neck problems were applied Arikawa taping approaches. The flexor or extensor patterns were determined by Arikawa method at first. if the symptoms and patterns were similar, the taping was attached same point. After taping on major muscles of causing neck pain levator scapula, scalenus medius, sternocleidomastoid, splenius capitis, semispinalis capitis. - we found neck pain released and cervical ROM increased. In conclusion, we determined cervical movement related to rotation of splenius capitis, extention of semispinalis capitis, levator scapula, flexion and rotation of scalenus medius.
Purpose: This study compared the effectiveness of three methods, fascial distortion model (FDM), myofascial release (MFR), self-myofascial release (SMR), on the neck range of motion and pain. Methods: In this study, the collected data were processed statistically using SPSS version 22.0 for Windows. Descriptive statistics were used to analyze the general characteristics of the subjects. Repeated measure ANOVA was conducted to analyze the range of motion of the neck of the group and VAS, and Contras was used to see the difference in significance over time. One-way ANOVA was used to compare the differences among the groups and a post-hoc test was used. The significance level (${\alpha}$) was 0.05. Results: In the range of motion, the flexion and extension of the neck, right rotation, and left rotation were significantly different in the SMR, FDM, and MFR groups. The right lateral flexion showed significant differences in the FDM, MFR, and SMR groups. The VAS was similar in the groups at 2 and 4 weeks, but there was a significant difference among the FDM, MFR, and SMR groups at 6 weeks. Conclusion: In this study, MFR and MSR as well as FDM were effective in controlling the range of motion and pain control of the neck. Further studies will be needed to determine the effects of long-lasting treatments other than pain control. These studies and the present study will be used as a basis for ongoing research into the duration and method of application for musculoskeletal therapies.

The purpose of this study is to investigate effects of Maitland orthopedic manipulative physiotherapy and stretching on pain, cervical range of motion, and muscle tone of adults with forward neck posture. A total 40 subjects were divided into a Maitland OMPT group(n=20) and a stretching group(n=20), performing joint mobilization exercise and stretching three times per week for six weeks. As for changes in pain, statistically significant decrease were found before and after the exercise within group comparison(p<.01), while no statistically significant difference was observed between-group comparison. In changes in cervical range of motion before and after the exercise, the Maitland OMPT group showed statistically significant increase(p<.01) in flexion, (left lateral flexion(p<.05), extension, left rotation, right rotation, and right lateral flexion, while the stretching group showed statistically significant increase(p<.05) in extension(p<.01), left rotation, left lateral flexion, right rotation, and right lateral flexion. However, no significant differences in between group comparison in flexion, extension, right rotation, left rotation, right lateral flexion and left lateral flexion. The results of measuring muscle tone changes showed that the Maitland OMPT group and the stretching group did not show significance in within and between group comparison(p<.05). In conclusion, the Maitland OMPT and stretching were effective on improving pain and range of motion.
PURPOSE: This study aimed to provide basic clinical data by analyzing the impact of motion observation training and stretching exercises for improving postures on the neck alignment and balance of stroke patients to enable them to accurately recognize the correct exercise method. METHODS: After sampling 20 stroke patients who met the selection criteria, this study randomly assigned 10 people who were administered the stretching exercise with observation training to the experimental group and 10 who received only the stretching exercise intervention to the control group by drawing lots. Next, neck alignment and balance were pre-tested. All interventions were conducted for 30 minutes, 3 times a week for 4 weeks, and when all the interventions were completed after 4 weeks, neck alignment and balance were re-measured in the same way as the pre-test. RESULTS: The comparison of changes in neck alignment and balance within the experimental and control groups showed statistically significant differences in the craniovertebral angle, cranial rotation angle, and balance (p < .05) (p < .01). Between the groups, statistically significant differences were found in the craniovertebral angle, cranial rotation angle, and balance (p < .05) (p < .01). CONCLUSION: A statistically significant difference in neck alignment and balance was observed in the group that underwent stretching exercises combined with observational training and a statistically significant difference was found between the groups. Therefore, it is believed that observation training should be used in clinical practice to improve forward head posture and restore balance in stroke patients.
