안면골 변형을 동반한 림프관종의 치험례 (Treatment of Lymphangioma combined with Facial Bone Deformity)

  • Lymphangioma is a benign, growth of lymphatic tissue that is present at birth or develops in early childhood, which may cause serious alterations in growth and developmemt. The problems with facial lymphangioma is usually releated directly to their size and to the area of the face which is involved. The lesions themselves may range from small, localized blemishes to huge facial masses involving both soft tissue and underlying bone and causing great distortion and asymmetry. The facial bones are seldom involved, but the natutal evolution of an individual lesion often cannot be accurately predicted when the child is first seen. Any changes in the underlying facial bone could be due either to a direct growth of the lesion into the bone, or secondary to pressure of the lesion growing outside the bone itself. A case of cystic lymphangioma extending from the neck to the tongue is reported. A six-year-old female was admitted because of swelling of the tongue. At that time, the tongue reportedly reached the extraoral size of 7x5x2.5cm and a soft, diffuse swelling of left anterior neck was revealed. The removal of cystic mass including left neck dissection and partial glossectomy were undertaken. The another case of lymphangioma is located on mandibular cheek. A twenty nine-year-old male was admitted because of palpable mass of the left mandibular area and fissure of palate. The radical excision of mass with mandibulectomy of body were undertuken. Thus we reported such a rare case and reviewed the lymphangioma.

두경부 영역에서 연골성 종양 치험례 (Two Cases of Cartilagenous Tumor in the Head & Neck)

  • Chondrosarcomas are uncommon disease that are noted almostly in the pelvis, sternum, long bone. Chondrosarcoma in the head & neck was very rare. It can occur in nearly every bone in the head and neck. Lesion of the maxilla is most frequent site in head and neck region, but temporomandibular joint is rare site. In chondrosarcoma of temporomandibular joint, its danger lies in its local invasiveness and potential to seed into the meninges. Histologically, the tumor exhibits myxoid feature, which must be differentiated from chordoma and chondroid chordoma. The cranial nerve palsies frequently observed with the tumors are related to the anatomical location. Chondroma is a benign tumor that most frequently found in the hand, foot bone, but can be originated in any cartilagenous area of body. When it occurs in one site, it is named as solitary enchondroma, and when it occurs in multiple site, it is named as multiple enchondromatosis. In the head & neck, it may occur in the nasal cavity, nasopharynx, nasal septum, eustachian tube, palate. But chondroma that occur as neck mass is extremely rare. Recently, the authors experienced a case of chondrosarcoma confirmed pathology affecting 35-years-old female presenting mass in left temporomadibular joint and a case of chondroma confirmed pathology affecting 26-years-old female presenting neck mass, left.

단일성 안와 골병변으로 확인된 갑상선 여포암의 원격 전이 1예 (A Case of Solitary Metastatic Deposit in the Orbital Rim from Follicular Thyroid Cancer)

  • Follicular thyroid carcinoma (FTC) is the second most common thyroid cancer, following papillary carcinoma. Metastasis to the orbital rim from FTC is very rare. We recently experienced a case of FTC with metastasis to the orbital rim in a 74-year-old woman, who initially presented with a huge thyroid mass and an asymptomatic solitary orbital rim lesion. The solitary orbital rim lesion was suspected to be a separate disease entity such as lymphoma from the preoperative imaging, but bone metastasis from FTC was finally confirmed after orbital rim resection and total thyroidectomy. During follow-up, the patient presented multiple bone metastasis, so the solitary orbital rim lesion was considered a clinical sign of systemic metastasis from FTC. Therefore, we present this unique case with a review of the literature.

길리씨 접근법을 통한 협골궁 골절의 정복 (Reduction of Isolated Zygomatic Arch Fractures with Gillies Approach)

  • Background and Objectives The incidence of facial bone fracture is increasing. The zygomatic bone, due to its anatomical prominence, is the second most common site of all facial bone fractures. In this study, we present the clinical experiences of zygomatic arch fracture in a tertiary hospital and introduce the Gillies approach for reduction and its outcome results. Subjects and Method We collected data from retrospective chart reviews of patients who underwent surgeries from 2010 to 2017 for zygomatic arch fractures at Chungbuk National University Hospital. Data were analyzed according to age, gender, cause of trauma, location of trauma, and clinical symptoms including trismus. All surgery was performed under general anesthesia and via the use of Gillies approach. The result of surgery was evaluated by postoperative facial computed tomography. Results Sixteen patients underwent surgery for zygomatic arch fracture. The patients had the average age of 41.3 years, a male predominance of 15:1 and physical assault as the most common cause of trauma. The time lag between injury and surgical reduction was 5.5 days. The surgical outcomes were assessed 'good' in 14 cases and 'moderate' in one case. Patients who had trismus preoperatively were resolved of it in all cases after operation. Postoperative complications were absent. Conclusion The Gillies approach proved to be a relatively easy, safe, and reliable method, and its surgical outcomes was satisfactory in our experiences.

두유를 통한 이소플라본 공급이 저체중 여대생의 골밀도에 미치는 영향 (The Effect of Isoflavone Supplementation by Soymilk on Bone Mineral Density in Underweight College Women)

  • The purpose of this study was to investigate the effect of isoflavone supplementation using soymilk on bone mineral density in underweight college women aged 19-22 years with lower than average bone mass. The bone mineral density (BMD) of the lumbar spine and femoral neck of a number of underweight (BMI < 20) college women was measured. From this group, 33 subjects whose T-score value was below -1 were selected. Anthropometric measurements, dietary recall and BMD were measured before the study began and at the end of the 10th week. The 33 subjects were given 400 ml of soymilk containing 60 mg of isoflavones daily for 10 weeks. After 12 weeks at the end of 10 weeks treatment, the BMD of the subjects was reassessed. The average age of the subjects was 21.1 yrs. At the baseline, average height, weight, and BMI of the subjects were 161 cm, 49.1 kg, 18.8 kg/$m^2$, respectively. Mean daily intake of energy and protein was 1637.2 kcal and 61.2 g, respectively. Mean dietary isoflavone intake was 17.4 mg. Despite the 400 ml soymilk supplementation, there was no significant change in nutrient intake after the 10-week treatment. However, there were significant increases in lumbar spine and femoral neck BMD. After the study, the BMD of the subjects' spine and femoral neck was lower than that before the study. In conclusion, soymilk intake was associated with a significant increase in the BMD of the lumbar spine and femoral neck in underweight college women with lower than average bone mass. 임프란트 고정체와 지대주 직경의 차이가 응력분포에 미치는 영향 (THE EFFECT OF THE DIFFERENCE OF THE IMPLANT FIXTURE AND ABUTMENT DIAMETER FOR STRESS DISTRIBUTION)

  • Statement of problem : Stress concentration on the neck bone affects the bone resorption, and finally the implant survival. Purpose: In order to examine the stress distribution on the neck bone and prosthesis abutment for implants, decreasing abutment sizes were used. Material and methods : Axisymmetric models were used to obtain the data required. These models were composed of 4mm implants with 3.4mm and 4mm abutments, 5mm implants with 3.4mm and 5mm abutments and 6mm implants with 3.4mm and 6mm abutments. All abutments were designed to received a 10mm high by 10mm diameter gold crown. Functional element analysis was used to obtain these results using data that consisted of 50 N vertical and 45 degree inclination forces. Results : 1. Changing the diameter of the abutment on the implant affects the effect of the inclination forces more than the effect of the vortical forces. 2. Changing the diameter of the abutment on the implant affect the effect of the inclination forces more than the effect of the vertical forces. 3. Experimentation showed that the larger diameter implants provided a decreased neck bone stress, whereas a larger diameter abutment provided a decrease marginal abutment stress. 4. Experimentation showed that the neck bone and abutment received more stress from inclination forces than vertical forces, Conclusions: By decreasing the size of the abutment on the implant we were able to diminishneck bone stress.

외측연결형 임플란트 고정체의 경부 디자인이 임플란트 주위조직에 미치는 영향에 대한 전향적 예비 임상연구 (Effect of neck design on peri-implant tissue responses in external connection type implant : a prospective pilot clinical study)

  • This clinical study was conducted to evaluate the clinical effects of a concave neck of external connection type implant fixture designed for platform switching on the peri-implant tissue responses. Two types of implants with different neck designs were implanted in 20 patients. For the experimental group, the bioseal(BS) implant fixtures with 's' shaped concave profile on the neck were used, and non-bioseal(NBS) implant fixtures with a straight profile on the neck were used as control(Total of 40 implants, NBS: n = 19, BS: n=21). During the one-year period after implant placement, implant survival rate, marginal bone resorption, bleeding, plaque, and complications were evaluated. The survival rate of NBS and BS group was 94.74% and 90.48%, respectively. There was no significant difference on marginal bone resorption, bleeding and plaque between the two groups (P>.05). Within the limits of the present study, implants with a concave neck design showed similar clinical results to implants with a straight neck design on the peri-implant tissue responses. Longitudinal clinical studies are necessary to confirm more effective clinical results.

한국인 남성 운동 선수군에서 Calcitonin Receptor 유전자의 AluI RFLP 분석 (AluI RFLP Analysis of the Calcitonin Receptor Gene in the Korean Athletic Men)

  • Bone mineral density (BMD) is influenced by genetic and environmental factors. Among genetic study; calcitonin receptor (CTR) gene is a good candidate influencing the inter-individual difference in BMD because CTR is involved in calcium and bone metabolism. Thus, we investigated the distribution of C1377T polymorphism in the CTR gene among male Korean elite athletic and control groups, respectively and also an association with BMD in lumbar spine and femoral neck. Our results suggested that this polymorphism of CTR gene was not significantly associated with lumbar spine or femoral neck BMDs in the both groups, respectively. However, we found that there was the racial difference in genotype distribution of this polymorphism between Caucasian and Asian populations. Though we could not detect the significant association between C1377T polymorphism of CTR gene and lumbar spine or femoral neck BMDs, further studies using other ethnic groups are necessary to clarify the precise role in BMD of CTR gene.

폐경 전 여성의 모유수유기간과 골밀도와의 관련성 연구 (The Relationship of Bone Mineral Densities and Period of Breast feeding in Premenopausal Women)

  • To determine whether personal history of lactation in premenopausal women influence bone mineral density, a cross-sectional study was conducted. One hundred eighty-four premenopausal women were selected from women who had been checked for bone mineral density by dual energy x-ray absortiometry in lumbar spine, femoral neck, Ward's triangle, and trochanteric site at general hospitals in Seoul and Pusan. They completed a questionnaire including life style factors and reproductive history. In the data analysis, Pearson correlation coefficients were used to test any association between individual variables and bone mineral density and a statistical comparisons between long term lactation(>24 months) and short term lactation(<24 months) were made by one way analysis of covariance. The results were summarized as follows: 1) There was no significant difference in the bone mineral density of the lumbar vertebrae in premenopausal women between the long term lactation group(>24months) and the short term lactation group(<24months). 2) There was no significant difference in the bone mineral density of the femur neck, Ward's triangle, and trochanteric site in premenopausal women between the long term lactation group (>24months) and the short term lactation group (<24months). Considering these results, we suggest prospective studies that measure bone mineral density before and after, in addition to those during lactation. We also suggest the further study with premenopausal women less than 35 who have achieved peak adult bone mass.

한국 남자의 연령별 골밀도에 영향을 미치는 영양요인 분석 (Nutritional Factors Related to Bone Mineral Density in the Different Age Groups of Korean Men)

  • Nutritional factors affecting bone mineral density (BMD) in the different age groups of Korean men were investigated to obtain baseline data for maintaining bone health. Information on diet and anthropometry was collected in 80 elementary school children, 83 high school students, 87 adults aged 25 to 35 years and 98 elderly people over 60 years of age. Data for food and nutrient intake were obtained by 24-hour recall method. BMDs of lumbar spine (L$_2$- L$_4$) and femoral neck were measured by dual energy x-ray absorptiometry. The relationship between BMD and nutritional factors were analyzed. In the femoral neck, 5.7% of adults was classified as osteopenia and 47.9% and 37.8% of the elderly were classified as osteopenia and osteoporosis. It was shown that plant protein, Ca, p, Fe, thiamin, riboflavin and vitamin C intakes were related with BMD in all age groups. As for the mean adequacy ratio (MAR) of nutrients, the lowest quartile group of BMD showed significantly lower MAR among children. The RDA percent of nutrients was a strong influential factor on BMD. Subjects who consumed below 75% of Korean RDA in energy, vitamin A, thiamin, and vitamin C showed lower BMD. Stepwise multiple regression analysis revealed that MAR in children, vegetable Ca in adolescents, and vitamin C in adults and elderly people were the highest influential factor on BMD. Therefore, the above results demonstrated that not only calcium but also other nutrients such as protein, iron, vitamin A, riboflavin, and vitamin C were necessary in order to keep the healthy bone status. In addition, although there were various dietary factors that influenced bone density, MAR was identified as the major factor that affected bone density. Thus, a balanced diet that includes all nutrients is necessary for a healthy bone density. (Korean J Nutrition 37(2) : 132-142, 2004)
