This study aims to estimate the consumption of antibiotics in Korea and to suggest the further studies. To measure the amount of antibiotics consumption, we referred to the statistic of NFHI(National Federation of Health Insurance) and a private institute of pharmaceutical information(Korea Intercontinental Medical Statistics; IMS Korea). There were 1,563 antibiotics produced in Korea in 1997. The total amount of antibiotics production was 1,197 billion won in 1997. Antibiotics accounted for 17.6% of the total pharmaceutical productions in 1997. Cephalosporins have taken the largest part of antibiotics production since 1992. The estimation using NFHI data showed that the total expenditure of antibiotics used in health facilities was 268 billion won, 608 billion won, 911 billion won in 1990, 1994, 1997 respectively. Tertiary hospitals spent 246 billion won, general hospitals 287 billion won, hospitals 78 billion won, clinics 300 billion won in 1997. The amount of expenditure and the intensity of antibiotics consumption in hospitals have increased more steeply than any other health facilities. The total expenditure of antibiotics consumption in health facilities and pharmacies was 778 billion won when estimated using the data from IMS Korea, and 999 billion won from NFHI. Cephalosporins was the fast growing antibiotics group in all of the market- hospitals, clinics, pharmacies since 1991. To measure the amount and patterns of antibiotics consumption more precisely, a pharmaceutical monitoring or surveillance system is needed.
RIB(Rigid Inflatable Boat) is widely used for coastal transportation in the commercial use and for ISR(Intelligence, Surveillance, Reconnaissance) in the military use. Since RIB is around 10 meters in length and over 30 knots in speed, its motion characteristics in waves is quite different from a large scale ship. When it turns, large roll occurs and heeling direction is opposite to the large ship's case. Currently, many countries are developing USV(Unmanned Surface Vehicle) of which type is RIB. In order to develop high performance autopilot and way point controller, it is very important to identify RIB's motion characteristics. In this paper, sea trial test results of a 7-meter RIB such as speed, turning, zig-zag, and way point control tests were represented and its resistance and propulsion model was identified by using sea trial data and Savitsky's formula. In addition, the state space model which will be used in the identification of the four-degree-of-freedom dynamics in the next step was formulated and the identification procedure was proposed.
Purpose: Recently an experience in the MERS crisis focused on the importance of infection control in hospitals. According to Korean National healthcare-associated Infection Surveillance System (KONIS) of the KSICP, a great number of 498 people, 841 people, and 1021 people were infected by pneumonia, urinary tract infection, and bacteremia respectively from 94 hospital ICUs during the year of 2014. Therefore, the purpose of this study is to investigate the configuration and design guidelines for the ICU rooms to minimize the nosocominal infections. Methods: Based on the several infection control guidelines and revised Medical Law, consequent analyses which classified the planning and operational behavior in the ICUs of seven hospitals, were performed to reduce the cross-infection. Results: The results of this study are offering a space, configuration and design guidelines for effective infection control in the intensive care units through the unit-bed area, the bed-to-bed distance, the isolation room, etc. Implications: It is expected that this study propose the direction of architectural planning and guideline for the ICU room in order to realize the intension of revised Medical Law.
This study aimed to assess the quality of outpatient antibiotic consumption in Korea compared with other countries. We used the National Health Insurance claims data for outpatient services in March, June, September, and December in 2012 and calculated nine indicator values based on the 12 European Surveillance of Antimicrobial Consumption (ESAC) drug-specific quality indicators. Indicator values in this study reflect the yearly use of antibiotic expressed in defined daily doses for 1,000 inhabitants per day (DID) and the use of main subclasses of antibiotics expressed in DID and as percentage of the total antibiotic use. Korea showed lower quality in the consumption of total antibiotics (J01), especially in the use of Cephalosporins (J01D) expressed in DID. Korea also showed low quality with regard to the use of narrow/broad spectrum antibiotics. The percentage of the use of narrow-spectrum Penicillins (J01CE) was lowest in Korea. The quality on the use of the third- and fourth-generation cephalosporin (J01(DD+DE)) was the fourth lowest among 26 countries. High rates of antibiotic resistance and payment system based on fee-for-service might have influenced on the high consumption of the broad spectrum antibiotics in Korea. It needs to further investigate the use of broad-spectrum antibiotics to identify the target of strategies promoting quality use of antibiotics in Korea.
Jejudo has unique geographical and climatologic characteristics compared with other areas in Korea. The aim was to evaluate differences of the nationwide notifiable infectious diseases (NND) occurred in Jejudo, 2011~2018. The source of data was on the Infectious Disease Statistics System operated by Centers for Diseases Control & Prevention in Korea between 1 January 2001 and 30 September 2018. After calculating the proportion of sum of years showing a higher incidence in Jejudo than nationwide divided by sum of comparing years(PSYH), authors defined the infectious diseases for control in Jejudo (IDCJ) with PSYH over 75%. In addition, the infectious diseases showing increasing trends during recent 2 years(2015~2018) were defined as sudden increase Infectious diseases for control (SIDCJ). IDCJ in 2001~2018 included 3 NNDs such as chicken pox, syphilis, and Streptococcus pneumoniae. The results support main evidences to plan strategies and distribute community resources for control and prevention of NNDs in Jejudo, Korea.
The relationship between the climate and the number of heat-related patients in South Korea was analysed in this study. The number of the patients was 1,612 during the summer 2011 to 2015 according to the Heat-related Illness (HRI) surveillance system. The coefficient of determination between the number of the patients and the daily maximum temperature was higher than that between the number of them and the other elements: the daily mean/minimum temperature and relative humidity. The thresholds of daily maximum and minimum temperature in metropolitan cities (MC) were higher than those in regions except for MC (RMC). The higher the maximum and minimum temperature became, the more frequently the heat-related illness rate was observed. The regional difference of this rate was that the rate in RMC was higher than that in MC. Prolonged heat wave and tropical night tended to cause more patients, which continued for 20 days and 31 days of maximum values, respectively. On the other hand, the relative humidity was not proportional to the number of the patients which was rather decreasing at over 70% of relative humidity.
Background: Monoclonal antibodies for the treatment of patients with different types of cancer, such as cetuximab, have been widely used for the past 10 years in oncology. Although drug information package insert contains some representative adverse events which were observed in the clinical trials for drug approval, the overall adverse event patterns on the real-world cetuximab use were less investigated. Also, there have been no published papers that deal with the full spectrums of adverse drug events of cetuximab using national-wide drug safety surveillance systems. Methods: In this study, we detected new adverse event signals of cetuximab in the Korea Adverse Event Reporting System (KAERS) by utilizing proportional reporting ratios, reporting odds ratios, and information components indices. Results: The KAERS database included 869,819 spontaneous adverse event reports, among which 2,116 reports contained cetuximab. We compared the labels of cetuximab among the United States, European Union, Australia, Japan, and Korea to compare the current labeling information and newly detected signals of our study. Some of the signals including hyperkeratosis, tenesmus, folliculitis, esophagitis, neuralgia, disseminated intravascular coagulopathy, and skin/throat tightness were not labeled in the five countries. Conclusion: We identified new signals that were not known at the time of market approval.
Objective: This study aims to analyze the characteristics of airborne radon and thoron level ($Bq/m^3$) generated from household products containing monazites, and estimate the effective doses (mSv/yr). Method: Radon & Thoron detector EQF3220 was used to monitor real-time airborne radon and thoron level ($Bq/m^3$), and their daughters ($Bq/m^3$) were recorded every two hours. Effective doses (mSv/yr) for radon and thoron were estimated according to models developed by International Commission on Radiological Protection (ICRP) and United Nations Scientific Committee on the Effects of Atomic Radiation (UNSCEAR). Results: The average levels of radon and thoron were $87.8Bq/m^3$ (range; $20.8-156.3Bq/m^3$) and $1,347.5Bq/m^3$ (range; $4-5,839.7Bq/m^3$), respectively. The average equilibrium factors (F) were 0.23 and 0.007, respectively. The levels of radon progeny were far higher than that thoron. Latex mattress showed the highest F (0.38). The average effective doses were estimated to be ICRP (1.9 mSv/yr) and UNSCER (1.3 mSv/yr) for radon and UNSCEAR (1.6 mSv/yr) for thoron. Conclusions: Our results have far exceeded the allowable effective dose for general population (1 mSv/yr). The government's actions such as the ban of use of consumer products containing monazite and the establishment of surveillance system to evaluate health effects for the people affected should be taken as early as possible.
The IUU Fishing Index is composed of 40 indicators. These indicators were grouped by state responsibilities (flag, coastal, port, and general including market) defined in the FAO IPOA-IUU (2001) and then by type into vulnerability, prevalence, and response. A total of 152 coastal nations was surveyed. Korea's total combined IUU Fishing Index was 2.49 in 2019 and 2.91 in 2021, indicating a drop in the ranking to the third worst out of 152 countries followed by China and Russia in 2021. The indicators that increased the IUU fishing risk in 2021 compared to 2019 included seven indicators of prevalence and two indicators of response while those reducing the risk included one prevalence and one response indicator. The IUU Fishing Index revealed that many fisheries observers and monitoring, control and surveillance (MCS) practitioners active in the waters of RFMOs jurisdiction where Korean distant water vessels operate have mentioned concerns about the compliance with RFMO conservation measures or fishing practices. It suggested that strengthening management intervention in the fishing sector is needed. The primary tool for management is the MCS system. Given the logistical difficulty of oversight from land, air and at-sea, there is a need to enhance MCS strategies through logbook data, at-sea observer and electronic monitoring program. It also suggested that MSC fisheries certification and fisheries improvement projects, which are widely used for improving fishing sector performance, could contribute to the eradication of IUU fishing and the promotion of sustainable distant water fisheries.
