Purpose: To determine the radiographic changes in forefoot geometry with weight-bearing. Materials and Methods: The forefoot radiographs of 100 normal Korean adults, 50 male and 50 female volunteers, were evaluated both in nonweight-bearing and weight-bearing. The mean age was 27 years with range of 21-39 years. Those with normal feet were selected from volunteers having no history of foot problems or other musculoskeletal diseases. Results: The changes of measured angle between phalanges and metatarsals with weight-bearing were as follows; Hallux valgus angle was noted to increase in 20% of the feet, decrease in 59%, and remained unchange in 21%. Intermetatarsal angle $1{\sim}2$ was noted to increase in 76% of the feet, decrease in 3%, and remained unchange in 21%. Intermetatarsal angle $1{\sim}5$ was noted to increase in 95% and remained unchange in 5%. Shift in medial sesamoid on weight-bearing was also not consistent. Lateral shift was noted in 27%, no shift in 66%, medial shift in 7%. Conclusion: The generalized concept that the angles between bones and shift of medial sesamoid in the forefoot will change consistently with weightbearing was not found.
Objectives : Juglandis Semen herbal acupuncture solution(JSS) has a broad array of clinical applications in oriental medicine, including treatment of chronic musculoskeletal diseases such as arthritis. This study was performed to investigate the global gene expression profiles using microarray assay in RAW 264.7 cell line treated with JSS and to advance our understanding of the pharmacologic effect of JSS. Methods : Change of the gene expression profile in RAW cell line following treatment with lipopolysaccharide(LPS) alone, or with LPS plus JSS was investigated with a cut-off level of 2 fold change in the expression. Especially, Change of the gene expression by treatment with LPS alone was compared with that by treatment with LPS plus JSS with a cut-off level of 1/2 fold change in the expression. Results: Of the 8170 genes profiled in this study, 51 were upragulated and 21 downregulated following LPS treatment, and 88 were upregulated and 69 downregulated following costimulation of JSS and LPS. Of the 51 genes upregulated following LPS treatment, 10 were downregulated following costimulation of JSS and LPS. Of the 21 genes downregulated following LPS treatment, 3 were upregulated following costimulation of JSS and LPS. Conclusion : JSS treatment induced upregulation of some genes including IL-10 and downregulation of that including MMP13 with its possible implication in an antiinflammatory action of JSS. However, further research on expression profile changes induced by JSS treatment is expected.
The purpose of this study was to examine the effects of hip joint mobilization (HJM) on walking ability, balance ability, and the joint range of motion in stroke patients to minimize the problems of the musculoskeletal system in patients with central nervous system diseases. All volunteers were randomly assigned to the HJM group ($n_1=14$) and the general neurodevelopment therapy (NDT) group ($n_2=16$). The HJM procedure involved applying Maitland mobilization techniques (distraction, lateral gliding, inferior gliding, and anterior gliding) by grade 3 to both hip joint. The mobilization process included mobilization and NDT for 15 min/day, 3 days a week for 4 weeks. The outcome measures were evaluated, including the hip joint passive range of motion (ROM) test and femur head anterior glide test (FHAG) using prone figure four test, dynamic and static balance abilities [timed up and go (TUG) test and center of pressure (COP) analysis], and walking ability [10-meter walking test (10MWT) and 6-min walking test (6MWT)]. Both the groups showed significant post-training differences in the hip joint ROM (FHAG and degree of hip extension) and 10MWT. The post-training improvements in the TUG test were significantly greater in patients of the HJM group than in the NDT group; however, there were no post-training improvements in COP in both groups. Patients in the HJM group showed post-training improvement in the 6MWT; however, statistically significant differences were not observed. Patients in the NDT group showed post-training improvements in the 6MWT. These results suggest that HJM improves hip joint ROM, dynamic balance ability, and walking speed in stroke patients. However, further studies are required to evaluate the long-term therapeutic efficacy of HJM in stroke patients.
Objectives : This study was aimed to provide the basic characteristic of patients who visited an Oriental medical hospital emergency room. Methods : We performed a retrospective study on 1,119 patients who visited Kyunghee University Oriental Medical Hospital Emergency Room from June 1, 2011 to March 31, 2012. Results and Conclusions : 1. In distribution of sex, the rate of males was 44.1%, and that of females was 55.9%. The male to female ratio was thus 1:1.26. The patients' average age was 54.64 and the 6th decade of life was the peak age group. 2. Most of the patients visited the Oriental medical hospital emergency room between 11:00 and 23:00. 3. In distribution by days of the week, the most common was Sundays (24.5%), followed by Mondays (16.7%). 4. In the month distribution, the most was December (12.4%), followed by November (10.0%). 5. The patients were grouped as follows: neurological diseases 58.9%, musculoskeletal disease 27.5%, internal medical disease 10.4%, and others 3.2%. 6. Admission rate of patients being treated was 38.8%, and the admission to the intensive care unit (ICU) was 4.5%. of total visiting patients.
Objectives : This study was aimed to promote the cooperative system of Korean and Western medicine in the dual health care system through a survey of physicians on recognition, problems and solution of the cooperative system. Methods : The research took place at Dongguk University Hospital from May 25 to 27, 2009 with 44 professors, residents, and interns employed by the hospital. Results : Of total 44 surveyed doctors, positive and moderate responses on the cooperative system between Korean and Western medicines were 40.9% and 43.2%, respectively. They scored it positive (62.5%) and moderate (31.3%) based on their experiences. These results can be supposed to represent the environment for the interdisciplinary medicine. Even in the interdisciplinary hospital of Korean and Western medicine, 68.2% of responders had no experience of the cooperative medical system. Expected interdisciplinary efficient departments were ordered rehabilitation medicine, neurology and orthopedics and associated with musculoskeletal disorders, the most frequent diseases treated by Korean traditional medicine. Conclusions : Korean and Western medical doctors, as medical personnel, intellectual persons as well as specialists, need a recognition and attitude to understand and respect each others' medicine. However, both groups also realize there are many complicated issues in the treatment of patients. It is difficult to require a change of Western doctors' perceptions and attitude toward Korean traditional medicine only with results of a survey. For the efficient cooperative system in the medical field, Korean medical doctors will need to study and consider specific problems mentioned by Western medicine.
Objectives: The purpose of the study is to investigate the musculo-skeletal pain prevalence and severity in the dental hygienists based on PRECEDE model. Methods: A self-reported questionnaire was completed by 483 dental hygienists in Gwangju from September 13 to October 12, 2013. Data were analyzed by frequency analysis, chi-square test, t-test, and multiple logistic regression analysis using SPSS 18.0 program. Musculo-skeletal pain severity was classified from 1 to 5 by PRECEDE model. The questionnaire consisted of six questions of the general characteristics of the subjects, one question of musculo-skeletal pain prevalence, one question of body part musculo-skeletal pain prevalence, one question of subjective health status, three questions of activities of daily living, six questions of working environment, one question of musculoskeletal system diseases knowledge, two questions of social support, two questions of education experience and data use method, and five questions of necessity of health education. Results: The prevalence rate of musculo-skeletal pain within a year was 83.9% and 22.8% of the dental hygienists complained of severe pain. The odds ratio of moderate pain severity was 1.99(95% CI, 1.10-3.60) and the odds ratio of unhealthiness was 3.27 (95% CI, 1.35-7.94). The odds ratio of pain severity in those working for 4-6 years was 0.21(95% CI, 0.08-0.57). The odds ratio of pain severity in those practicing 6-10 scaling cases per day was 0.33(95% CI, 0.17-0.65). The odds ratio of pain severity in wrist turning and bending was 3.56(95% CI, 1.19-10.62). Conclusions: The muscolu-skeletal pain severity in the dental hygienists was closely associated with subjective health condition, work duration, the number of scaling practice activity, and a treatment posture. Regular physical checkup for the dental hygienists will improve the musculo-skeletal pain due to scaling practice.
Discomfort as well as muscular pain and musculoskeletal diseases occur in cases of stressed posture due to prolonged pressure. Therefore, the American Academy of Orthopaedic Surgeons (AAOS) and Pediatric Orthopaedic Society of North America (POSNA) recommend that bags that weigh no more than 15 - 20% of the wearer's weight. However, despite the prolonged pressure from using baby carriers, there are no recommendations and limited studies on how to protect wearers. Therefore, this study investigates subjective fatigue according to the usage of waist support and hip seat. Based on this, muscle fatigue and secondary subjective fatigue according to three types of currently commercial baby carriers (X-type, H-type, and H-hip type) were measured. Subjective comfortability was evaluated on a 5 point Likert-scale and subjective fatigue was evaluated on Borg's CR-10 scale. Objective muscle fatigue was also compared by measuring and analyzing electromyogram (EMG). The results of primary subjective fatigue showed statistically significant changes in the shoulders, waist, calves, and soles of the feet according to the usage of hip seats and waist support. Electromyogram measurements also showed less muscle fatigue at the upper trapezius muscle and thoracic erector spine muscle for the H-hip seat baby carrier, which has both waist support and hip seat, compared to an X-type baby carrier, which has neither. However, results of subjective fatigue showed opposite results at the waist despite having same results at the shoulders and beneath the shoulders. This show discrepancies between objective muscle fatigue and subjective fatigue; therefore, that both aspects must be taken into consideration when developing ergonomic baby carriers.
Low back pain is generally defined as a complex symptom rather than a specific diseases entity, especially when musculoskeletal lesions of the low back, neurogenic, vasculogenic and discogenic lesions are present. Sixty to eighty percent of all human being have experienced low back pain once or more times during in their lifetime. Low back pain is a one of the commonest cause of physical impairment and induces disability the activities of everyday living or the activities of work. Today, low back pain plays important part for the loss of working capability from social, industrial field and individual daily life. But there are no study about incidence and prevention program of low back pain in rural peoples. This study was conducted to find out current status and characteristics related to low back pain in rural peoples(especially engaged in agriculture), so that could provide basic data to establish prevention program for them. The data were gathered from 301 rural adults(female 62.1%, male 37.9%) with low back pain by the structured interview in the 4 communities(south and north Gyeong-sang Do and Chung-cheong Do), and sampled at local hospital, local clinic, and health clinic center in May, 2000. The results were as followed. About one third of cases(35.5%) were aged from 60 to 69, 34.6% were smoker, and 91.4% not used to regular exercise. Low back pain frequently due to intervertebral articulopathy(42.5%), follewed by herniated intervertebral disk(19.9%), sprain(17.3%), vertebral dislocation(4.0%), and etc(16.2%). Above half of cases(56.5%) were a eight-hour workday, and 61.8% complained of low back pain during work. Mostly, period of agricultural experience was more than 20 years, and cultivated more than 2 crops. Low back pain is common to rural adults, therefore adequate resting time from work and lumbar strengthening exercise program needed to prevent low back pain in rural peoples.
Purpose. Walking depends on the speed and type of shoe to be worn, and the degree of impact varies with the muscle used. In addition, the speed can be changed by moving objects and using objects when walking. This study analyzed the change of walking speed by applying various factors influencing walking. Methods. A total of 60 patients who had not undergone musculoskeletal diseases during the last 1 year were included. Shoe type was divided into slippers and shoe heels. Behavioral types were divided into bagging, books, and cell phone use. The walking speed was measured by the general walking, the middle walking, and the fast walking. The time was measured using a 10M linear distance test. The collected data were analyzed with SPSS program for independent samples t-test, one-way ANOVA. Results. There was a statistically significant difference according to the type of shoes when walking. Walking speed was slow in shoe heel. In addition, There was statistically significant difference according to type of behavior task at walking. Walking speed was slow in task type using mobile phone during walking. Conclusions. The walking speed were appeared difference in each type of shoe heel, using mobile phone.
Objectives: The purpose of this study is to investigate clinical features of children and adolescents patients admitted to the oriental medical hospital. Methods: The study composed of 1284 inpatients aging from 0 to 19 who had been admitted in 4 oriental medical hospitals located in Daejeon and Chungcheong. Results: 1. In this study, 688 boys and 715 girls were examined, giving the ratio of 0.96:1. Average age of the children and adolescents was 11.5, and the number of inpatients has increased as getting older. 2. The percentage of children and adolescents inpatients has increased every year : 9.1% in 2005, 13.0% in 2006, 13.8% in 2007, 16.3% in 2008, 18.0% in 2009, with 22.2% in 2010, 7.8% until May 2011. The month scored the highest admittance was December and the lowest was March. 3. In systemic division, the musculoskeletal system was the most common, followed by neuromuscular system, respiratory system, digestive system, psychological system, and nervous system. 4. Most patients have admitted in acupuncture & moxibustion department, followed by oriental pediatrics, oriental rehabilitation medicine. 5. In Pediatrics, 196 boys and 166 girls were admitted. The age distribution showed that 6.4% were in infancy, 43.6% in preschool period, 30.1% in prepuberal period, 18.8% in early puberty and 1.1% in late puberty. The percentage of pediatrics inpatients has increased every year. Conclusions: More data of patients should be collated for further study. More detailed study with different kinds of diseases are expected based on this study.
