Purpose : The examine the reliability and validity of the modified Emory Functional Ambulation Profile(mEFAP) for assessing gait function in chronic stroke patients. Methods : A total of 45 stroke patients, who had a stroke more than 6 months, participated in the study. Reliability was determined by Intra-class Correlation Coefficient($ICC_{3,1}$), including Bland and Altman method (Standard Error of Measurement: SEM, Small Real Differences: SRD). Validity was examined by correlating results to the gait ability(mEFAP, Modified Motor Assessment Scale-Gait(MMAS-G), Scandinavian Stroke Scale-Gait(SSS-G), Functional Ambulation Category(FAC), 10m Waking Test(10m WT)), and Fugl Meyer-Lower/Extremity(FM-L/E), Berg Balance Scale(BBS). Results : Inter-rater reliability for the total mEFAP was High($ICC_{2,1}$=.998), and absolute reliability were excellent (SEM: 1.75, SRD: 4.85). Subjects without assistance factor performed better on all tests than did subjects who had stroke. There were significant correlations between the mEFAP and MMAS-G, SSS-G, FAC(r=-.66~-.79), 10 m WT(r=-.86), and FM-L/E, BBS(r=-.72~-.78), indicating good validity. Increased times on the mEFAP correlated with poor performance on the gait ability, motor function of lower extremity, BBS and slow gait speeds on the 10 m WT in stroke patients. Conclusion : The mEFAP can be administered easily and comprehensively. It is a reliable gait assessment tool for patients with stroke and correlated with known of function, the mEFAP may be a clinically useful measure of ambulation.

PURPOSE: Social participation is essential for stroke survivors. Although participation restrictions are affected by several factors, a few studies have focused on the effects of living in a particular residential area (urban versus rural) on the participation restrictions after a stroke. This study examined the factors affecting participation restrictions in stroke survivors according to the residential area. METHODS: One hundred sixty-six stroke patients (including 130 living in urban areas and 36 living in rural areas) were recruited for this study. The Korean Modified Barthel Index (K-MBI) was used to assess the activities of daily living (ADL). The Korean Mini-Mental Examination Status (MMSE-K) was used to assess cognition. The motor function was evaluated using the Fugl-Meyer function assessment (FMA). Path analysis was used to test the hypothesized model of participation restriction. RESULTS: The proposed path model showed good fit indices. In rural and urban areas, the direct effects were significant between the ADL and participation restrictions (β = -.673 and -.457, respectively). For urban areas, the direct effects were significant between cognition and participation restrictions (β = -.252). In both area types, motor function and cognition had a significant direct effect on the ADL. CONCLUSION: For urban community-dwelling stroke survivors, the ADL and cognition had direct effects on participation restrictions. For rural area stroke survivors, the ADL had direct effects on participation restrictions. Activity level exercise programs help reduce the participation restrictions. Moreover, it is essential to address cognition training to improve participation in urban community-dwelling stroke survivors.

Pursuing sustainable development throughout society and industry and the field of environmental policy, each international organization or nation has performed international standardization projects on environmental management activities for their system as well as environmental assessment for a product such as life cycle assessment (LCA) and life cycle inventory database (LCI DB), and the environmental aspects have been increasingly demanded as crucial evaluation specifications. Moreover, the conventional environmental policy, which represents the direct-control, has been more dependent on the market forces and product itself after the Climate Change Convention., and the Integrated Product Policy (IPP, EU) is applied vigorously to strengthen global competitiveness of a product and to achieve the effect of environmental improvement for it. According to change of the international railway market, the value of Eco-Design has been increasingly important in developed countries including EU. Thus, each country is establishing its own guidelines, software and database for each product, and developing new policies through Eco-Design with practical results in marketing. To react this and develop Korean railway as an environment-friendly industry with priority to other transportation system as well as maintain high place in technology, the direction of RACE software development of main function is introduced, which is exclusively used for EMU to assess both environmental and economic aspects with LCA and eco-efficiency (EE).

Objective : The purpose of this study was to investigate the effect of proprioceptive-vestibular based sensory integrative intervention on handwriting and fine motor function in elementary school students in grades 1 to 3. Methods : In this study, eight students in an elementary school in Busan were enrolled. The intervention was conducted twice a week from May to October, 2016, and a total of 14 intervention sessions were conducted. In order to evaluate the writing ability and the fine-motor exercise ability before and after the intervention, the fine-motor movement items of the Korean alphabet writing test and the Bruininks-Oseretsky Test of Motor Proficiency (BOTMP) evaluation were used. Results : There was a statistically significant difference between the total scores of consonant writing and the Korean alphabet writing assessment after the intervention. In BOTMP-Fine motor, the response speed items showed statistically significant difference. Visual-motor control scores increased during intervention, but the statistical significance was not found. Conclusion : Sensory integration interventions might have positive effects on elementary school students' writing skills and fine motor functions.
The aim of this study was to distinguish 3 concepts(capacity, capability, and performance) for the motor activities of children with cerebral palsy(CP) and examinate relation between capacity(can do in a standardized environment), capability(can do in daily environment), and performance(does do in daily environment). Cross-sectional analysis with a assessment record of children with CP (n=40; 19males, 21 females; mean age 6y 6mo, SD 3y 8mo) was performed. Levels of severity according to the Gross Motor Function Classification System(GMFCS) included level1(13%), level2(10%), level3(43%), level 4(33%), and level 5(3%). Motor activities capacity was assessed by the Gross Motor Function Measure (GMFM-66). Capability and performance were assessed using 2 scales(functional skill, caregiver assistance) of the Pediatric Evaluation of Disability Inventory(PEDI). Correlations between capacity and capability was 0.811(p < .05), and between capability and performance were high(r=0.711, p < .05). And the correlation between capacity and performance is the lowest(r=0.711, p < .05). Motor performance levels are only partly reflected by the motor capacity and motor capability levels in children with CP. Because performance is influenced by Contextual factors (particularly, social factors such as family function). This study suggests that it is necessary to distinguish and evaluate the capacity, capability, and performance in children with cerebral palsy.
The purpose of this study was to investigate the effect of mental practice on the upper extremity function of the hemiplegic Patients, 8 subjects with hemiplegia patients participated in this study. They did the Mental Practice programme for twenty minute, two times a week, three weeks. The Mental practice effects was evaluated by Fugl-Meyer Assessment(FMA), Action Research Arm Test(ARA), Stroke Rehabilitation Assessment of Movement(STREAM). The data were analyzed using paired t-test. The result of this study are as follows: FMA score were significant difference test-retest(p<.05), ARA score were statistically significant improvement between test-retest(p<.05), On the STREAM, the patients had the score improved on 4 of the 10 items of the upper-extremity Scale. The result suggest that mental practice is a potentially useful method of practicing motor skills. Mental practice may be a cost-effective, non-invasive tool with wuhich patients with stroke can receive additional practice of functional skills, and realize greater outcome, than if therapy alone were used.
Purpose : The aim of this study was to compare the effect of task-oriented training using the PNF(proprioceptive neuromuscular facilitation) and task-oriented training with chronic stroke patients. Methods : Sixteen chronic stroke patients participated. Participants were randomly assigned to the control and experimental group( 8 experimental, 8 control). All of participants were in-patients at local hospital and had been received training appropriate to the purpose of each group program. Experimental period was carried out 30 minutes/day, five days/week, during four weeks. The experimental group trained the task-oriented training using the PNF and control group trained the task-oriented training. Fugl-meyer assessment scale and Modified Barthel Index was measured to compare the upper arm function and activities of daily living. We were measured before and after the training. Results : The results of the study were as follow: Fugl-meyer assessment scale was significantly increased both groups(p<.05), and significant between groups(p<.05). Modified Barthel index was significantly increased both groups(p<.05) and between groups(p<.05). Conclusion : PNF can be effective in improving upper arm function and ability to perform daily life of chronic stroke patients.

Purpose : This study aimed to investigate the effect of trunk postural stability maintained wearing a spiral elastic band on the upper extremity function of patients with stroke who undertook a functional activity program and to provide fundamental data useful for the application of therapeutic interventions in the clinical environment of occupational therapy. Methods : The research subjects included 16 patients with stroke who were evenly divided into an experimental group and a control group. The eight-week intervention consisted of a functional activity program aimed at improving the function of the affected-side upper extremity. The experimental group performed the functional activity program while maintaining a stable trunk posture by wearing a spiral elastic band. The control group followed the program without wearing the band. Upper extremity function was assessed before and after the intervention using a manual function test (MFT) and a motor activity log (MAL), and the effectiveness on the upper extremity function of the affected side was confirmed. Results : First, in the within-group comparison of the upper extremity function scores before and after the intervention, the experimental group showed a statistically significant difference in the MFT and MAL assessment(p<.05). The control group showed a statistically significant difference in the MAL assessment(p<.05). Second, in the between-group comparison of the change in scores after the intervention, a statistically significant difference was observed between the two groups in all assessments(p<.05). Conclusion : This study confirmed the positive effect on upper extremity function of trunk stabilization posture using a spiral elastic band. The results could be useful when implementing a task-oriented training program in the clinical environment of occupational therapy in the future, as application of the training while maintaining trunk postural stability by wearing a spiral elastic band could be more effective for improving the upper extremity function of patients with stroke.
The purpose of this study was to examine the relation of motor function, depression, and fear of falling, and to identify factors influencing fear of falling in patients with Parkinson's disease in the community. The participants were 180 patients with Parkinson's disease who were selected by convenience sampling. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient and multiple linear regression. There was a significant difference in fear of falling according to gender, occupation, walking assistance device, number of falls, Parkinson's disease stage, duration of illness, antihypertensive drug, motor function, and depression. Fear of falling showed significant positive correlations with motor function, and depression. Depression, number of falls, Parkinson's disease stage, gender, antihypertensive drug, and motor function were significant predictors influencing fear of falling in patients with Parkinson's disease, and these variables accounted for 36.0% of the variance. Depression of the influencing factors was the strongest factor. The results of this study suggest that a variety of intervention strategies for preventing or mitigating depression with systematic nursing assessment of the influencing factors on fear of falling are needed to prevent fear of falling in patients with Parkinson's disease.
Objectives The aim of this study was to examine significant improvement of motor and non-motor symptoms after treatment with Sasang constitutional medicine treatment on a Soyangin patient with Parkinson disease having depression. Methods The patient was treated with Soyangin's constitutional medications treatment(herb and acupuncture) and scalp acupuncture.
