동통을 동반한 제2 중족 족지 관절 불안정성의 치료경험 - 1예 보고 - (Surgical Treatment for Painful Instability of the Second Metatarsophalangeal Joint - 1 Case Report -)

  • An unstable second metatarsophalangeal joint may produce pain in the forefoot. Plication of stretched lateral ligament and capsule and transfer of the extensor digitorum brevis under the transverse intermetatarsal ligament performed as the primary procedure to stabilize this painful joint. But the pain was not subsided and the proximal phalanx was resubluxated. So, we osteotomized the second metatarsal to restore a normal alignment of the second toe. Then the symptom was subsided. We report a case of painful instability of the metatarsophalangeal joint of the second toe.

  • Measurements of plantar pressure provide an indication of foot and ankle function during gait and other functional activities because the foot and ankle provide necessary support and flexibility for weight bearing and weight shifting while people are performing these activities. Plantar pressure is being increasingly used in both research and clinical practice to measure the effects of various footwear and physical therapy intervention. The influence of walking speed and ground inclination on plantar pressure parameters However has not been evaluated in detail. So, in this study to determine the effect of changes in walking speed and ground inclination on plantar pressure treadmills with different walking speeds and inclination were used. Plantar pressure parameters were measured with the Parotec system using the walking and running in 20 healthy participants(10 male, 10 female) aged $20{\sim}28$(mean 22.22, S.D.2.26 years) when slow walking and running. The result of this study with increased die walking speed, the peak pressure of 1st, 5th metatarsal head and total contact time and impulse total at the forefoot was affected by walking speed; however, die peak pressure, contact time and impulse total at the forefoot was not affected by ground inclination.

에너지 저장형 인공발의 개발 (Development of the Energy Storing Foot)

  • KESF-1 foot conceptually provides storage of potential energy and is converted to kinetic energy throughout the weight - bearing phase of the gait cycle. This stored energy is progressively released as the foot continues through the toe-off phase to rebound and propel the body forward. A weight deflects the keel through a predetermined range, then the keel "springs back" as weight is removed. Foot designs criteria were selected to guide development beyond the proof-of concept composite material keels; 1) store and return energy (1-3/4 " metatarsal deflection at 435 pounds vertical load), 2) natural feel and stability; 3) useful life of 3-years: 4) lightweight; 5) reduced production costs. The purpose of this study is developed the comfortable stable foot that fitted with Korean lifestyle and road condition. The results produced the realistic cosmetic foot cover with polyurethane form and the keel composed with composite materials of both glass fiber and carbon fiber.

생체역학적 분석을 통한 보행 부하 감소용 인솔 개발 (The Development of the Insole for Gait Load Decreasing by Biomechanics Analysis)

  • The mailman's shoes should be designed in due consideration of occupational features they spend most of times to walk. For that reason, the shoes required functions to reduce the foot fatigue and to protect body by dispersing the body weight to the whole foot. In this research, for the functional improvement of the insole, insoles are investigated and analyzed by biomechanics experimentation. Under the base of these experimental results, we develop insoles that can reduce the body load and muscular-skeletal disorder. The pressures are concentrated on the metatarsus and heel by the result of analyzing pressure distributions of the using shoes. Accordingly, we offer the prototype functional insole that is ranked from high pressure to low pressure on the base of a shock absorb function. This prototype functional insole is examined for statistical significance by pressure distribution areas. The experimental results show that pressure areas are dispersed to whole foot, for this reason, pressures of the metatarsus and heel are reduced. Results of this research can not only improve the function of insoles which is suitable for occupational features, but also be a base on constructing data bases for biomechanics gait insoles.

Reverdin 절골술을 이용한 무지외반증의 치료 (Reverdin Osteotomy for the Treatment of Hallux Valgus)

  • Authors studied 24 cases in 18 patients with the hallux valgus deformity treated by Reverdin osteotomy at Department of Orthopaedic Surgery, Wallace Memorial Baptist Hospital from January 1995 to December 1999. Reverdin osteotomy is one of the distal metatarsal osteotomies in the treatment of hallux valgus in adults. We performed Reverdin osteotomy on 24 cases (18 patients) for painful hallux valgus associated with an increased hallux valgus angle (> 35degrees) and first-second intermetatarsal angle(>13 degrees). The results were as follows. 1. The average first-second intermetatarsal angle was 16.8 degrees and average valgus angle was 37.3 degrees, preoperatively. they were corrected to 7.8 degrees and 12 degrees respectively. 2. The metatarsalgia of the great toe was subsided in all cases and they were satisfied with the result of their operations in respect to cosmetically and functionally. Avascular necrosis of capital fragment, nonunion, and dorsal angulation were not developed in any case. But one case had superficial wound infection and delayed union, presumably due to secondary infection.

가골 신연술을 이용한 단중족증의 치료 (Treatment for Brachymetatarsia by Callotasis)

  • Purpose: To evaluate the effectiveness of callotasis using the external fixator for the treatment of brachymetatarsia. Materials and Methods: Eleven patients(15 cases) who underwent callotasis were available. Follow-up averaged 23 months(15-38 months). Both the patients' postoperative satisfaction and the postoperative radiographic results were retrospectively evaluated. Results: The duration from applying the external fixator to remove averaged 15 weeks (8-21 weeks). We did osteotomy again in two cases because of early consolidation at the osteotomy site during distraction period. All cases were evaluated mild claw toe deformity and motional pain in metatarsophalangeal joint, but all patients satisfied the postoperative results except one complained motional pain and stiffness in metatarsophalangeal joint. The metatarsal shortening averaged 13mm(9-18mm) before operation. The amount of distraction for shortened metatarsals averaged 15mm(5-22mm). We found the 6 cases of narrowing of the metatarsophalangeal joint, 2 cases of subluxation of the metatarsophalangeal joint. Conclusion: We think that callotasis with external fixator is good operative method for the brachymetatarsia.

The Effects of Lower Limb Training Using Sliding Rehabilitation Machine on the Foot Motion and Stability in Stroke Patients

  • Purpose: The purpose of this study was to investigate the effect of lower limb training using a sliding rehabilitation machine on the foot motion and stability in stroke patients. Methods: Thirty participants were allocated to two groups: Training group (n=15) and Control group (n=15). Subjects in the control group received physical therapy for 30 minutes, five times per week, and those in the training group received lower limb training using a sliding rehabilitation machine for 30 minutes, five times per week, with physical therapy for 30 minutes, five times per week, during a period of six weeks. Heel rotation, hallux stiffness, foot balance, metatarsal load, toe out angle, and subtalar joint flexibility were measured by RS-scan. Results: Significant improvement of the foot motion (hallux stiffness, meta load) and the foot stability (toe out angle, subtalar joint flexibility) was observed in the training group. Conclusion: This study demonstrated that lower limb training using a sliding rehabilitation machine is an effective intervention to improve the foot motion and stability.

꽃사슴에서의 대퇴골(大腿骨) 절단술(切斷術) 1례(例) (A Case Repost of Femoral Amputation in Sika Deer)

  • 충남대학교 부속동물병원에 내원(來院)한 우측후지(右側後肢)의 부전골에 골절상(骨折傷)을 받은 6개월금(個月今)의 꽃사슴에 대하여 대퇴골절단술(大腿骨切斷術)을 실시한 증례(症例)를 보고한다. 마취는 체중 kg당 1.2mg의 xylazine을 근육주사하여 실시하였으며, 수술방법은 개에서의 대퇴골절단술(大腿骨切斷術)과 같은 방법(方法)을 시도하였다. 수술직후 일어서기, 걷기, 달리기등의 운동에 불편을 보였지만 시간이 경과함에 따라 숙달되는 것을 관찰할 수 있었으며, 절단수술후(切斷手術後) 현재(現在)까지 (약(約) 6개월(個月)) 병적(病的) 미후(微候)는 인정되지 않았다.

  • 저자들은 미숙아에서 이학적 검사상 AHO를 동반하지 않고, 생화학 검사에서는 저칼슘혈증, 고인산혈증, 혈청 PTH의 증가와 24시간 소변의 기저 cAMP의 증가를 보이면서, 대퇴골의 자발골절을 동반했던 가성부갑성선 기능저하증 1례를 문헌 고찰과 함께 보고하는 바이다.

  • Tarsal tunnel syndrome is often misdiagnosed it shares clinical features with other diseases of the lower extremity such as lumbar radiculopathy and arthritis. Failure to diagnose and treat neuropathies effectively can cause permanent neuropathic pain. We report a case of a ganglion cyst detected early using ultrasonography and MRI in a 51-year-old man who complained of rapidly aggravating pain, numbness and paresthesia on the great toe and the first metatarsal area in the sole, symptoms that resembled tarsal tunnel syndrome. Surgical treatment could be performed early with no permanent sequelae. We could also avoid repeated steroid injections or prescription medications.
