Two Cases of Herpes Zoster Following Varicella Vaccination in Immunocompetent Young Children: One Case Caused by Vaccine-Strain (건강한 어린 소아에서 수두 백신 접종 후 발생한 대상포진 2예: 백신주에 의한 1예)

  • Herpes zoster (HZ) has been reported in immunocompetent children who received the varicella vaccine. In vaccinated children, HZ can be caused by vaccine-strain or by wild-type varicella-zoster virus (VZV). Like wild-type VZV, varicella vaccine virus can establish latency and reactivate as HZ. We report two cases of HZ in otherwise healthy 16- and 14-month-old boys who received varicella vaccine at 12 months of age. They presented with a vesicular rash on their upper extremities three to four months after varicella vaccination. In one case, a swab was obtained by abrading skin vesicles and VZV was detected in skin specimens by polymerase chain reaction. The VZV open-reading frame 62 was sequenced and single nucleotide polymorphism analysis confirmed that the virus from skin specimen was vaccine-strain. This is the first HZ case following varicella vaccination confirmed to be caused by vaccine-strain VZV in the immunocompetent children in Korea. Pediatricians should be aware of the potential for varicella vaccine virus reactivation in vaccinated young children.

  • Purpose: Rabies is a fatal but preventable disease with proper pre-exposure anti-rabies vaccination (ARV). Dogs, as household pets and strays, are the reservoir and vector of the disease, and dog bites have been associated with human rabies cases in Sri Lanka over the past few years. However, other susceptible species having frequent contact with humans may be a source of infection. One such species is sheep and immunity following ARV has never been tested in sheep reared in Sri Lanka. Materials and Methods: We have tested serum samples from sheep reared in the Animal Centre, Medical Research Institute of Sri Lanka for the presence of anti-rabies antibodies following ARV. Sheep serum samples were tested with Bio-Pro Rabies enzyme-linked immunosorbent assay (ELISA) antibody kits used for the first time in Sri Lanka and our results were verified by a seroneutralization method on cells (fluorescent antibody virus neutralization, FAVN test) currently recommended by World Organization for Animal Health and World Health Organization. Results: Sheep received annual ARV and maintained high neutralizing antibody titers in their serum. No maternal antibodies were detected in lamb around 6 months of age. Agreement between the ELISA and FAVN test, i.e., coefficient concordance was 83.87%. Conclusion: Annual vaccination in sheep has an effect on maintaining adequate protection against rabies by measurements of anti-rabies antibody response. Lambs need to be vaccinated earlier than 6 months of age to achieve protective levels of neutralizing antibodies in their serum. Introducing this ELISA in Sri Lanka will be a good opportunity to determine the level of anti-rabies antibodies in animal serum samples.

  • Since the introduction of the Foreign Patient Assistance Law in 2009, more than 300,000 foreign patients are visiting Korea each year due to the efforts of overseas patient attraction organizations, host companies and governments. Despite these achievements, however, the company is confronted with limitations in product development and the rate of patient population growth is slowing. Therefore, this study suggests the importance and meaning of various product development through the hierarchical analysis (AHP) on the major factors of product development of medical tourism services, and suggests direction of product development in domestic medical tourism industry in the future.

  • The range of problems that can be handled by the activation of big data and the development of hardware has been rapidly expanded and machine learning such as deep learning has become a very versatile technology. In this paper, mnist data set is used as experimental data, and the Cross Entropy function is used as a loss model for evaluating the efficiency of machine learning, and the value of the loss function in the steepest descent method is We applied the GradientDescentOptimize algorithm to minimize and updated weight and bias via backpropagation. In this way we analyze optimal reliability value corresponding to the number of exercises and optimal reliability value without overfitting. And comparing the overfitting time according to the number of data changes based on the number of training times, when the training frequency was 1110 times, we obtained the result of 92%, which is the optimal reliability value without overfitting.

  • On March 11, 2011, the Korea National Assembly finally passed the bill on the Damage Relief on the Medical Malpractice and Mediation for Medical Dispute. One of the features of this Act is including "The Subrogation Payment System for Damage (abbreviated SPSD)". This System is that 'Korean Medical Dispute Mediation-Arbitration Board' pays the damages, instead of the health care provider, for the patient who isn't paid damages by the health care provider despite of the Mediation or ruling. The purpose of this study is to search the problems and make improvement on SPSD. This System was introduced extreamly to the patients in order to induce them to the mediation. However,there remains several problems. In this articles, I have examined thoroughly the legal issues on SPSD. There are legal issues about the methods and ratio of the financial burden. In this connection, wide discretionary authority has been granted to administrative agencies specifically. On this account, this System clearly contains elements of a violation against the Constitutional Law. Moreover, this System can be broadly applied to the case of court ruling or the Korea Consumer Agency's mediation. But these measures go against the aim of legislation that the medical dispute can be resolved through the mediation or arbitration by this Act. In the end, these problems must be revised through the additional discussion.

Evaluation of the Quality of Occupational Health and Safety Management Systems Based on Key Performance Indicators in Certified Organizations

  • Background: Occupational Health and Safety Management Systems are becoming more widespread in organizations. Consequently, their effectiveness has become a core topic for researchers. This paper evaluates the performance of the Occupational Health and Safety Assessment Series 18001 specification in certified companies in Iran. Methods: The evaluation is based on a comparison of specific criteria and indictors related to occupational health and safety management practices in three certified and three noncertified companies. Results: Findings indicate that the performance of certified companies with respect to occupational health and safety management practices is significantly better than that of noncertified companies. Conclusion: Occupational Health and Safety Assessment Series 18001-certified companies have a better level of occupational health and safety; this supports the argument that Occupational Health and Safety Management Systems play an important strategic role in health and safety in the workplace.

The Effects of Medical Staffing Level on Length of Stay (의료 인력의 확보가 환자 입원일수에 미치는 영향)

  • Purpose: The objective of this study was to analyze the effects of medical staffing level as bed-to-medical staff ratio on patient outcomes as length of stay (LOS) among hospitals in Korea. Methods: Two hundred and fifty one hospitals participated in the study between January and March 2008. Data for the study was requested by an electronic data interchange from the Health Insurance Review Agency in 2008. In data analysis, SPSS WIN 15.0 program was utilized for descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and multiple regression. Results: The mean score for length of stay was 13.6 days. The mean of operating bed-to-nurse ratio was 7.93:1. The predicting factors for LOS were bed-to-nurse's aide ratio, bed-to doctor's ratio, severely ill patient rate, and hospital type. These factors explained 28.9% of the variance in patient outcomes. Conclusion: This study results indicate that the relationship between medical staffing level and patient outcomes is important in the improvement of the quality of patient care. Thus, improvements in the quality of the nurse practice environment could improve patient outcomes for hospitalized patients.

  • The coronavirus disease 2019 (COVID-19) pandemic made it necessary for medical schools to restructure their curriculum by switching from face-to-face instruction to various forms of flexible learning. Flexible learning is a student-centered approach to learning that has received interest in many educational sectors. It is a critical strategy for expanding access to higher education during the pandemic. As flexible learning includes online, blended, hybrid, and hyflex learning options, learners have the opportunity to select an instruction modality based on their needs and interests. The shift to flexible learning in medical education took place rapidly in response to the COVID-19 pandemic, and learners, instructors, and schools were not prepared for this instructional change. Through the lens of the technology acceptance model, human agency, and a social constructivist perspective, I examine students, instructors, and educational institutions' roles in successfully navigating the digital transformation era. The pandemic has also accelerated the use of advanced information and communication technologies, such as artificial intelligence and virtual reality, in learning. Through a review of the literature, this paper aimed to reflect on current flexible learning practices from the instructional design and educational technology perspective and explore emerging technologies that may be implemented in future medical education.

  • In this paper, I tried to pursue health and social welfare together through medical convergence based on the main subject of a culture of convergence. Artists' drug accidents are never ending. Now Korea is also out of the drug cleansing country. it is impossible to get rid of them with the national public power. It is time for a treatment plan for these. They need a certain period of time and regular periods of rest and control over sports. Our humanities are researching to understand the changing human images of today. In parallel, medical convergence will also have to be transformed in various ways for human healing. Recently, we can see the case of healing with the combination of oriental medicine, natural healing and western medicine. Furthermore, the structure of medical convergence for the fight against disease can be analyzed as an example. South Korea is also preparing for various convergence programs focusing on natural sciences such as engineering, medical care, and the environment. In order to prevent drug addiction it is important to determine the department responsible for handling the problem of substance abuse. we need to improve the environment that they can be combined with Ondol therapy and natural healing therapies. Furthermore, I expect that fusion medicine will contribute to improving the quality of life of drug addicts and become a successful model to revitalize local economies in particular.

Experimental Evaluation of Scattered X-Ray Spectra due to X-Ray Therapeutic and Diagnosis Equipment for Eye Lens Dosimetry of Medical Staff

  • Background: For proper monitoring of the eye lens dose, an appropriate calibration factor of a dosimeter and information about the mean energies of X-rays are indispensable. The scattered X-ray energy spectra should be well characterized in medical practices where eye lenses of medical staffs might be high. Materials and Methods: Scattered X-ray energy spectra were experimentally derived for three different types of X-ray diagnostic and therapeutic equipment, i.e., the computed tomography (CT) scan, the angiography and the fluoroscopy. A commercially available CdZnTe (CZT) spectrometer with a lead collimator was employed for the measurement of scattered X-rays, which was performed in the usual manner. Results and Discussion: From the obtained energy spectra, the mean energies of the scattered X-rays lied between 40 and 60 keV. This also agreed with that obtained by the conventional half value layer method. Conclusion: The scattered X-rays to which medical workers may be exposed in the region around the eyes were characterized by means of spectrometry. The obtained mean energies of the scattered X-rays were found to match the flat region of the dosimeter response.
