  • Purpose: The purposes of this study were to examine the relationships between health literacy, self-efficacy, social support, and medication adherence and to determine its impact on medication adherence in the elderly. Methods: This study was a descriptive research study. Structured questionnaires were used for survey with a convenience sample of 220 elderly in a community. Data were analyzed using descriptive statistics, ANOVA, Pearson's correlation coefficient, and stepwise multiple regression with IBM SPSS 18.0. Results: The mean scores of health literacy, self-efficacy, social support, and medication adherence were $35.89{\pm}19.39$, $55.34{\pm}7.44$, $86.54{\pm}15.90$ and $4.82{\pm}1.83$, respectively. There were significant correlations between health literacy, self-efficacy, social support, and medication adherence. Social support (${\beta}=.23$, p=.001) and health literacy (${\beta}=.21$, p=.002) were significantly associated factors with medication adherence. Conclusion: This finding showed that nursing interventions to promote medication adherence in the elderly group should focus on social support program and health literacy intervention.

  • To improve the competitiveness of the hospital provides high quality medical services in a hospital coordinator role is emphasized. This study on customer orientation of the role ambiguity in order to identify the impact of degree of customer orientation were analyzed for demographic differences. Dependent variable, customer orientation affects role ambiguity as independent variables, and regression analysis were set. And the control variables are set to support situational factors, customer orientation on the role ambiguity and hierarchical regression analysis was performed. Obtained through empirical results are as follows: First, according to the demographic characteristics of the hospital coordinator customer orientation, the difference between gender and medical subjects are not shown. Age, education, work experience, job title, and the hospital on the pattern of customer orientation has shown a difference. Second, according to the hospital coordinator role ambiguity about its impact on customer orientation analysis can be a role implementation, job implementation, opinion communication in achieving customer orientation was negatively affected. Third, role ambiguity, and customer orientation factors for the moderating effects of organizational support for the role of customer orientation can role implementation, job implementation, opinion communication was a statistically significant. Fourth, the role ambiguity factors and customer orientation for the administrative support for the moderating effect of customer orientation and role implementation is significant, but job implementation, opinion communication were statistically significant. Fifth, the role ambiguity factors and customer support for customer orientation and customer orientation for the moderating effects of role performance and the opinion communication was not statistically significant. However, job implementation was statistically significant. The limitations of this study are as follows: First, role ambiguity, situational factors and support due to limitations of the variable factors that may affect the customer orientation of a number of factors were excluded. So many exogenous variables in the measurement process can affect. Second, the variables measured as problems of self-assessment by the variable measuring the respondent's bias may occur. Third, This study is difficult to generalize. In other words, several areas of the province conducted by the empirical results of the survey as a limit on the overall generalization can follow.

  • Recently, medical technology and IT technology have been trying to converge to overcome limitations of the time and the space in medical technology application. In this study, an app was developed to support remote medical direction for emergency medical services. The developed app allows doctors in remote locations to receive real-time emergency patient situations from emergency paramedics. It can also guide the patient's condition diagnosis and emergency treatment and enable rapid response from the emergency room.

  • This study aims to develop suitable and appropriate support models for ADHD children to provide effective and efficient support for the target children. The diverse characteristics of target area, educational, social conditions, and supporting organization and delivery systems have been comprehensively considered. Domestic and overseas literature review has been performed to analyze the status of support services for ADHD children to develop the model. The FGI was adapted to identify eligibility and expectations for the qualification of ADHD children and the awareness and expectations of relevant professionals. Based on the theoretical rationale and empirical analysis results derived from these systematic procedures, suitable and appropriate support models were developed for ADHD children as a special educational subjects. The support model includes the development of individualized support programs for the needs of the entire ADHD student population, which require screening and assessment of the entire student population and requiring support for special needs. Support methods were developed in a system that supports family support, educational support, medical assistance, and medical support.

  • Purpose: To evaluate several risk factors related to re-ulceration of diabetic foot including psychosocial aspects such as familial support and degree of independence of patients' activity. Materials and Methods: We reviewed medical records and performed telephone interview with eighty-five patients who had a history of hospitalization in our hospital due to diabetic foot ulceration from year 2002 to 2010. Based on the collected data, we analyzed several factors such as age, gender, prevalence duration, accompanying diseases, HbA1c level, degree of independence and familial support. Results: The mean age was 61.4 years and most common in the 4th decade. There were 57 cases (67%) of recurrence, predominance of male. Eleven patients with recurrent diabetic foot ulceration had undergone major amputations. Psychosocial problems such as depression, insufficient familial support and mortality were more frequently observed in recurrent group. Conclusion: This study shows that psychosocial factor such as familial support for patient with diabetic foot could be important to reduce the recurrence rate of diabetic foot ulceration. Therefore, we should pay attention to strategic plans for prevention, screening, treatment, and aftercare through the prospective studies including psychosocial risk factor in diabetic foot ulceration.

  • Objective: Undernutrition is common amongst esophageal cancer patients and therefore appropriate nutrition support is critical. Nevertheless, the effectiveness of enteral nutrition (EN) versus parenteral nutrition (PN) is still controversial. The aim of this study was to investigate the effect of EN and PN on the nutritional state and the length of hospital stay for patients who underwent an Ivor-Lewis (IL) esophagectomy. Method: A retrospective clinical analysis was performed that utilized the electronic medical records of patients who underwent IL esophagectomy during a 3-year period between January 2010 and December 2012 at a tertiary teaching hospital located in Seoul, Korea. The EN group and PN group were analyzed by comparing the nutrition supply, postoperative complications, length of hospital stay, and weight variation. Results: After an IL esophagectomy, the complication rate between the EN group and PN group was insignificant and the length of hospital stay was significantly shorter for the PN group compared to the EN group (14 vs. 16 days, respectively; p<0.001). At the time of discharge, those in the PN group lost less weight postoperatively (p=0.003). Conclusion: PN may be considered as safe nutrition support for esophageal cancer patients who underwent an esophagectomy.

  • With the advancement of information technology and increasing diversity in medical field, there are ongoing researches on ontology based intelligent medical system in Oriental medicine field. Intelligent diagnostic support system uses ontology to give a structure to complex medical knowledge and personal medical history so that we can make diagnosis more scientific, and provide better medical services. In this paper, we suggest an ontology that structuralize three knowledge types basic medical data, clinical trial data, and personal health information, which can be used as important information for individually tailored diagnosis. Especially in Oriental medicine diagnosis, both patient's symptoms of illness and physical constitution play a great role; it can lead to distinct diagnosis depending on their combination. Thus, it is much needed to have a diagnostic support system that uses personal health history and physical constitution along with basic medical data and clinical trial data in the field. In this paper, we implemented an Oriental medicine diagnostic support system that provides individualized diagnosis service to each patient by building an ontology on Oriental medicine focused on individual physical constitution and disease information.

  • The purpose of this paper is to suggest diagnosis support system for diseases of mind so that users can make effective decisions without professional knowledge by developing efficient knowledge system and utilizing ontology with which questions and logic inference are possible to diagnose diseases of mind. Furthermore, this diagnosis support system could be applied to supplement previous diagnosis method which depends on experiences by activating the diagnose of mind diseases thru ontology and determining state of mind effectively without technical knowledge. As a result of this experiment, diagnosis support system for diseases of mind was found to be accordance with the result of consulting instructions and show additional relevance thru utility extension.
