Objectives: The purpose of this study was to clarify the association between serum carcinoembryonic antigen (CEA) and type of work in the shipbuilding industry. Methods: 1,072 final study subjects were admitted to a general hospital from April through July 2010 for the purpose of medical examination. Data on general characteristics such as age, smoking history, alcohol history and exercise habits was gathered through structured self-administered questionnaires. Information on job factors was collected from a medical examination, by interview and through company personnel data. Serum CEA levels were measured after eight hours' fasting and were analyzed by a radioimmunoassay. Results: On univariate analysis, the mean serum CEA level was significantly higher among married (p=0.02), older age (p<0.01), longer work time (p<0.01), smokers (p<0.01), lower education (p<0.01), and indirect and direct exposure groups (p<0.01). On multiple regression analysis, serum CEA level was influenced by smoking (p=0.001), duration of work (p=0.019), and direct exposure group (p<0.001). However, among the direct exposure group, serum CEA level was not significantly different between welding, mounting, electro-device constructive work, grinding and cleaning, and painting. Conclusions: The goal of this research was to determine if there were differences between serum CEA levels according to occupational role among shipyard workers. The direct exposure group of shipyard workers had a relatively higher level of serum CEA than did the indirect exposure group and office workers, most likely due to occupational exposure.
Purpose: The purpose of this study was to identify the health status and request of community residents. This study is part of development of efficient health life enhancement programs. Methods: The subjects of this study were 586 households in a rural community and well-trained interviewers visited every household in the designated area and individually interviewed heads of households for general information and health service utilization. The data were collected using a questionnaire for seven months from March 25 to September 24, 2002. The data were analyzed using frequency. percentage by SPSSWIN (v 10.0). Results: 1. 58.9% of subjects were above sixty, and 60.8% of them were women. 2. The most serious problems they identified were health problems (45.6%), and the second was(##-was+were) economic problems(22.0%). 3. Almost half of the study population regarded themselves as healthy (46.2%). The types of illness they had were neuralgia, arthritis, hypertension, diabetes, and heart disease. 4. Most of the study population answered that they didn't do anything special for health enhancement. 5. Among medical facilities, hospitals or medical clinics (55.3%) were most commonly utilized when they were sick. 6. The highest request of the subjects was 'Home visiting service (45.4%)', and 'free treatment connected to the service of hospitals' was the second. What they needed most regarding health enhancement programs in health life enhancement centers was 'free health examination (58.2%), and heath education programs (57.7%). 7. As a result of investigating the subjects' demand for nursing services in health life enhancement center, 'the clinical examination' was the most requested, and next was 'blood pressure check', 'emergency treatment', and 'rehabilitation service', in that order.
치과의사는 일반 의사들과 다르게 진단과 치료를 위한 거의 모든 과정을 혼자서 처리하는 것이 일반적이다. 이러한 환경에서 치과의사의 진단효율을 향상시키기 위해서는 진단자료와 방사선사진이 동시에 제시되는 효율적인 진료환경이 필요하다. 본 논문에서는 치과병원에서의 진단과 치료를 위한 통합환경을 제안한다 제안된 시스템은 치과병원의 주요업무인 환자기록생성, 진단을 위한 영상취득과 분석, 치료계획수립, 시술시뮬레이션 등을 하나의 시스템으로 유기적으로 결합함으로서 필름과 종이가 없는 병원환경을 실현하였다. 또한 모든 임상자료의 신속한 검색과 정량적인 분석을 지원함으로서 환자에게 보다 정확하고 예측이 가능한 치료를 제공할 수 있도록 하였다.
Background: Evidence supporting an association between the 8q24 rs4242382-A polymorphism and prostate cancer (PCa) risk has been reported in North American and Europe populations, though data from Asian populations remain limited. We therefore investigated this association by clinical detection in China, and meta-analysis in Asian, Caucasian and African-American populations. Materials and Methods: Blood samples and clinical information were collected from ethnically Chinese men from Northern China with histologically-confirmed PCa (n=335) and from age-matched normal controls (n=347). The 8q24 (rs4242382) gene polymorphism was genotyped by polymerase chain reaction-high-resolution melting analysis. We initially analyzed the associations between the risk allele and PCa and clinical covariates. A meta-analysis was then performed using genotyping data from a total of 1,793 PCa cases and 1,864 controls from our study and previously published studies in American and European populations, to determine the association between PCa and risk genotype. Results: The incidence of the risk allele was higher in PCa cases than controls (0.222 vs 0.140, $P=7.3{\times}10^{-5}$), suggesting that the 8q24 rs4242382-A polymorphism was associated with PCa risk in Chinese men. The genotypes in subjects were in accordance with a dominant genetic model (ORadj=2.03, 95%CI: 1.42-2.91, $Padj=1.1{\times}10^{-4}$). Presence of the risk allele rs4242382-A at 8q24 was also associated with clinical covariates including age at diagnosis ${\geq}65$ years, prostate specific antigen >10 ng/ml, Gleason score <8, tumor stage and aggressive PCa, compared with the non-risk genotype ($P=4.6{\times}10^{-5}-3.0{\times}10^{-2}$). Meta-analysis confirmed the association between 8q24 rs4242382-A polymorphism and PCa risk (OR=1.62, 95%CI: 1.39-1.88, $P=1.0{\times}10^{-5}$) across Asian, Caucasian and African American populations. Conclusions: The replicated data suggest that the 8q24 rs4242382-A variation might be associated with increased PCa susceptibility in Asian, Caucasian and African American populations. These results imply that this polymorphism may be a useful risk biomarker for PCa in multi-ethnic populations.
Objectives: To identify the cost effectiveness of early dementia diagnosis using the 2014 Health Insurance Review & Assessment Service-National Patients Sample (HIRA-NPS). Methods: The medical costs of dementia between Western medical care and Korean medical care were compared through the reimbursement and non-reimbursement item code for dementia examination. In addition, the medical expenses of patients with dementia and mild cognitive impairment were compared and analyzed with respect to Western and Korean medical care. Results: There were 87,434 claims, of which 16,101 patients were diagnosed with dementia and 38,680,789,560 won was found to be the medical expenses. 12,881 patients (80.0%) with dementia, 3,144 patients (19.5%) with mild cognitive impairment, and 76 patients (0.5%) progressing from mild cognitive impairment to dementia. The proportion of medical expenses was 97.6% for dementia patients, 2.3% for mild cognitive impairment, and 1% for patients progressing from mild cognitive impairment to dementia. 86,070 claims (98.4%) were from Western medical care, with16,824 patients (98.2%), and the medical expenses was 38,546,895,400 won (99.7%). 1,361 claims (1.6%) were from Korean medical care, with 303 patients (1.8%), and the medical expenses was 133,894,160 won (0.3%). Conclusions: This study compared and analyzed the medical costs of dementia patients and the diagnosis of both Korean and Western medical care. The results of this study are expected to be used as basic research data for investigating cost effectiveness of developing early diagnosis of dementia.

Infection with human papillomavirus (HPV) could affect genesis of both cervical and esophageal cancers. The type-specific distribution of HPV in cervical cytology abnormalities of women has remained unclear in Shantou, an esophageal cancer high-incidence area of China. Data from 22,617 women who were subjected to cervical HPV DNA testing with simultaneous cervical cytological examination during 2009-2013 were therefore here retrospectively evaluated in a hospital-based study. Overall, 16.2% (3,584/22,114)of women with normal cytology were HR-HPV positive, with HPV-52 (4.07%) as the most common type followed by -16 (3.63%), and -58 (2.46%). Prevalence of HR-HPV was 50.3% (253/503) in women with cervical cytological abnormalities, of which in ASC-H 71.4%, ASC-US 39.1%, HSIL 80.3% and LSIL 73.7%. HPV-58 (14.12%) was the most common type for all cervical cytological abnormalities, followed by HPV-16 (13.72%), and -52 (12.72%), while the more common HPV-16 type in ASC-H (42.9%) and HSIL (36.1%), HPV-52 and -58 were the most common types for ASC-US (10.3%) and LSIL (25%), respectively. Multiple HPV co-infections were identified in 33.2% (84/253) cytology abnormalities with positive HR-HPV, and the highest prevalence of HPV-58/16 combination in HSIL (28.6%, 6/21) was observed. Our data indicated a relative high prevalence of HPV-58 and -52 in women with cervical cytological abnormalities, which should be considered in the development of next-generation vaccines for Shantou.
Purpose: Until now, students wrote only differential diagnoses and plans during the interstation work (IW) portion of clinical performance examinations (CPX). The contents of history taking (Hx) and the physical examination (PE) were only evaluated using checklists. This study addressed the correlations of Hx and PE scores from the checklists and the subsequent IW to research the necessity of adding Hx and PE sections in to the IW. Methods: One hundred and thrity-three students participated in a ten case CPX, and eight cases were used in this study. Immediately following the 12 minute patient encounter, students were given 6 minutes to write the IW. For the Hx and PE section, students were instructed to write the findings that they considered key information in approaching the patient's problem. The Hx and PE sections of the IW were scored using the same items that appeared on the SP checklist. Calculations of Spearman's rho were performed to find the relationship between the checklist scores and IW scores of Hx and PE sections. Results: Correlations of Hx and PE scores between the SP checklists and IW were low to moderate. Across the eight cases, the median Spearman's rho was 0.37 with a range of 0.14 to 0.54. Conclusion: We suggest that, because SP checklist scores measure only what information the students gather, but not their reasons for doing so (e.g.: relative importance), students tend gather very specific information from the patients. By analyzing of their IW, we were able to explore how students prioritize the important information they have obtained from the patients.
The purpose of this study was to obtain the information of the home treatment state for children with delayed development and to identify stress and coping pattern in mothers of the children. Data were collected from 211 mothers of delayed developmental children(DDC) by means of structured questionnaires. The results of this study were that in the home treatment mothers had technical insufficiencies to treat their children. so they had a difficulties in treating their children at home. But the home treatment were effective and mothers were assisted by their family and others in their treatment. The mothers had stress by anxiety of prognosis. hurts, responsibility, technical insufficiencies and negligent in housework. And acquisition of home treatment technique, the existence of assistant, and emotion in treatment also influenced on stress in the mother. The coping pattern in the stress were regular medical examination. communication with medical team and other mothers with DDC, faith of recovery and treatment. confidence in home treatment, family coorperation. and leisure time without concern for treatment. The younger mother received the more help from other mothers with DDC. And the mothers with severe, complex DDC had the more help from faith of recovery. Regular medical examination and faith of recovery were helpful coping pattern in mothers who had no treatment skill. Confidence in family coorperation and leisure time without concern for treatment were particularly helpful in mothers whose treatment duration is longer, and home treatment was not effective.
Objectives: This study established a practical direction for the prevention and management of metabolic syndrome by evaluating the health status, nutrition intake level, and diet quality according to metabolic syndrome and related drug treatment in Korean adults. Methods: The data from the 2017 KNHANES (Korea National Health and Nutrition Examination Survey) was analyzed. The analysis included 2,978 adults, classified into the normal, metabolic syndrome (MetS), metabolic syndrome with medicines (MetS-M), and without medicines (MetS-noM) groups. The nutrient intake, NAR (nutrient adequacy ratio), INQ (index of nutritional quality), and DDS (dietary diversity score) were analyzed. Results: The mean BMI was significantly higher in the MetS group than in the normal group for all subjects. Subjects of the MetS group tended to consume less energy and major nutrients, while males aged 50 ~ 64 and all females showed less intake of nutrients in the MetS-M group. The energy intake ratio was within 55 ~ 65 : 7~ 20 : 15 ~ 30 of KDRI (Korean Dietary Recommended Intake), but the carbohydrate energy ratio of all subjects aged 50 to 64 was over 65%. The NAR of the major nutrients was lower in the MetS-M group, the average INQ was around 0.8, especially the INQ of calcium and vitamin A was less than 1, and the total DDS score was less than 4 points. Conclusions: This study confirmed that the nutrient intake and diet quality differed among subjects diagnosed with metabolic syndrome and managed with medical care. The intakes of energy and many nutrients, the quality of diets, and the diversity of food groups in the MetS-M group were lower than in the normal group. Therefore, these will be an important basis for establishing a specific direction of diet education for preventing and managing metabolic syndrome according to gender, age, metabolic syndrome, and drug treatment.
Objectives: This study was carried out to identify relation of health care utilization due to skin disease(ICD-10, L00-L99) and characteristics(sex, age, exposure status, type of industry, size of enterprise) of workers. Methods: We made new database composed of 30,536 workers' health examination results in one specific health examination institute in Inchon and data of medical insurance utilization due to skin disease in 4 medical insurance associations for enterprise from January, 1995 to December, 1997 And we analyzed determinants of health care utilization due to skin disease of workers. Results: Among 30,536 study subjects, 8,837(28.9%) workers and 4,181 (13.7%) workers utilized medical insurance due to total skin disease(ICD-10, L00-L99) and contact dermatitis(ICD-10, L23-L25), respectively. Female workers(p<.001), workers exposed to organic solvents(p<.05), workers if manufacturing industries(p<.05, p<.01, respectively) and in large scale enterprises(p<.001) utilized more medical insurance due to total skin disease and contact dermatitis than male workers, workers not exposed to organic solvents, workers in non-manufacturing industries and small scale enterprises. With multiple logistic regression analysis, significant explanatory variables affecting workers' medical utilization due to total skin disease and dermatitis and eczema(ICD-10, L20-L30) in total workers were sex, age, specific chemicals and size of enterprises. And age, type of industries, organic solvents, specific chemicals and size of enterprises were significant explanatory variables related to medical insurance utilization due to dermatitis and eczema and contact dermatitis in male workers. Conclusion: From the above results, we found that workers exposed to organic solvents utilized more medical insurance due to skin disease than workers not exposed to. And, comparing to workers in large scale enterprises, workers in small scale enterprises may have unmet medical care utilization due to skin disease. Therefore we have to establish counterplan to manage occupational skin disease of high risk group(organic solvent exposure group) and to satisfy unmet medical care utilization of workers in small scale enterprises.
