뒤시엔느 근이영양증 환자에게 기계적 기침보조기법 적용의 임상적 의의 (Clinical Implication of Mechanical Insufflation-Exsufflation Method in Patients with Duchenne muscular dystrophy)

  • Purpose : The aim of this study was to clarify the lung capacity, maximal insufflation capacity, and peak cough flow when a mechanical insufflation-exsufflation(MIE) method was used to increase Duchenne muscular dystrophy patients' lung function. Methods : The subjects of the study were 21 patients with Duchenne muscular dystrophy. They were randomly selected from patients within the boundaries of the selection criteria, and divided into two groups; The subject group(n=11) used the mechanical insufflation-exsufflation method with traditional therapeutic exercise. The control group(n=10) used only traditional therapeutic exercise. Results :The results indicated that maximal insufflation capacity, unassisted peak cough flow and assisted peak cough flow significantly increased in the subject group(p<.05). By contrast, in the control group, the results didn't indicate the significant differences from the variable. There were significant differences in maximal insufflation capacity and assisted peak cough flow between the subject group and the control group before and after the application of the mechanical insufflation-exsufflation method. Conclusion : A mechanical insufflation-exsufflation method has positive effects on the improvements of cough functions and that of pulmonary functions such as lung volume, lung elasticity in patients with Duchenne muscular dystrophy.

  • Let Mf(x) be the Hardy-Littlewood maximal function on $\mathbb{R}^n$. Let $\Phi$ and $\Psi$ be functions satisfying $\Phi$(t) = ${\int^t}_0$a(s)ds and $\Psi(t)$ = ${\int^t}_0$b(s)ds, where a(s) and b(s) are positive continuous such that ${\int^\infty}_0\frac{a(s)}{s}ds$ = $\infty$ and b(s) is quasi-increasing. We show that if there exists a constant $c_1$ so that ${\int^s}_0\frac{a(t)}{t}dt\;c_1b(c_1s)$ for all $s\geq0$, then there exists a constant $c_1$ such that(0.1) $\int_{\mathbb{R^{n}}$ $\Phi(Mf(x))dx\;\leq\;c_2$ $\int_\mathbb{R^{n}}$$\Psi(c_2\midf(x)\mid)dx$ for all $f\epsilonL^1(R^n_$. Conversely, if there exists a constant $c_2$ satisfying the condition (0.1), then there exists a constant $c_1$ so that ${\int^s}_\delta\frac{a(t)}{t}dt=;\leq\;c_1b(c_1s$ for all $\delta$ > 0 and $s\geq\delta$.

  • In order to evaluate the accuracy of the measurement of maximal oxygen intake (MOI), the MOI in seven subjects was determined 3 to 4 times in each individual. Following a 10 minute warm-up on treadmill (4.3 km/hr with 9 degree grade), the subject was asked to run at a speed of 8.73 km/hr on treadmill for a period of 3 minutes at a given grade which was elevated in a step-wise manner from zero to the level of exhaustion. Following a 3 minute run, the subject was allowed to rest for a period of 3 to 5 minutes. During each period of running, several cardio-pulmonary functions were determined and the range of variability for each measurement was computed. The oxygen consumption during the maximal work load was taken as the MOI. The results may be summarized as follows: (1) The minute volume, the oxygen consumption and the heart rate increased linearly until the grade was elevated to 9 degree above which these values were leveled off. (2) The minute volume and the heart rate during maximal exorcise were $87.4{\pm}8.10\;1/min\;and\;187{\pm}3.7$ per minute, respectively. (3) The maximal oxygen intake which corresponds to the oxygen consumption during maximal exercise was averaged to 3.04 1/min. (4) The coefficient of variance for the maximal oxygen intake was 6.32% while the corresponding values for the minute volume and the heart rate during maximal exercise were 5.22 and 2. 14%, respectively.

  • In this paper, the concept of maximal ideals relative to a filter on posets is introduced and examined. An intrinsic characterization of distributive lattices is obtained. In addition, we also give a characterization of pseudo primes in semicontinuous lattices and a characterization of semicontinuous lattices. Functions of semicontinuous lattices which are order preserving and semicontinuous are studied. A new concept of semiarithmetic lattices is introduced and examined.

  • In this paper we consider the holomorphic curves (or derived holomorphic curves introduced by Toda in [15]) with maximal defect sum in the complex plane. Some well-known theorems on meromorphic functions of finite order with maximal sum of defects are extended to holomorphic curves in projective space.

  • In this paper, let q ∈ (0, 1]. We establish the boundedness of intrinsic g-functions from the Hardy-Lorentz spaces with variable exponent Hp(·),q(ℝn) into Lorentz spaces with variable exponent Lp(·),q(ℝn). Then, for any q ∈ (0, 1], via some estimates on a discrete Littlewood-Paley g-function and a Peetre-type maximal function, we obtain several equivalent characterizations of Hp(·),q(ℝn) in terms of wavelets.

  • In this paper we establish the Lp boundedness of Marcinkiewicz integral operators on product domains with rough kernels satisfying a weak size condition. We assume that our kernels are supported on surfaces generated by curves more general than polynomials and convex functions. This generalizes and extends previous results.

  • Let $\{u(t)\}$ and $\{u(dt)\}$ be two maximal length sequences of period $2^n-1$. The cross-correlation is defined by $C_d({\tau})=\sum{_{t=0}^{2^n-2}}(-1)^{u(t+{\tau})+v(t)$ for ${\tau}=0,1,{\cdots},2^n-2$. In this paper, we propose a new proof for finding the values and the number of occurrences of each value of $C_d({\tau})$ when $d=2^{k-2}(2^k+3)$, where $n=2k$, $k$ is a positive integer.

  • In this paper, we prove Lp estimates of a class of singular integral operators on product domains along surfaces defined by mappings that are more general than polynomials and convex functions. We assume that the kernels are in L(log L)2 (𝕊n-1 × 𝕊m-1). Furthermore, we prove Lp estimates of the related class of Marcinkiewicz integral operators. Our results extend as well as improve previously known results.

A Horizontal Partition of the Object-Oriented Database for Efficient Clustering

  • The partitioning of related objects should be performed before clustering for an efficient access in object-oriented databases. In this paper, a horizontal partition of related objects in object-oriented databases is presented. All subclass nodes in a class inheritance hierarchy of a schema graph are shrunk to a class node in the graph that is called condensed schema graph because the aggregation hierarchy has more influence on the partition than the class inheritance hierarchy. A set function and an accessibility function are defined to find a maximal subset of related objects among the set of objects in a class. A set function maps a subset of the domain class objects to a subset of the range class objects. An accessibility function maps a subset of the objects of a class into a subset of the objects of the same class through a composition of set functions. The algorithm derived in this paper is to find the related objects of a condensed schema graph using accessibility functions and set functions. The existence of a maximal subset of the related objects in a class is proved to show the validity of the partition algorithm using the accessibility function.
