발 반사마사지가 유방암 환자의 수술 후 통증 및 기분에 미치는 효과 (Effect of Foot Reflexology on Patient's Pain and Mood Following a Mastectomy)

  • The purpose of this study was to test the effects of foot reflexology on pain and mood in patients who have just had a mastectomy. The research design was a nonequivalent control group, non-synchronized design. The participants for this study were 28 patients with breast cancer on first or second day after mastectomy surgery. The first 12 patients were assigned to the control group and the second 16 to the experimental group. The study was conducted from October 28, 2001 to December 22, 2001 on a surgical unit of Y Medical Center in Seoul, Korea. Pain intensity, blood pressure, pulse rate and mood were measured as dependent variables. The instrument used for pain intensity and mood was a 10 cm visual analogue scale. Cronbach alpha for this study was .70. For the experimental group pre and post tests were done immediately before and 15 minutes after completion of foot reflexology. Foot reflexology was performed once for 20 minutes. After the pretest, patients in the control group were instructed to have a quiet time for a 35 minute period and then the post-test was done. The number of participants was small so the data were analyzed by Mann-Whitney U test, and Wilcoxon signed ranks test. The results of this study are as follow : 1. In the experimental group there were significant decreases between the pre and post tests for pain intensity (z=-3.47, p<.001), diastolic blood pressure (z=-2.67, p<.01) and pulse rate (z=-3.44, p<.001) but there was no difference between the two groups on the post test. 2. The systolic blood pressure, diastolic blood pressure and pulse rate were also not significantly different between the experimental group and the control group on the post test. 3. The mood score was significantly improved after foot reflexology(z=-3.52, p=.000) and the difference between the two groups was also significant (u=49.0, p=.03). In conclusion, there was a statistically significant difference in mood between the two groups, but no difference for pain intensity, blood pressure, or pulse rate. However, there were significant differences between the pretest and post-test for pain intensity, diastolic blood pressure and pulse rate in the experimental group. On the basis of the above findings, this study suggests that foot reflexology can be an effective method of relieving pain and improving mood in clinical practice in women who have had mastectomy surgery.

유방절제술을 받은 여성의 건강증진행위에 관한 연구 (A Study on Health Promoting Behavior In Post-Mastectomy Patients)

  • The purpose of this study was to examine the relationship among perceived health status, self-esteem, self-efficacy and health promoting behavior, and to determine the predictors of health promoting behavior in post-mastectomy patients. The study, a descriptive correlational study, was done with structural questionnaires. A total of 51 post-mastectomy subjects from C university hospital in Kwang-ju, South Korea completed mail-in self-reporting questionnaires during a three month period from March to June, 1999. The data were collected using Lawstone's(1982) perceived health status scale, Rosenberg's(1965) self-esteem scale, the modified self-efficacy scale(Shere et al, 1982), and the modified health promoting lifestyle profile (Walker et al, 1987). The data obtained were analyzed according to percentage, mean and standard deviation, principal component analysis, varimax rotation, t-test, ANOVA, Pearson's correlation, and stepwise multiple regression. The results were as follows: 1. The health promoting behavior measurement resulted in six factors. Each factor was labelled as follows: self-actualization, nutrition, stress management, exercise, health responsibility and interpersonal support. The total percent of variance explained by the six factors was 58.4%. 2. The mean score of health promoting behavior was 85.92(range 58~117). The scores of six factor were nutrition 3.20, self-actualization 2.59, stress management 2.58, interpersonal support 2.58, health responsibility 2.49, and exercise 2.34 on a four point scale. 3. When the score of health promoting behavior factors were compared by general characteristics. Factor I: self-actualization, differed significantly by the frequency of pregnancy (F=3.06, p=.037). Factor II: nutrition differed significantly by drinking experience(t=-2.26, p=.028) and the pre- or post stage of menopause(F=2.69, p=078). FactorIII: stress management differed significantly depending on regularity of mensturation(t=-2.12, p= .042). FactorIV: exercise differed significantly by type of religion (F=2.49, p=.072), marital status(F=5.03, p=.010), and feeding type (F=2.64, p=.036). Factor V: health responsibility differed significantly by regularity of mensturation(t=2.18, p=.037). 4. The total health promoting behavior score was significantly related to self-esteem and perceived health status(r=.610, p.006; r= .378, p=.006). The score of selfactualization also corresponded with selfesteem and perceived health status(r=.556, p=.001; r=.343, p=.013). 5. The predictor to explain the score of health promoting behavior was self-esteem, which accounted for 37.1% of the total variance. The predictor to explain the score of self-actualization was self-esteem, which accounted for 30.9% of the total variance. The score of nutrition was primarily affected by both premenopause and drinking experience, which accounted for 13.1% and 9.5% respectively. Finally, the score of exercise was dictated by marriage, Buddhism, no experience of breast feeding, which accounted for 17%, 9.8%, & 5.2% respectively. In conclusion, self-esteem is the main predictor for health promoting behavior in post-mastectomy women. These findings suggest a need for nursing strategies which promote self-esteem in such patients.

Evaluation of Eligibility and Utilization of Breast Conservation Treatment in an Asian Context

  • Background: Breast conservation treatment (BCT) has long been recognised to provide survival outcomes equivalent to mastectomy for the treatment of breast cancer. However, published reports of BCT rates in Asian communities are lower than those from Western countries. This study sought to investigate the eligibility and utilisation of BCT in a predominantly Asian population. Materials and Methods: All patients treated surgically by a single surgeon at a private medical facility between 2009 and 2011 were included in the study. Patients were deemed to have successful BCT if they underwent breast conserving surgery with pathologic clear margins and completed all recommended adjuvant treatment. Those who did not complete adjuvant treatment were excluded from the analysis. Results: Data from a total of 161 patients who underwent treatment during the study period were analysed. The mean age was 48.8 years. One hundred and six patients (65.8%) were of Chinese ethnicity, 12 were Indian (7.5%), 11 were Malay (6.8%), 18 were Caucasian (11.2%) and 14 (8.7%) were of other Asian ethnicity. One hundred and thirty-eight women (85.7%) underwent BCT. Of the 23 (14.3%) who underwent mastectomy, 8 (5.4%) elected to undergo a mastectomy despite being eligible for BCT. In total, it was assessed that 146 of 161 patients (90.7%) were eligible for BCT and utilisation was 94.5%. Conclusions: In this study, eligibility, utilisation of BCT and eventual successful breast conservation rates are similar to published rates in Western communities. Additional research is needed to investigate the reasons for the lower published BCT rates in Asian countries and determine ways to improve them.

유방암 환자의 상지감각변화와 어깨관절기능 및 우울 (Change in the Sensation of the Arms, the Range of Motion in the Shoulders and Depression in Breast Cancer Patients)

  • Purpose: The purpose of this study was to examine the association among change in the sensation of the arms, the range of motion in the shoulders and depression in breast cancer patients. Method: This is a descriptive study on correlation. The participants were 132 patients who had been diagnosed with breast cancer and had mastectomy at a university hospital, and had participated in a breast cancer self-help group. The degree of depression was assessed by SCL-90-R scores. Collected data were analyzed with SPSS 12.0 for windows. Results: The participants complained of 'heaviness' as the most frequent and serious symptom of change in the sensation of the arms. 'Completely zip up the dress with a back-fastening zipper' was the most difficult motion of the shoulders. Participants who were in a worse stage of disease were in a worse condition in shoulder functions, and on a higher degree of depression. The degree of change in sensation and shoulder functions showed a positive correlation with each other, and both of them showed a negative correlation with the degree of depression. Conclusion: Sensation and motion change in the arms and the shoulders were common phenomena that affected depression in patients who had mastectomy. Nurses should consider not only depression but also discomfort of the arms and the shoulders for patients with mastectomy.

Improved Long-term Survival with Contralateral Prophylactic Mastectomy among Young Women

  • Background: Despite mixed survival data, the utilization of contralateral prophylactic mastectomy (CPM) for the prevention of a contralateral breast cancer (CBC) has increased significantly over the last 15 years, especially among women less than 40. We set out to look at our own experience with CPM, focusing on outcomes in women less than 40, the sub-population with the highest cumulative lifetime risk of developing CBC. With an extended follow-up, we hoped to demonstrate differences in the long-term disease free survival (DFS) and overall survival (OS) among groups who underwent the procedure (CPM) versus those that did not (NCPM). Materials and Methods: We performed a retrospective review of all breast cancer patients less than age 40 diagnosed at Mount Sinai Medical Center between January 1, 1980 and December 31, 2010 (n=481). Among these patients, 42 were identified as having undergone CPM, while 195 were confirmed as being CPM-free during the observation period. A univariate and multivariate analyses were performed. Results: The CPM group had a significantly higher percentage of patients who were diagnosed between 2000 and 2010 (95.2% vs 40%, p=0.0001). The CPM group had significantly smaller tumors (0-2cm.: 41.7% vs 24.8%, p=0.04). Among the entire group of patients, the overall five- and 10-year DFS were 81.3% and 73.3%, respectively. CPM was significantly associated [HR 2.35 (1.02, 5.41); p=0.046] with 10-year OS, although a similar effect was not observed for five-year OS. Conclusions: We found that CPM has increased dramatically over the last 15 years, especially among white women with locally advanced disease. In patients less than 40, who are thought to be at greatest cumulative risk of secondary breast cancer, CPM provided an OS advantage, regardless of genetics, tumor or patient characteristics, and which was only seen after 10 years of follow-up.

유방암 수술 후 조기 환자를 위한 타이치(Tai Chi) 운동 프로그램의 적용 효과 (Effects of a TaiChi Program for Early Mastectomy Patients)

  • Purpose: There is a need to apply the Tai Chi program to help early mastectomy patients improve their upper limb functions and for the emotional state as well. Method: Participants were recruited at breast cancer centers of two general hospitals from February 17 to April 25, 2005. Twenty-five subjects in the experimental group and 23 subjects in the control group participated for 12 weeks. The Tai chi program consisted of breast cancer self management education and a Tai Chi exercise program. Range of motion of the shoulder joint was determined by the Uni-level inclinometer, and functional assessment of the shoulder was evaluated with the tool. Data was analyzed by SPSS 11. Results: The experimental group had significantly increased shoulder range of motion, especially in external rotation, upper limb function and emotional state compared with the control group after program for 12 weeks. Conclusion: This program makes it possible to be independent and have a positive everyday life. In addition, it can help keep a healthy lifestyle in mastectomy women.

Local and regional recurrence following mastectomy in breast cancer patients with 1-3 positive nodes: implications for postmastectomy radiotherapy volume

  • Purpose: To determine the necessity of postmastectomy radiotherapy (PMRT) and which regions would be at risk for recurrence, we evaluated local and regional recurrence in breast cancer patients with 1-3 positive nodes and a tumor size of <5 cm. Materials and Methods: We retrospectively analyzed data of 133 female breast cancer patients with 1-3 positive nodes, and a tumor size of <5 cm who were treated with mastectomy followed by adjuvant systemic therapy between 2007 and 2016. The median follow-up period was 57 months (range, 12 to 115 months). Most patients (82.7%) were treated with axillary lymph node dissection. Adjuvant chemotherapy, endocrine therapy, and trastuzumab therapy were administered to 124 patients (93.2%), 112 (84.2%), and 33 (24.8%), respectively. The most common chemotherapy regimen was anthracycline and cyclophosphamide followed by taxane (71.4%). Results: Three patients (2.3%), 8 (6.0%), and 12 (9.0%) experienced local, regional, and distant failures, respectively. The 5-year cumulative risk of local recurrence, regional recurrence, distant metastasis, and disease-free survival was 3.1%, 8.0%, 11.7%, and 83.4%, respectively. There were no statistically significant clinicopathologic factors associated with local recurrence. Lymphovascular invasion (univariate p = 0.015 and multivariate p = 0.054) was associated with an increased risk of regional recurrence. Conclusion: Our study showed a very low local recurrence in patients with 1-3 positive nodes and tumor size of <5 cm who were treated with mastectomy and modern adjuvant systemic treatment. The PMRT volume need to be tailored for each patient's given risk for local and regional recurrence, and possible radiation-related toxicities.

  • 본 연구는 '유방완전절제술을 받은 여성의 심리사회적 경험은 무엇인가'라는 연구 질문을 가지고 유방완전절제 후 자조집단에 참여하는 여성의 심리사회적 경험을 심층면담과 참여관찰로 탐색한 것이다. 특히 현상학적 이론과 분석방법을 바탕으로 여성이 갖는 유방의 의미와 본질 그리고 유방완전절제술을 받은 후 겪게 되는 독특한 경험을 연구하고자 하였다. 연구 분석을 통해 '삶과 죽음과의 기로', '성(性) 정체성으로 인한 아픔', '사회의 이중적 시각에 억울함', '신체적 장애와 심리적 장애 사이의 갈등', '지지체계에 대한 고마움과 아쉬움', '현실적으로 바라는 소망과 바램'의 본질을 찾게 되었다. 연구 참여자는 여성으로서의 정체성 혼란을 가지지만 삶에 대한 열망으로 자아와 타인과의 끊임없는 내면적 투쟁을 경험하고 있다. 연구 결과에 대한 요약과 제언은 계속 증가하는 유방암 환자들에 대한 정부의 정책과 사회복지 서비스를 현실화 하고 구체화 하는데 유용한 자료가 될 것으로 기대한다.

기공(氣功)이 유방절제술을 받은 여성의 건강증진에 미치는 영향 (The Effect of Qigong on the Health Promotion in Post-mastectomy Women)

  • Objective : Qigong has been reported as an effective for post-mastectomy women. It consists of a series of postures combined with slow, smooth, graceful movements, and is considered a low intensity exercise. In this study, a specialist of Qigong developed a Qigong program for post-mastectomy women in a community. The object of this research is to evaluate the effect of Qigong on a subjective symptom scale, on the grade of discomfort and pain in everyday life, BMI, fat(%), muscular strength, shoulder joint movement, in post-mastectomy women. Methods : The study used a non-equivalent control group non-synchronized design. The experimental group received a Qigong 3 times a week for 12 weeks from September 7th to December 28th in 2009. The subjects were 40 patients are divided into 25 persons in experimental groups and 15 persons in contrastive group. The data were analyzed with descriptive statistics, chi-square test, t-test, Mann-Whitney U test. A P-value less than .05 was considered significant by 2-tailed test. All statistical analyses were performed with SPSS win(ver 12.0). Results : The results were as follows : The score in the subjective symptom(p=.040), and score of anxiety (p=.024), fat(%)(p=.007) were significantly decreased after Qigong program. The values for shoulder flexibility (left : p=.010, right : p=.008), and Muscular strength(grip power)(left : p=.021, right : p=.029) significantly increased after Qigong program. And Flexion(left ; p=.029), Extension(left : p=.001, right : p=.038), Adduction(left ; p=.001, right ; p<0.001) were also significantly increased after Qigong Conclusions : The Qigong is an effective health promotion program for post-mastectomy women.

웃음요법이 유방절제술 후 환자의 통증과 상태불안에 미치는 영향 (The Effects of Laughter Therapy on Pain and Anxiety among Patients with Postmastectomy)

  • Purpose: The design of this study is a nonequivalent control group, non-synchronized quasi-experimental study. The purpose of this study is to examine the effects of laughter therapy on pain and anxiety of patients with postmastectomy. Methods: The participants were 33 patients admitted for mastectomy in a college affiliated hospital (17 patients in experimental group and 16 patients in control group from November, 2008 to March, 2009). The registered nurse who were certified as the 1st degree laughter specialist implemented the intervention once a day for 30 minutes from the 2nd day to the 6th day after surgery when visited them. Date were analyzed using t, Mann-Whitney U test, repeated measures of ANOVA, paired t-test, or ANCOVA. Results: The level of pain was significantly lower in the experimental group than in the control group. Since the level of pain in all participants was reduced as time passed, the interaction effect between time and group existed. The level of anxiety was significantly lower in the experimental group than in the control group. Conclusion: Based on the results, the laughter therapy may be recommended as an useful intervention to effectively reduce the levels of pain and anxiety among patients with postmastectomy.
