The efficacy of photobiomodulation on dental injection pain: a systematic review of randomized clinical trials

  • Dental injections are routinely performed and can result in pain and anxiety in patients. This systematic review aimed to evaluate the efficacy of photobiomodulation therapy (PBMT) in dental injections for pain management in patients undergoing dental treatment. Indexed databases, including PubMed, EMBASE, Scopus, ISI Web of Knowledge, and Cochrane Library, were electronically searched without a time limit up to February 2024. A risk of bias evaluation was performed using the Cochrane tool. A preliminary investigation using electronic and manual methods yielded 4,920,881 manuscripts. Based on the eligibility requirements, 13 randomized control trials (RCTs) were included. Self-assessed pain was determined using the visual analog scale, Face, Legs, Activity, Cry, Controllability scale, or Wong-Baker face pain scale. Eight RCTs demonstrated a notable decrease in needle pain in patients undergoing dental needle injections using PBMT. Based on current evidence, PBMT may help reduce needle pain related to dental anesthesia. Further standardized studies are needed to assess the significance of PBMT for postoperative pain in patients undergoing dental injections.

Analyzing Operation Deviation in the Deasphalting Process Using Multivariate Statistics Analysis Method

  • In the case of system like MES, various sensors collect the data in real time and save it as a big data to monitor the process. However, if there is big data mining in distributed computing system, whole processing process can be improved. In this paper, system to analyze the cause of operation deviation was built using the big data which has been collected from deasphalting process at the two different plants. By applying multivariate statistical analysis to the big data which has been collected through MES(Manufacturing Execution System), main cause of operation deviation was analyzed. We present the example of analyzing the operation deviation of deasphalting process using the big data which collected from MES by using multivariate statistics analysis method. As a result of regression analysis of the forward stepwise method, regression equation has been found which can explain 52% increase of performance compare to existing model. Through this suggested method, the existing petrochemical process can be replaced which is manual analysis method and has the risk of being subjective according to the tester. The new method can provide the objective analysis method based on numbers and statistic.

A study on the perception and management of dental technician's infection prevention (치과기공사의 감염에 대한 인식과 관리 실태 연구)

  • Purpose: The purpose of this study was to investigate dental technicians' awareness of infections and to enhance their interest and commitment towards infection prevention. Methods: A self-reported questionnaire survey was conducted among dental technicians. Total of 195 responses were used for analysis. A cross-tabulation analysis was used to compare dental technicians' awareness of infections according to their levels of education on infection prevention (α=0.05). Results: Dental technicians were educated on infection prevention (40.5%). Most participants received infection prevention education from school (29.8%). Dental laboratories had an infection control guideline (34.9%). Dental technicians were not aware of infection risks at their workplaces or believed that they were not at risk of infections at their workplaces (59.3%). Conclusion: Dental technicians must be consistently educated on infection prevention and control through systematic education at school and wider promotion through media. Research is needed to economically and efficiently improve equipment used by dental technicians and to protect them from infectious diseases. A budget for infection prevention must also be determined. Institutional measures such as providing support for dental technicians at the policy level and developing a basic infection prevention manual are necessary. Solving these issues not only protects dental technicians from infections but also allows them to provide high-quality medical services.

Effects of Box Color and Precision Demand on the Muscles' Recruitment Pattern in Repetitive Lifting Tasks (대칭형 들기 작업에서 사회심리적 요인이 근육 동원 형태에 미치는 영향 분석)

  • In this study, the effects of psychosocial stress (box color and precision demand) on muscle activity were evaluated in laboratory setting. Eight subjects performed sagittally symmetric lifting tasks. Box color (yellow, black), precision demand (yes, no), and box weight (5%MVC, 10%MVC, 15 %MVC) were varied and surface EMG signals from seven muscles(medial deltoid right, biceps brachii right, lateral triceps right, latissimus dorsi right, erector spinae right, external oblique right, internal oblique right) were recorded. EMG signals were band-pass filtered($10{\sim}400\;Hz$), rectified, RMS smoothed and normalized (NEMG). Analysis of variance tests were conducted on the total NEMG (TNEMG: the sum of the seven muscles' NEMGs) and on the individual muscle's NEMGs. Box color had no effect on the TNEMG and on the seven muscles activities(p>0.05). When precision demand was required at the end point of lifts, the mean NEMG showed higher values than no precision demand conditions: TNEMG (14% increase) and medial deltoid(40% increase), biceps brachii(10% increase), lateral triceps(26% increase), latissimus dorsi(25% increase) muscles. Those increases showed more conspicuous as the box weight increased in the muscles of medial deltoid, lateral triceps, and latissimus dorsi.

A Result of Field Survey for Ergonomic Work Risk Factors in General Hospital (의료업종의 작업위험요인에 대한 실태조사 결과)

  • The purposes of this research are to survey work-related risk factors of musculoskeletal disorders(MSDs) in various departments and tasks at general hospitals in non-manufacturing sectors, and to use basic data derived from the survey results in preventing work-related MSDs in hospital workers. Investigation started in March of 2006 and continued for 6 months in 220 general hospitals at Seoul, Incheon, Kyeongi, and Kangwon area. Investigators visited and interviewed workers in hospitals to identify the presence of tasks of musculoskeletal burden, the investigation results of ergonomic risk factors required by the Occupational Safety and Health Law, statistical analysis from questionnaire for musculoskeletal symptoms, and major departments and tasks that have such risk factors. Twenty-seven percents of hospitals finished the investigation of ergonomic risk factors, and 69% did not do the investigation while remained 4% did not have such factors in their hospitals. The rank order of major departments that had such musculoskeletal burdens was kitchen rooms of 143, managing departments/computer rooms/dispensaries of 137, physical treatment rooms of 109, nursing departments of 96, radiological and clinical laboratories of 63. Eighteen hospitals that did not hold legal duties by the section 148 of labor minister decree practiced prevention programs of MSDs according to the labor-management cooperation. Nursing departments ranked in the first place for the numbers of musculoskeletal symptoms of 438. Managing departments/computer rooms/dispensaries, kitchen rooms, and medical treatment departments held 127, 52 and 45 symptoms, respectively. The magnitude order of physical symptom areas followed shoulder of 185, backs and waist of 166, hand wrists of 120, necks of 110, and legs/feet of 106. The departments and jobs that had major work-related ergonomic risks were patient transporting, central supplying, patient nursing (moving patients into wheel-chairs, changing of patient posture and sheet alteration), manual transporting, operation, and managing/computer departments.

The Role of Bloodletting and Cupping in Severe Acute Urticaria and Angioedema as Skin Emergencies in Persian Medicine

  • Objectives: Some dermatological diseases can be life-threatening. Urticaria and angioedema are common reasons for patients to seek treatment at an emergency department. Severe, generalized urticaria and angioedema can endanger patients' lives by involving the airways and causing anaphylactic shock. The humor-based Persian Medicine (PM) concepts of Shara and Mashara, referring to two kinds of skin lesions, have similarities to urticaria and angioedema, respectively. This article aims to provide scientific evidence regarding the application of PM as an early intervention strategy in the emergency management of urticaria and angioedema. Methods: This was a narrative review of PM studies identified by searching medical databases using search terms related to these diseases, as well as risk-associated keywords such as "fatal", "death", "life-threatening", "emergency", "cupping", and "bloodletting". Data were then compared, interpreted, and analyzed. Results: PM scholars consider the human body as a unified whole and believe in an inner power (Nature) which stems from the body. When the presence of excessive hot substances cause an imbalance of bodily humors, Nature directs their heated vapors sharply toward the skin, thus causing Shara and Mashara. If there is a high risk of inflammation spreading to vital organs under severe conditions, urgent manual interventions are crucial. Conclusion: In serious conditions of urticaria and angioedema, Fasd or bloodletting and Hijama can be effective in speeding up the control of lesions and reducing morbidity and mortality. Consequently, the development of integrated Persian and conventional medicines may provide new therapeutic pathways for skin emergencies.

  • A BPO is an irrevocable undertaking given by an Obligor Bank acting on behalf of the buyer to a Recipient Bank acting on behalf of the seller to pay on satisfaction of certain specified conditions. There have been a Trade Service Utility(TSU), a Transaction Matching Application(TMA) and ISO 20022 TSMT messages that core electronic technical systems linked to the BPO. As a key usefulness of the BPO, it brings to market an alternative means of satisfying the risk mitigation, financing and information management needs of banks and businesses engaged in trade. That is, corporates can spread the risk among multiple Obligor Banks by requesting multiple BPOs for the same trade transaction not to exceed the value of the initial transaction. Since the BPO replaces the manual document checking process with the electronic matching of data, buyers and sellers will benefit from significantly increased accuracy and objectivity. By using BPO, buyers and sellers can become trusted counterparties by demonstrating reliability and giving sellers the assurance of being paid on time as per the payment terms and conditions agreed. When compared to L/C, the BPO requires submission of data only. This requires a change of practice of those accustomed to dealing with physical documentation. The beneficiary of a BPO is the Recipient Bank which is always the Seller's Bank. In the meantime, banks will need to agree between themselves that the URBPO 750 will be applied, modified or excluded. However, customers of banks will face low transactional costs due to a fully automated process and standardized data formats used in international trade documentation. There is no amendment and cancellation rules in the URBPO 750s, but only have Assignment of Proceeds rules. As a result, the BPO is likely to enable new business opportunities, lower costs, fast transaction process and strengthen key customer relationships.

Tea Consumption, Alcohol Drinking and Physical Activity Associations with Breast Cancer Risk among Chinese Females: a Systematic Review and Meta-analysis

  • Objective: To evaluate associations between tea consumption, alcohol drinking and physical activity and breast cancer risk among Chinese females. Methods: Three English databases (PubMed, ScienceDirect and Wiley) and three Chinese databases (CNKI, WanFang and VIP) were independently searched by 2 reviewers up to December 2012, complemented by manual searches. The quality of included studies was assessed with the Newcastle-Ottawa Scale items. Random-effects models were used to estimate the pooled odds ratios (ORs) and 95% confidence intervals (CIs). Potential publication bias was estimated through Egger's and Begg's tests. Heterogeneity between studies was evaluated with $I^2$ statistics. Results: Thirty-nine studies involving 13,204 breast cancer cases and 87,248 controls were identified. Compared with non-drinkers, regular tea drinkers had decreased risk (OR=0.79, 95%CIs: 0.65-0.95; $I^2$=84.9%; N=16). An inverse association was also found between regular physical activity and breast cancer risk (OR=0.73, 95%CIs: 0.63-0.85; $I^2$=77.3%; N=15). However, there was no significant association between alcohol drinking and breast cancer risk (OR=0.85, 95%CIs: 0.72-1.02; $I^2$=63.8%; N=26). Most of the results from the subgroup analysis were consistent with the main results. Conclusion: Tea consumption and physical activity are significantly associated with a decreased risk of breast cancer in Chinese females. However, alcohol drinking may not be associated with any elevation of risk.

Risk Perception of the Firefighters Responsible for Nuclear Power Plants: Construct Validity (원자력발전소 화재에 대한 관할 지역 소방관의 위험인식: 측정도구의 개발과 타당화)

  • As the importance of first responses for fire accidents has grown in the safety management of nuclear power plants, a systematic approach to measure firefighters' psychological states and competence is needed. The current study investigated the construct of the risk perception of the firefighters working near nuclear power plant sites, and then developed and validated a new scale to measure firefighters' risk perception regarding nuclear power plant accidents. The scale items were developed on the basis of literature review and interviews with the firefighters working near nuclear power plant sites. In order to validate the new scale, we recruited 180 firefighters from five fire stations in the vicinity of the nuclear power plants in Jeonnam Province, Gyeongbuk Province, and Busan. The results of exploratory factor analyses revealed that the scale consisted of five factors: "manual" reflecting a lack of response guidelines and manuals for fire incidents and radioactive material release; "fear" reflecting a fear of fire incidents in the nuclear power plants and their catastrophic consequences; "resource" reflecting a lack of protective equipment and manpower for responding to fire incidents in the nuclear power plants; "trust" reflecting trust and cooperation with the counterpart institutions for firefighting in the nuclear power plants; and "knowledge" reflecting the knowledge of radioactivity and firefighting in the nuclear power plants. Further analyses provided statistical evidence supporting for the 15-item scale's internal consistency and construct validity. Finally, We discussed the implication and limitations of the current research.

A Study on the safety measures for the protection of hydrogen cooling system of generator (수소를 냉각매체로 하는 발전기 안전대책에 관한 연구)

  • This paper provided a counter measures against the troubles and accidents that are likely to take place in the power plant using hydrogen gas as a coolant for the cooling system of the generator. Because of the extremely wide flammability limits of hydrogen in comparison to the other flammable gases, the safety measures against the hydrogen accidents is very important to ensure the normal operation of electric-power facility. This study's purpose was a presentation of standard model of safety management of hydrogen equipments in the coal firing power plant such as following items: 1) providing the technical prevention manual of the hydrogen explosions and hydrogen fires occurring in the cooling system of power generator; 2) the selection of explosion-proof equipments in terms of the risk level of operating environment; 3) the establishment of regulations and counter measures, such as the incorporation of gas leakage alarm device, for preventing the accidents from arising, 4) the establishment of safety management system to ensure the normal operation of the power plant.
