Usefullness of CT Gastrography and Vurtual Gastroscopy using Computed Tomography in Detection of Gastric Cancer (위암 진단에 있어서의 CT 위장 조영술과 상부위장관 조영술과의 비교)

  • Purpose: Advancement of computed tomography (CT) hardware and software has allowed thin section scanning and reconstruction of fascinating 2-dimentional (2D) and 3- dimentional (3D) images. Especially, the reconstruction of 3D images of gastrointestinal tract has been used in the detection and diagnosis of pre-malignant and malignant diseases. To compare the efficacy of CT gastrography with conventional upper gastrointenstinal series (UGIs) in gastric cancer patients. Materials and Methods: During Nov. 2002 and Mar. 2003, twenty-seven patients who had gastric cancer received both double contrast upper GI series and CT gastrography prior to radical surgery. Among these patients, nineteen had early gastric cancer (EGC) and 8 had advanced gastric cancer (AGC). Fifteen patients were male and 12 were female. The mean age was 54 yrs (range, $27\∼75$ yrs). The patients were placed on NPO and Stomach was distended with gas in fasting state prior to CT scanning. Double contrast upper GI series were performed as routine manual. CT scan was conducted in all patients using 8 or 16-channel multidetector CT in this study. The collimation and reconstruction for CT scanning were set at 2.5 mm and 1.25 mm, respectively. CT scanning was performed in the supine position. For image processing, CT gastrography, in which raysum and surface rendering images were constructed, virtual and 2D image in coronal and sagittal images were performed. The detectability of gastric cancer was assessed between UGIs and CT gastrography. Results: In AGCs, the detection rate of cancer using CT gastrography and virtual gastroscopy was higher than EGC cases. However, CT gastrography and virtual gastroscopy showed less favorable results than UGIs. Even though only a small number of cases had been studied, we might conclude that CT gastrography and virtual gastroscopy could replace UGIs in the detection of AGC cases. Conclusion: The detection rate used with CT gastrography and Virtual gastroscopy is not better than that of UGIs in early gastric cancer, however, in advanced gastric cancer cases, it is nearly equal to that of UGIs.

A Systematic Review from the Journals under the Korean Oriental Medical Society (대한한의학회지 및 대한한의학회 산하 정회원 학회의 학술진흥재단 등재지에 대한 비염 임상연구의 체계적 고찰)

  • Objectives: This study aimed to evaluate any clinical studies regarding rhinitis published by the Korean Oriental Medical Society and its sub-societies for future rigorous clinical research. Methods: Every article relevant to rhinitis was initially obtained from journals of the Korean Oriental Medical Society and its sub-societies by electronic search at journal web sites or manual searches. Journals were limited to those registered with the Korea Research Foundation. From initial findings, two independent reviewers selected clinical articles and these articles were further analyzed separately by predefined criteria according to prospective and retrospective studies. For prospective studies, quality assessment was also conducted. Results: From 36 initially obtained articles, 17 were finally analyzed. 2 articles were randomized controlled studies, 6 articles were prospective whereas 9 were retrospective. In the prospective articles, there were no randomized controlled trials and the other non-randomized studies had no control group with several problems of quality regarding pre-calculated study size and unbiased assessment. None of the retrospective studies described rhinitis diagnosis objectively and outcome measures were either non-relevant to rhinitis or non-validated. Conclusions: Further well-designed randomized controlled studies for rhinitis are mandatory and more rigorous non-randomized controlled studies should be conducted.

Development and Evaluation of an Automated Stainer for Mycobacterium Tuberculosis (결핵균 자동염색기의 개발 및 평가)

  • The detection of tubercle bacilli (TB) from sputum smear is one of the fast and inexpensive methods for diagnosis of tuberculosis. For this method. sputum smears are usually flexed by heating and stained by acid-fast staining method, and then examined under an optical microscope. Two Procedures are commonly used fur TB staining. One is hot staining and the other is cold staining method. The Ziehl-Neelsen method which is a hot staining method is widely used in Korea because its stained color is more vivid However, the conventional automated stainer has to fix the sputum smear on a slide manually and the stain is not so vivid because it has not heating function. In an effort to save labor and minimize variations in manual staining Procedure. we developed an automated stainer with heating function. The entire staining process is fully automated. from fixation to final washing and drying. With the automated methods, five slides can be flexed and stained in 21 minutes at consistent high quality We compared the concordance rate between the two methods for 91 sputum samples to validate the stain quality of the developed automated stainer. As the results, the concordant rate between the two methods was 95% and there was no significant difference (p>0.05)

A study on Quality Control of Mammography Equipment in the Gwang-Ju (일부지역 유방 촬영 장비의 정도관리 실태조사 연구)

  • As breast cancer shows the highest incidence among women's cancers, the use of mammography is also increasing as a screening test. Mammography should produce high-quality images for accurate diagnosis. For this, it is necessary to manage the performance and image quality of mammography. Thus, in order to investigate quality control, the present study conducted a questionnaire survey of 37 hospitals registered as breast cancer medical examination centers in Gwang-Ju, concerning their quality control of mammography. In the results of surveying the characteristics of apparatus use for mammography, many respondents did not know about the equipment that they were using. Of the hospitals, 19 (49%) were using film, 19 (49%) CR, and 1 DR. In the results of asking how to do quality control, only 38% answered that they inspected according to the manual. In addition, all the surveyed hospitals had specialized agencies do quality control for them. As to the reason for using agencies, 65% mentioned limitations in personnel, time, distance, etc, and 44% mentioned the absence of machines and materials. These results suggest that quality control is being performed perfunctorily, as well as passively and indifferently as it relies on specialized agencies. Accordingly, it is necessary to provide regular education for enhancing people's perception of quality control and to perform quality control adequately in the presence of Radiologist.

  • Diabetic gastroparesis is a disorder in a diabetic of delayed gastric emptying in the absence of mechanical obstruction. It is estimated that about 20%-50% of diabetics suffer from gastroparesis. Clinical Sympoms include early satiety, bloating, nausea, anorexia, vomiting, abdominal pain, and weight loss. Severe gastroparesis might result in recurrent hospitalization, malnutrition, and even death, but, no exact treatment has yet been established. Electrogastrography has been known to be a simple, non-invasive, and effective method in assessing gastric motility and EGG has been used to diagnosis diabetic gastroparesis and to estimate its prognosis. Recently, two cases of diabetic gastroparesis were observed. For this cases, manual acupucture, electroacupucture, lumbar skin warming and herb medicine were applied. After application of these therapies, gastrointestinal symptoms improved and these therapeutic effects were confirmed in EGG. Moreover, blood glucose control improved, so patients were able to discontinue insulin injection and change to p.o.medication. In light of this encouraging application of oriental medicine, this is reported along with investigation of the literature.

Monitoring of the mortalities in the aquaculture farms of South Korea (한국 주요 양식종의 수산동물 폐사피해 모니터링)

  • The practical monitoring was performed to survey the mortalities in aquaculture farms of olive flounder, rockfish, rainbow trout, Japanese eel, white shrimp and abalone in South Korea from May to November, 2011. The aquatic organism disease inspectors, who have the national licenses for the diagnosis and prevention of aquatic disease and have close relationship with the farms, investigated the rates and causes of mortalities according to the standard manual. In 70 flounder farms, the cumulative mortalities rate was 27.9%, and the mortalities were caused by scuticociliatosis, streptococcosis, VHS, non-infectious loss, vibriosis and gliding bacterial disease. The moralities rate of 30 rockfish farms was 13.6%, and those were mainly contributed by gill flukes and streptococcosis. Most of mortalities of rainbow trout were caused by non-infectious loss and protozoan white spot disease. The mortalities rate of Japanese eel was 0.6% by edwardsiellosis, protozoan white spot disease and gill flukes. The loss rate of white shrimp was 71.2%, and most of them was related with non-infectious ones, such as carnivalization, transportation loss, and the rest was caused by viral white spot disease. The mortalities rate in the abalone farms was 10.7% and all of them were related with non-infectious loss.

Quantitative image processing analysis for handwriting legibility evaluation (글씨쓰기 명료도 평가의 정량적 영상처리 분석)

  • Although evaluation of writing disabilities identification and timely intervention are required, clinicians adopt a manual scoring method and there is a possibility of error due to subjective evaluation. In this study, the size ratio and position of letters are digitized and quantified through image processing of offline handwritten characters. We tried to evaluate objectively and accurately the performance of writing through comparison with existing methods. From November 12th to 16th, 2018, 20 adults without neurological injury were selected. They used a pencil to follow the 10 words, 2 sentence stimuli after keeping the usual habit, and we collected the writing test data. The results showed that the height of the word was 1.2 times larger than the width and it tilted to the lower left. The spacing interval was 9mm on average. In the Paired T test, a high correlation was showed between our system and existing methods in the word and sentence 2. This demonstrated the possibility as a testing tool. This study evaluated objectively and precisely writing performance of offline handwritten characters through image processing and provided preliminary data for performance standards. In the future, it can be suggested as a basic data on writing diagnosis of various ages.

The Effect of The Forward Head Posture on The Maximum Mouth Opening of The Temporomandibular Joint, The Muscle Activity and The Asymmetry Rate of The Temporalis and Masseter (머리전방자세가 턱관절의 최대 개구량과 관자근 및 깨물근의 근활성도와 비대칭률에 미치는 영향)

  • This study examined the effects of the forward head posture on the maximum mouth opening of the jaw joint and the muscle activity and asymmetry of the temporalis and masseter. The craniovertebral angle was measured in 50 subjects (23.10±2.73 years) who met the selection requirements of the study. The maximum mouth opening, muscle activity, and muscle asymmetry were measured by reducing 0%, 10%, and 20%. As the craniovertebral angle decreased, the maximum mouth opening decreased (p<.001), the muscle activity of the right and left temporalis decreased (p<.001), and the muscle activity of the left temporalis also decreased. (p<.01). The asymmetry rate of the added muscles of the left and right temporalis, left and right temporalis, and masseter increased according to the change in condition (p<.01). The results of this study are expected to help establish a treatment strategy and comprehensive diagnosis for the temporomandibular joint and present a theoretical basis of manual therapy and therapeutic exercises used for the treatment of TMD.

Association of Pre- and Perinatal Risk Factors With Tourette Syndrome or Chronic Tic Disorders in a Korean School-Age Population

  • Objectives: Tic disorders are highly heritable; however, growing evidence suggests that environmental factors play a significant role in their pathogenesis. Studies on these factors have been inconsistent, with conflicting results. Therefore, this study aimed to examine the associations of pre- and perinatal exposure to Tourette syndrome (TS) or chronic tic disorders (CTD) in Korean school-aged children. Methods: This case-control study used data from a large prospective cohort study. The primary outcome was TS/CTD diagnosis according to the Diagnostic and Statistical Manual of Mental Disorders, 5th edition (DSM-5) criteria and Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version-Korean Version. Demographic, pre-, and perinatal information was obtained from the maternal questionnaires. Data between the TS/CTD and control groups were compared using the chi-squared or Student's t-test, as appropriate. Two-step logistic regression analyses were used to test the association between TS/CTD and pre- and perinatal risk factors. Results: We included of 223 children (78 with TS/CTD and 145 controls). Significant differences in the demographic data between the two groups were observed. The male sex ratio, mean parental age, parental final education level, and family history of tics were included as confounders. In the final adjusted multivariable model, TS/CTD was significantly associated with antiemetic exposure during pregnancy (odds ratio [OR]=16.61, 95% confidence interval [CI] 1.49-185.22, p=0.02) and medically assisted reproduction (OR=7.89, 95% CI 2.28-27.28, p=0.01). Conclusion: Antiemetic exposure and medically assisted reproduction are significantly associated with the risk of TS/CTD. These results should be replicated in future prospective and gene-by-environment studies.

A Research on Applicability of Drone Photogrammetry for Dam Safety Inspection (드론 Photogrammetry 기반 댐 시설물 안전점검 적용성 연구)

  • Large dams, which are critical infrastructures for disaster prevention, are exposed to various risks such as aging, floods, and earthquakes. Better dam safety inspection and diagnosis using digital transformation technologies are needed. Traditional visual inspection methods by human inspectors have several limitations, including many inaccessible areas, danger of working at heights, and know-how based subjective inspections. In this study, drone photogrammetry was performed on two large dams to evaluate the applicability of digital data-based dam safety inspection and propose a data management methodology for continuous use. High-quality 3D digital models with GSD (ground sampling distance) within 2.5 cm/pixel were generated by flat double grid missions and manual photography methods, despite reservoir water surface and electromagnetic interferences, and severe altitude differences ranging from 42 m to 99.9 m of dam heights. Geometry profiles of the as-built conditions were easily extracted from the generated 3D mesh models, orthomosaic images, and digital surface models. The effectiveness of monitoring dam deformation by photogrammetry was confirmed. Cracks and deterioration of dam concrete structures, such as spillways and intake towers, were detected and visualized efficiently using the digital 3D models. This can be used for safe inspection of inaccessible areas and avoiding risky tasks at heights. Furthermore, a methodology for mapping the inspection result onto the 3D digital model and structuring a relational database for managing deterioration information history was proposed. As a result of measuring the labor and time required for safety inspection at the SYG Dam spillway, the drone photogrammetry method was found to have a 48% productivity improvement effect compared to the conventional manpower visual inspection method. The drone photogrammetry-based dam safety inspection is considered very effective in improving work productivity and data reliability.
