치아형태이상을 가진 터너 증후군 환자의 치과적 관리 (Dental Management in a Patient with Turner Syndrome with Dental Anomalies : A Case Report)

  • 터너 증후군이란 한 개의 X 염색체와 전부 혹은 부분이 결여된 X 염색체를 나타내는 염색체 이상 질환이다. 치근 형태 이상이 터너 증후군 환자에서 정상에서보다 호발한다고 보고되었는데, 치근의 이러한 특징은 근관치료를 까다롭게 만들고, 특수한 처치를 요하기도 한다. 또한 터너 증후군 환자들은 종종 심장 또는 신장의 기형과 같은 전신적인 문제를 동반하기도 하므로, 임상의는 터너 증후군의 특징적인 구내 소견을 인지하고, 사전에 구내 합병증을 예방하기 위해 환자들이 정기적으로 치과 검진을 받을 수 있게 해야 한다. 10세 때에 터너증후군 진단을 받은 12세 여자 환자가 하악 양측 소구치 부위의 불편감을 주소로 내원했다. 특징적으로 하악의 모든소구치에서 우상치와 치외치가 관찰되었다. 하악 양측 제1소구치는 세 개의 치근을, 근단부 병소를 보이는 하악 양측 제2소구치는 두 개의 치근을 가지고 있었다. 근관의 복잡한 해부학적 특징 때문에 치과 현미경을 이용한 적절한 시야를 확보하여 근관치료를 마무리 했다. 2년간의 정기적인 관찰 결과 해당치아와 관련된 임상적 징후나 증상, 치근단 병소는 관찰되지 않았다. 본 증례 보고를 통해 터너 증후군 환자의 구강내 특징과 이의 치과적 관리에 대해 보고하고자 하였다.

  • Apert syndrome은 첨두합지증(acrocephalosyndactly)중의 하나로, 1906년 Apert에 의해 보고된 증후군이며 관상봉합의 조기유합에 의한 첨두증(acrocephaly)과 합지증이 공존하는 선천성 유전 질환이다. 임상적으로 뾰족한 고깔모양의 머리 형태를 하고 있으며, 손과 발이 합지증을 보이는 것이 가장 특징적이다. 상염색체 우성의 유전을 하며 중안면부의 발육부전으로 인한 안구돌출 경도의 양안이개 및 상악골 발육부전을 나타내고, 고구개, 연구개열을 동반하는 경우가 있으며, 하악 발육은 정상이나 상대적으로 하악 전돌의 양상을 나타내고 좁은 상악과 하악궁에 심한 총생이 나타나고, 골격적 이상에 의한 부정 교합과, 전방부의 개교를 유발한다. 본 증례는 성형외과에서 상악과 하악에 대한 교정적 처치의 가능성에 대하여 의뢰된 3세 1개월의 Apert syndrome 남아에 대한 것으로 의학적 기왕력과 향후 치과적 처치방향에 대해 과거의 문헌을 고찰하고, 교정적 진단과 치료계획의 수립과정 중에 다소의 지견을 얻었기에 이에 대해 보고하는 바이다.

Morphologic analysis of C-shaped root using 3-D reconstruction

  • C-shaped canal configuration is very difficult to treat because that clues about preoperative canal anatomy cannot be ascertained from clinical crown morphology and limited information can be derived from the radiographic examination. This study was done to get more informations about the root and canal configuration of C-shaped root by 3-dimentionally reconstructing for the purpose of enhancing success rate of endodontic treatment. 30 mandibular molars with C-shaped root were selected. Teeth had been extracted from periodontal problems with sound crown and root portion, including teeth with caries lesion limited crown portion only.(omitted)

  • Anatomical anomaly of temporo-madiblar joint and its dysfunction is becoming one of the important problem in dentistry because the number of these cases are increasing rapidly. Applying 'Lateral Transcranial Technic', 'Updegrave method' and 'Denar Accuard 100' to skull and adult with normal occlusion the author obtained following results: 1. Grewcock method combied with cephalostat as one of 'Lateral Transcranial Technic' revealed clear picture but the image of condyle head was tend to incline downward. 2. Direction of central radiation in 'Updegrave method' should be 2 recommended inch upward from auditorial mearus. 3. For functional analysis and correct diagnosis 'Denar Accuard 100' should be highly recommended.

  • Jesogammarus (Jesogammarus) hinumensis Morino, 1993 was discovered firstly from a brackish water region in Jeju Island, Korea. To identification of the specimens we conducted both of morphological and molecular analyses. This species is characterized by having large eyes and a robust seta on the mandibular palp article 1. The morphology of this Korean specimens was well matched with the original description without variation. The mitochondrial cytochrome c oxidase subunit I(COI) sequences of the present specimens were also completely identical to the sequences of J. (J.) hinumensis collected from the type locality of the species. Thus, we concluded that the Jesogammarus species from Jeju Island is J.(J.) hinumensis, based on both morphological and molecular data.

The evaluation of healing patterns in surgically created circumferential gap defects around dental implants according to implant surface, defect width and defect morphology

  • Purpose: The aim of this study was to evaluate the factors affecting healing patterns of surgically created circumferential gap defects around implants in dogs. Materials and Methods: In four mongrel dogs, all mandibular premolars were extracted. After 8 weeks of healing periods, implants were submerged. According to the surface treatment, turned surface was designated as a group A and rough surface as a group B. In each dog, surgical defects on the left side were made with a customized tapered step drill and on the right with a customized paralleled drill. Groups were also divided according to the width of the coronal gaps: 1.0mm, 1.5mm, or 2.0mm. The dogs were sacrificed following 8 weeks and the specimens were analyzed histologically and histomorphometrically. Results: During the postoperative period, healing was uneventful and implants were well-maintained. As the size of the coronal gap was increased, the amount of bone-to-implant contact was decreased. The bone healing was greater in rough surface implants compared to the turned ones. About the defect morphology, tapered shape showed much bone healing and direct bone to implant contact even in the smooth surface implants. Conclusion: Healing of the circumferential defect around dental implant is influenced by the implant surface, defect width and the morphology of the defect. When using rough surface implants, circumferential gap defects within 2 mm do not need any kinds of regenerative procedures and the healing appeared to be faster in the tapered defect morphology than the paralleled one.

상악전돌에 관한 방사선 두부계측학적 연구 (A ROENTGENOCEPHALOMETRIC STUDY ON MAXILLARY PROTRUSION)

  • This study was undertaken to compare the craniofacial morphology of Class II, Division 1 malocclusion with that of normal occlusion in children, and to investigate the incidence of various Class II, Division 1 craniofacial skeletal patterns. The subjects consist of thirty seven boys and fifty three girls with Class II, Division 1 malocclusion, and forty six boys and eighty one girls 10-15 years with normal occlusion. Measurements were recorded, tabulated and analyzed on the lateral cephalograms by the degree of SNA, SNB and ANB. The following characteristics of the Class II, Division 1 skeletal pattern were observed. 1. The anteroposterior relationship of the maxilla to the cranium in the Class II, Division 1 was very similar to that of normal occlusion. 2, Mandible of the Class II, Division 1 malocclusion was in the posterior position in relation to the cranial anatomy when compared to normal. 3. The chin point as measured by SN Pog and NS Gn showed distal positioning in relation to normal occlusion. 4. SN to mandibular plane angle was large in Class II, Division 1 malocclusion. 5. Mandibular incisor inclination was not significantly different between Class II, Division 1 malocclusion and normal occlusion, but maxillary incisors inclined and positioned labially and consequently overjet was large in Class II, Division 1 malocclusion. 6. Class II, Division 1 malocclusion was divided into four types of craniofacial skeletal pattern. The most common Class II, Division 1 pattern was found to be type C in which SN-Mand. Pl. was above mean range of normal occlusion. The next frequent pattern was found to be type A in which maxilla and mandible were within normal range of protrusion while upper incisors were severly labially inclined.

Relationship between chin deviation and the position and morphology of the mandible in individuals with a unilateral cleft lip and palate

  • Objective: In this study, we aimed to examine the relationship between chin deviation and the positional and morphological features of the mandible and to determine the factors that contributed to chin deviation in individuals with a unilateral cleft lip and palate (UCLP). Methods: Cone-beam computed tomography (CBCT) images of 28 adults with UCLP were analyzed in this study. Segmented three-dimensional temporomandibular fossa and mandible images were reconstructed, and angular, linear, and volumetric parameters were measured. Results: For all 28 individuals, the chin was found to deviate to the cleft side by 1.59 mm. Moreover, among these 28 individuals, only 7 showed distinct (more than 4 mm) chin deviation, which was toward the cleft side. Compared to the non-cleft side, the mandibular body length, frontal ramal inclination, and vertical position of the condyle were lower and inclination of the temporomandibular fossa was steeper on the cleft side. Furthermore, the differences in inclination of the temporomandibular fossa, mandibular body length, ramus length, and condylar volume ratio (non-deviated/deviated) were positively correlated with chin deviation. Conclusions: UCLP individuals show mild chin deviation to the cleft side. Statistical differences were noted in the parameters that represented positional and morphological asymmetries of the mandible and temporomandibular fossa; however, these differences were too small to indicate clinical significance.
