단방성 법랑모세포종의 방사선학적 연구 (Radiographic study of the unicystic ameloblastoma)

  • Purpose : To acquire the useful diagnostic information through the analysis of the clinical and radiological characteristics of mandibular unicystic ameloblastoma. Materials and Methods : The researchers compared and analysed the clinical and radiological features of 79 cases of mandibular unicystic ameloblastoma confirmed by histopathlogic examination. Results : The results of our research showed that unicystic ameloblastomas occurred more frequently in males than in females and the incidence is the highest in the 2nd and 3rd decades. These lesions occurred most frequently in angle-ramus area, mandibular molar area, and anterior area in descending order respectively $68.4\%$, $24\%$, and $7.6\%$. These lesions had smooth ($54.4\%$) rather than undulating ($45.6\%$) borders. These lesions showed well- defined borders in $59.5\%$ among all the cases. Root resorption and loss of lamina dura were observed as $55.7\%$ and $58.5\%$ respectively. Displacement of mandibular canal was observed in $55.9\%$ among all the related cases. Internal pattern of lesional radiolucency was observed as even ($49.4\%$) or uneven ($50.6\%$). Conclusion : These results would be helpful in diagnosing of mandibular unicystic ameloblastoma. 하악 대구치에서 치근단 병소가 치근 이개부 감염에 미치는 영향 (The influence of periapical lesion on furcation involvement in mandibular molars)

  • The purpose of the study was to investigate the influence of an endodontic infection on presence of furcation involvement in periodontally-involved mandibular molars. All first and second mandibualr molars in 45 patients were selected if at least one was root-filled or had a possible periapical radiolucency. The sample consisted of patients from a referral population at a periodotnal clinic which represented an adult population with a mean age of 47.5 years (range 31 to 63) For mandibular molars with periapical destruction at both roots, frequency of horizontal furcation depth ${\geqq}$ 3 mm was significantly more compared to teeth without periapical destruction. Mean periodontal probing depth was significantly greater at mandibular molars with periapical destruction. It is suggested that a root canal infection in periodontitis-involved molars may potentiate periodontitis progression by spreading of endodontic pathgens through patent accessory canals and dentinal tubules. In conclusion, an endodontic infection in mandibular molars was found to be associated with additional attachment loss in the furcation area, and may thus be considered to be one of several risk factors influencing the prognosis of molars in periodontitis-prone patients.

  • Fusion is an abnormality of tooth development defined as the union of two developing dental germs, resulting in a single large dental structure. This irregular tooth morphology is associated with a high predisposition to dental caries and periodontal diseases. As a result of recurring inflammatory periodontal processes, disorders such as periodontal pocket, pericoronitis, and paradental cysts may develop. A rare mandibular anatomic variation is the retromolar canal, which is very significant for surgical procedures. The fusion of a paramolar and mandibular third molar associated with a paradental cyst co-occurring with the presence of a retromolar canal is rare, and the aim of the present study is to describe the evaluation of this anatomical configuration using cone-beam computed tomography.

한국환자에 매식된 ITI 임플란트에 관한 조사 (An analysis of ITI implants placed in korean patients)

  • The aim of this study was to analyze various data of 120 non-submerged ITI implants placed in 64 korean adults. The data were retrieved from patients' charts and registered in the computer and coded for a statistical analysis. The results revealed that the most common type of edentulism was a distal extension case, and the largest number of implants were placed in the mandibular first molar position. Implants were placed more frequently in mandibular, posterior position than maxillary, anterior position, respectively. With respect to the implant diameter and length, an implant with 4.1mm in diameter and more than 10mm in length was the most common. Since a molar position was the most prevalent position for implant placement in korean patients, clinicians should be careful about implant diameter which can resist a strong occlusal force expected in the molar position. In addition, the distance to the mandibular canal and the floor of maxillary sinus should be considered to avoid possible damages, such as nerve injury and sinus perforation in selecting a proper implant length.

Mandibular bone necrosis after use of paraformaldehyde-containing paste

  • Paraformaldehyde has been used in the past as a pulpotomy agent. However, it has a severe cytotoxic effect and may cause alveolar bone necrosis. Depulpin, a devitalizing agent containing 49% paraformaldehyde, is no longer used frequently due to its severe side effects. In the two cases described in the present study, Depulpin was used as a devitalizing agent during root canal treatment. It caused a gradual loss of sensibility in adjacent teeth, gingival necrosis, and osteomyelitis. This case report demonstrates the serious side effects of using a paraformaldehyde-containing paste as a devitalizing agent for pulp, particularly mandibular bone necrosis.

Cone-beam Computed Tomography Measurement of the Position of the Inferior Alveolar Nerve Canal in Mandibular Prognathism

  • Background and Objectives: To determine the anatomic position of the inferior alveolar nerve (IAN) canal in patients with mandibular prognathism using the cone-beam CT (CBCT). Materials and Methods: Fifty rami from 25 patients were evaluated. The images were taken by i-CAT and reconstructed 3-dimensionally using the Simplant 11 program. The linear distances between the IAN canal to the buccal cortex (a, $a^{\dag}$ and $a^{\ddag}$), from the IAN canal to the alveolar crest (b and $b^{\dag}$) and the anterior margin ($b^{\ddag}$) and finally the buccal cortical thickness (c, $c^{\dag}$ and $c^{\ddag}$) were measured at three reference planes (VP, OP and HP). Results: On the left side, the average distance of a, b and c were 7.12, 15.96 and 3.60 mm on the VP plane, respectively. On the OP, the distance of $a^{\dag}$, $b^{\dag}$ and $c^{\dag}$ was 6.11, 8.83 and 2.63 mm. For the HP, the distance of $a^{\ddag}$, $b^{\ddag}$ and $c^{\ddag}$ was 4.84, 10.11 and 2.30 mm. On the right side, the distance of a, b and c, on the VP, was 7.10, 16.13 and 3.42 mm, respectively. On the OP, the distance of $a^{\dag}$, $b^{\dag}$ and $c^{\dag}$ was 4.77, 8.75 and 2.68 mm. On the HP, the distance of $a^{\dag}$, $b^{\dag}$ and $c^{\ddag}$ was 4.55, 9.84 and 2.38 mm. Regarding the difference between genders, the distance in male's was longer than female's on the VP (p=0.019), and was thicker in males than females on the HP (p=0.002). Conclusion: The CBCT data provided accurate information about the location and course of the IAN.

하악 대구치에서 근관세척방법에 따른 잔사제거효과에 대한 평가 (IN VITRO EVALUATION OF CLEANING EFFICACY OF VARIOUS IRRIGATION METHODS IN MANDIBULAR MOLARS)

  • 본 연구의 목적은 isthmus가 존재하는 하악 대구치의 근심 근관에서 근관세척방법에 따른 잔사제거효과를 평가하는 것이다. 45개의 하악 대구치의 근심근관을 #30 Profile .06까지 확대하였다. 각 근관을 3개의 군으로 나누고 conventional irrigation syringe와 초음파, RinsEndo를 사용하여 1분간 최종 세척하였다. 조직표본을 제작한 후, 광학현미경으로 치근단 1, 3, 5 mm 부위의 단면을 관찰하였다. 근관과 isthmus의 잔사 양으로 청결도를 계산하고 Mann-Whitney U test로 검정하였다. 1. 초음파 군과 syringe군은 5 mm부위의 isthmus를 제외한 나머지 부위에서 청결도의 유의한 차이가 없었다. 2. RinsEndo군은 syringe군에 비해 1 mm, 3 mm부위의 근관과, 모든 부위의 isthmus에서 유의하게 높은 청결도를 나타내었다 (p<0.05). 3.초음파 군과 RinsEndo군은 3mm부위의 근관을 제외한 나머지 부위에서 청결도의 유의한 차이가 없었다. 임상적으로 RinsEndo를 이용한 근관세척방법의 부가적인 적용이 하악 대구치의 근심근관의 잔사제거에 효과가 있을 것으로 사료된다.

Assessment of endodontically treated teeth by using different radiographic methods: an ex vivo comparison between CBCT and other radiographic techniques

  • Purpose: To compare different radiographic methods for assessing endodontically treated teeth. Materials and Methods: Root canal treatments were applied in 120 extracted mandibular teeth, which were divided into four groups: (1) ideal root canal treatment (60 teeth), (2) insufficient lateral condensation (20 teeth), (3) root canals filled short of the apex (20 teeth), (4) overfilled root canal treatment (20 teeth). The teeth were imaged using intraoral film, panoramic film, digital intraoral systems (CCD and PSP), CCD obtained with portable X-ray source, digital panoramic, and CBCT images obtained at 0.3 $mm^3$ and 0.2 $mm^3$ voxel size. Images were evaluated separately by three observers, twice. Kappa coefficients were calculated. The percentage of correct readings obtained from each modality was calculated and compared using a t-test (p<0.05). Results: The intra-observer kappa for each observer ranged between 0.327 and 0.849. The inter-observer kappa for each observer for both readings ranged between 0.312 and 0.749. For the ideal root canal treatment group, CBCT with 0.2 $mm^3$ voxel images revealed the best results. For insufficient lateral condensation, the best readings were found with periapical film followed by CCD and PSP. The assessment of teeth with root canals filled short of the apex showed the highest percentage of correct readings by CBCT and CCD. For the overfilled canal treatment group, PSP images and conventional periapical film radiographs had the best scores. Conclusion: CBCT was found to be successful in the assessment of teeth with ideal root canal treatment and teeth with canals filled short of the apex.

Retrieval of a separated nickel-titanium instrument using a modified 18-guage needle and cyanoacrylate glue: a case report

  • During root canal preparation procedures, the potential for instrument breakage is always present. When instrument breakage occurs, it leads to anxiety of the clinician and as well as a metallic obstruction of the canal which hinders further cleaning and shaping. Separated instruments must always be attempted for retrieval and if retrieval is not possible bypass should be tried. With the increased use of nickel-titanium (NiTi) instruments the incidence of separated instruments has increased. A considerable amount of research has been done to understand the various factors related to the fracture of NiTi instruments to minimize its occurrence. This paper presents a review of the literature regarding the fracture of NiTi instruments and also describes a case report showing the use of a modified 18-guage needle and cyanoacrylate glue to retrieve a separated NiTi instrument from the mesiolingual canal of a mandibular first molar.

CBCT를 이용한 유구치의 치근 및 근관의 형태학적 평가 (Assessment of Root and Root Canal Morphology of Human Primary Molars using CBCT)

  • 이 연구의 목적은 CBCT를 사용하여 유구치 치근 및 근관의 형태학적 특징을 평가하는 것이다. 과잉치 발거를 위해 전남대학교 치과병원 소아치과에 내원하여 CBCT를 촬영한 만 4 - 5세 환자 총 68명의 영상으로 연구를 진행하였다. 치근 흡수가 되지 않은 상악 제1유구치, 상악 제2유구치, 하악 제1유구치, 하악 제2유구치로 구분하여 분석하였다. 치근 및 근관의 수, 치근 및 근관의 길이와 각도 등의 다양한 변수들이 분석되었다. 상악 유구치는 모두 3개의 치근을 가졌고 원심 협측 치근과 구개 치근은 단일 근관을 가졌다. 한 치근에 2개의 근관이 있는 경우는 오직 상악 제2유구치의 근심 협측 치근에서 발견되었다. 하악 유구치는 대부분 근심과 원심 2개의 치근을 가졌다. 하악의 근심 치근은 대부분 2개의 근관을 가졌다. 치근의 길이와 관련하여, 상악의 구개 치근의 길이가 가장 긴 반면, 원심 협측 치근의 길이는 가장 짧은 것으로 밝혀졌다. 하악 유구치는 근심의 치근이 더 길었다. 상악 유구치에서는 구개 치근의 각도가 가장 컸으나 하악은 근심의 각도가 더 작았다. 상악 유구치의 치근 및 근관이 곡선형이 많은 반면 하악 유구치는 직선형을 더 많이 볼 수 있었다.
