스테레오 매칭 기법을 이용한 영상유도시술 시스템 (Image-guided Surgery System Using the Stereo Matching Method)

  • 자기공명영상은 뛰어난 해상도의 해부학적 구조 정보를 제공하여 임상적인 외과수술에 매우 유용하게 적용되고 있다. 영상처리 기법과 MRI 영상유도기법을 이용한 뇌수술은 외과 전문의에게 많은 도움을 줄 수 있다. 본 논문에서는 스테레오 매칭 기법을 이용하여 중재적 시술이 가능한 유도영상시술 시스템의 개발에 관하여 소개하였다. 생검을 수행하기 위하여, MRI 마커, 카메라 마커, 탐침 프로브 마커를 정밀하게 제작하였고 시스템의 정확성을 입증하기 위하여 팬텀을 제작하였다. 제작된 마커와 팬텀을 이용하여 1.5 Tesla MRI 시스템으로 실험을 수행하였다. 구현된 시스템의 오차범위는 팬텀 실험에서 약 1.5%였고, 동물실험에서는 오차가 3mm 이내로 임상적용이 가능한 수준임을 착인하였다. 본 연구에서 제시한 스테레오 매칭기법을 이용한 유도영상시술 시스템은 기존의 방법보다 우수한 성능을 보여주었다.

MRI-guided Wire Localization Open Biopsy is Safe and Effective for Suspicious Cancer on Breast MRI

  • Background: Magnetic resonance imaging of breast, reported to be a high sensitivity of 94% to 100%, is the most sensitive method for detection of breast cancer. The purpose of this study was to investigate our clinical experience in MRI-guided breast lesion wire localization in Chinese women. Materials and Methods: A total of 44 patients with 46 lesions undergoing MRI-guided breast lesion localization were prospectively entered into this study between November 2013 and September 2014. Samples were collected using a 1.5-T magnet with a special MR biopsy positioning frame device. We evaluated clinical lesion characteristics on pre-biopsy MRI, pathologic results, and dynamic curve type baseline analysis. Results: Of the total of 46 wire localization excision biopsied lesions carried out in 44 female patients, pathology revealed fourteen malignancies (14/46, 30.4%) and thirty-two benign lesions (32/46, 69.6%). All lesions were successfully localized followed by excision biopsy and assessed for morphologic features highly suggestive of malignancy according to the American College of Radiology Breast Imaging Reporting and Data System (BI-RADS) category of MRI (C4a=18, C4b=17, C4c=8,C5=3). Of 46 lesions, 37 were masses and 9 were non-mass enhancement lesions. Thirty-two lesions showed a continuous kinetics curve, 11 were plateau and 3 were washout. Conclusions: Our study showed success in MRI-guided breast lesion wire localization with a satisfactory cancer diagnosis rate of 30.4%. MRI-guided wire localization breast lesion open biopsy is a safe and effective tool for the workup of suspicious lesions seen on breast MRI alone without major complications. This may contribute to increasing the diagnosis rate of early breast cancer and improve the prognosis in Chinese women.

  • 국내에 자기공명영상의 보급의 증가와 함께 유방 자기공명영상의 건수도 지속적으로 증가하고 있다. 하지만 이에 비해 자기공명영상 유도하 조직생검술 및 침위치결정술과 같은 유방의 중재적 시술은 초음파 유도하 시술이나 입체정위생검술에 비해 많이 이루어지지 않는다. 유방 자기공명영상은 다른 유방 영상 검사들에 비해 높은 민감도를 보이는 검사법이지만 그 특이도는 제한되는 검사법이기 때문에, 자기공명영상에서만 발견된 병변들은 자기공명영상 유도하 조직생검술이나 자기공명영상 유도하 침위치결정술을 통한 수술적 절제를 통하여 병리적 진단이 이루어져야 한다. 이러한 배경을 바탕으로, 본 종설에서는 자기공명영상 유도하 유방의 중재적 시술들의 적응증, 시술 방법, 시술 과정에서의 여러 고려 사항 및 제한점들에 대해 다루고자 한다.

Multiparametric MRI in Active Surveillance of Prostate Cancer: An Overview and a Practical Approach

  • MRI has become important for the detection of prostate cancer. MRI-guided biopsy is superior to conventional systematic biopsy in patients suspected with prostate cancer. MRI is also increasingly used for monitoring patients with low-risk prostate cancer during active surveillance. It improves patient selection for active surveillance at diagnosis, although its role during follow-up is unclear. We aim to review existing evidence and propose a practical approach for incorporating MRI into active surveillance protocols.

  • The authors congratulate the cerebrations for the 30 years of the Korean Society of Medical Physics (http://www.ksmp.or.kr/). The paper is published to recognize the anniversary. Geon-Ho Jahng invited Professor Z. H. Cho to join to submit this manuscript because he has been one of the leaders in the field of magnetic resonance imaging (MRI) during the last 40 years. In this review, we describe the development and clinical histories of MRI internationally and domestically. We also discuss diffusion and perfusion MRI, molecular imaging using MRI and MR spectroscopy (MRS), and the hybrid systems, such as positron emission tomography-MRI (PET-MRI), MR-guided focused ultrasound surgery (MRgFUS), and MRI-guided linear accelerators (MRI-LINACs). In each part, we discuss the historical evolution of the developments, technical developments, and clinical applications.

영상의학과 의사의 시각에서 본 자기공명영상 기반 전립선 표적 생검 (MR-Guided Targeted Prostate Biopsy from Radiologists' Perspective)

  • 전립선암의 진단은 전립선을 10-12구역으로 나눠서 무작위로 조직을 얻는 체계적 생검 방식이 표준적으로 사용되어 왔다. 그러나 종괴의 위치나 유무와 상관없이 조직의 획득이 이루어지기 때문에 일부 전립선암을 놓치거나 임상적으로 중요하지 않은 암을 과도하게 발견한다는 점에서 비판을 받고 있다. 이를 보완하기 위해 자기공명영상 기반 전립선 표적 생검이 제안되었으며 이는 자기공명영상에서 의심 병변을 발견 후 이를 토대로 초음파 유도 혹은 MRI 유도하에 표적 생검을 하는 방식이다. 본 종설에서는 자기공명영상 기반 표적 생검의 방식에 대해서 살펴보고 최근 전립선암 진단에 있어서의 가이드라인과 최근 동향에 대해 다루고자 한다.

  • Transrectal ultrasound (TRUS)-guided systematic biopsy, the current gold standard for the detection of prostate cancer, suffers from low sensitivity for clinically significant cancer. The use of diagnostic multiparametric MRI has increased the relevance of targeted biopsy techniques such as MRI-TRUS fusion biopsy and direct (in-bore) MRI-guided biopsy, which have higher detection rate for clinically significant cancer. Although primarily used in patients who remain at high clinical suspicion for prostate cancer despite a negative systematic biopsy, with the increasing use of upfront diagnostic MRI, these biopsies are expected to replace routine systematic biopsies. This pictorial essay aims to enhance our understanding of the concepts of these biopsy techniques so that they can be performed safely and provide maximum diagnostic yield.

세라믹 LDH 나노하이브리드를 이용한 MRI 조영제의 최신 연구동향 (Recent Progress in MRI Contrast Agent with Ceramic LDH Nanohybrids)

  • Ceramic layered double hydroxide (LDH) nanohybrids have attracted considerable interest in biomedical science due to their unique structural feature and characteristics in biological condition. Many studies on LDH nanoparticles have been reported in diagnosis applications including magnetic resonance imaging (MRI) contrast agents in order to not only provide better imaging performance through multimodal imaging strategy, but realize therapeutic function which treat cancers in one platform. This review highlights the recent progress in MRI T1 contrast agent, dual modal imaging system, and MRI-guided drug delivery systems ranging from synthetic method and characterization to evaluation in vitro and in vivo based on the ceramic LDH nanohybrids. Future research directions are also suggested for next-generation bio-imaging contrast agent.

Magnetic Resonance Imaging Meets Fiber Optics: a Brief Investigation of Multimodal Studies on Fiber Optics-Based Diagnostic / Therapeutic Techniques and Magnetic Resonance Imaging

  • Due to their high degree of freedom to transfer and acquire light, fiber optics can be used in the presence of strong magnetic fields. Hence, optical sensing and imaging based on fiber optics can be integrated with magnetic resonance imaging (MRI) diagnostic systems to acquire valuable information on biological tissues and organs based on a magnetic field. In this article, we explored the combination of MRI and optical sensing/imaging techniques by classifying them into the following topics: 1) functional near-infrared spectroscopy with functional MRI for brain studies and brain disease diagnoses, 2) integration of fiber-optic molecular imaging and optogenetic stimulation with MRI, and 3) optical therapeutic applications with an MRI guidance system. Through these investigations, we believe that a combination of MRI and optical sensing/imaging techniques can be employed as both research methods for multidisciplinary studies and clinical diagnostic/therapeutic devices.

Paradigm Shift in Prostate Cancer Diagnosis: Pre-Biopsy Prostate Magnetic Resonance Imaging and Targeted Biopsy

  • With regard to the indolent clinical characteristics of prostate cancer (PCa), the more selective detection of clinically significant PCa (CSC) has been emphasized in its diagnosis and management. Magnetic resonance imaging (MRI) has advanced technically, and recent international cooperation has provided a standardized imaging and reporting system for prostate MRI. Accordingly, prostate MRI has recently been investigated and utilized as a triage tool before biopsy to guide tissue sampling to increase the detection rate of CSC beyond the staging tool for patients in whom PCa was already confirmed on conventional systematic biopsy. Radiologists must understand the current paradigm shift for better PCa diagnosis and management. This article reviewed the recent literature, demonstrating the diagnostic value of pre-biopsy prostate MRI with targeted biopsy and discussed unsolved issues regarding the paradigm shift in the diagnosis of PCa.
