Characteristics of Sitting Balance and Trunk Muscle Endurance in Patients With Adolescent Idiopathic Scoliosis

  • The purpose of this study was to compare the static balance in a sitting position between a group with adolescent idiopathic scoliosis (AIS) and a normal aged-matched group. Forty-nine subjects were included in this study. Thirty-one healthy subjects and eighteen AIS subjects were participated. Each group was tested with the Lumbar Trunk Muscle Endurance Test (LTMET) and Balance Performance Monitor (BPM). The parameters for static balance were sway area, sway path, mean balance, maximum velocity, anterior-posterior angle, and left-right angle of each group with eyes opened and closed. Results from the LTMET showed significantly more increase in the normal group than in the AIS group in the flexor and extensor endurance. The BPM tested showed significantly difference beteen the groups in parameters of sitting balance such as maximum velocity and anterior-posterior sway angle. For the AIS subjects, there were no significant differences in all parameters of sitting balance between eyes opened and eyes closed. In comparisons of the groups with eyes opened there were no significant differences in all parameters of sitting balance. In comparisons of the groups with eyes closed there were significant differences in the sway area, maximum velocity, anterior-posterior sway angle and left-right sway angle. These results suggest that the AIS group relies much more on proprioception than on vision, and develops compensatory passive postures of the spine. Further study is needed to measure many AIS patients with morphologic and electromyographic data for clinical application.

Effects of Gastrocnemius and Lumbar Back Muscle Exercise on Standing Balance

  • The purpose of this study was to test the effect of Gastrocnemius and Low Back-muscle isotonic exercise on static dynamic standing balance during the period of 4 weeks. This study was two groups pretest-posttest design. Nineteen subjects who were over 22 years old were randomly assigned to either the experimental group that received the Gastrocnemius muscle exercise(n=9) or the low back muscle exercise(n=10) : The former group performed isotonic exercise(plantar flexion), the latter group performed isotonic exercise(trunk extension) a total of 18 times for three times per week for four weeks. Two groups also performed static and dynamic balance before the exercise and 4weeks after the exercise. The data were analyzed by using the paired t-test and independent t-test. The results were as follows: As compared with change of dynamic balance performance capacity at two groups, a significant difference was shown in the test(p<.05), but not in static balance(p>.05). Also, a significant difference of balance between groups was not shown in the test. In this study indicated that gastrocnemius and low back muscle isotonic exercise will have positive impact on standing balance.

The association between pain, balance, fall, and disability in patients with lumbar spinal stenosis with vascular claudication

  • Background: The effect of lumbar spinal stenosis (LSS) and peripheral vascular disease (PVD), which occurs with similar degenerative conditions, when seen together, has not been studied. The aim of this study is to examine and compare the relationship between pain, balance, disability, fear of falling, and kinesiophobia in LSS patients with intermittent vascular claudication (IVC). Methods: Seventy-two patients diagnosed with LSS using magnetic resonance imaging participated in this study. Thirty-five patients with IVC symptoms and showing vascular lesions by lower extremity venous and arterial Doppler ultrasonography imaging were included in the IVC-LSS group. The pain, static balance, dynamic balance, disability, fear of falling, and kinesiophobia were evaluated using the numeric rating scale, single leg stance test, Time Up and Go (TUG), the Oswestry Disability Index (ODI), Fall Efficacy Scale-International (FES-I), and Tampa Scale for Kinesiophobia (TSK), respectively. Results: Age and female sex were found to be higher in the IVC-LSS group (P = 0.024; P = 0.012). The IVC-LSS group had a shorter single leg stance time and TUG test duration, pain intensity, ODI, FES-I, and TSK scores were higher than patients with LSS (P = 0.001). Pain, fear of falling, and kinesiophobia were moderately correlated with disability in the IVC-LSS group. No relationship was found between pain and dynamic balance. Also, the pain was not related to kinesiophobia. Conclusions: The findings indicated that IVC causes loss of balance and an increase in pain, disability, fear of falling, and kinesophobia in patients with LSS.

진동자극 저항운동이 허리통증 경험자의 균형과 보행에 미치는 영향 (The Effect of Resistance Exercise with Vibration Stimulation on Balance and Gait of Experienced Back Pain Adults)

  • 본 연구는 진동자극 저항운동이 허리통증 경험자의 균형 및 보행에 미치는 영향을 연구하였다. 30명의 히리통증 경험이 있는 성인 대학생을 대상으로 8 Hz 진동자극 저항운동(n=10), 30 Hz 진동자극 저항운동(n=10), 허리 안정화운동(n=10)으로 무작위로 나누었다. 각 그룹의 중재는 6주동안 주 3회, 1회에 30분을 적용하였다. 실험 전에 정적균형과 보행패턴을 측정하였고, 6주 후에 실험전과 동일한 방법으로 측정하였다. 연구결과 세 그룹 모두에서 운동전과 후의 시간에 따른 정적균형, 디딤기율, 흔듦기율, 한걸음 길이, 한발짝률의 변화는 통계적으로 유의한 차이가 있었다(p<.05). 허리 안정화운동이 정적균형과 디딤기율에서는 그룹 간의 유의한 차이가 있었고(p<.05), 흔듦기율 및 한걸음 길이와 한발짝률은 그룹 간의 차이가 없었다. 결론적으로 진동자극 저항운동이 근수축 유발 기전에 의해 다리근육의 근력을 강화하였고, 강화된 다리 근력은 균형능력에 긍정적인 영향을 미쳤으며, 향상된 균형능력은 신체가 움직이는 동안 안정적으로 자세를 조절할 수 있게 하여 보행능력의 긍정적 측면에 영향을 미친 것으로 판단된다.

Combined training improves body composition, balance, and muscle function in sarcopenia elderly

  • Purpose: Sarcopenia is defined as a decrease in muscle mass, strength, and function with age that affects overall body function. We aimed to investigate the effect of combined training on body composition, balance, and muscle function in sarcopenia elderly. Research design, data, and methodology: Twenty-eight sarcopenia elderly (age 74.9±4.5 years) were randomly assigned to an exercise, EG (n=14), or a control, CG (n=14), group. The EG performed an intervention consisting of combined exercise training (60-75 min) for a total of 12 weeks, three times a week. The CG maintained their usual daily lifestyle during the intervention period. We measured body weight, body mass index (BMI), % body fat, free fat mass, balance ability, peak torque in shoulder, knee, and lumbar joints normalized for bodyweight in one second. Results: The EG showed improved body composition (i.e., BMI, fat-free body mass, fat mass; all p < 0.031, η2 > 0.179), balance (i.e., right and left of static and dynamic balance and fast 10 m walk; all p < 0.049, η2 > 0.152), and muscular function (i.e., 90°/sec and 180°/sec peak power per kg bodyweight, 90°/sec average power per kg bodyweight, 180°/sec total work, and 180°/sec endurance ratio; all p < 0.045, η2 > 0.158). Conclusions: Combined exercise training improves muscle mass and strength, body composition, balance, and muscle function in sarcopenia elderly.

청소년기 경직형 뇌성마비아동의 체간강화 운동이 척추분절 및 보행에 미치는 영향 (The Effects of Trunk Strengthening Program in Gait and Spinal Segment with Spastic Cerebral Palsy)

  • Purpose : This study aims to examine effects of trunk strengthening exercise in three rigid cerebral palsy adolescents aged between 11 and 13 on their spinal segments and gait. Methods : 2A trunk strengthening program for proprioceptive neuromuscular facilitation was made focusing on improvement in their school life and mental and social activities during their school years, and then trunk strengthening exercise was applied to the three subjects for 30 minutes five times per week for four weeks. With the subjects in a static state, each group's gait analysis was made and maximum anterior flexion, maximum extension, maximum left and right lateral flexion, and cross rotation of the trunk were measured using Sonosens (Germany) prior to and after the intervention. Results : Assessment of spinal segment mobility with the subjects in a static state showed that the trunk muscle strengthening exercise increased their maximum joint movement angles from the right side to the center and rotational abilities of the cervical and lumbar spines. Gait analysis indicated increased movements in the thoracic and lumbar spines and relatively decreased anterior-posterior movement of the cervical spine. Conclusion : Trunk strengthening program is effective in enhancing spinal segment mobility and balance ability of cerebral palsy adolescents and considered able to be used together with diverse treatment interventions.

가상현실 체험형 운동과 요부안정화운동에 따른 뇌졸중환자의 낙상관련 신체·심리적 비교 분석 (Comparative Analysis of Fall-Related Physicopsychological according to Virtual Exercise and Lumbar Stabilization Exercise in the Patient with Stroke)

  • 이 연구는 뇌졸중 환자를 대상으로 낙상예방을 위하여 가상현실 체험형 게임 운동(Nintendo-Wii Sport-NWS)과 요부안정화 운동을 실시하고 낙상관련 신체 심리 기능 변화를 측정하였다. 뇌졸중환자 30명을 대상으로 신체적 기능은 정적, 동적균형으로 측정하였고 심리적기능은 낙상효능감을 측정하여 운동 전과 운동 후, 각 운동군간을 비교분석하였다. 정적균형은 BBS, FRT, 동적균형은 TUG, 10m 보행검사, 낙상효능감은 낙상 효능감 지수로 측정되었다. 그 결과 두군 모두 신체 심리적기능에서 유의한 개선을 나타내었다. 그러나 BBS, FRT, 10m보행에서 요부안정화 운동군이 더 통계적으로 개선된 것으로 나타났으며 낙상효능감은 가상현실 체험형 게임 운동 군에서 더욱 개선된 것으로 나타났다. 결론적으로 가상현실 체험형 게임운동은 뇌졸중환자의 신체 심리적 기능 증진에 긍정적인 영향을 미친다.

필라테스 운동이 여중생의 요통과 체간 등속성 근력 및 평형성에 미치는 영향 (Effects of Pilates Exercises on Isokinetic Trunk Strength and Balance in Female Middle School Students with Lumbago)

  • Purpose: The purpose of this research was to investigate the effects of performing Pilates exercises for eight weeks on the isokinetic trunk strength and balance of female middle school students with lumbago. Methods: Twenty-four female students who met the test requirements were recruited and randomly divided into a control group (CG, n = 16) and a Pilates exercise group (Pilates group; PG, n = 8). The PG performed a Pilates program, which consisted of lumbar muscle strengthening exercises, for 60 minutes three times a week for eight weeks. Data analysis was conducted by two-way repeated ANOVA, and a Bonferroni test was carried out when significant differences appeared. The alpha level was set at 0.05. Results: Following the experimental treatment, the PG showed an increased trunk flexion peak torque of 17% and 13% at angular velocities of 180°/sec and 300°/sec, respectively. The intergroup comparison showed no significant difference at pre-measurement, while the PG increased about 31% (180°/sec) and 15% (300°/sec) higher than the control group at post-measurement. Following the experimental treatment, the PG showed an increased trunk extensor peak torque of approximately 29% and 21% at angular velocities of 180°/sec and 300°/sec, respectively. The intergroup comparison showed no significant difference at pre-measurement, while the PG increased approximately 27% (180°/sec) and 15% (300°/sec) higher than the control group at post-measurement. Both the CG and the PG showed a 20% and 50% decrease in pain index after the experimental treatment, respectively. After the experimental treatment, the PG had about 50% lower pain than the CG. The average error rate of static balance in the PG reduced by 19% from 3.28±0.45 to 2.65±0.36, confirming a significant improvement. Conclusion: Regular Pilates exercise for eight weeks improved the isokinetic trunk muscle strength of female middle school students with lumbago and relieved their pain. Pilates was also shown to be effective in improving balance.

체중부하 자세에 따른 요통환자와 정상인의 양측골반높이에 대한 연구 (Comparison of the Pelvic Height Difference in Subjects with Lower Back Pain and in Normal Subjects in Different Postures)

  • Background: Lumbar joint dysfunction is reported to be the main cause of lower back pain (LBP). The purpose of this study was to evaluate the effect of joint dysfunction on the postural balance of the lower hack and pelvis in different normal activities such as walking or stair management. Also it was studied whether the status of LBP (intensity and duration of LBP, length of treatment) contributes to die pelvic height difference (PHD) in various postures. Subjects: 28 patients with LBP and 32 normal adult volunteers, 60 years of age or younger, who came to the Community Health Center and orthopedic clinics in Incheon, South Korea. Methods: In order to determine the accuracy of the manual angulometer method in measuring the PHD, it was compared to the pelvic x-ray method in selected subjects. In the manual angulometer method, the arm of the angulometer was placed on the top of both iliac crests. The PHD was measured in static upright stance, then one-legged stance, on the affected leg or unaffected leg each time. Information regarding the disease status was obtained through interviews. Visual assessment scale was used to grade the intensity of LBP. Data analysis was performed using SPSS 10.0/PC program. Homogeneity between the two groups was tested by 2-test and t-test. To compare the PHD of the subgroups, we used t-test, F-test and two-way ANOVA. Relationships among dependent variables were analyzed by Pearson correlation analysis. Conclusion: In patients with LBP, lumbar joint dysfunction causes lumbar and pelvic postural asymmetry during normal activities.
