A Study on the Usefulness of Copper Filter in Single X-ray Whole Spine Lateral using 3D Printer (단일조사 whole spine Lateral 검사에서 3D 프린터로 제작한 구리 필터 유용성 연구)

  • The WSS lateral examination is important for diagnosing spinal disorders. Recently, long-length detectors for large-area diagnose have been popularized to effectively reduce the exposure dose and examination time. It can be applied very efficiently to examinations of patients with high risk of falls, children, and adolescents. However, since the image is acquired through a single irradiation, the volume of cervical vertebra is relatively smaller than the lumbar due to the geometrical anatomy of the spine. Therefore, this study intends to fabricate an additional filter using 3D printing technology and copper filament to obtain uniform image quality in the WSS lateral examination and to analyze the results. 3D printing technology is able to easily print a desired shape, so it is widely used in the entire industrial field, and recently, a copper filament has been developed to confirm the possibility as an additional filter. In the WSS lateral examination, CNR and SNR were excellently measured when the additional filter was applied, confirming the possibility of using the additional filter.

Raynaud`s Disease: One Case Report (Raynaud 씨 병: 1 치험례)

  • Raynaud attempted to clarify the situation concerning symmetrical and spontaneous gangrene in a thesis published in 1952. The disease was defined as Raynaud`s phenomenon without associated and contributing conditions or disease, and described as two types, such as, `locale syncope and asphyxia` and `gangrene`. Predilection of Raynaud`s disease for female under 40 years of age wi-thout any vascular occlusive disease beginning in the early decades of life and typical color changes in the skin of the extremities incited by coldness are outstanding features in this disease. One typical case of Raynaud`s disease is presented with relating references. Patient was 24 year old female single patient, who noted pain, numbness, and cyanosis of the finger tips of both hands for 6 years previously in the winter season, and recently such symptoms were aggravated including her both feet for two years even in the summer after exposure to cold water. Physical and laboratory examination revealed nothing specific except slightly glistening tight face and hypertrophy of both finger tips with clammy coldness. FamiliaI and past history revealed nothing specific abnormal contributory factors. Biopsy of skin on the dorsum of right foot one year before this admission revealed no evidence of scleroderma. Treatment was aimed to relieve vasospasmodic reaction to coldness and was very successful with bilateral lumbar and thoracic sympathectomy. Patient is free of symptoms relating to the Raynaud’s phenomenon after sympathectomy for 6 months including winter season.

Investigation on the correlation Improvement Rate of Symptoms with Moire Topography Analytic Improvement Rate (추나요법을 시행한 요통환자의 호전도와 Moire 영상 개선도와의 상관관계)

  • The present study retrospectively investigated clinical outcome at patients with low back pain or sciatica during Chuna treatment (flexion-distraction technique). The study population consisted of 29 patients. Discogenic group consisted of 21 patients who were already diagnosed as HNP of lumbar spine with serial MRIs(magnetic resonance imaging) or CTs(computerized tomography). Simple LBP group consisted of 8 patients with low back pain & sciatica who were non-specific disorder on radiologic examination. All patients were treated with flexion-distraction technique, one of Chuna technique, under analysis of Moire Topography. And the evaluation of clinical outcome was done twice during this study by Moire Topography Analytic Point and Low Back Pain Assesment, Visual Analogue Scale. The results were summarized as follows; Total improvement rate of Moire Topography was $25.8{\pm}17.8%$, and the rate of Low Back Pain Assesment was $56.5{\pm}23.0%$, Visual Analogue Scale of post-treatment was $32.6{\pm}22.5$ Between Improvement rate of Moire Topography and improvement rate of Low Back Pain Assesment, significant correlation was proved(Person's coefficient was 0.381, p<0.05). After all, it is certain improvement of Moire Topography represents symptom's improvement.

Crush Cytologic Findings of Myxopapillary Ependymoma in Spinal Cord - A Case Report - (점액 유두상 상의세포종의 압착도말 세포학적 소견 - 1예 보고 -)

  • Myxopapillary ependymoma generally arise in the conus medullaris and filum terminale of adult spinal cord. These tumors are readily recognized due to unique histopathologic features, however, their cytologic features are not well described. When only a tiny sample is obtained, cytologic examination using crush preparation may be a useful diagnostic tool to help appropriate intraoperative diagnosis. We present the crush cytologic features of myxopapillary ependymoma arising in thoracic and lumbar spinal cord of a 13-year-old boy. The patient had complained of paraparesis and back pain for 1 month. The MRI image revealed a relatively well demarcated intramedullary mass in T11-L1 levels. Crush preparation for cytology were peformed by biopsy material. Crush cytologic findings revealed high cellularity and small sized branching papillary clusters on fibrillary or mucinous background. The tumor cells had uniform round or elongated nuclei. The cytoplasmic process of tumor cells were attached to the vascular wall. Between the tumor cells and vascular walls, the perivascular collar of globoid acellular stroma with metachromatic reaction on toluidin blue stain was noted. The crush preparation of myxopapillary ependymoma is considered as a simple and highly accurate diagnostic tool for differentiation from other intramedullary neoplasms of central nervous system.

Analysis of the 3D-Surface Topography in Scoliosis (측만증 환자의 3차원 체표면 영상분석)

  • This study understood measured items of significantly difference through the surface topographical comparison of dorsal trunk's surface between normal and scoliosis subjects. 300 subjects for this study were participated (middle, high school and college students). Normal 28(males 12, females 16) and scoliosis subjects 16(males 4, females 12) were chosen by assessment tools (physical examination, 3D-surface topography) among subjects at Dongshin University Oriental Hospital. The results were as follow: 1. It was shown that the significantly difference was observed by distance of peak to curve with scoliosis between two groups(p<0.001). 2. It was shown that top difference of between groups in anterior-posterior view was the significantly difference at cervicothoracic, thoracic, thoracolumbar and dorsal part of lumbar(p<0.001). These results suggest that method to use 3D-surface topography measurement would be useful diagnosis method and clinical use of physical therapy.

  • The pelvic girdle function as an integrated unit with all three bones moving at all three joints, are influenced by the lower extremities below and vertebral column and trunk above sacroiliac movements are caused by spinal motion, whereas iliosacral movements are caused by movements of the lower limbs. Concept of normal functional integration among the lumbar spine, pelvic and hip joint is basic to the understanding of dysfunction in this region and also functional movement of the lumbo-pelvic-hip region are part of the clinical examination, consequently the integrated biomechanics of these region need to be understood. The purpose of this review is to ascertain the integrated biomechanics among the lumbo-pelvic-hip complex by consideration of literature and to give sufficient information to be able to render accurate assessment and treatment for the syndromes described.

A Clinical Trial for Two Adolescent Patients with Structural Spinal Scoliosis (청소년기의 구조적 척추 측만증 치험 2례)

  • Objectives: The purpose of this study is to report two cases of adolescent patient with structural spinal scoliosis treated by Chuna therapy. Methods: We examined two adolescent patients with an X-ray examination. These patients were diagnosed with structural scoliosis, and we used acupuncture and Chuna method at these patients. Cobb's angle and VAS(Visual Analogue Scale) were used to evaluate effects of the treatment. Results and Conclusions: After the treatment, we got these improvement results : In one case, the Cobb's angle was improved by $8.4^{\circ}$ ($19^{\circ}$ to $10.6^{\circ}$) at thoracic level (44.2% correctability) and VAS has been changed from 5 to 1-2. In another case, the Cobb's angle was improved by $4.3^{\circ}$($11.3^{\circ}$ to $7^{\circ}$) at lumbar level(38.0% correctability) and VAS has been changed from 7 to 1-2.

Diagnostic Imaging and Treatment of Lumbar Vertebral Fracture in a Dog (개에서 발생한 요추골절의 영상진단과 치료 1례)

  • A female, nine-month-old Rottweiler was presented with bilateral hind limbs lameness. Physical examination, survey radiography, computed tomography (CT) and magnetic resonance imaging (MRI) were performed. Oblique fracture of L6 vertebral body and abnormal alignment was observed in survey radiographs, CT, and MRI. No significant lesion was found in the intervertebral disc adjacent to fracture site. The patient was applied by bandage and medical management including corticosteroid administration was performed. The patient was able to walk after 4 weeks and prognosis was good.

Spinal Arachnoiditis after Continuous Epidural Block (지속적 경막외 차단술 후 발생한 척수거미막염)

  • A 35-year-old female patient was referred to our hospital with neurologic symptoms after continuous epidural block performed 2 days earlier. She die not have any prior no previous lumbar surgery or experience trauma, intraspinal hemorrhage, infections or other known causative factors to associate with neurologic symptoms. Continuous epidural block is widely used for postoperative pain control. Complications can occur with this block including postduralpuncture headache, epidural abscess and rare cases of arachnoiditis etc. We experienced such a case of spinal arachnoiditis after continuous epidural block. Neurologic examination revealed painful bilateral hypoesthesia below $S_2$ level dermatomes, urinary and fecal incontinence and various degrees of leg weakness. The following day, the patient was noted to have bilateral sacral radiculopathies and lesion on proximal portion of both tibial nerve. CSF study reported: protein 264 mg/dl, sugar 64 mg/dl, WBC $7/mm^3$. L-spine MyeloCTscan results were unremarkable. She was discharged after a month of hospitalization and has regular checkups but her neurologic symptoms show no signs of improvement.

Successful Treatment of a Symptomatic Discal Cyst by Percutaneous C-arm Guided Aspiration

  • Although discal cysts are a rare cause of low back pain and radiculopathy. Currently, surgical excision is usually the first-line treatment for discal cysts. However, alternative treatment methods have been suggested, as in some cases symptoms have improved with interventional therapies. A 27-year-old man presented with an acute onset of severe pain, and was found to have a discal cyst after an open discectomy. The patient underwent cyst aspiration and steroid injection through the facet joint under C-arm guidance. After the procedure, the patient's pain improved to NRS 0-1. On outpatient physical examination 1 week, and 1 and 3 months later, no abnormal neurological symptoms were present, and pain did not persist; thus, follow-up observation was terminated. When a discal cyst is diagnosed, it is more appropriate to consider interventional management instead of surgery as a first-line treatment, while planning for surgical resection if the symptoms do not improve or accompanying neurologic deficits progress.
