대사증후군 상태 개선을 위한 생활습관 중재프로그램의 프로그램 이론 평가 (Program Theory Evaluation of a Lifestyle Intervention Program for the Prevention and Treatment of Metabolic Syndrome)

  • Objectives: The purpose of this study is to evaluate the program theory of a lifestyle intervention program for the prevention and treatment of metabolic syndrome. Methods: The program evaluated is a tailored intervention for multiple health behavior associated with metabolic syndrome which is informed by theoretical constructs from the Intervention Mapping and Transtheoretical model. The program components include one-to-one health counseling, a self-management handbook, and a health diary. To evaluate program impact theory we examined the logic of program goals and objectives, intervention methods and strategies, and the theoretical constructs of program materials through document review and matrix building. Results: This evaluation has found that the intervention program applied social cognitive theory constructs to design intervention methods and strategies in addition to the Transtheoretical model: self-monitoring for goal setting and monitoring skill, outcome expectation for the benefits of health behavior change, and interaction with environment for observational learning through modeling. While the intervention addresses multiple determinants and behaviors, it is limited to an individual level and lacks social and environmental approaches. Following the Transtheoretical framework, the contents of the intervention materials were developed utilizing consciousness raising as a main strategy for earlier stages of change, and counterconditioning and stimulus control for later stages of change. Conclusion: Program theory evaluation can be a process of enhancing program validity. It would also be necessary for providing basis for efficient program implementation. When comparisons of program theory between similar programs are possible, program theory and validity will be strengthened when comparisons of program theories between similar programs are possible.

무릎의 신체 기능 개선을 위한 생활방식 중재 재활 전략 (Rehabilitation Strategies in Lifestyle Intervention for Improving the Physical Function of the Knee)

  • Purpose: Lifestyle intervention (LSI) provides basic recommendations that improve the quality of life and health of patients with minor disabilities. The LSI intervention strategies are associated with active living, healthy weight, healthy eating, and emotional stability. These intervention strategies can change an unhealthy lifestyle to a healthy lifestyle and provide important health care information. Main issue: This study focused on a new LSI-based knee rehabilitation protocol and proved the effect of exercise prescription on the knee. The clinical significance of this study demonstrated that continuous rehabilitation, effective rehabilitation, and recurrence prevention can be achieved by prescribing the appropriate exercise for patients after discharge. Therefore, practical lifestyle medicine knowledge and information are provided by the home-based rehabilitation self-exercise program with the new LSI-based knee rehabilitation protocol. Conclusions: The LSI-based protocol can improve and maintain health conditions and knee function. With the aim of improving self-care abilities, this program is expected to make significant contributions recurrence prevention, reduced mortality, and improved quality of life, physical function, and fitness.

생활습관개선 프로그램이 성인의 식이행동과 신체활동 및 심혈관위험요인에 미치는 효과: 중재 3개월 결과를 중심으로 (Short-term Effects of a Lifestyle Intervention Program on Eating Behaviors, Physical Activity and Cardiovascular Risks in Korean Adults)

  • Objectives: The aim of this study was to evaluate the effects of a lifestyle modification program for Korean adults with cardiovascular disease risk factors on their health behaviors and health status. Methods: A total of 448 adults with abdominal obesity and additional cardiovascular disease risk factors(high blood pressure, low HDL-cholesterol, high triglyceride or high blood glucose) were randomly assigned to either an intensive intervention group (IIG, n=216) or a minimal intervention group(MIG, n=232). Participants in the IIG received lifestyle modification program which consisted of health counseling with nutrition assessment, health booklet and health diary, while those in MIG received minimal information. Results: The participants in the IIG significantly improved dietary habits(p<.05), retrained eating(p<.001), external eating(p<.01) behaviors, leisure time physical activity(p<.05), dietary self-efficacy(p<.01), exercise self-efficacy(p<.01) and MetS score(p<.001) after 3 months. In addition, the participants in the IIG showed more improvement in dietary habits(p<.05) compared with those in the MIG. Conclusion: The lifestyle modification program was effective in improving some health behaviors, behavioral determinants and cardiovascular risk factors for a short term.

대학생을 위한 라이프스타일 재설계 프로그램 개발 (Development of a Lifestyle Redesign Program for College Students)

  • 본 연구는 대학생을 대상으로 적용 가능한 라이프스타일 재설계 프로그램을 개발하고자 하였다. 프로그램 개발을 위해 전문가 8명을 대상으로 델파이 연구방법을 시행하였다. 라이프스타일 및 시간 사용에 대한 선행연구를 기반으로, 1차 델파이 조사에서는 평가도구, 라이프스타일 유형 파악, 생활시간조사표를 통한 활동의 우선순위 결정에 대한 조사가 이루어졌다. 2차 델파이 조사에서는 전문가 의견을 바탕으로 구성하였고, 내용타당도 지수가 낮은 경우에는 수정 및 보완하여 재구성하였다. 3차 델파이 조사에서는 2차 내용의 틀을 유지하면서 전문가 자신의 2차 때 응답과 다른 전문가와의 2차 평균 점수를 함께 제시하였다. 연구결과 최종적으로 평균이 3.88~4.00점, 내용타당도 지수가 모두 1.00, 수렴도가 모두 0.00인 프로그램을 도출하였다. 대학생을 위한 라이프스타일 재설계 프로그램은 1단계 중재 전 평가로 라이프스타일 유형 파악과 범용 평가도구의 적용, 활동을 선택을 하고, 2단계는 중재실행으로 라이프스타일을 재설계하며 진행자가 개입하여 대학생들이 잘 실천할 수 있도록 독려하고, 3단계는 중재결과로 라이프스타일 유형의 변화를 확인하며 중재 후 범용 평가도구를 적용하고, 마지막 4단계는 프로그램을 종결하는 것으로 프로그램이 개발되었다. 이는 대학생들 자신이 원하는 생활을 성공적으로 수행하기 위해, 그들의 수행패턴을 변화시키는 하나의 중재로 라이프스타일 재설계를 활용하도록 하는 근거를 제공하였다.

  • Objectives: The purpose of the this study was to evaluate lifestyle intervention program for the treatment of the metabolic syndrome. Methods: Subjects of this study were 700 adults with metabolic syndrome who took health examinations in health promotion centers of Korea Association of Health Promotion between May 1 and June 30, 2006. Subjects were randomly assigned to an experimental group and a comparison group. Participants in the experimental group received intensive 3-month lifestyle modification intervention and participants in the comparison group received minimal information on lifestyle modification. Pre test and Post test were carried out to evaluate the effectiveness of the intervention program. Results: After the intensive intervention on lifestyle modification(healthy diet, physical activity, moderate drinking, stress management, and smoking cessation), the levels of blood pressure, waist circumference, fasting blood glucose and triglycerides were significantly reduced for those in experimental group(P<0.001). For those in comparison group, the levels of blood pressure(P<0.001), waist circumference(P<0.001), and triglycerides(P<0.01) were significantly reduced after the intervention. No change in the levels of high-density lipoprotein cholesterol were observed in both groups. After 3-month intervention, the prevalence of metabolic syndrome was reduced to 35.7% in experimental group and 48.5% in comparison group. Conclusion: This study has demonstrated the efficacy of therapeutic lifestyle intervention for the management of metabolic syndrome.

뇌졸중 재발예방을 위한 생활양식조절 코칭 프로그램의 효과 (The Lifestyle Modification Coaching Program for Secondary Stroke Prevention)

  • Purpose: The purpose of this study was to investigate the effects of the lifestyle modification coaching program on self efficacy, lifestyle and physiologic indexes related to the recurrence of stroke in patients with stroke. Methods: Sixty-one patients with stroke registered with a stroke center participated in this study. Of the participants, 32 were assigned to the experimental group and 29 to the control group. Self efficacy, alcohol drinking, smoking, physical activity, BMI (body mass index), WHR (waist-hip ratio), blood lipid level, and blood pressure were measured both for the baseline, as well as after intervention. The lifestyle modification coaching program consisted of an 8-week telecoaching session following face-to-face education. The control group received only the face-to-face education. Results: There were significant differences in physical activities, WHR, blood pressure within and between groups after intervention. The lifestyle modification coaching program had significant influences on blood pressure even after gender, age, and physical activity had been adjusted. Conclusion: The results of the study indicate that lifestyle modification coaching program is effective for physical activity, abdominal obesity and blood pressure. Therefore it can be used by nurses in hospitals and communities as one of the secondary stroke prevention programs for patients with stroke.

과체중 및 비만임부를 위한 생활습관중재 프로그램의 효과 (Effect of Lifestyle Intervention Program for Overweight and Obesity Pregnant Women)

  • Purpose: This study was conducted to identify the effects of a lifestyle intervention program on weight gain, dietary habits, fatigue and pregnancy stress, blood pressure, and neonatal birth weight, using Cox's interaction model of client health behavior for overweight and obese women. Methods: This was a quasi-experimental research with a non-equivalent control group pre-post test design. A total of 52 patients who met the selection criteria, including 25 in the experimental group and 27 in the control group, were the subjects of the study; they comprised overweight and obese pregnant women who were receiving prenatal care at A and B women's hospital in J province. The lifestyle intervention program ran for 12 weeks in total and consisted of interactions involving affective support, health information, and professional/technical competencies. The data collection period was from February 1, 2017 to August 31, 2017. Results: This study showed differences in the appropriate weight gain rate (χ2=6.17, p=.013), suppression of an increase in fatigue (t=-2.32, p=.012), and an increase in pregnancy stress (t=-1.87, p=.034). Yet, no differences in physical activity, dietary habits change, blood pressure, and neonatal birth weight (p>.05) were found. Conclusion: The study findings indicate that this program could be an effective intervention for the control of appropriate weight gain, fatigue, and pregnancy stress. Therefore, a lifestyle intervention program based on Cox's interaction model of client health behavior could be an efficient strategy for a positive health outcome of overweight and obesity pregnant women.

비만 성인 대상 체중관리 프로그램 "Healthy Life Plan"의 시범운용 효과 (The Effects of "Healthy Life Plan" Program on the Management of Obesity for Korean Adults: A Pilot Study)

  • Objectives: This study aims to evaluate the effectiveness of a lifestyle intervention on the improvement of obesity, health behaviors and behavioral determinants among Korean adults. Methods: Obese adults aged 30 to 59 years (n=76) were randomly assigned to the intervention group (IG) or the comparison group (CG). The IG received a 3-month multi-component lifestyle intervention, while the CG received minimal information on obesity. The program consisted of health counseling, health education booklet and health diary. Health examination and self-administered survey were conducted before and after the intervention to determine the effectiveness of the program. Results: After the intervention. health indices of the IG were significantly improved in weight(p=.003), waist circumference(p=.011), % body fat(p=.021), and total abdominal fat area(p=.041). The reduction of waist circumstance among IG participants was better than that of those in the CG(p=.017). The IG demonstrated significant improvements in dietary behaviors(p=.013), periodic measurement of waist circumstance(p=.005), pros of weight control(p<.001) and awareness of one's current biomarkers(p=.038) better than the CG did. The proportion of normalized participants in waist circumference was 21.1% in the IG and 8.7% in the CG(p=.017). Conclusions: Lifestyle intervention program can improve the obese status and health behaviors in adults.

심뇌혈관질환 예방을 위한 생활습관 개선 프로그램에 관한 문헌분석 연구 (The Literature Review on Life style Intervention Program for the Prevention of Cardiovascular Disease)

  • 본 연구는 심뇌혈관질환 예방을 위한 생활습관 중재 프로그램의 특성을 파악하기 위하여 프로그램 중재 연구논문을 분석한 문헌분석 연구이다. 연구대상 논문은 Pubmed 등 전자 데이터베이스를 통해 "심뇌혈관질환", "중재" 등의 검색어를 적용하여 최종 18편의 논문이 선정되었다. 대상 문헌에서의 프로그램 참여 대상자 선정기준은 주로 신체생리학적 기준이 이용되었고, 신체활동과 식습관 생활습관 영역을 중재한 프로그램이 많았다. 중재유형은 교육이 가장 많이 이용되었고, 여러 중재를 병합하여 적용하는 연구가 많은 것으로 나타났다. 중재기간은 평균 22주 이상의 장기개입이 많았고, 중재효과를 측정하는 효과지표는 신체생리학적 지표가 가장 많이 활용되었다. 본 연구를 통해 여러 생활습관의 상호관련성을 고려한 중재 프로그램의 개발과 건강한 생활습관을 유지하기 위한 환경 및 사회적 지원이 필요함을 제언한다.

지역사회 거주 노인의 건강관리를 위한 작업기반 라이프스타일 중재 프로그램의 효과 (Effectiveness of Occupation Based Lifestyle Intervention Program for Health Management of Community Dwelling Older Adults)

  • 목적 : 본 연구는 작업기반 라이프스타일 중재 프로그램이 지역사회 노인에게 미치는 효과를 알아보고 적용가능성을 알아보기 위해 진행되었다. 연구방법 : 지역사회에 거주하고 있는 노인 9명을 대상으로 진행되었으며 단일집단 사전-사후 검사 설계를 사용하여 연구를 진행하였다. 참가자들은 해외 연구를 통해 효과가 확인된 라이프스타일 리디자인 프로그램(Lifestyle redesign program)을 기반으로 한 작업기반 라이프스타일 중재 프로그램을 그룹회기 12회, 개인회기 1회로 총 7주에 걸쳐 제공받았다. 효과를 알아보기 위해 작업균형, 시간사용, 활동참여 수준, 활동 수행도 및 만족도, 우울, 건강관련 삶의 질에 대한 평가를 프로그램 전, 후로 진행하고 평가 결과를 분석하였다. 또한 후기평가 시 프로그램에 대한 설문을 진행하여 참가자들이 생각하는 주관적인 프로그램의 타당성, 필요성, 효과성에 대해 알아보았다. 결과 : 초기평가와 후기평가를 통해 얻은 자료를 분석한 결과 전반적인 작업균형(p=.012)과 하위영역인 건강(p=.017), 관계(p=.012), 도전 및 흥미(p=.012), 자아정체성(p=.012)에 대한 작업균형 그리고 활동참여수준(p=.008), 활동 수행도(p=.012) 및 만족도(p=.008), 전반적인 건강관련 삶의 질(p=.034) 및 하위영역 중 신체적 건강(p=.041)이 향상되었으며, 여가(p=.008)와 휴식(p=.008)에 대한 시간사용에 변화가 있었고, 마지막으로 우울(p=.012)이 감소한 것을 확인할 수 있었다. 또한 프로그램에 대한 설문 결과 참가자들이 생각하는 프로그램에 대한 주관적 타당성, 필요성, 효과성 모두 적절한 수준을 나타내는 것을 확인하였다. 결론 : 작업기반 라이프스타일 중재프로그램은 지역사회 노인에게 충분히 받아들여질 수 있으며 건강에 긍정적인 영향을 미칠 수 있는 중재법으로 사료된다.
