Hydrogen refueling stations that use compressed hydrogen at high pressure provide safety distances between facilities in order to ensure safety. Most accidents occurring in hydrogen stations are accidental leaks. When a leak occurs, various types of ignition sources generate a jet flame. Therefore, the analysis of leaked gas diffusion and jet flame due to high pressure hydrogen leakage is one of the most important factor for setting the safety distance. In this study, the leakage accidents that occur in the hydrogen refueling station operated in high pressure environment are simulated for various leakage source sizes. The results of this study will be used as a reference for the future safety standards.

On offshore platforms, oil and gas leaks are apt to be the initial events of major accidents that may result in significant loss of life and property damage. To prevent accidents induced by leakage, it is vital to perform a case-specific and accurate risk assessment. This paper presents an integrated method of Ddynamic Qquantitative Rrisk Aassessment (DQRA)-using the Decision Making Trial and Evaluation Laboratory (DEMATEL)-Bayesian Network (BN)-for evaluation of the system vulnerabilities and prediction of the occurrence probabilities of accidents induced by leakage. In the method, three-level indicators are established to identify factors, events, and subsystems that may lead to leakage, fire, and explosion. The critical indicators that directly influence the evolution of risk are identified using DEMATEL. Then, a sequential model is developed to describe the escalation of initial events using an Event Tree (ET), which is converted into a BN to calculate the posterior probabilities of indicators. Using the newly introduced accident precursor data, the failure probabilities of safety barriers and basic factors, and the occurrence probabilities of different consequences can be updated using the BN. The proposed method overcomes the limitations of traditional methods that cannot effectively utilize the operational data of platforms. This work shows trends of accident risks over time and provides useful information for risk control of floating marine platforms.
As the meter reading method of gas meters develops, smart gas meters capable of remote meter reading (leak checking meters and multi-function safety meters) are being used. These meters have a function to judge leakage by utilizing the flow rate and pressure data collected as an additional function. Leakage judgment function using flow rate data has valid cases in the actual field, but the pressure data-based leakage judgment standard is based on not only the pressure value change due to leakage but also various factors (pressure regulator pressure, connection with adjacent meters, usage of adjacent houses, location of meters, etc.). There is a problem with pressure magnitude changes (levels). In this paper, as a technique that can judge leaks by using pressure data collected from smart gas meters, it was developed through preprocessing of pressure data, criteria for pressure value ranges related to leaks, leak judgment techniques, and application case verification.
This study was experimentally performed in order to estimate the errors due to the leakage of transmitter gaugelines in the orifice flow meter for natural gas. It would be a serious problem in safety if a large quantity of leak was occurred at the tubes or fittings like valve. But in most cases the safety problems might be rarely happened because the gas leak detectors could be operated in advance and the various kinds of inspection would be also fulfilled periodically. If the leakage was occurred continuously with an undetectable amount at the gaugelines for measuring the pressure or the differential pressure(DP), the amount of leakage might be an error or an unaccounted flow(UAE). In addition if the measuring value of pressure or DP were affected by the leakage, it might also be a measurement error. The experiments were performed to estimate the amount of leakage and to check the DP changes if it exited. First, through the measurement of the air pressure changes in the airtight container connected to a transmitter with gaugelines as the time passed, the amount of leakage causing from the fittings of gaugelines was roughly estimated. As changing the leak position of the gaugeline, the leak was intentionally made to break out. The variance of DP was checked as controlling the extent of leakage and compared to no leak conditions. Consequently, under the normal maintenance conditions, the result represented that the amount of leakage causing from the gaugelines was insignificant and also the DP changes on leakage conditions were too small to cause the errors of measurements.

Hydrogen is a fuel of fuel cell system, which has powerful explosion possibility. Hence, the fuel cell system needs safety evaluation to prevent risk of hydrogen leakage. We use a actual size chamber of a common fuel cell module to analyze hydrogen. Hydrogen injection holes are located in lower part of the chamber in order to simulated hydrogen leakage. The hydrogen sensor can detect range of 0${\sim}$4%. Since the hydrogen gas, of which leaked amount is controled by MFC, are injected at the bottom holes, the transient sensor signals are measured. At a condition of 10cc/s of hydrogen leakage, the sensor detects hydrogen leakage after 22sec and there is also several seconds of time delay depending on the position of the sensor. This experimental data can be applied for the design of the hydrogen detection system and ventilation system of a residential fuel cell system.
Temperature variation according to space and time on the inner parts of engineering constructions(e.g.: dam, slope) can be a basic information for diagnosing their safety problem. In general, as constructions become superannuated, structural deformation(e.g.: cracks, defects) could be occurred by various factors. Seepage or leakage of water through these cracks or defects in old dams will directly cause temperature anomaly. Groundwater level also can be easily observed by abrupt change of temperature on the level. This study shows that the position of seepage or leakage in dam body can be detected by multi-channel temperature monitoring using thermal line sensor. For this, diverse temperature monitoring experiments for a leakage physical model were performed in the laboratory. In field application of an old earth fill dam, temperature variations for water depth and for inner parts of boreholes located at downstream slope were measured. Temperature monitoring results for a long time at the bottom of downstream slope of the dam showed the possibility that temperature monitoring can provide the synthetic information about flowing path and quantity of seepage of leakage in dam body.
Due to the rapid spread and low minimum ignition energy of hydrogen, rupture is highly likely to cause fire, explosion and major accidents. The self-ignition of high-pressure hydrogen is highly likely to ignite immediately when it leaks from an open space, resulting in jet fire. Results of the diffusion and leakage simulation show that jet effect occurs from the leakage source to a certain distance. And at the end of location, the vapor cloud explosion can be occurred due to the formation of hydrogen vapor clouds by built-up. In the result, it is important that depending on the time of ignition, a jet fire or a vapor cloud explosion may occur. Therefore, it is necessary to take into account jet effect by location of leakage source and establish a damage minimizing plan for the possible jet fire or vapor cloud explosion. And it is required to any kind of measurements such as an interlock system to prevent hydrogen leakage or minimize the amount of leakage when detecting leakage of gas.

The pore water pressure in the embankment ranged from $0.035{\sim}1.116kg/cm^2$. The seepage that piping showed, as well as the leakage from the reservoirs are acceptable for the safety management of the reservoir. The maximum horizontal displacement and direction analyzed from the measured inclinometer data gives us very effective information to evaluate the safety in reservoirs. The resistivity monitoring technique, which is obtained on the reservoir crest, is an efficient tool to detect leakage zone. The safety index (SI) was predicted by the resistivity monitoring, and was evaluated to have a safety level of 0.8-1.0 at all reservoirs. Safety evaluations of reservoir through instrument systems are effective when studying the embankment, when the results of the instrument system have been analyzed compositively.

As the simple empirical and phenomenological model applied to the analysis of leakage and explosion of chemical substances does not regard numerous variables, such as positional density of installations and equipment, turbulence, atmospheric conditions, obstacles, and wind effects, there is a significant gap between actual accident consequence and computation. Therefore, the risk management of a chemical plant based on such a computation surely has low reliability. Since a process plant is required to have outcomes more similar to the actual outcomes to secure highly reliable safety, this study was designed to apply the CFD (computational fluid dynamics) simulation technique to analyze a virtual prediction under numerous variables of leakages and explosions very similarly to reality, in order to review the computation technique of the practical safety distance at a process plant.
