외부계측법에 의한 상악골 이동의 위치적 정확도에 대한 평가 연구 (A STUDY ON ACCURACY OF MAXILLARY REPOSITIONING BY EXTERNAL MEASURING TECHIQUE)

  • 하악골의 발육이상과 더불어 상악골의 수평적 및 수직적 발육이상이 동반되어 있는 대부분의 악안면 기형 환자에서 상하악골을 동시에 재위치시키는 "상하악골 동시 이동 수술"은 현재 악교정 수술에서 보편화되어 있는 술식으므로, 이 경우 수술중에 일차로 상악골의 정확한 위치를 얻는 것이 매우 중요하다. Le Fort I 수평골절단술을 이용한 상악골의 재위치시 위치 이동의 정확성을 기하기 위하여 고전적으로 사용되어온 계측방법인 내부계측법(Intrnal measuring technique)은 최근에 이르러 계측상의 삼차원적 오차에 따른 정확도에 많은 의문이 제기되어 왔으며, 따라서 이를 보완하기 위하여 1985년 Johnson 이 처음으로 외부계측법(External measuring technique)을 소개한 이후로, Van Sickels 등의 여러 학자들에 의하여 그 사용의 유용성 및 정확성이 입증되어 왔다. 이에 저자등은 최근 2년간 본 교실에서 경험상 상${\cdot}$하악골동시이동 악교정수술환자중, 외부계측법에만 의존하여 상악골 이동량을 술중 계측했던 17명의 환자에서 수술전 이동계측량과 수술후 이동된 결과를 Lateral cephalometics상에서 전치부와 구치부의 수직 및 수평적 이동 오차 관계로, P-A cephalogram 상에서 수평적 이동 및 상악치열의 중심선 변위에 대한 이동 오차 관계로 분석하여 외부계측법에 의한 수술적 이동의 정확도를 평가하므로써 향후 이 방법의 사용에 대한 신뢰성을 보고자 본 연구를 시행하였다. 연구 결과로써, 상악골의 수직 및 수평적 이동시 외부계측법 만을 이용한 경우, 수술전 이동량에 비해 비교적 신뢰성 있게 상악골을 원하는 위치로 이동시킬 수 있는 것으로 사료되나, 대체적으로 전하방 및 우측으로 위치되기 쉬운 경향을 보여주었으며, 정면상의 수평기울기(transverse plane)에 대한 편차는 수술자에 의해 크게 영향을 받지 않았으나 상악치열의 중심위는 수술자에 의해 영향을 받은것 같다.

반안면 왜소증 환자에서의 골신장술: 증례보고 (THE USE OF DISTRACTION OSTEOGENESIS TO TREAT HEMIFACIAL MICROSOMIA: A CASE REPORT)

  • Distraction osteogenesis(DO) is a surgical method of bone formation that involves an osteotomy and sequential stretching of the healing callus by gradual movement and subsequent remodeling. DO is used to correct facial asymmetry, such as in patients with hemifacial microsomia, maxillary or mandibular retrusion, cleft lip and palate, alveolar defects, and craniofacial deficiency. It is accomplished with the aid of a distraction device, which is secured with screws placed directly into bone, for a predetermined length of time. Hemifacial microsomia is characterized by unilateral facial hypoplasia, often with unilateral shortening of the mandible and subsequent malocclusion. Patients with hemifacial microsomia and facial asymmetry have a vertically short maxilla, tilted occlusal plane, and short mandible. Early treatment is necessary to avoid subsequent impaired midfacial growth. The standard treatment of these malformations consists of the application of bone grafts, which can lead to unpredictable growth. The new bone-lengthening procedure represents a limited surgical intervention and opens up a new perspective for treatment, especially in younger children with severe deformities. This report describes a case of hemifacial microsomia(Type-II left-sided hemifacial microsomia). The patient, a 10-year-old child, visited our clinic for facial asymmetry correction. He had a hypoplastic mandible, displaced ear lobe, 10 mm canting on the right side, and malocclusion. We planned DO to lengthen the left mandible in conjunction with a Le Fort I osteotomy for decanting and then perform a right intraoral vertical ramus osteotomy(IVRO). Progressive distraction at a rate of 0.5 mm/12 hours was initiated 7 days postoperatively. The duration of DO was 17 days. The consolidation period was 3 months. Satisfactory results were obtained in our case, indicating that DO can be used successfully for functional, aesthetic reconstruction of the mandible. We report a case involving DO in conjunction with orthognathic surgery for correcting mandibular hypoplasia with a review of the literature.

악교정 수술환자의 술전후 하악운동 양상변화에 관한 임상적 연구 (A CLINICAL STUDY ON MANDIBULAR MOVEMENT AFTER ORTHOGNATHIC SURGERY)

  • 저자는 1999년 6월부터 2000년 4월까지 경북대학교병원 구강악안면외과에서 하악골 후퇴를 위해 SSRO 혹은 LeFort I 골 절단술을 동반한 SSRO를 시행받은 환자 42명을 대상으로 술전, 술후 1개월, 술후 6개월에 각각 MKG를 이용하여 하악운동량 및 양상을 조사하고 이를 몇 가지 요소에 따라 군으로 나누어 계측치를 측정, 연구한 바 아래와 같은 결과를 얻었다. 1. 남녀 성별에 따른 두 군 사이에서, 좌측운동량의 변화 및 최대 개구속도의 변화량은 유의한 차이가 있었으며(p<0.05), 그 외의 항목은 차이가 없었다. 2. SSRO만을 시행 받거나 혹은 LeFort I을 동반하여 SSRO를 시행받을 경우, 수술방법에 따른 하악운동량의 변화는 유의성이 없었다.(p>0.05) 3. 하악골이동량에 따른 세 군에서 각 군사이의 하악운동량의 변화는 좌측 측방운동량에서는 $6{\sim}10mm$의 이동군이 가장 우수하였으며(p<0.05), 그 외의 항목은 통계적 유의성이 없었다. 4. 하악골의 개폐구 양상은 전두면 상에서 술전이 복잡편향형, 단순편향형, 복잡편위형, 단순편위형, 직선형 순이었고 술후 1개월에서는 단순편향형, 단순편위형, 복잡편위형, 직선형, 복잡편향형 순이며, 술후 6개월에서는 술전과 같은 순서였다. 또한 시상면상에서는 술전에 비일치형이, 술후 1개월에서는 일치형이 술후 6개월에서는 다시 비일치형이 우세하였다. 5. 술전 관절증의 증상유무에 따른 두 군사이의 하악운동량의 변화에는 통계적 유의성이 없었다(p>0.05). 6. 과두재위치장치를 적용한 군과 적용하지 않은 군에서, 두 군사이의 하악운동량의 변화에는 유의성이 없었다(p>0.05). 7. 술전 관절증이 있었던 환자중 술후 관절증의 완화가 나타난 경우가 63% 였으며, 과두재위치장치를 적용하지 않은 군에서 술후 관절증의 완화가 더 유의성있게 나타났다(p<0.05).를 보였으며 복합 레진은 유의차가 없었다. 상악 견치 (8%), 하악 제 1소구치 (5%) 순이었다. 10. I군에서 추정되는 낭의 발생 원인으로는 치수절단술을 받은 유치가 59.2%로 가장 많았고 이외에 심한 우식 및 치료 받지 않은 외상 병력등 기타 가능한 원인들이 있었다. 11.함치성 낭의 치료법으로는 I군의 경우 61.2%에서 조대술이 시행되었고, II군과 III군의 경우 61.1%, 80.0%에서 적출술이 시행되었다.mH I처치시 SM1과 SM2는 4조각의 절편으로 절단되어 같은 양상을 보였고, GS톤의 경우는 3조각의 절편으로 절단되었다. Kpn I, Sma I, Xho I 그리고 Pst I에는 절단되지 않았다.s subsp. salicinius와 유전자 유사치가 99.60%, 99.73%를 보여 Lactobacillus salivarius subsp. salicinius로 동정되었다. 이상의 결과를 종합하면 치아우식증이 없는 소아의 타액에서 분리된 유산균 중 과산화수소를 분비하여 인공치태 형성과 휘발성 유황화합물 생성을 억제하는 분리균주는 Lactobacillus salivarius subsp. salicinius로 동정되었다.적으로 낮은 수축률과 우수한 물성을 보였으며, 나노필러를 사용한 복합레진의 경우, 기존의 hybrid 필러를 이용한 레진에 비하여 수축응력을 감소시키지는 못하였다. 나노필러를 이용한 복합레진은 개발의 초기단계이며, 물성의 증가를 위한 연구가 필요할 것으로 사료된다.또 다른 약물인 glycyrrhetinic acid($100{\mu}M$)도 CCh 자극으로 인한 타액분비를 억제하였다. 이상의 결과로 미루어 gap junction은 흰쥐 악하선 세포로부터의 타액분비 조절에 중요한 역할을 하는데, 이는 gap junction이 세포막 $Ca^{2+}$ 통로를 조절함으로써 수용체 자극으로 유발된 세포내

Comparison of time and cost between conventional surgical planning and virtual surgical planning in orthognathic surgery in Korea

  • Background: The purpose of this study was to measure the time of the conventional surgical planning (CSP) and virtual surgical planning (VSP) in orthognathic surgery and to compare them in terms of cost. Material and method: This is a retrospective study of the patients who underwent orthognathic surgery at the Pusan National University Dental Hospital from December 2017 to August 2018. All the patients were analyzed through both CSP and VSP, and all the surgical stents were fabricated through manual and three-dimensional (3D) printing. The predictor variables were the planning method (CSP vs. VSP) and the surgery type (group I: Le Fort I osteotomy + bilateral sagittal split osteotomy [LFI+BSSO] or group II: only bilateral sagittal split osteotomy [BSSO]), and the outcomes were the time and cost. The results were analyzed using the paired t test. Results: Thirty patients (12 females, 18 males) met the inclusion criteria, and 17 patients were excluded from the study due to missing or incomplete data. There were 20 group I patients (LFI+BSSO regardless of genioplasty) and 10 group II patients (BSSO regardless of genioplasty). The average time of CSP for group I was 385 ± 7.8 min, and that for group II was 195 ± 8.33 min. The time reduction rate of VSP compared with CSP was 62.8% in group I and 41.5% in group II. On the other hand, there was no statistically significant cost reduction. Conclusions: The time investment in VSP in this study was significantly smaller than that in CSP, and the difference was greater in group I than in group II.

상악골 재위치술 시행 시 골편의 이동량에 따른 내측기준점의 변화 - 3차원 가상수술 프로그램을 이용한 연구 (Positional Changes of the Internal Reference Points Followed by Reposition of the Maxilla - A Study of a 3D Virtual Surgery Program)

  • Purpose: Reposition of the maxilla is a common technique for correction of midfacial deformities. To achieve the goal of the surgery, the maxilla should be repositioned based on the precisely planned position during surgery. The internal reference points (IRPs) and the external reference points (ERPs) are usually used to determine vertical dimension of maxilla, which is an important factor for confirming maxillary position. However, the IRPs are known to be inaccurate in determining the vertical dimension. In this study, we investigated the correlation of positional change of the modified IRPs with repositioned maxilla. Methods: The study group consisted of 26 patients with dentofacial deformities. For the simulation of the surgery, patient maxillary CT data and 3-D virtual surgery programs (V-$Works^{(R)}$ and V-$Surgery^{(R)}$) were used. IRPs of this study were set on both the lateral wall of piriform aperture, inferior margin of both infraorbital foramen, and the labial surfaces of the canine and first molar. The distance from the point on lateral wall of the piriform aperture to the point on the buccal surface of the canine was defined as IRP-C, and the distance from the point on the inferior margin of the infraorbital foramen to the point on the buccal surface of the $1^{st}$ molar was defined as IRP-M. After the virtual simulation of Le Fort I osteotomy, the changes in IRP-C and IRP-M were compared with the maxillary movement. All measures were analyzed statistically. Results: With respect to vertical movements, the IRP-C (approximately 98%) and the IRP-M (approximately 96%) represented the movement of the canine and the $1^{st}$ molar. Regarding rotating movement, the IRPs changed according to the movement of the canine and the $1^{st}$ molar. In particular, the IRP-C was changed in accordance with the canine. Conclusion: IRPs could be good indicators for predicting vertical movements of the maxilla during surgery.

Bone and Soft Tissue Changes after Two-Jaw Surgery in Cleft Patients

  • Background Orthognathic surgery is required in 25% to 35% of patients with a cleft lip and palate, for whom functional recovery and aesthetic improvement after surgery are important. The aim of this study was to examine maxillary and mandibular changes, along with concomitant soft tissue changes, in cleft patients who underwent LeFort I osteotomy and sagittal split ramus osteotomy (two-jaw surgery). Methods Twenty-eight cleft patients who underwent two-jaw surgery between August 2008 and November 2013 were included. Cephalometric analysis was conducted before and after surgery. Preoperative and postoperative measurements of the bone and soft tissue were compared. Results The mean horizontal advancement of the maxilla (point A) was 6.12 mm, while that of the mandible (point B) was -5.19 mm. The mean point A-nasion-point B angle was $-4.1^{\circ}$ before surgery, and increased to $2.5^{\circ}$ after surgery. The mean nasolabial angle was $72.7^{\circ}$ before surgery, and increased to $88.7^{\circ}$ after surgery. The mean minimal distance between Rickett's E-line and the upper lip was 6.52 mm before surgery and 1.81 mm after surgery. The ratio of soft tissue change to bone change was 0.55 between point A and point A' and 0.93 between point B and point B'. Conclusions Patients with cleft lip and palate who underwent two-jaw surgery showed optimal soft tissue changes. The position of the soft tissue (point A') was shifted by a distance equal to 55% of the change in the maxillary bone. Therefore, bone surgery without soft tissue correction can achieve good aesthetic results.

하악전돌증환자의 악교정수술후 안면측모 변화에 관한 두부방사선 계측학적 연구 (A CEPHALOMETRIC STUDY OF PROFILE CHANGES FOLLOWING ORTHOGNATHIC SURGERY IN PATIENTS WITH MANDIBULAR PROGNATHISM)

  • The purpose of this study was to examine soft tissue and hardtissue changes following orthognathic surgery in patients with mandibular prognathism lateral cephalometric films were obtained immediate before surgery, 48 hours following surgery, and 6 months following surgery. 18 patients were selected (10 men, and 6 women) for this study, who had received orthognathic surgery. Statistical analysis for the each time interval differences were performed with the SPSS package The results were as follows, *In the cases of mandibular sagittal split osteotomy 1 LI point was moved backward (average 7.55mm) 48 hours following surgery. 6 months later, it was returned forward (average 1.1mm) Relapse rate was 14.6% 2 Pog was moved backward (average 8.3mm) 48 hours following surgery The ratio of horizontal change of soft tissue to hard tissue at pog is 0.95 1 *In the cases of maxillary Le-Fort I osteotomy & mandibular sagittal split osteotomy. 3. A point was moved forward (average 3.31mm) 48 hours following surgery. 6 months later, it was returned backward (average 0.31) Relapse rate was 9 4% 4 6 months later, the ratio of facial convexity angle change of soft tissue to hard tissue is 0.63 1.

Factor XI deficiency and orthognathic surgery: a case report on anesthesia management

  • Factor XI deficiency (Hemophilia C) is a very rare autosomal recessive bleeding disorder. Patients with factor XI deficiency do not typically show any spontaneous bleeding or specific symptoms. Sometimes those who have this disorder are identified during special situations such as trauma or surgery. Orthognathic surgery is particularly associated with a high bleeding risk. Therefore, great care must be taken when treating patients with bleeding disorders such as factor XI deficiency. There are a few reports that address the management of patients with bleeding disorders during orthognathic surgery. The current report describes a patient with factor XI deficiency who underwent Le Fort I osteotomy together with bilateral sagittal split osteotomy. The patient's condition was assessed using both rotation thromboelastometry ($ROTEM^{TM}$) and noninvasive measurements of total hemoglobin (SpHb) using Masimo Radical 7 (Masimo Co. CA, USA).

자가결찰 브라켓과 골신장술을 이용한 구순구개열 환자의 치험례 (Cleft lip and palate patient treatment using self-ligating bracket and distraction osteogenesis: A case report)

  • It is difficult to perform orthodontic treatment for cleft lip and palate patient. Although there are many orthodontic appliances to expand narrowed maxillary arch, results are rarely successful and the possibility of relapse is increased due to severe scars. Self-ligating bracket, recently used in orthodontic treatment, suggests solution of crowding by expansion of dental arches. Light and continuous force could apply for orthodontic movement due to characteristic low friction of self ligating bracket, which gives expansion force until dentition reaches its new equilibrium position and it can be expressed as spontaneous lateral expansion with heavy labial tension. This kind of expansion force is thought to be a possibility of expanding the constricted maxillary arch of cleft lip and palate patient. Repositioning of the maxilla by Le Fort I osteotomy in case of severe maxillary deficiency, increases the possibility of relapse because of limitation in anterior movement and adaptation of soft tissue. In these cases, distraction osteogenesis(DO) can be applied for stable result. We report a case of cleft lip and palate patient with narrowed maxillary arch and maxillary deficiency using self ligating bracket and DO.

  • Missing of the upper posterior dentition can cause alveolar bone resorption & pneumatization of Maxillary sinus wall, which makes traditional implant placement impossible, The solution includes various methods to the posterior maxilla to provide adequate bone support for implant installation and long-term survival. -- sinus floor elevation, sinus-lift graft, inlay graft using LeFort I osteotomy, onlay graft, This is a clinical Sr. retrospective study on implant surgery & prosthodontic restoration with upper edentulous posterior jaw from Jan. 1990. to Jun. 1997 at implant clinic of Chonbuk National University Hospital. The results obtained were as follows: 1. Six hundred ninety-nine implants were placed on upper posterior jaw of two hundred seventeen patients, among them one hundred sixty-five implants were placed in forty-four patients with sinus lift. 2. The height of the remained alveolar bone was classified on the base of Misch's concept. This included seventy-nine SA-1s, ninety-seven SA-2s, sixty-five SA-3s and sixty SA-4s. 3. Ninety percent of implants were successfully integrated in non-grafted area and eighty-seven percent of implants were successully integrated in sinus lift area.
